Impact of training periods in the emergency department on the motivation of health care students to learn

Objectives, Motivation is one of the most important factors for learning and achievement. The perceived value of the task, perceptions of self-efficacy and beliefs about control of learning are the main determinants of motivation. They are highly influenced by the individual's personal history and especially by significant past experiences. We assessed the impact of training periods in the emergency department on the motivation of health care students to learn in the field of emergency medicine. Methods, A survey was conducted in 2008 with 112 undergraduate medical students and 201 undergraduate nursing students attending an emergency medicine academic programme. At the beginning of the course, the students completed an anonymous 26-item questionnaire to assess their motivational orientations. Results, Perceived task value was higher for students who had previously attended a training period in the emergency department (P = 0.002). Perceived self-efficacy was depressed when the respondent had been confronted with negative outcome events (P < 0.001). Control of learning beliefs was affected negatively in students who had attended a training period in the emergency department (P < 0.001). Conclusions, Motivation is a major contributor to the success of learning. Training periods in the emergency department can have positive and negative impacts on the learning motivation of medical and nursing students in the field of emergency medicine. Ideally, and in terms of increasing motivation, health care students should gain experiential learning in the emergency department before attending a corresponding academic course. During this period, tutors should provide appropriate supervision and feedback in order to support self-efficacy perception and learning control beliefs. [source]


Conjugate limb coordination after experience with an interlimb yoke: Evidence for motor learning in the rat fetus

Abstract This study investigated the capacity of the E20 rat fetus to adaptively alter patterns of interlimb coordination in a prenatal model of motor learning. Fetal limb movement was manipulated with an interlimb yoke, consisting of a fine thread attached at the ankles, which created a physical linkage between two limbs. Exposure to the yoke resulted in a gradual increase in conjugate movements of the yoked limbs during a 30-min training period, which persisted after removal of the yoke. Training effects were evident when the yoke was applied to two hindlimbs, two forelimbs, or a homolateral forelimb,hindlimb pair. A savings in the rate of acquisition also was observed when fetuses experienced yoke training in a second session. These data argue that the rat fetus can respond to kinesthetic feedback resulting from variation in motor performance, which suggests that experience contributes to the development of coordinated motor behavior before birth. © 2005 Wiley Periodicals, Inc. Exercise training in late middle-aged male Fischer 344 × Brown Norway F1-hybrid rats improves skeletal muscle aerobic function

The Fischer 344 × Brown Norway F1-hybrid (F344BN) rat has become an increasingly popular and useful strain for studying age-related declines in skeletal muscle function because this strain lives long enough to experience significant declines in muscle mass. Since exercise is often considered a mechanism to combat age-related declines in muscle function, determining the utility of this strain of rat for studying the effects of exercise on the ageing process is necessary. The purpose of this study was to evaluate the plasticity of skeletal muscle aerobic function in late middle-aged male rats following 7 weeks of treadmill exercise training. Training consisted of 60 min per day, 5 days per week with velocity gradually increasing over the training period according to the capabilities of individual rats. The final 3 weeks involved 2 min high-intensity intervals to increase the training stimulus. We used in situ skeletal muscle aerobic metabolic responses and in vitro assessment of muscle mitochondrial oxidative capacity to describe the adaptations of aerobic function from the training. Training increased running endurance from 11.3 ± 0.6 to 15.5 ± 0.8 min, an improvement of ,60%. Similarly, distal hindlimb muscles from trained rats exhibited a higher maximal oxygen consumption in situ (23.2 ± 1.3 versus 19.7 ± 0.8 ,mol min,1 for trained versus sedentary rats, respectively) and greater citrate synthase and complex IV enzyme activities in gastrocnemius (29 and 19%, respectively) and plantaris muscles (24 and 28%, respectively) compared with age-matched sedentary control animals. Our results demonstrate that skeletal muscles from late middle-aged rats adapt to treadmill exercise by improving skeletal muscle aerobic function and mitochondrial enzyme activities. This rat strain seems suitable for further investigations using exercise as an intervention to combat ageing-related declines of skeletal muscle aerobic function. [source]


