Selected AbstractsInsulin therapy in type 2 diabetes: what is the evidence?DIABETES OBESITY & METABOLISM, Issue 5 2009Mariëlle J. P. Van Avendonk Aim:, To systematically review the literature regarding insulin use in patients with type 2 diabetes mellitus Methods:, A Medline and Embase search was performed to identify randomized controlled trials (RCT) published in English between 1 January 2000 and 1 April 2008, involving insulin therapy in adults with type 2 diabetes mellitus. The RCTs must comprise at least glycaemic control (glycosylated haemoglobin (HbA1c), postprandial plasma glucose and /or fasting blood glucose (FBG)) and hypoglycaemic events as outcome measurements. Results:, The Pubmed search resulted in 943 hits; the Embase search gave 692 hits. A total of 116 RCTs were selected by title or abstract. Eventually 78 trials met the inclusion criteria. The studies were very diverse and of different quality. They comprised all possible insulin regimens with and without combination with oral medication. Continuing metformin and/or sulphonylurea after start of therapy with basal long-acting insulin results in better glycaemic control with less insulin requirements, less weight gain and less hypoglycaemic events. Long-acting insulin analogues in combination with oral medication are associated with similar glycaemic control but fewer hypoglycaemic episodes compared with NPH insulin. Most of the trials demonstrated better glycaemic control with premix insulin therapy than with a long-acting insulin once daily, but premix insulin causes more hypoglycaemic episodes. Analogue premix provides similar HbA1c, but lower postprandial glucose levels compared with human premix, without increase in hypoglycaemic events or weight gain. Drawing conclusions from the limited number of studies concerning basal,bolus regimen seems not possible. Some studies showed that rapid-acting insulin analogues frequently result in a better HbA1c or postprandial glucose without increase of hypoglycaemia than regular human insulin. Conclusion:, A once-daily basal insulin regimen added to oral medication is an ideal starting point. All next steps, from one to two or even more injections per day should be taken very carefully and in thorough deliberation with the patient, who has to comply with such a regimen for many years. López Arrieta Abstract Alzheimer's disease (AD) is a growing worldwide medical, social, and economic problem. In all countries, both prevalence and incidence of this disorder increase with age. The task of translating scientific clinical research into effective interventions for dementia has proved to be a difficult challenge. Data about the effects of therapeutic interventions come from several sources of evidence, ranging from studies with little potential for systematic bias and minimal random error, such as well-designed randomized controlled trials, through controlled but nonrandomized cohort and case-control studies, all the way to opinions based on laboratory evidence or theory. Although clinical trials are widespread in AD, there is increasing recognition that the results of studies do not necessarily apply to the type of patients that are seen by clinicians because of differences in patient characteristics, comorbidities, cotherapies, severity of disease, compliance, local circumstances, and patients preferences, which may differ sufficiently from those in the trial situation to attenuate or change the benefit-to-risk ratio. There are several methods to address those issues, like pragmatic trials and n-of-1 trials. When data from randomized clinical trials do not provide clear answers from sufficiently similar studies in the magnitude of effect sizes, lack of statistical significance, or identification of subgroups, systematic reviews and meta-analysis may help to provide a better summary of the data. A major difference between a traditional review and a systematic is the systematic nature in which studies are chosen and appraised. Traditional reviews are written by experts in the field who use differing and often subjective criteria to decide what studies to include and what weight to give them, and hence the conclusions are often very diverse, depending on the reviewer. Publication and selection bias is a major concern of traditional reviews. Systematic reviews and meta-analysis are being increasingly used in dementia, propelled by the Cochrane Dementia and Cognitive Improvement Group, to make decisions about treatment, management, and care and to guide future research. [source] Niche heterogeneity determines bacterial community structure in the termite gut (Reticulitermes santonensis)ENVIRONMENTAL MICROBIOLOGY, Issue 7 2005Hong Yang Summary Differences in microenvironment and interactions of microorganisms within and across