Rethinking Social Security in Latin America

In the past decade, many Latin American governments have radically restructured their old age income security systems, following the lead of Chile, which undertook its major pension reform in 1981. The defining characteristic of the reforms has been a shift in the basis of public pensions from social to individual responsibility: instead of the widely used system that "collectivized" or pooled the risk of being without the capacity to earn while aged, numerous countries in the region have adopted a system that relies on individual savings accounts. The reforms have maintained a role for a modified version of public pooling; this combination of individual and social savings to finance pensions is known as the "multipillar" approach. This article is based on a report prepared for the Office of the Chief Economist of the Latin America and Caribbean Region of the World Bank (Gill, Packard and Yermo, 2004).1 The report recognizes that the system of individual accounts, the essential aspect of the reform, has been a necessary and positive development, and one that is consistent with the economics of insurance and social welfare objectives. Beyond this recognition, however, the results of reform are much more complex. Each country has implemented its own version of the multipillar system. The article therefore draws on country evidence in order to determine: How has the new approach to public pensions in Latin America fared? In particular, have the changes left workers and their families in reform countries better off? The first section provides a brief description of the reforms. The second discusses the main macroeconomic concerns and effects. The third describes the impact on coverage levels, and other social welfare implications. The fourth evaluates the stagnation of coverage levels and presents various possible explanations. The fifth makes specific proposals to improve the multipillar pension system in Latin America. A study examining inter-rater and intrarater reliability of a novel instrument for assessment of psoriasis: the Copenhagen Psoriasis Severity Index

Summary Background, There is a perceived need for a better method for clinical assessment of the severity of psoriasis vulgaris. The most frequently used system is the Psoriasis Area and Severity Index (PASI), which has significant disadvantages, including the requirement for assessment of the percentage of skin affected, an inability to separate milder cases, and a lack of linearity. The Copenhagen Psoriasis Severity Index (CoPSI) is a novel approach which comprises assessment of three signs: erythema, plaque thickness and scaling, each on a four-point scale (0, none; 1, mild; 2, moderate; 3, severe), at each of 10 sites: face, scalp, upper limbs (excluding hands and wrists), hands and wrists, chest and abdomen, back, buttocks and sacral area, genitalia, lower limbs (excluding feet and ankles), feet and ankles. Objectives, To evaluate the inter-rater and intrarater reliability of the CoPSI and to provide comparative data from the PASI and a Physician's Global Assessment (PGA) used in recent clinical trials on psoriasis vulgaris. Methods, On the day before the study, 14 dermatologists (raters) with an interest in psoriasis participated in a detailed training session and discussion (2·5 h) on use of the scales. On the study day, each rater evaluated 16 adults with chronic plaque psoriasis in the morning and again in the afternoon. Raters were randomly assigned to assess subjects using the scales in a specific sequence, either PGA, CoPSI, PASI or PGA, PASI, CoPSI. Each rater used one sequence in the morning and the other in the afternoon. The primary endpoint was the inter-rater and intrarater reliability as determined by intraclass correlation coefficients (ICCs). Results, All three scales demonstrated ,substantial' (a priori defined as ICC > 80%) intrarater reliability. The inter-rater reliability for each of the CoPSI and PASI was also ,substantial' and for the PGA was ,moderate' (ICC 61%). The CoPSI was better at distinguishing between milder cases. Conclusions, The CoPSI and the PASI both provided reproducible psoriasis severity assessments. In terms of both intrarater and inter-rater reliability values, the CoPSI and the PASI are superior to the PGA. The CoPSI may overcome several of the problems associated with the PASI. In particular, the CoPSI avoids the need to estimate a percentage of skin involved, is able to separate milder cases where the PASI lacks sensitivity, and is also more linear and simpler. The CoPSI also incorporates more meaningful weighting of different anatomical areas. [source]

Summary Background, There is a lack of consensus as to the best way of monitoring psoriasis severity in clinical trials. The Psoriasis Area and Severity Index (PASI) is the most frequently used system and the Physician's Global Assessment (PGA) is also often used. However, both instruments have some drawbacks and neither has been fully evaluated in terms of ,validity' and ,reliability' as a psoriasis rating scale. The Lattice System Physician's Global Assessment (LS-PGA) scale has recently been developed to address some disadvantages of the PASI and PGA. Would Different Routine Precordial Electrode Positions Be More Useful?

Abstract There are reasons to wonder if the current precordial electrode positions are the best ones to study the electrical forces of the heart. Theoretically, more horizontal electrode positions and several superior and inferior electrode positions that are perpendicular to the horizontally placed electrode positions should make a superior lead system. A study was designed to determine if that was true. The result of the study indicated that such a system did not solve all of the problems that were posed by the current system. A performance study of job management systems

Abstract Job Management Systems (JMSs) efficiently schedule and monitor jobs in parallel and distributed computing environments. Therefore, they are critical for improving the utilization of expensive resources in high-performance computing systems and centers, and an important component of Grid software infrastructure. With many JMSs available commercially and in the public domain, it is difficult to choose an optimum JMS for a given computing environment. In this paper, we present the results of the first empirical study of JMSs reported in the literature. Four commonly used systems, LSF, PBS Pro, Sun Grid Engine/CODINE, and Condor were considered. The study has revealed important strengths and weaknesses of these JMSs under different operational conditions. For example, LSF was shown to exhibit excellent throughput for a wide range of job types and submission rates. Alternatively, CODINE appeared to outperform other systems in terms of the average turn-around time for small jobs, and PBS appeared to excel in terms of turn-around time for relatively larger jobs. Pinpointing users with location estimation techniques and Wi-Fi hotspot technology