Exploring the predictability of the ,Short Rains' at the coast of East Africa

Abstract The boreal autumn ,Short Rains' at the coast of East Africa are deficient when there is weak development of a zonal circulation cell along the Indian Ocean equator, an anomalously low sea-surface temperature in the western portion of the basin, and in the high phase of the southern oscillation. Such large-scale circulation departures and their precursors are described by compact indices. September values of these indices for the period 1958,96 are used to explore the predictability of an index (RON) of October,November rainfall at the coast of East Africa. Regressions with cross-validation over the entire 1958,96 period are evaluated for the early (1958,77) and late (1978,96) halves of the record. In complementary experiments, the entire record is separated into 1958,77 as a training period and 1978,96 as a verification period. In all experiments, correlation of calculated versus observed rainfall is high for the early record and low for the late half of the record, a behaviour not noted in cross-validation over the entire 39 year time span. The 11-year sliding correlations of the indicated circulation indices with RON all reveal a drastic deterioration of relationships from the early to the late half of the record, although the equatorial zonal circulation cell appears to remain strong throughout. Service User Outcomes of Staff Training in Positive Behaviour Support Using Person-Focused Training: A Control Group Study

Background, Effectively supporting individuals with intellectual disabilities who display challenging behaviours continues to be a priority for service providers. Person-focused training (PFT) is a model of service delivery which provides staff with skills in functional assessment and intervention development. Existing longitudinal data from a study of 138 cases suggest that implementation of staff-developed behaviour support plans through PFT is effective in reducing challenging behaviour in approximately 77% of cases [McClean et al.Journal of Intellectual Disability Research (2005) vol. 49, pp. 340,353]. However, no control group was used in this study. Method, The current study involves the use of a control group of individuals with challenging behaviours matched against those selected for PFT over a 6-month period. Groups were matched on type of challenging behaviour, duration of challenging behaviour, gender and level of disability. Information on the frequency, management difficulty and severity of challenging behaviour was collected pre- and post-training using the Checklist of Challenging Behaviours (CCB) for both groups. Observational data were collected for the target group alone. Rates of psychotropic medication were tracked across the training period. Results, Significant reductions in the frequency, management difficulty and severity of challenging behaviour were found for service users in the target group but not in the control group after 6 months. No significant changes were found in the use of psychotropic medication for either group over the 6-month period. Conclusion, Overall results suggest that PFT is an effective model for providing support to individuals with challenging behaviours. [source]


Reduced lipolysis and increased lipogenesis in adipose tissue from pinealectomized rats adapted to training

Abstract:, The current study investigated the effects of chronic training and pinealectomy on the lipogenic and lipolytic activity of adipose tissue. Pinealectomized and sham-operated adult male Wistar rats were distributed in to four subgroups: pinealectomized untrained, pinealectomized trained, control untrained and control trained. At the end of the training period (8 wk) the rats were killed. Blood samples were collected for glucose, insulin and leptin determinations. Peri-epididymal adipocytes were isolated for measurement of in vitro rates of lipolysis and incorporation of substrates (d -[U- 14C]-glucose, l -[U- 14C]-lactate, [2- 14C]-acetate and [1- 14C]-palmitate) into lipids, and samples of epididymal adipose tissue were homogenized for evaluation of glucose-6-phosphate dehydrogenase maximal activity. Pinealectomy resulted in a significantly increased lipolytic capacity in response to isoproterenol and a decrease in circulating leptin levels without affecting the rates of incorporation of different substrates into lipids. However, only in the intact control group did training promote a higher basal and isoproterenol-stimulated lipolysis, increase the incorporation of palmitate (esterification), decrease the incorporation of acetate (lipogenesis) into lipids and diminish circulating leptin levels. These effects of exercise training were not seen in pinealectomized rats. However, pinealectomized trained animals showed a marked reduction in lipolysis and an increased rate of acetate incorporation. In conclusion, we demonstrated for the first time that the pineal gland plays an important role in the regulation of lipid metabolism in such a way that its absence caused a severe alteration in the balance between lipogenesis and lipolysis, which becomes evident with the adaptation to exercise training. TRAINING EFFECTS ON PERFORMANCE OF DESCRIPTIVE PANELISTS

ABSTRACT The amount of training necessary to adequately "train" a descriptive panel is a matter of contention. The objectives of this research were to compare the performance of descriptive panelists after short-term (4 h), moderate (60 h) and extensive training (120 h). Seven screened panelists were chosen to evaluate three commercial tomato pasta sauces after each training period. Panelist performance improved with increased training. Sample differences were observed in all texture attributes and some flavor attributes, even after the shortest training time (4 h). However, more differences were found with at least 60 h of product training. After 120 h of training, product differences for all texture attributes and most of the flavor attributes studied could be ascertained by the trained panelists. These results suggested that only limited training may be necessary to find differences among products for many texture attributes and some flavor attributes. However, extensive training may be required to reduce variation among panelists and increase the discriminant abilities of panelists. [source]