habitat boundaries should influence structure and diversity of the microbial communities within an ecosystem. We tested this hypothesis using the well characterized gut tract of the European subterranean termite Reticulitermes santonensis as a model. By cloning and sequencing analysis and molecular fingerprinting (terminal restriction fragment length polymorphism), we characterized the bacterial microbiota in the major intestinal habitats , the midgut, the wall of the hindgut paunch, the hindgut fluid and the intestinal protozoa. The bacterial community was very diverse (> 200 ribotypes) and comprised representatives of several phyla, including Firmicutes (mainly clostridia, streptococci and Mycoplasmatales -related clones), Bacteroidetes, Spirochaetes and a number of Proteobacteria, all of which were unevenly distributed among the four habitats. The largest group of clones fell into the so-called Termite group 1 (TG-1) phylum, which has no cultivated representatives. The majority of the TG-1 clones were associated with the protozoa and formed two phylogenetically distinct clusters, which consisted exclusively of clones previously retrieved from the gut of this and other Reticulitermes species. Also the other clones represented lineages of microorganisms that were exclusively recovered from the intestinal tract of termites. The termite specificity of these lineages was underscored by the finding that the closest relatives of the bacterial clones obtained from R. santonensis were usually derived also from the most closely related termites. Overall, differences in diversity between the different gut habitats and the uneven distribution of individual phylotypes support conclusively that niche heterogeneity is a strong determinant of the structure and spatial organization of the microbial community in the termite gut. [source] How good is the fossil record of arthropods?GEOLOGICAL JOURNAL, Issue 3-4 2001An assessment using the stratigraphic congruence of cladograms Abstract A total of 179 published cladograms of arthropods was tested for their congruence with the palaeontological record. Congruence for data sets including 510 tetrapod and 157 fish trees was also calculated for comparison. Results provide support for the hypothesis that the fossil record of arthropods is less complete (at levels of stratigraphic and taxonomic resolution pertinent to most macroevolutionary studies) than the records of several other taxa. Differences in tree size, shape, balance and the distribution of first stratigraphic appearances (potential sources of bias) were controlled for by various randomization procedures. Most measures of congruence for arthropods were statistically poorer than those for other groups, even when the sample was divided into broad temporal bins. Many of the most robust and widely reproduced sister groupings of arthropods are attended by ghost ranges of many tens or hundreds of millions of years. Fossils of a number of presently very diverse and abundant arthropod groups are conspicuously absent from the record, despite many spectacular examples of the detailed preservation of others. There is probably no single reason for the apparently poor record of arthropods. Low preservation potential, small size of individuals, small numbers of individuals, and restricted palaeobiogeography almost certainly play a role in particular cases. The architectures obtained are determined by the size and concentration ratios of the microspheres in the dispersions, are very diverse, and can be obtained reproducibly over millimeter length scales. [source] Pharmaceutical antibiotic compounds in soils , a reviewJOURNAL OF PLANT NUTRITION AND SOIL SCIENCE, Issue 2 2003Sören Thiele-Bruhn Antibiotics are highly effective, bioactive substances. As a result of their consumption, excretion, and persistence, they are disseminated mostly via excrements and enter the soils and other environmental compartments. Resulting residual concentrations in soils range from a few ,g upto g kg,1 and correspond to those found for pesticides. Numerous antibiotic molecules comprise of a non-polar core combined with polar functional moieties. Many antibiotics are amphiphilic or amphoteric and ionize. However, physicochemical properties vary widely among compounds from the various structural classes. Existing analytical methods for environmental samples often combine an extraction with acidic buffered solvents and the use of LC-MS for determination. In soils, adsorption of antibiotics to the organic and mineral exchange sites is mostly due to charge transfer and ion interactions and not to hydrophobic partitioning. Sorption is strongly influenced by the pH of the medium and governs the mobility and transport of the antibiotics. In particular for the strongly adsorbed