Oral mesalamine and clinical remission are associated with a decrease in the extent of long-standing ulcerative colitis

Abstract Objective: To compare colonoscopy alone with surveillance biopsy for the determination of anatomic extent in long-standing ulcerative colitis (UC). To assess the influences of mesalamine use and clinical disease activity on the change of histologic extent with time. Materials and Methods: Disease extent (proctosigmoiditis, left-sided colitis, or pancolitis) measured by colonoscopy and surveillance biopsy was compared among 212 consecutive patients with long-standing UC. Among the 102 patients who had 2 consecutive colonoscopies with surveillance biopsies, the following influences on change in histologic extent were determined: disease activity, mesalamine use, age at disease onset, folic acid, corticosteroid and azathioprine/6-mercaptopurine use, and time between colonoscopies. Results: Agreement between gross and microscopic findings was poor (, = 0.39). Colonoscopy underestimated and overestimated extent in 25.9% and 8.5%, respectively. Microscopic distribution between consecutive colonoscopies remained the same in 60.8%. Where distribution changed, an increase was twice as common as a decrease in extent. There was no difference in age at onset, time between colonoscopies, or disease duration among those with an increase, decrease, or no change in extent. Clinical remission and oral mesalamine were independently associated with 10.7 and 5.8 times the odds of a decrease in disease extent, respectively. Folic acid, topical mesalamine, corticosteroids, and immunomodulators did not influence change in extent. Conclusions: UC extent is best determined by surveillance biopsy. Among patients with long-standing UC, histologic extent fluctuates with time. Disease remission and oral mesalamine were independently associated with decreases in disease extent. [source]


Are parametric models suitable for estimating avian growth rates?

For many bird species, growth is negative or equivocal during development. Traditional, parametric growth curves assume growth follows a sigmoidal form with prescribed inflection points and is positive until asymptotic size. Accordingly, these curves will not accurately capture the variable, sometimes considerable, fluctuations in avian growth over the course of the trajectory. We evaluated the fit of three traditional growth curves (logistic, Gompertz, and von Bertalanffy) and a nonparametric spline estimator to simulated growth data of six different specified forms over a range of sample sizes. For all sample sizes, the spline best fit the simulated model that exhibited negative growth during a portion of the trajectory. The Gompertz curve was the most flexible for fitting simulated models that were strictly sigmoidal in form, yet the fit of the spline was comparable to that of the Gompertz curve as sample size increased. Importantly, confidence intervals for all of the fitted, traditional growth curves were wholly inaccurate, negating the apparent robustness of the Gompertz curve, while confidence intervals of the spline were acceptable. We further evaluated the fit of traditional growth curves and the spline to a large data set of wood thrush Hylocichla mustelina mass and wing chord observations. The spline fit the wood thrush data better than the traditional growth curves, produced estimates that did not differ from known observations, and described negative growth rates at relevant life history stages that were not detected by the growth curves. The common rationale for using parametric growth curves, which compress growth information into a few parameters, is to predict an expected size or growth rate at some age or to compare estimated growth with other published estimates. The suitability of these traditional growth curves may be compromised by several factors, however, including variability in the true growth trajectory. Nonparametric methods, such as the spline, provide a precise description of empirical growth yet do not produce such parameter estimates. Selection of a growth descriptor is best determined by the question being asked but may be constrained by inherent patterns in the growth data. [source]


Stellar populations and surface brightness fluctuations: new observations and models