COMPARISON OF PROCESS-BASED AND ARTIFICIAL NEURAL NETWORK APPROACHES FOR STREAMFLOW MODELING IN AN AGRICULTURAL WATERSHED,

ABSTRACT: The performance of the Soil and Water Assessment Tool (SWAT) and artificial neural network (ANN) models in simulating hydrologic response was assessed in an agricultural watershed in southeastern Pennsylvania. All of the performance evaluation measures including Nash-Sutcliffe coefficient of efficiency (E) and coefficient of determination (R2) suggest that the ANN monthly predictions were closer to the observed flows than the monthly predictions from the SWAT model. More specifically, monthly streamflow E and R2 were 0.54 and 0.57, respectively, for the SWAT model calibration period, and 0.71 and 0.75, respectively, for the ANN model training period. For the validation period, these values were ,0.17 and 0.34 for the SWAT and 0.43 and 0.45 for the ANN model. SWAT model performance was affected by snowmelt events during winter months and by the model's inability to adequately simulate base flows. Even though this and other studies using ANN models suggest that these models provide a viable alternative approach for hydrologic and water quality modeling, ANN models in their current form are not spatially distributed watershed modeling systems. However, considering the promising performance of the simple ANN model, this study suggests that the ANN approach warrants further development to explicitly address the spatial distribution of hydrologic/water quality processes within watersheds. The main neutrophil and neutrophil-related functions may compensate for each other following exercise,a finding from training in university judoists

Abstract In order to clarify the relationship between exercise and neutrophil function, we measured three major neutrophil and neutrophil-related functions, viz. the reactive oxygen species (ROS) production capability and phagocytic activity (PA) of neutrophils and serum opsonic activity (SOA), simultaneously before and after a unified loading exercise under three different sets of conditions. Thirteen female collegiate judoists were examined with a unified exercise loading (2 h) immediately before and after a 64 day training period. Immediately thereafter, the athletes took part in a 6 day intensified training camp, following which the same exercise loading was repeated. Responses from circulating neutrophils were estimated by comparing the two sets of values obtained before and after the two instances of exercise loading. The parameters assessed included neutrophil count, SOA, PA and ROS production capability. ROS production increased after the exercise loading performed immediately before and after the 64 day training period just before the camp, (p < 0.01) but decreased following the exercise loading performed after the camp (p < 0.05). This suggested depressed bacteriocidal capability of the circulating neutrophils. PA decreased after the exercise loading sessions imposed prior to and after the 64 day training period (p < 0.01) but did not change in the loading session after the camp. No changes were seen in SOA produced with the loading exercise either before the 64 day exercise period or before the camp, but increased significantly following the post-camp session (p < 0.05). In conclusion, athletic training-induced changes in immune functional activities of neutrophils, such as ROS production and PA, and neutrophil-related factors, such as SOA, may compensate for each other to maintain the overall integrity of the neutrophil immune function. Combined cuff and suture technique for orthotopic whole intestinal transplantation in rats

For the purpose of immunological study on small intestinal transplantation (SIT), rat SIT models using direct suture technique widely have been used, which requires at least several months of training for microsurgery. Alternatively, a simple cuff technique for SIT has been mainly used by us, which reduces warm-ischemic time and the training period, but the entire intestinal grafts usually obtain a limited blood supply. This report describes a modification of a combined cuff and suture technique for rat SIT to aid beginning microsurgical transplantation researchers. The advantages are 1) use of only arterial suturing, making it easier for beginners, with the cuff technique applied to the more difficult vein anastomosis; 2) achievement of better arterial inflow and graft survival than when the arterial cuff technique is used; and 3) doing only partial clamping of the aorta, which improves animal survival and success of the procedure. Detecting postoperative urinary retention with an ultrasound scanner

