Assessment and Intervention to Teach Age Recognition Skills: a Suggested Programme Using the Example of a Case Study to Illustrate

A 43-year-old man with mild intellectual disabilities was referred after several contacts with the law when he had been seen in the street talking to children who were unknown to him. Some staff members feared that he had a sexual interest in children. Other colleagues considered that the behaviour was innocent, reflecting his friendly nature, but that it was a behaviour which was misinterpreted by others. The present paper charts the assessment of this man's difficulties and abilities over a wide range of relevant areas, and a reassessment following an intensive period of one-to-one training on age-recognition skills. Although the training was unsuccessful, the measures used assisted in making a detailed assessment and providing clear instructions to care staff, all of which was to the subject's long-term benefit. The present paper also illustrates the way in which difficulties of this nature can be assessed and uses allied assessment measures which may be helpful in looking at the whole picture of any person with such problems. It also considers how these areas may then be addressed. Although, in this case, the subject was unable to learn to discriminate specific age groups despite intensive training, he was consequently able to be advised in a manner which should keep him from getting into trouble in the future and the now detailed knowledge of his abilities in all these areas is valuable for his long-term support in the community. Seizing possibilities for positive family caregiving in nursing homes

Aims., This paper explores the ways family members reconstruct meaning through seizing possibilities for positive caregiving in nursing homes. Background., The importance of the ability of family caregivers to adapt and accommodate has been well documented in international family caregiving research. Through engagement in caregiving activities, families learn to modify, adapt and accommodate to changes in their situation and relationships. The support family caregivers experience in learning to accommodate change is crucial in enabling them to reconstruct positive aspects of caregiving in a long-term aged care context. Method., In this study, a hermeneutic phenomenological approach was adopted informed by the philosophical world views of Heidegger and Gadamer. Data collected by in-depth interviews and participant observations, from a purposeful sample of 14 family caregivers, underwent hermeneutic analysis. Results., Five shared meanings associated with seizing possibilities for positive caregiving were revealed: accommodating new and different ways of caring; feeling a part of the nursing home community; seeing the whole picture; learning to care in stress-reducing ways and learning to seize possibilities for self. Conclusion., This paper illustrates how families, through caregiving experience in nursing homes, learn to become active managers, negotiating, accommodating and redeveloping a sense of future viewed with hope, strength and positive anticipation. Relevance to clinical practice., By highlighting what is attributed significance by families, a critical examination of the difficult issues which obstruct the development of meaningful partnerships among nurses, family and their relatives is facilitated. In particular, an examination of tensions at an ideological level supports the need for future research to focus its efforts on examining the ways of implementing nursing care that facilitates partnerships that incorporate and build upon positive and equal relations among staff, families and residents in the context of the nursing home setting. Creation and Construction: On the Theological Appropriation of Postmodern Theory

Postmodern theory can be appreciated by theological anthropology along the following lines: it interprets the cultural conditions that shape personal identity, including the elements of construction and contingency in identity-formation. It emphasizes the necessity for a multifaceted approach to the question about what it means to be human, and for avoiding closure. This is expressed in the doctrine of the human as created, as sinner and as restored,as none of these perspectives captures the whole picture. Postmodern theory also focuses on the importance of otherness for establishing identity, thereby offering a new way of interpreting human beings as created in the image of God. [source]


Value-based segmentation of luxury consumption behavior

Following a broader perspective in exploring customer perceptions of and motives for purchasing luxury brands, it is not sufficient to explain the whole picture of luxury consumption in terms of socially oriented consumer motives and the desire to impress others. The main contribution here is to explore a multidimensional framework of luxury value as a general basis for identifying value-based consumer segments. Roles of lipid-soluble vitamins during ontogeny of marine fish larvae

Abstract The roles of lipid-soluble vitamins during ontogeny of marine fish larvae are a subject topic where only fragments of the whole picture are known. Most of the research has been focussed on the larval requirements and the availability of these vitamins in the live feed organisms used for early-stage larvae, while the function of the vitamins in the larvae themselves is largely unknown. Our knowledge is mostly extrapolated from research on other vertebrates and also in part from juvenile and adult fish. Vitamin A is known to be essential for establishing body and organ axes in vertebrate embryos and interacts with other nutrients such as vitamin D and fatty acids through the steroid/thyroid nuclear hormone receptor family. In marine fish larvae, excess vitamin A stimulates pigmentation, but at the same time induces vertebral deformities. Live feed organisms contain very little vitamin A but marine fish larvae appear to convert carotenoids in Artemia and copepods to vitamin A, while rotifers, which contain little carotenoids, should be enriched with vitamin A. Vitamin E acts as an antioxidant and is important for the protection of marine fish larvae against the oxidation pressure probably present in intensive rearing systems. Vitamin E may also have other roles connected to its modulation of cell and tissue red-ox balance. In marine fish larvae and juveniles, vitamin E has been shown to enhance the symptoms of vitamin C deficiency, while protecting against the oxidative effect of n-3 fatty acids. Vitamin D is important for the modulation of calcium and phosphorus homeostasis and for the development of the vertebrate skeleton. Vitamin K influences bone development and coagulation of the blood. There is little information on vitamins D and K connected to the ontogeny of marine fish larvae. [source]