We investigate the use of surface brightness fluctuations (SBF) measurements in optical and near-IR bandpasses for both stellar population and distance studies. New V -band SBF data are reported for five galaxies in the Fornax cluster and combined with literature data to define a V -band SBF distance indicator, calibrated against Cepheid distances to the Leo group and the Virgo and Fornax clusters. The colour dependence of the V -band SBF indicator is only ,15 per cent steeper than that found for the I band, and the mean ,fluctuation colour' of the galaxies is We use new stellar population models, based on the latest Padua isochrones transformed empirically to the observational plane, to predict optical and near-IR SBF magnitudes and integrated colours for a wide range of population ages and metallicities. We examine the sensitivity of the predicted SBF,colour relations to changes in the isochrones, stellar transformations, and initial mass function. The new models reproduce fairly well the weak dependence of V and I SBF in globular clusters on metallicity, especially if the more metal-rich globulars are younger. Below solar metallicity, the near-IR SBF magnitudes depend mainly on age, while the integrated colours depend mainly on metallicity. This could prove a powerful new approach to the age,metallicity degeneracy problem; near-IR SBF observations of globular clusters would be an important test of the models. The models also help in understanding the and fluctuation colours of elliptical galaxies, with much less need for composite stellar populations than in previous models. However, in order to obtain theoretical calibrations of the SBF distance indicators, we combine the homogeneous population models into composite models and select out those ones with fluctuation colours consistent with observations. We are able to reproduce the observed range of elliptical galaxy colours, the slopes of the V and I SBF distance indicators against (fainter SBF in redder populations), and the flattening of the I -band relation for The models also match the observed slope of I -band SBF against the Mg2 absorption index and explain the steep colour dependence found by Ajhar et al. for the HST/WFPC2 F814W-band SBF measurements. In contrast to previous models, ours predict that the near-IR SBF magnitudes will also continue to grow fainter for redder populations. The theoretical V -band SBF zero-point predicted by these models agrees well with the Cepheid-calibrated V -band empirical zero-point. However, the model zero-point is 0.15,0.27 mag too faint in the I band and 0.24,0.36 mag too faint in K. The zero-points for the I band (empirically the best determined) would come into close agreement if the Cepheid distance scale were revised to agree with the recent dynamical distance measured to NGC 4258. We note that the theoretical SBF calibrations are sensitive to the uncertain details of stellar evolution, and conclude that the empirical calibrations remain more secure. However, the sensitivity of SBF to these finer details potentially makes it a powerful, relatively unexploited, constraint for stellar evolution and population synthesis. [source]


The sonographic diagnosis of chorionicity

Abstract The differentiation between mono- and dichorionic placentation in twin pregnancies is of clinical importance because of the significant difference in perinatal morbidity and mortality between the two, and the increased surveillance indicated in monochorionic gestations. Application of ultrasonography has enabled very precise prenatal determination of chorionicity. While this is best performed in the first trimester when accuracy approaches 100%, even in the third trimester, using a composite cascade of available sonographic features, accuracy has been reported to approach 97%. While two clearly separate placentae or discordant fetal gender conform to dichorionicity, in most twin pregnancies other features need to be assessed to determine chorionicity. The presence of the ,lambda' or the ,T' sign in the presence of a single placenta, best determined in the first trimester, is the most reliable indicator of chorionicity, with measurements of the inter-twin membrane thickness and counting of the membrane layers being less reliable. Usefulness of iodine/creatinine ratio from spot-urine samples to evaluate the effectiveness of low-iodine diet preparation for radioiodine therapy

Summary Objective, The success of a low-iodine diet (LID) is best determined by measurement of 24-h urine iodine (U-I) excretion. The aim of this study was to determine reliable estimates for 24-h U-I based on spot-urine samples and to provide cut-offs to determine the effectiveness of LID preparation. Design, We prospectively measured iodine levels in 193 patients based on 24-h- and spot-urine samples before radioactive iodine therapy. The iodine was expressed as the 24-h U-I excretion (,g/day) and as two different indices from spot urine, simple iodine concentration (simple I) and the iodine/creatinine (I/Cr) ratio. Poor LID preparation was defined as I excretion of >150 ,g/day according to the 24-h U-I measurement. Results, The measured 24-h U-I was significantly higher than the two indices from spot urine (P < 0·001). However, there were statistically significant correlations between the 24-h U-I values and the two spot-urine-based indices; the correlation coefficient was 0·539 for simple I and 0·773 for I/Cr ratio (P < 0·001). The cut-off of I/Cr ratio for poor LID preparation was >66·2 ,g/g Cr (sensitivity 96·4%, specificity 83·6%, positive predictive value 50·0% and negative predictive value 99·3%). Conclusions, We demonstrated that the I/Cr ratio from spot urine could serve as a useful and reliable alternative to 24-h urine collection as it has acceptable diagnostic values for detecting poor LID preparation.