Background: Retention of urine is a common postoperative problem associated with risk of overdistention and permanent detrusor damage. Prevention of urinary retention by insertion of indwelling catheter may increase the risk of urinary infection. We have performed a reliability test of an ultrasound scanner, implemented in the postoperative monitoring equipment. Methods: Patients were monitored after different types of surgery under spinal anesthesia with an ultrasound scanner in the postanesthesia care unit (PACU). Patients: Patients who according to current guidelines required a urinary bladder catheter, were scanned before a catheter was inserted and urine volume was measured. These two urine volumes were compared and analyzed for agreement. Results: Nineteen female and 17 male patients were included. The mean difference between ultrasound estimates and catheter urine volume measurements was ,,21.5 mL, and limits of agreement, calculated as a 95% confidence interval, were ,,147 and +,104 mL. This means that the urine volume estimated by ultrasound was on average 21.5 mL smaller than the urine volume when the bladder was emptied. Conclusion: This study confirms a good agreement between the ultrasound scanner estimates of urinary bladder volume and urine volume measured after emptying the bladder. Nurses in the PACU could operate the ultrasound scanner after a brief instruction and training period. Considering the potentially serious long-term consequences of undiagnosed postoperative urinary retention, introducing this equipment for routine monitoring of urinary bladder volume should be considered. Functional Electrical Stimulation-Supported Interval Training Following Sensorimotor-Complete Spinal Cord Injury: A Case Series

ABSTRACT Objective.,To investigate the effect of interval training supported by Functional Electrical Stimulation (FES) on ambulation ability in complete spinal cord injury (SCI). Methods.,We trained four men with sensorimotor-complete (ASIA A) SCI, who achieved gait through FES of the quadriceps femoris, gluteus maximus, and common peroneal nerve on each side on a motorized treadmill. Training involved progressive interval walking exercise, consisting of periods of activity followed by equal periods of rest, repeated until muscle fatigue. We used time to muscle fatigue during continuous treadmill ambulation as the primary outcome measure. We also recorded the patterns of incremental stimulation for all training and testing sessions. Results.,All subjects increased their ambulation capacity; however, the responses varied from subject to subject. Some subjects increased the total distance walked by as much as 300% with progressive improvement over the entire training period; however, others made more modest gains and appeared to reach a performance plateau within a few training sessions. Conclusions.,FES-supported interval training offers a useful and effective strategy for strength-endurance improvement in the large muscle groups of the lower limb in motor-complete SCI. We believe that this training protocol offers a viable alternative to that of continuous walking training in people with SCI using FES to aid ambulation. [source]


Changes in body composition after a 12-wk aerobic exercise program in obese boys

Previous studies have shown that vigorous aerobic training programs for obese children result in minimal weight changes, and concluded that they may not be beneficial. Weight change alone may not detect important beneficial changes in body composition associated with vigorous training in these children. Fifteen obese boys (aged 9,12 yr, body mass index (BMI) 31.8±6.5, average percent body fat (%BF) 41±4.2) underwent a supervised aerobic and resistance training program (12 wk, 2 days/wk for 30 min/session), to investigate the effects on weight and body composition. After the 3-month training period, weight loss averaged only 1.5±1.0 kg (not significant), but total body fat decreased by 4.1±1.8 kg (p<0.05) and fat-free mass (FFM) increased by 2.6±1.1 kg (p<0.05) based on hydrostatic weighing. As a result, %BF fell by 10% (p<0.01). There was a 5.8±2.8 mL/kg/min (p<0.05) increase in peak volume of oxygen uptake (VO2), along with a 248±120 kcal/d (p<0.05) increase in resting energy expenditure (REE). Activity questionnaires showed a significant increase in high intensity recreational activities (6.5±1.5 vs 3.5±0.5 h physical activity/wk; p<0.01) in the home and a significant decrease in low intensity activities (7±2.0 vs 12±3.5 h TV viewing/wk; p<0.01). Conclusions: Vigorous supervised aerobic training in obese boys has beneficial effects on body composition, fitness and leisure time activities that are not apparent by measurement of changes in body weight alone. Exposure to respirable dust and crystalline silica in bricklaying education at Dutch vocational training centers

Abstract Background Construction workers are educated at vocational training centers before they begin their working lives. Future bricklayers and their instructors are exposed to respirable dust and possibly to hazardous respirable crystalline silica from trial mortar. Methods Thirty-six personal air samples were collected at six training centers to estimate exposure to respirable dust for both students and teachers. A selection of 22 samples was analyzed for crystalline silica. Results Average respirable dust exposures ranged from 0.59,mg/m3 for teachers to 1.45,mg/m3 for students performing recycling and cleaning tasks. In 45% of the analyzed samples, respirable crystalline silica was detected. Exposure to silica remained below the Dutch OEL (75,µg/m3). Exposure was significantly less for teachers than for students. This effect was found in both types of vocational training centers present in the Netherlands. Dry sweeping, as performed at all locations in this study, contributed considerably to the exposure to respirable dust. A first step in reducing exposure to dust and silica at training centers would therefore be to avoid dry sweeping. The presence of a dust extraction system, although not optimally designed, also significantly lowered exposure. Conclusions To assess a construction worker's lifetime exposure to respirable dust and crystalline silica, the vocational training period should also be taken into account. Acute heavy-resistance exercise,induced pain and neuromuscular fatigue in elderly women with fibromyalgia and in healthy controls: Effects of strength training

