Update: Effects of Antioxidant and Non-Antioxidant Vitamin Supplementation on Immune Function

The purpose of this manuscript is to review the impact of supplementation with vitamins E and C, carotenoids, and the B vitamins on parameters of innate and adaptive immune function as reported from clinical trials in humans. There is evidence to support causal effects of supplementation with vitamins E and C and the carotenoids singly and in combination on selected aspects of immunity, including the functional capacity of innate immune cells, lymphocyte proliferation, and the delayed-type hypersensitivity (DTH) response. Controlled intervention trials of B vitamin-containing multivitamin supplements suggest beneficial effects on immune parameters and clinical outcomes in HIV-positive individuals [source]


Multivitamin supplementation in HIV-positive pregnant women: impact on depression and quality of life in a resource-poor setting

Objectives The primary objective of this study was to examine the effect of vitamin supplementation on health-related quality of life and the risk of elevated depressive symptoms comparable to major depressive disorder (MDD) in HIV-positive pregnant women in Dar es Salaam, Tanzania. Methods From April 1995 to July 1997, 1078 HIV-positive pregnant women were enrolled in a randomized controlled trial. We examined the effects of vitamin supplementation on quality of life and the risk of elevated depressive symptoms, assessed longitudinally every 6,12 months. Results A substantial prevalence of elevated depressive symptoms (42%) was observed in HIV-positive pregnant women. Multivitamin supplementation (B-complex, C and E) demonstrated a protective effect on depression [relative risk (RR)=0.78; P=0.005] and quality of life [RR=0.72 for social functioning (P=0.001) and vitality (P=0.0001); RR=0.70 for role-physical (P=0.002)]; however, vitamin A showed no effect on these outcomes. Conclusions Multivitamin supplementation (B-complex, C and E) resulted in a reduction in risk of elevated depressive symptoms comparable to MDD and improvement in quality of life in HIV-positive pregnant women in Tanzania. Effect of prepared diet and vitamins A, E and C supplementation on the reproductive performance of cage-reared bighead carp Aristichthys nobilis (Richardson)

Twenty-month-old bighead carp, Aristichthys nobilis (Richardson), were fed prepared dry diets for 20 months in cages in Laguna de Bay, Philippines, to determine the effect on reproductive performance. The experimental diets were similar in composition except for the combinations of vitamins being tested. Diet 1 was supplemented with vitamins A, E and C; diets 2, 3 and 4 each lacked one of the supplementary vitamins; and diet 5 did not include any vitamin supplementation. Bighead carp that relied solely on natural food without a prepared diet served as a control. The total of six treatments each had two replicates. Results showed that the onset of gonad maturation was 2,3 months earlier in the fish that were fed the prepared diets regardless of vitamin supplementation, when compared with the fish that were fed natural food (control). Moreover, the prepared diets enhanced egg hatchability which was significantly higher in fish that were fed diet 1 (+ vitamins A, E and C, 80.5 ± 18.1%) and diet 3 (, vitamin E, 78.5 ± 1.1%) than in those fish that were fed natural food (control) (36.5 ± 31.3%). Mean number of 3-day-old larvae was highest in fish fed on diet 1 (34 525 ± 1732), followed closely by fish that were fed diet 3 (32 420 ± 3909). A low number of 3-day-old larvae was obtained from fish fed the natural diet (14 490 ± 4331) as well as in fish that were fed diet 2 (, vitamin A, 14 347 ± 4863), diet 4 (, vitamin C, 21 407 ± 5840) and diet 5 (, vitamin A, E and C, 12 191 ± 1439). Other criteria for reproduction such as relative fecundity, fertilization rate, and hatching rate did not differ significantly (P >,0.05) among treatments. The addition of vitamins also had no significant effects on weight gain of adult fish. [source]


Clinical trial: B vitamins improve health in patients with coeliac disease living on a gluten-free diet