Location awareness is becoming an important capability for mobile computing; however, it has not been possible until now to provide cheap pervasive positioning systems. Wide area coverage is most famously achieved by using global positioning systems (GPS). A constellation of low-orbit satellites cover the earth's surface. Unfortunately GPS does not work indoors and has limited success in big cities because of the ,urban canyon' effect. PlaceLab is a research project that attempts to solve the ubiquity issues surrounding 802.11-based location estimation. PlaceLab, like RADAR, uses a device's 802.11 interface; however, it does not require the area to be pre-calibrated. It predicts location via the known positions of the access points detected by the device. Commonly used systems have a number of drawbacks, including cost, accuracy and the ability to work indoors. PlaceLab is a piece of open source software developed by Intel Research that can pinpoint a user within a Wi-Fi network. We set out here to investigate whether PlaceLab can be used as a means of establishing a user's position. This type of investigation could, if successful, pave the way for the development of other location-based applications. This report documents the efforts to answer the above question. PlaceLab was found to work, but only in ideal locations where factors such as the number of floors and the lack of available APs did not affect its use. Colloidal soft matter as drug delivery system

Abstract Growing interest is being dedicated to soft matter because of its potential in delivering any type of drugs. Since hydrophilic, lipophilic, small and big molecules can be loaded into these colloidal systems and administered through the parenteral or nonparenteral route, soft matter systems have been used to solve many biomedical and pharmaceutical problems. In fact, they make possible to overcome difficulties in the formulation and delivery of poorly water-soluble drug molecules, settle some stability issues typical of biological drug molecules, design parenteral sustained release forms and provide functionalized soft particles that are very effective in drug targeting. This review deals with the important role that colloids play in the drug delivery and targeting, with particular attention to the more currently used systems such as microemulsions, organogels, liposomes, micelles, and dendrimers. Though significant progress has been made in drug targeting, some challenges still remain. Further efforts will be required to better understand the characteristics of targets and to discover new ones. Quantitative structure/property relationship analysis of Caco-2 permeability using a genetic algorithm-based partial least squares method

Abstract Caco-2 cell monolayers are widely used systems for predicting human intestinal absorption. This study was carried out to develop a quantitative structure,property relationship (QSPR) model of Caco-2 permeability using a novel genetic algorithm-based partial least squares (GA-PLS) method. The Caco-2 permeability data for 73 compounds were taken from the literature. Molconn-Z descriptors of these compounds were calculated as molecular descriptors, and the optimal subset of the descriptors was explored by GA-PLS analysis. A fitness function considering both goodness-of-fit to the training data and predictability of the testing data was adopted throughout the genetic algorithm-driven optimization procedure. The final PLS model consisting of 24 descriptors gave a correlation coefficient (r) of 0.886 for the entire dataset and a predictive correlation coefficient (rpred) of 0.825 that was evaluated by a leave-some-out cross-validation procedure. Non-carious cervical lesions on permanent anterior teeth: a new morphological classification

Abstract Background:, Various cross-sectional forms of non-carious cervical lesions have been described but no formal morphological classification system has been developed. The aims of this study were to describe the spectrum of common morphological forms of non-carious cervical lesions observed within a large sample of extracted human permanent anterior teeth and to develop a system for classification based on morphological features. Methods:, Over 15,000 extracted permanent anterior teeth were examined macroscopically under illumination at 2× magnification. Well-defined, descriptive categories were formed, based on observable non-carious cervical lesion features and using terminology currently reported in the literature. The lesions were then sorted into these categories. Results:, Five hundred and forty-two non-carious cervical lesions were identified on 15,289 teeth extracted in the early to mid 20th century, representing a frequency of 3.5%. The main categories developed were "shallow", "concave", "wedge-shaped", "notched", and "irregular". Conclusions:, The new morphological classification system provides an alternative to presently used systems based on aetiology, and should facilitate future research on non-carious cervical lesions. SUMMARY 1. The ectopic expression of genes has proven to be an extremely valuable tool for biologists. The most widely used systems involve electrically or chemically mediated transfer of genes to immortalized cell lines and, at the other end of the spectrum, transgenic animal models. As would be expected, there are compromises to be made when using either of these broad approaches. Immortalized cell lines have limited ,physiological relevance' and transgenic approaches are costly and out of the reach of many laboratories. There is also significant time required for the de novo generation of a transgenic animal. 2. As a viable alternative to these approaches, we describe the use of recombinant adenovirus and Sindbis virus to deliver genes to cells and tissues. 3. We exemplify this approach with studies from our laboratories: (i) an investigation of Ca2+ handling deficits in cardiac myocytes of hypertrophied hearts using infection with recombinant adenovirus encoding either green fluorescent protein (GFP) or the sarcoplasmic/endoplasmic reticulum calcium-ATPase (Serca2a); (ii) a study of the mechanism of macrophage/microglial migration by infection of embryonic phagocytes with a GFP-encoding virus and coculture with brain slices to then track the movement of labelled cells; and (iii) we are also exploiting the natural tropism of the Sindbis virus to label neurons in hippocampal brain slices in culture to resolve high-resolution structure and to map neuronal connectivity. 4. Further development of these approaches should open new avenues of investigation for the study of physiology in a range of cells and tissues. [source]