Objective To examine heavy-resistance exercise,induced acute neuromuscular fatigue, blood lactate concentration, and muscle pain in elderly women with fibromyalgia (FM) and in healthy controls before and after a period of strength training. Methods Thirteen elderly women with FM (mean ± SD age 60 ± 2 years) and 10 healthy women (mean ± SD age 64 ± 3 years) performed a heavy-resistance fatiguing protocol (5 sets of leg presses with 10 repetitions maximum) before and after a 21-week strength training period. Maximal isometric force and electromyography (EMG) activity of leg extensors and blood lactate concentration were measured during the loading. Pain was assessed by visual analog scale. Results The strength training led to large increases in maximal force and EMG activity of the muscles and contributed to the improvement in loading performance (average load/set) at week 21. The fatiguing loading sessions typically applied in strength training before and after the experimental period caused remarkable and comparable acute decreases in maximal force and increases in blood lactate concentration in both groups. Acute exercise-induced muscle pain increased similarly in both groups, and the pain level in women with FM was lowered after the 21-week training period. Conclusion The increased strength in women with FM improved high-load performance and also seemed to attenuate perceived pain. Acute exercise-induced neuromuscular changes and the time course of muscle pain in women with FM were comparable with findings in healthy controls, which suggests a typical fatiguing process and a similar trainability of the muscles in elderly women with FM. Perception of risk of adverse drug reactions by medical students: influence of a 1 year pharmacological course

What is already known about this subject ,,Previous studies have pointed out the question of effective training and information for health professionals on adverse drug reactions (ADRs). ,,This lack of training is known to induce inadequate use of drugs and noncompliance of patients. What this study adds ,,Our study was the first to evaluate the perceived risk of ADRs among young medical students and to investigate the impact of university pharmacology courses on their perception of this risk. ,,The aim of our study was not to assess a definite level of perception of risk for the different drug classes but to determine whether the perceived risk of ADRs differs after attending pharmacology courses. ,,Our results show that the pharmacological training allows young medical students to be aware of potentially serious ADRs, especially related to drugs considered as relatively safe, such as NSAIDs and aspirin. Aims To investigate how adverse drug reactions (ADRs) to several classes of drugs are perceived by young medical students before and after a 1 year pharmacology course. Methods The whole cohort of 92 medical students (63 females and 29 males) was questioned during their third year. A visual analogue scale was used to define a score (ranging from 0 to 10) of perceived risk of ADRs associated with each drug class before and at the end of the pharmacological training period. Results Before the pharmacology course, hypnotics were ranked as the most dangerous drugs by the medical students, followed by antidepressants and anticoagulants. Contraceptive pills were listed in the last position. After pharmacological training, antidepressants moved into the first position, followed by anticoagulants and hypnotics. When all different drug classes were taken as a whole, the mean (±SD) of median scores of the perceived risk were 4.8 (±1.3) before and 5.8 (±1.5) at the end of the pharmacology course (P < 0.0001). Except for antidiabetics, antihypertensive drugs, tranquillizers, corticosteroids and hypnotics, the perceived risk significantly increased after the pharmacology course for the other drugs. The highest increases were observed for contraceptive pills (+104%, P < 0.01), NSAIDs (+86%, P < 0.01) and aspirin (+56%, P < 0.01). Conclusions Pharmacological training allows young medical students to be aware of potentially serious ADRs associated with drugs, in particular with drugs considered relatively safe (such as NSAIDs and aspirin) by nonhealth professionals. Effects of detraining on muscle strength and mass after high or moderate intensity of resistance training in older adults