Summary Background, Patients with coeliac disease living on a gluten-free diet show vitamin deficiency and reduced subjective health status. Aim, To study the biochemical and clinical effects of B vitamin supplementation in adults with longstanding coeliac disease. Methods, In a double blind placebo controlled multicentre trial, 65 coeliac patients (61% women) aged 45,64 years on a strict gluten-free diet for several years were randomized to a daily dose of 0.8 mg folic acid,0.5 mg cyanocobalamin and 3 mg pyridoxine or placebo for 6 months. The outcome measures were psychological general well-being (PGWB) and the plasma total homocysteine (tHcy) level, marker of B vitamin status. Results, Fifty-seven patients (88%) completed the trial. The tHcy level was baseline median 11.7 ,mol/L (7.4,23.0), significantly higher than in matched population controls [10.2 ,mol/L (6.7,22.6) (P < 0.01)]. Following vitamin supplementation, tHcy dropped a median of 34% (P < 0.001), accompanied by significant improvement in well-being (P < 0.01), notably Anxiety (P < 0.05) and Depressed Mood (P < 0.05) for patients with poor well-being. Conclusions, Adults with longstanding coeliac disease taking extra B vitamins for 6 months showed normalized tHcy and significant improvement in general well-being, suggesting that B vitamins should be considered in people advised to follow a gluten-free diet. Iron and Colorectal Cancer Risk: Human Studies

Some reports have associated iron with cancer risk, particularly of the colorectum. This review will focus on the human studies that have investigated this association. Comparative studies were sought in which people with and without colorectal neoplastic lesions, either cancers or adenomatous polyps, were assessed for iron exposure. Iron exposure variables included dietary iron intake, iron vitamin supplementation, body iron stores as measured by ferritin or transferrin saturation, and gene status for hereditary hemochromatosis. Medline was searched for published reports using the key words iron, cancer, colon, rectum, ferritin, transferrin, and hemochromatosis. In addition, the Cochrane Library was searched for relevant studies and several authors were contacted to investigate their awareness of unpublished studies. Studies were categorized by study design and ranked for quality of innovation in design, sample size, and thoroughness of iron status ascertainment. Thirty-three studies were reviewed in 26 publications. Of the larger studies, approximately three-quarters supported the association of iron, in all three strata of exposure, with colorectal neoplasia risk. Because iron is broadly supplemented in the American diet, the benefits of iron supplementation need to be measured against the long-term risks of increased iron exposure, one of which may be increased risk of colorectal cancer. Vitamin supplement use among children with Down's syndrome and risk of leukaemia: a Children's Oncology Group (COG) study

Summary Vitamin supplements have been proposed for children with Down's syndrome (DS) with claims of improving cognitive abilities, or immune or thyroid function. Several studies have shown decreased levels of zinc in this population. Because children with DS have a 50-fold increased risk of developing acute leukaemia during the first 5 years of life, we explored the relation between child vitamin and herbal supplement use and the risk of leukaemia in a case-control study. During the period 1997,2002, we enrolled 158 children with DS aged 0,18 years that were diagnosed with acute lymphoblastic leukaemia (ALL) (n = 97) or acute myeloid leukaemia (AML) (n = 61) at participating Children's Oncology Group institutions. We enrolled 173 DS children without leukaemia (controls), selected from the cases' primary care clinic and frequency-matched on age. Data were collected via telephone interviews with mothers of the index child regarding use of multivitamins, zinc, vitamin C, iron and herbal supplements, including age at first use, frequency and duration. Among controls, 57% reported regular multivitamin use (,3 times/week for ,3 months) compared with 48% of ALL cases and 61% of AML cases. We found no evidence of an association between children's regular multivitamin use and ALL or AML (adjusted odds ratios [OR] = 0.94 [95% CI 0.52, 1.70] and 1.90 [0.73, 4.91] respectively). There was a suggestion of an increased risk for AML associated with regular multivitamin use during the first year of life or for an extended duration (ORs = 2.38 [0.94, 5.76] and 2.59 [1.02, 6.59] respectively). Despite being the largest study of DS-leukaemia, our sample size was small, resulting in imprecise effect estimates. Future research should include larger sample sizes as well as a full assessment of diet including vitamin supplementation to adequately examine the relation between nutritional status and childhood leukaemia. BT03 SILASTIC RING MINI GASTRIC BYPASS FOR MORBID OBESITY: THE NEW ZEALAND EXPERIENCE