antibiotics, fast leaching through soils by macropore or preferential transport facilitated by dissolved soil colloids seems to be the major transport process. Antibiotics of numerous classes are photodegraded. However, on soil surfaces this process if of minor influence. Compared to this, biotransformation yields a more effective degradation and inactivation of antibiotics. However, some metabolites still comprise of an antibiotic potency. Degradation of antibiotics is hampered by fixation to the soil matrix; persisting antibiotics were already determined in soils. Effects on soil organisms are very diverse, although all antibiotics are highly bioactive. The absence of effects might in parts be due to a lack of suitable test methods. However, dose and persistence time related effects especially on soil microorganisms are often observed that might cause shifts of the microbial community. Significant effects on soil fauna were only determined for anthelmintics. Due to the antibiotic effect, resistance in soil microorganisms can be provoked by antibiotics. Additionally, the administration of antibiotics mostly causes the formation of resistant microorganisms within the treated body. Hence, resistant microorganisms reach directly the soils with contaminated excrements. When pathogens are resistant or acquire resistance from commensal microorganisms via gene transfer, humans and animals are endangered to suffer from infections that cannot be treated with pharmacotherapy. The uptake into plants even of mobile antibiotics is small. However, effects on plant growth were determined for some species and antibiotics. Hingegen kommt es insbesondere durch biologische Transformationsprozesse zu einer intensiven Degradation und Inaktivierung der Antibiotika. Verschiedene Metaboliten weisen jedoch ebenfalls ein antibiotisches Potential auf. Der Abbau der Antibiotika wird durch die Festlegung in Böden gehemmt; dementsprechend wurde eine Persistenz verschiedener Antibiotika nachgewiesen. Trotz der starken bioaktiven Wirkung aller Antibiotika sind die festgestellten Effekte auf Bodenorganismen sehr unterschiedlich. Dies liegt nicht zuletzt an einem Mangel an geeigneten Testmethoden. In der Regel sind jedoch von Dosis und Wirkungsdauer abhängige Effekte insbesondere auf Mikroorganismen festzustellen, die zu Veränderungen der Mikroorganismenpopulation führen können. Lediglich durch Anthelmintika wurden deutliche Wirkungen auf Vertreter der Bodenfauna hervorgerufen. Infolge der antibiotischen Wirkung der Pharmazeutika kann eine Resistenzbildung bei Bodenorganismen ausgelöst werden. Zudem hat die Medikation von Antibiotika die Bildung resistenter Mikroorganismen bereits im behandelten Organismus zur Folge. Durch deren anschließende Ausscheidung gelangen resistente Keime auch direkt in die Böden. Handelt es sich um resistente Pathogene oder kommt es zur Übertragung der Resistenzgene zwischen kommensalen und pathogenen Mikroorganismen, so besteht das erhebliche Risiko einer nicht therapierbaren Infektion von Mensch und Tier. Die Aufnahme selbst mobiler Antibiotika in die Pflanzen ist sehr gering. Dennoch wurden bei einigen Pflanzenarten Wirkungen von Antibiotika auf das Wachstum nachgewiesen. [source] Glaciomarine deposition around the Irish Sea basin: some problems and solutionsJOURNAL OF QUATERNARY SCIENCE, Issue 5 2001Dr. Jasper Knight Abstract Reconstructing the depositional processes and setting (marine or terrestrial depositional environment) of late Devensian age glacigenic sediments around the Irish Sea basin (ISB) is critical for developing an all-embracing and consistent glacial model that can account for all observed field evidence. Identifying episodes of marine and terrestrial glacial deposition from field data is considered the first step in achieving this goal. Criteria for distinguishing marine and terrestrial glacial environments in the ISB include the geomorphology, sedimentology and faunal content (biofacies) of the associated deposits. Exposures of glacigenic sediments around the ISB are very diverse in terms of their morphosedimentary characteristics and associated biofacies, and thus inferred depositional processes and setting. Possible reasons for the diverse geological record include the differing geometry of eastern and western ISB coasts, time-transgressive ice retreat, and differential land rebound effects as a result of forebulge collapse and neotectonics. Poor geochronometric control on ice sheet events has not helped the correlation of ISB events with glacial and climatic events elsewhere. Future investigations of glacial sites around the ISB should use an integrated methodological approach involving a range of geomorphological, sedimentary and biofacies data, and dating control where possible. [source] Phenotypic characteristics