Experiences and concerns about ,returning to work' for women breast cancer survivors: a literature review

Abstract Objective: To explore how female breast cancer patients experience work incapacity during the treatment and return-to-work phases and how interactions between patients and stakeholders affect this experience. Method: Database search for full text articles published between January 1995 and January 2008 that focused on employed female breast cancer patients, factors related to work incapacity, and returning to work. Only results based on self-report data were included. Studies focusing on treatment, financial factors, rate of return, or absence were excluded. Results: Six articles met the inclusion criteria. Women with breast cancer receive varied reactions but little advice about returning to work. Women were primarily concerned with disclosing the diagnosis to their employer and to relatives. Uncertainties about physical appearance, ability to work, and possible job loss affected the women's decisions about working during the treatment phase. After treatment, most women wanted to regain their ,normal life', but concentration and arm or fatigue problems potentially interfered. Although supportive work environments were helpful, the individual needs of women differed. Employers and employees need to find a balance in defining accommodating work. Many women received favourable support, but some reported feeling discriminated against. Many women re-evaluated the role of work in their lives after being confronted with breast cancer. Conclusion: Work adjustments could help women to keep their jobs during illness and recovery. To resolve women's concerns about returning to work, employers, physicians, and insurance institutions should consider increasing and improving communication with breast cancer patients and playing a more active and supportive role. Expatriate Social Ties: Personality Antecedents and Consequences for Adjustment

This expaloratory study examines the relationship between personality characteristics (extraversion, core self evaluations), social tie characteristics (number, breadth, depth), and three types of expatriate adjustment (general, interaction, and work). Data was collected at two points in time from 75 expatriate employees from one organization on international assignments around the world. Results indicate that core self-evaluations, but not extraversion, are positively related to the number of ties formed with other expatriates and host country nationals. Social ties with other expatriates were found to provide greater social support, but similar access to information, than those with host country nationals (HCNs). In general, depth and breadth of relationships with other expatriates predicted general and work adjustment; whereas, breadth and total number of relationships with HCNs predicted all three types of adjustment. Overall, these results provide initial support for the importance of social ties in facilitating expatriate adjustment. [source]


Occupational status and social adjustment six months after hospitalization early in the course of bipolar disorder: a prospective study

Journal compilation © 2010 John Wiley & Sons A/S. Objectives:, Bipolar disorder is often accompanied by poor functional outcomes, the determinants of which are not fully understood. We assessed patients with bipolar disorder undergoing a hospital admission early in the illness course and identified predictors of occupational status, overall social adjustment, and work adjustment six months later. Methods:, This was a prospective longitudinal cohort study. During hospitalization patients were evaluated with a cognitive battery; symptoms, occupational history, and other clinical factors were also assessed. At six-month follow-up, patients' symptom remission status was assessed; they were also evaluated as to their occupational status, overall social adjustment, and work adjustment. Multivariate analyses were used to identify predictors of these outcomes. Results:, Among the 52 participants, the average rating of overall social adjustment at follow-up was between mild and moderate maladjustment. While 51 had a history of working full time, only 28 (54%) worked full time at follow-up. A total of 24 (46%) had symptoms that met criteria for a full depression or mania syndrome. In multivariate analyses, full-time occupational status at follow-up was predicted by the absence of baseline substance abuse. Better overall social adjustment was predicted by better performance on cognitive tasks of processing speed and by symptom remission; the latter variable also predicted work adjustment. Conclusions:, Persons with bipolar disorder have limited occupational recovery and overall social adjustment six months after a hospital admission early in the illness course. Predictors vary among outcomes; performance on tasks of processing speed and the extent of symptom remission are independently associated with functional outcomes. [source]


Personality style, psychological adaptation and expectations of trainee clinical psychologists

The literature suggests that personality affects how individuals experience stress, cope, utilize social support and psychologically adapt. This study examined the personality style, psychological adaptation and expectations of 364 UK trainee clinical psychologists. Personality traits characterizing the sample were: enhancing, modifying, extraversing, outgoing, and agreeing. Mean overall personality adjustment was significantly better than the normal population. The sample scored significantly poorer on self-esteem, anxiety and depression, but well outside the defined range for poor adaptation. A small percentage (8%, n = 31) had ,poor' personality adjustment scores, scored worse on indicators of psychological adaptation and reported greater shortfall of course aspects compared with expectations. However, 41% scored as having a significant problem on one or more of: anxiety, depression, low self-esteem and work adjustment. About one-third had a probable substance use problem. In regression analyses, poor personality adjustment, and different areas of expectation-shortfall, especially impact of training on life variously predicted anxiety, depression, and poor work adjustment. Satisfaction with social support was not predictive of adaptation after personality adjustment was accounted for. Implications for clinical psychology training include the need for enhanced expectation-management through pre-course marketing, and better attention to trainees' personal and professional development throughout training.