Purpose: , The laparoscopic mini gastric bypass (LMGB) is purportedly a technically simpler, yet equally effective operation to the laparoscopic Roux-en-Y gastric bypass as treatment for morbid obesity. This study reports the early results of LMGB in a major New Zealand bariatric centre. Methodology: , Clinical data was prospectively collected on all patients undergoing LMGB over a two year period. Results: , 142 patients were studied. 77% were females. Mean age was 43.8. Pre-operative mean body weight and body mass index (BMI) were 121.3 kg and 45.4 kg/m2 respectively. Mean BMI at one and two years follow up had decreased to 27.35 and 25.72 kg/m2 respectively. 83% of patients reported obesity associated co-morbidities pre-operatively. Post-operatively, 78% of patients reported a reduction in medication requirement. All surgery was performed laparoscopically. There were no anastomotic leaks and zero mortality. 8% of patients required further operations for complications or revision to a Roux-en-Y gastric bypass. 20% of patients required subsequent endoscopic interventions, the majority for investigation of vomiting and/or pain. 22% of patients required re-admission. 14% of patients reported new onset reflux or worsening of pre-existing reflux after LMGB. 82% of patients reported increased exercise capability post LMGB. 54% of patients required vitamin supplementation. Conclusion: , LMGB achieves significant weight loss and resolution of obesity related co-morbidities with a low short term complication rate. LMGB should be considered as a safe and simple surgical option for morbid obesity. [source]


Dietary vitamin C and E supplementation and reproduction of milkfish Chanos chanos Forsskal

Milkfish Chanos chanos Forsskal broodstock (11 years old, average body weight 5.23,5.73 kg) reared in 10-m-diameter by 3-m-deep floating net cages (31,36 fish per cage) at SEAFDEC AQD's Igang Marine Substation in Guimaras Island, central Philippines, were fed daily at 3% of total body weight formulated diets (36% protein, 7,8% lipid) supplemented with 0.1% vitamin C, 0.05% vitamin E, both vitamin C and E or no vitamin supplementation (control) for 3 years. Reproductive performance was assessed in an attempt to determine the optimum nutrition for successful spawning of milkfish. The total egg production, mean number of eggs per spawning, number of spawns and mean egg diameter were not affected by dietary vitamin C and E supplementation. However, broodstock given dietary supplementation of vitamin C alone or in combination with vitamin E had a higher percentage of spawns with higher (> 90%) percentage egg viability, hatching and cumulative survival rate than those of the control. Broodstock given dietary vitamin E supplementation alone had few spawns, which made the results difficult to analyse. The results confirm the essentiality of vitamin C supplementation in producing more spawns with good egg and larval quality. The production of an adequate volume of good quality eggs and larvae to support hatchery operation is necessary to offset the huge investment in broodstock development, as it takes at least 5 years for milkfish to attain sexual maturation and spawning. Association between hyperhomocysteinaemia and abdominal aortic aneurysm

Background: Hyperhomocysteinaemia is associated with occlusive vascular disease. In vitro evidence has demonstrated the induction of a serine elastase by homocysteine in vascular smooth muscle. Anecdotal evidence from case reports and post-mortem studies has suggested an association with abdominal aortic aneurysm (AAA). The aim was to determine the prevalence of hyperhomocysteinaemia in patients with AAA. Methods: Some 120 subjects (60 controls and 60 patients with AAA) were studied prospectively. Epidemiological, clinical and haematological data were collected. Patients were defined as having AAA if ultrasonographic measurement of the aorta was greater than 4·5 cm. Those with evidence of occlusive peripheral vascular disease or an ankle: brachial pressure index lower than 0·8 were excluded. Homocysteine was measured with a commercial high-pressure liquid chromatography analyser. The reference range from age-matched controls was 8·9,14·3 µmol l,1. Results: The median(s.d.) value of homocysteine for patients was significantly higher than that for the control group: 13·1(7·88) versus 10·9(5·07) µmol l,1 (P = 0·03, Mann,Whitney U test). Hyperhomocysteinaemia (homocysteine concentration greater than 14·3 µu;mol l,1) was present in 48 per cent of patients with AAA, compared with 24 per cent of the control population (P < 0·01, ,2 test). There were no significant differences between groups with regard to age, folate levels, vitamin B12 concentration or renal function. Conclusion: These results strongly suggest an association between hyperhomocysteinaemia and AAA. If studies currently ongoing demonstrate a causal relationship between hyperhomocysteinaemia and vascular disease progression, it raises the possibility of treating small aneurysms with vitamin supplementation to slow their growth. © 2001 British Journal of Surgery Society Ltd [source]