of rhizobia isolates nodulating Acacia species in the arid and Saharan regions of MoroccoLETTERS IN APPLIED MICROBIOLOGY, Issue 5 2000K. Zerhari The phenotypic characteristics of 48 isolates obtained from root nodules of four Acacia species (Acacia cyanophylla, A. gummifera, A. horrida and A. raddiana) growing in soils collected from the arid and Saharan regions of Morocco were studied. The rhizobia were very diverse with respect to their cross-nodulation patterns, as well as their physiological and biochemical properties. Dendrograms obtained through computer numerical analysis of 52 phenotypic characteristics showed that isolates could fit into four clusters below the boundary level of 0·85 average distance and that they were very distinct from the reference strains. Some interesting isolates for inoculation trials have been identified. They were able to grow at pH ranging from 4 to 9, tolerate a high salt concentration (3% NaCl) and grew at a maximum temperature between 35 and 40 °C. [source] Acquisition of double-stranded DNA-binding ability in a hybrid protein between Escherichia coli CspA and the cold shock domain of human YB-1MOLECULAR MICROBIOLOGY, Issue 3 2000Nan Wang Escherichia coli CspA, a major cold shock protein, is dramatically induced upon temperature downshift. As it binds co-operatively to single-stranded DNA (ssDNA) and RNA without apparent sequence specificity, it has been proposed that CspA acts as an RNA chaperone to facilitate transcription and translation at low temperature. CspA consists of a five-stranded ,-barrel structure containing two RNA-binding motifs, RNP1 and RNP2. Eukaryotic Y-box proteins, such as human YB-1, are a family of nucleic acid-binding proteins that share a region of high homology with CspA (43% identity), termed the cold shock domain (CSD). Their cellular functions are very diverse and are associated with growth-related processes. Here, we replaced the six-residue loop region of CspA between the ,3 and ,4 strands with the corresponding region of the CSD of human YB-1 protein. The resulting hybrid protein became capable of binding to double-stranded DNA (dsDNA) in addition to ssDNA and RNA. The dsDNA-binding ability of an RNP1 point mutant (F20L) of the hybrid was almost unchanged. On the other hand, the dsDNA-binding ability of the hybrid protein was abolished in high salt concentrations in contrast to its ssDNA-binding ability. These results indicate that the loop region between the ,3 and ,4 strands of Y-box proteins, which is a little longer and more basic than that of CspA, plays an important role in their binding to dsDNA. [source] Pepino mosaic virus: a successful pathogen that rapidly evolved from emerging to endemic in tomato cropsMOLECULAR PLANT PATHOLOGY, Issue 2 2010INGE M. HANSSEN SUMMARY Taxonomy:Pepino mosaic virus (PepMV) belongs to the Potexvirus genus of the Flexiviridae family. Physical properties: PepMV virions are nonenveloped flexuous rods that contain a monopartite, positive-sense, single-stranded RNA genome of 6.4 kb with a 3, poly-A tail. The genome contains five major open reading frames (ORFs) encoding a 164-kDa RNA-dependent RNA polymerase (RdRp), three triple gene block proteins of 26, 14 and 9 kDa, and a 25-kDa coat protein. Genome diversity: Four PepMV genotypes, with an intergenotype RNA sequence identity ranging from 78% to 95%, can be distinguished: the original Peruvian genotype (LP); the European (tomato) genotype (EU); the American genotype US1; and the Chilean genotype CH2. Transmission: PepMV is very efficiently transmitted mechanically, and a low seed transmission rate has been demonstrated. In addition, bumblebees have been associated with viral transmission. Host range: Similar to other Potexviruses, PepMV has a rather narrow host range that is thought to be largely restricted to species of the Solanaceae family. After originally being isolated from pepino (Solanum muricatum), PepMV has been identified in natural infections of the wild tomato species S. chilense, S. chmielewskii, S. parviflorum and S. peruvianum. PepMV is causing significant problems in the cultivation of the glasshouse tomato Solanum lycopersicum, and has been identified in weeds belonging to various plant families in the vicinity of tomato glasshouses. Symptomatology: PepMV symptoms can be very diverse. Fruit marbling is the most typical and economically devastating symptom. In addition, fruit discoloration, open fruit, nettle-heads, leaf blistering or bubbling, leaf chlorosis and yellow angular leaf spots, leaf mosaic and leaf or stem necrosis have been associated with PepMV. The severity of PepMV symptoms is thought to be dependent on environmental conditions, as well as on the properties of the viral isolate. Minor nucleotide sequence differences between isolates from the