The cell death machinery governed by the p53 tumor suppressor in response to DNA damage

The cellular response to genotoxic stress that damages DNA includes cell cycle arrest, activation of DNA repair, and in the event of irreparable damage, induction of apoptosis. However, the signals that determine cell fate, that is, survival or apoptosis, are largely unclear. The tumor suppressor p53 has been implicated in many important cellular processes, including regulation of apoptotic cell death. When cells encounter genotoxic stress, certain sensors for DNA lesions eventually stabilize and activate p53. Subsequently, p53 exerts its tumor suppressor function by transactivating numerous target genes. Active p53 is subjected to a complex and diverse array of covalent post-translational modifications, which selectively influence the expression of p53 target genes. In this regard, the molecular basis for how p53 induces apoptosis has been extensively studied; however, the relative contribution of each downstream effecter is still to be explored. Moreover, little is known about precise mechanisms by which modified p53 is capable of apoptosis induction. A thorough understanding for the whole picture of p53 modification in apoptosis will be extremely valuable in the development of highly effective and specific therapies for caner patients. 2325: Dynamic retinal vessel analysis , how different parameters create the whole picture

Purpose Dynamic vessel analysis is usually associated with the observation of the reaction of retinal vessels to a defined stimulus. The data which is generated this way may be further analysed with respect to the dynamic unstimulated and stimulated vessel behaviour. Assessment of different parameters may highlight different aspects of the underlying disease. Methods Vessel diameters of retinal vessel segments were assessed by Dynamic Vessel Analyzer (DVA) in healthy volunteers of different age groups and patients with diabetes, glaucoma and systemic hypertension. Mathematical analysis of unstimulated vessels was used to describe vessel wall characteristics. Methods of signal analysis including Fourier Transformation, spectral filtration, auto- and cross correlation were applied to evaluate characteristic oscillations and pulse wave propagation along the vessel. Results Characteristic different vessel behaviour and vessel wall conformation are obtained by dynamic quantitative evaluations from the unstimulated vessels in physiologic aging and disease. Conclusion Dynamic vessel analysis includes information which may lead to further understanding of the vascular status and underlying disease pathology. It is also feasible to assess pulse wave velocities in retinal arterioles und thus clinically characterize the elasticity of the upstream vasculature in health and disease. [source]


The magnocellular theory of developmental dyslexia

Abstract Low literacy is termed ,developmental dyslexia' when reading is significantly behind that expected from the intelligence quotient (IQ) in the presence of other symptoms,incoordination, left,right confusions, poor sequencing,that characterize it as a neurological syndrome. 5,10% of children, particularly boys, are found to be dyslexic. Reading requires the acquisition of good orthographic skills for recognising the visual form of words which allows one to access their meaning directly. It also requires the development of good phonological skills for sounding out unfamiliar words using knowledge of letter sound conversion rules. In the dyslexic brain, temporoparietal language areas on the two sides are symmetrical without the normal left-sided advantage. Also brain ,warts' (ectopias) are found, particularly clustered round the left temporoparietal language areas. The visual magnocellular system is responsible for timing visual events when reading. It therefore signals any visual motion that occurs if unintended movements lead to images moving off the fovea (,retinal slip'). These signals are then used to bring the eyes back on target. Thus, sensitivity to visual motion seems to help determine how well orthographic skill can develop in both good and bad readers. In dyslexics, the development of the visual magnocellular system is impaired: development of the magnocellular layers of the dyslexic lateral geniculate nucleus (LGN) is abnormal; their motion sensitivity is reduced; many dyslexics show unsteady binocular fixation; hence poor visual localization, particularly on the left side (left neglect). Dyslexics' binocular instability and visual perceptual instability, therefore, can cause the letters they are trying to read to appear to move around and cross over each other. Hence, blanking one eye (monocular occlusion) can improve reading. Thus, good magnocellular function is essential for high motion sensitivity and stable binocular fixation, hence proper development of orthographic skills. Many dyslexics also have auditory/phonological problems. Distinguishing letter sounds depends on picking up the changes in sound frequency and amplitude that characterize them. Thus, high frequency (FM) and amplitude modulation (AM) sensitivity helps the development of good phonological skill, and low sensitivity impedes the acquisition of these skills. Thus dyslexics' sensitivity to FM and AM is significantly lower than that of good readers and this explains their problems with phonology. The cerebellum is the head ganglion of magnocellular systems; it contributes to binocular fixation and to inner speech for sounding out words, and it is clearly defective in dyslexics. Thus, there is evidence that most reading problems have a fundamental sensorimotor cause. But why do magnocellular systems fail to develop properly? There is a clear genetic basis for impaired development of magnocells throughout the brain. The best understood linkage is to the region of the Major Histocompatibility Complex (MHC) Class 1 on the short arm of chromosome 6 which helps to control the production of antibodies. The development of magnocells may be impaired by autoantibodies affecting the developing brain. Magnocells also need high amounts of polyunsaturated fatty acids to preserve the membrane flexibility that permits the rapid conformational changes of channel proteins which underlie their transient sensitivity. But the genes that underlie magnocellular weakness would not be so common unless there were compensating advantages to dyslexia. In developmental dyslexics there may be heightened development of parvocellular systems that underlie their holistic, artistic, ,seeing the whole picture' and entrepreneurial talents.
