Performance of recombinant ESAT-6 antigen (ML0049) for detection of leprosy patients

Abstract Aims:, The study was aimed to evaluate the Mycobacterium leprae recombinant early secreted antigenic target-6 (rESAT-6) for its serological performance in leprosy patients. Methods and Results:, Employing enzyme-linked immunosorbent assay (ELISA), serum samples were tested for prevalence of immunoglobulin G antibodies against M. leprae rESAT-6. The results revealed that the sensitivity of the assay for smear-positive leprosy patients was 82·4% (14 of 17) while for smear-negative patients it was 19·4% (six of 31). Interestingly, the performance of ESAT-6-based assay was statistically comparable with anti-phenolic glycolipid-I antibody-detecting ELISA, a most widely studied serological assay in leprosy. Regarding specificity, none of the 48 controls was positive indicating that antibody response to ESAT-6 was highly specific. Moreover, a high concordance between bacterial index and anti-ESAT-6 antibody-detecting assay was noted. Conclusions:, Recombinant ESAT-6 seems to be a potential serological reagent for detection of M. leprae infection. Significance and Impact of the Study:, ESAT-6 serology may have utility for (i) early diagnosis, particularly, of highly infectious form (multibacillary, MB) of leprosy, (ii) monitoring the response in smear-positive leprosy patients during the course of the chemotherapy, (iii) classification of leprosy patients into MB and paucibacillary groups for treatment purpose. Hence, further research on these lines is warranted. [source]


On the justification for civil commitment

This paper explores some of the controversies in the debate regarding the justification of civil commitment. The sometimes conflicting values reflected in the mental health legislation, human rights principles, moral philosophy and psychiatric professional standards are discussed. In spite of the often substantial use of civil commitment in many countries, there are almost no scientifically sound studies addressing the outcome of coercive treatment. The paper establishes that the traditional arguments in favour of civil commitment, like lack of insight and competence as well as the effectiveness of civil commitment, are poorly founded. The paper concludes that there seems to be a general agreement that civil commitment of patients who are dangerous to themselves or others should be the responsibility of the mental health care system, while civil commitment for treatment purposes is more controversial and hard to justify. Teaching of Anatomy of Genital Organs in the Large Animals

At the University of Missouri-Columbia, USA, teaching the anatomy of genital organs in large (and in small) animals is clinically oriented. In the male horse, ox, and pig, the descriptive anatomy of the structures is taught in that order as they are listed in Nomina Anatomica Veterinaria. Clinical correlates are immediately mentioned in relation to each of the following structures. The testicle, epididymis, ductus deferens, spermatic cord, and tunics of spermatic cord and testis are correlated to the castration, criptorchidism, ectopic testicles, and inguinal herniae, as well as to the landmarks and approaches to different clinical techniques. The penis and male urethra are correlated to the prolapse of the prepuce, ulcerative posthitis, balanitis, penile deviation, penischisis, persistent penile frenulum, short retractor penis muscle, catheterization of the urethra, the contagious equine metritis (CEM), etc. In the female horse, ox and pig, following the similar order as in the male species, the ovary and the salpinx are correlated to the diagnosis of pregnancy by rectal exploration, ovarian hypoplasia, ectopic pregnancy, and ovariectomy. The uterus is correlated to the different aspects of metritis and endometritis, to the retained placenta, pyometra, uterine torsion, uterine prolapse and eversion, Caesarian section, diagnosis of pregnancy and different stages of oestrus by rectal exploration, double external ostium of the cervix, distocia, and the assessment of possible difficulties in the birth process. The vagina and vestibulum vaginae are correlated to the gaertneritis, vaginal and cervical prolapse, vaginitis, catheterization of the urinary bladder, and pelvimetry. The vulva and clitoris are correlated to vulvitis, balanitis, distocia, episiotomy, and transmissible genital diseases. Different kinds of udder diseases (mastitis) are based on the thorough knowledge of the anatomy of the udder including the blood and nerve supply and the lymphatic system. The two techniques of epidural anesthesia (Magda and Farquharson in ruminants) and subsacral anaesthesia (Popescu) for diagnosis and treatment purposes are explained in detail. [source]


Medications used in overdose and how they are acquired , an investigation of cases attending an inner Melbourne emergency department

Abstract Objective: This study aimed to investigate which categories of medication are most commonly implicated in overdose, to compare this information with prescription data and to explore how the medications used in overdoses are typically acquired. Methods: A 12-month audit (11/2003,10/2004) of all medication overdose presentations to an inner-Melbourne ED was conducted and the medications compared to published population-based prescription data. Interviews were conducted with 31 patients who attended the ED following a medication overdose and typical stories regarding the acquisition of medications reported. Results: The same broad categories of medications identified in earlier studies were found to contribute to the majority of overdoses in this study, namely benzodiazepines, antidepressants, analgesics and antipsychotics. Two benzodiazepine medications, diazepam and alprazolam, appeared to be over-represented in the overdose data relative to their population rates of prescription. Patient interviews revealed three main reasons for the original acquisition of the medications used in overdose: treatment purposes (77%); recreational use (16%); and overdose (7%). The most common source of medications (68%) used in overdose was prescription by the patient's usual doctor. Conclusion: The high representation of benzodiazepines among medications used in overdose is of ongoing concern. Implications: The time of medication prescription and dispensing may be an ideal opportunity for overdose prevention, through judicious prescribing, consideration of treatment alternatives, patient education and encouraging the safe disposal of unused medications. Deep-seated, well differentiated lipomatous tumors of the chest wall and extremities

The role of cytogenetics in classification, prognostication
Abstract BACKGROUND Intramuscular lipomas and atypical lipomatous tumors (ALT) are common deep-seated lipomatous tumors of the chest wall and extremities. Distinguishing between these two entities can be difficult based on histologic analysis alone. However, the cytogenetic profiles of ALT and intramuscular lipomas are distinct. Correct classification is important, because aggressive local disease recurrence occurs more frequently in patients with ALT than in patients with intramuscular lipoma. The authors examined their single institutional experience and correlated their classification with clinical features and outcome. METHODS In the current study, 106 patients with deep-seated, well differentiated adipose tumors of the chest wall and extremities were classified as having ALT or intramuscular lipoma using a combined approach of histology and cytogenetics, if available. The classification was correlated with clinicopathologic features and follow-up data. RESULTS Fifty-five patients were classified as having intramuscular lipoma and 51 were classified as having ALT. Classification did not correlate with age and gender (P = 0.28 and P = 0.96, respectively). Intramuscular lipomas were smaller than ALTs (P < 0.0001), but there was significant overlap between the 2 groups. ALT occurred preferentially in the lower extremity (P < 0.0009). Four percent of patients with intramuscular lipomas and 27% of patients with ALTs developed local disease recurrence (P = 0.0006). Disease recurrence did not correlate with patient age at diagnosis, patient gender, tumor size, and tumor location (P = 0.45, P = 0.26, P = 0.49, and P = 0.28, respectively). Within the subset of patients with ALTs, disease recurrence did not correlate with patient age at diagnosis, patient gender, or tumor location (P = 0.38, P = 0.54, and P = 0.86, respectively). CONCLUSIONS Classification of deep-seated, well differentiated lipomatous tumors of the extremities and chest wall using a combined approach of histology and cytogenetics correlated well with biologic behavior/disease recurrence.