same genotype have been shown to lead to enhanced aggressiveness and symptomatology. Control: Prevention of infection through strict hygiene measures is currently the major strategy for the control of PepMV in tomato production. Cross-protection can be effective, but only under well-defined and well-controlled conditions, and the effectiveness depends strongly on the PepMV genotype. [source] Diversity in fertility patterns in GuatemalaPOPULATION, SPACE AND PLACE (PREVIOUSLY:-INT JOURNAL OF POPULATION GEOGRAPHY), Issue 6 2006Sofie De Broe Abstract This study investigates urban and rural fertility trends in Guatemala up to 2002. It also aims to establish, using the theory of diffusion as its theoretical framework, the extent to which ethnicity and ethnic diversity are associated with geographical patterns in local-level fertility after controlling for socio-economic indicators. Data from the Demographic and Health Surveys of 1987, 1995,96 and 1998,99, the National Maternal and Child Health Survey of 2002 and the Census of 2002 were used. P/F ratios were calculated and used as an analytical tool and quality control measure of the data in order to establish the timing of changes in fertility patterns as measured by age-specific fertility rates (ASFRs) based on exact exposure in four-year periods from 1972 to 2002. Finally, the 2002 census data were used to analyse and model fertility at the municipio- level using ordinary least squares regression. The results suggest a steady but very slow decline in fertility from 1972 until the mid-1990s. Both the P/F ratios and ASFRs calculated using the Maternal and Child Health Survey and Census of 2002 show a sharp decline in fertility since 1998. The regression results for the census data suggest an independent and significant effect of ,proportion of indigenous people' and an almost significant effect of ethnic diversity on fertility at the municipio -level. The very slow decline in fertility in Guatemala until fairly recently can be attributed to the fact that Guatemala has been lagging behind in terms of socio-economic development and the additional challenge of having a culturally very diverse and segregated population, preventing the spread of modern reproductive ideas and behaviour. The accelerated fertility decline since the end of the 1990s seems likely to be associated with the widespread availability and increased uptake of family planning following declining fertility desires among its indigenous population. [source] Shipwrecks and founder effects: Divergent demographic histories reflected in Caribbean mtDNAAMERICAN JOURNAL OF PHYSICAL ANTHROPOLOGY, Issue 4 2005Antonio Salas Abstract During the period of the Atlantic slave trade (15th,19th centuries), millions of people were forced to move from Africa to many American destinations, changing dramatically the human landscape of the Americas. Here, we analyze mitochondrial DNA from two different American populations with African ancestry, with hitherto unknown European and Native American components. On the basis of historical records, African-Americans from Chocó (Colombia) and the Garífunas (or "Black Carib") of Honduras are likely to have had very different demographic histories, with a significant founder effect in the formation of the latter. Both the common features and differences are reflected in their mtDNA composition. Both show a minor component (,16%) from Native Central/South Americans and a larger component (,84%) from sub-Saharan Africans. The latter component is very diverse in the African-Americans from Chocó, similar to that of sub-Saharan Africans, but much less so in the Garífunas, with several mtDNA types elevated to high frequency, suggesting the action of genetic drift. [source] Magnetometric prospection of various types of large ditched enclosures in BohemiaARCHAEOLOGICAL PROSPECTION, Issue 1 2006Roman K, ivánekArticle first published online: 6 JUN 200 Abstract A systematic combination of aerial and subsequent geophysical surveys over the six years of the ,Settlement Patterns in Prehistoric Bohemia' project yielded much new and often surprising information about the nature of archaeological sites, features and locations in the agricultural lowlands of Bohemia. Newly discovered and verified large ditches represent a group of very diverse and often, in Bohemian archaeology, atypical linear features. The results of large-area magnetometric surveys aided in mapping locations more precisely and in the overall separation of linear features, as well as in choosing among probable archaeological interpretations of the sites. Future archaeological verification of more of these identified atypical situations by test trenches would provide more precise dating, and perhaps unambiguous interpretations of prehistoric sites.