Summary This study examined the effects of a 12 weeks detraining period on muscle strength and mass in older adults who had previously participated in a 12 weeks resistance training programme of high [80% of one repetition maximum (1-RM)] or moderate (60% of 1-RM) intensity. Twenty older adults (60,74 years), separated into a high (HI; n = 10; age: 65 ± 5 years) and a moderate (MI; n = 10; age: 66 ± 4 years) intensity resistance training group, were measured in the 1-RM knee extension and flexion strength, and the midthigh cross sectional areas (CSAs) of quadriceps, hamstrings and total thigh before and after a 12 weeks training period as well as after a 12 weeks detraining period. Maximum knee extension and flexion strength and the CSAs of all muscles decreased significantly (P<0·05) with detraining but remained higher (P<0·05) than pretraining levels for both groups. The HI group had a greater decrement (P<0·05) in maximum strength and the CSA of total thigh compared to the MI group but strength levels and the CSA following detraining were higher (P<0·05) for the HI group. The above data suggest that after a short detraining period of 12 weeks, muscle strength and hypertrophy levels of older adults decrease but remain greater than pretraining irrespective of training intensity. Greater declines in muscle strength are observed following HI training but still muscular strength and muscle mass are retained at a higher level than with MI probably due to the higher gains achieved during the training period. Comparative effects of resistance training on peak isometric torque, muscle hypertrophy, voluntary activation and surface EMG between young and elderly women

Summary We compared the effect of a 10-week resistance training program on peak isometric torque, muscle hypertrophy, voluntary activation and electromyogram signal amplitude (EMG) of the knee extensors between young and elderly women. Nine young women (YW; range 20,30 years) and eight elderly women (EW; 64,78 years) performed three sets of ten repetitions at 75% 1 repetition maximum for the bilateral leg extension and bilateral leg curl 3 days per week for 10 weeks. Peak isometric torque, EMG and voluntary activation were assessed before, during, and after the training period, while knee extensor lean muscle cross-sectional area (LCSA) and lean muscle volume (LMV) were assessed before and after the training period only. Similar increases in peak isometric torque (16% and 18%), LCSA (13% and 12%), LMV (10% and 9%) and EMG (19% and 21%) were observed between YW and EW, respectively, at the completion of training (P<0·05), while the increase in voluntary activation in YW (1·9%) and EW (2·1%) was not significant (P>0·05). These findings provide evidence to indicate that participation in regular resistance exercise can have significant neuromuscular benefits in women independent of age. The lack of change in voluntary activation following resistance training in both age groups despite the increase in EMG may be related to differences between measurements in their ability to detect resistance training-induced changes in motor unit activity. However, it is possible that neural adaptation did not occur and that the increase in EMG was due to peripheral adaptations. [source]


Prevention and treatment of pressure ulcers in a university hospital centre: A correlational study examining nurses' knowledge and best practice

International Journal of Nursing Practice 2010; 16: 183,187 Prevention and treatment of pressure ulcers in a university hospital centre: A correlational study examining nurses' knowledge and best practice This descriptive correlational study had the goal of exploring if relationships existed between the level of knowledge of nurses concerning pressure ulcers, certain nurses' characteristics and the preventive care they applied. A multi-method approach was taken using a questionnaire to measure the level of knowledge of nurses (n = 256) and chart audits (n = 235) to identify the preventive care applied. The results show that the level of knowledge of the nurses is insufficient. They also show a correlation between a higher level of knowledge and (i) the sector of activities in which the nurses are working, (ii) the training periods provided by the university hospital centre, and a (iii) good perception by the nurses of their level of knowledge. However, training on its own cannot guarantee the provision of quality health care, as there is a wide discrepancy between what nurses know and what they put into practice. [source]


Operative experience in the Victorian general surgical training programme

Background: The Royal Australasian College of Surgeons recommends minimum operative and endoscopy training requirements for advanced trainees in general surgery. This study examines the influences of trainee seniority, geographical location of surgical rotation, and surgeons' remuneration arrangements on the operative and endoscopy experiences of general surgical trainees in Victoria. Methods: Trainee log books covering two 6-month training periods in the year 2001 were analysed for depth and breadth of reported operative and endoscopy experience. Results: A total of 81 trainee log books was reviewed. Senior trainees were significantly more likely to have a higher primary operator experience. Total operative caseload and primary operator experience, in terms of major operative cases, were greater in rural rotations. Endoscopy experience was significantly greater in rural rotations. Operative exposure to gynaecological, laparoscopic, orthopaedic, paediatric and plastic surgical procedures was significantly greater in rural rotations. Surgeons' remuneration arrangements were not significantly related to trainee operative experience. Conclusions: Rural surgical rotations enhance the depth and breadth of operative experience of Victorian general surgical registrars, allowing greater exposure to subspecialty surgical procedures.
