    Adjusted Scaling of FDG Positron Emission Tomography Images for Statistical Evaluation in Patients With Suspected Alzheimer's Disease

    ABSTRACT Background and Purpose. Statistical parametric mapping (SPM) gained increasing acceptance for the voxel-based statistical evaluation of brain positron emission tomography (PET) with the glucose analog 2-[18F]-fluoro-2-deoxy-d-glucose (FDG) in patients with suspected Alzheimer's disease (AD). To increase the sensitivity for detection of local changes, individual differences of total brain FDG uptake are usually compensated for by proportional scaling. However, in cases of extensive hypometabolic areas, proportional scaling overestimates scaled uptake. This may cause significant underestimation of the extent of hypometabolic areas by the statistical test. Methods. To detect this problem, the authors tested for hyper metabolism. In patients with no visual evidence of true focal hypermetabolism, significant clusters of hypermetabolism in the presence of extended hypometabolism were interpreted as false-positive findings, indicating relevant overestimation of scaled uptake. In this case, scaled uptake was reduced step by step until there were no more significant clusters of hypermetabolism. Results. In 22 consecutive patients with suspected AD, proportional scaling resulted in relevant overestimation of scaled uptake in 9 patients. Scaled uptake had to be reduced by 11.1%± 5.3% in these cases to eliminate the artifacts. Adjusted scaling resulted in extension of existing and appearance of new clusters of hypometabolism. Total volume of the additional voxels with significant hypometabolism depended linearly on the extent of the additional scaling and was 202 ± 118 mL on average. Conclusions. Adjusted scaling helps to identify characteristic metabolic patterns in patients with suspected AD. It is expected to increase specificity of FDGPET in this group of patients. [source]


    Ultrathin planar inverted-F antenna for multistandard handsets

    Abstract A compact multiband internal antenna has been proposed for multistandard personal communication handsets. The antenna simultaneously covers various frequency bands such as GSM, DCS, PCS, UMTS, WiBro, Bluetooth, S-DMB, and WLAN. Total volume of the antenna is 0.78 cm3 (2.6 cm × 1.5 cm × 0.2 cm) that makes it attractive for multiband slim handsets.     Beyond Reaping the First Harvest: Management Objectives for Timber Production in the Brazilian Amazon

Additional millions of hectares of large, privately owned forests and smaller areas of community forests are certified as well managed by the Forest Stewardship Council, based on certification standards that will be reviewed in 2007. Forest size and ownership are two key factors that influence management objectives and the capacity of forest managers to achieve them. Current best ecological practices for timber production from Brazil's native Amazon forests are limited to reduced-impact logging (RIL) systems that minimize the environmental impacts of harvest operations and that obey legal restrictions regarding minimum diameters, rare species, retention of seed trees, maximum logging intensity, preservation of riparian buffers, fire protection, and wildlife conservation. Compared with conventional, predatory harvesting that constitutes >90% of the region's timber production, RIL dramatically reduces logging damage and helps maintain forest cover and the presence of rare tree species, but current RIL guidelines do not assure that the volume of timber removed can be sustained in future harvests. We believe it is counterproductive to expect smallholders to subscribe to additional harvest limitations beyond RIL, that larger private forested landholdings managed for timber production should be sustainable with respect to the total volume of timber harvested per unit area per cutting cycle, and that large public forests should sustain volume production of individual harvested species. These additional requirements would improve the ecological sustainability of forest management and help create a stable forest-based sector of the region's economy, but would involve costs associated with lengthened cutting cycles, reduced harvest intensities, and/or postharvest silviculture to promote adequate growth and regeneration. Resumen:,Bajo la nueva ley forestal aprobada en 2006 por el Congreso Brasileño, millones de hectáreas de bosques públicos están destinadas a constituir futuras concesiones madereras. Millones de hectáreas adicionales de extensos bosques privados y áreas reducidas de bosques comunitarios están certificadas por el Forest Stewardship Council por su buen manejo, con base en estándares de certificación que serán revisados en 2007. La extensión y tenencia del bosque son dos factores clave que influyen en los objetivos de manejo y en la capacidad de los manejadores para alcanzarlos. Las mejores prácticas ecológicas actuales para la producción de madera en los bosques de la Amazonía Brasileña están limitadas a sistemas de tala de impacto reducido (TIR) que minimizan los impactos ambientales de las operaciones de cosecha y que obedecen restricciones legales en relación con los diámetros mínimos, las especies raras, la retención de árboles semilla, la máxima intensidad de tala, la preservación de amortiguamientos ribereños, la protección del fuego y la conservación de vida silvestre. En comparación con la cosecha convencional, depredadora, mediante la cual se obtiene >90% de la producción de madera en la región, la TIR dramáticamente reduce el daño y ayuda a mantener la cobertura del bosque y la presencia de especies de árboles raras, pero los actuales lineamientos de TIR no aseguran que el volumen de madera removida pueda ser sostenido en futuras cosechas. Consideramos que es contraproducente esperar que los pequeños propietarios suscriban límites a la cosecha más allá de la TIR; que los bosques privados manejados para la producción de madera debieran ser sustentables respecto al volumen total de madera cosechada por unidad de área por ciclo de corte; y que los bosques públicos deberían sustentar el volumen de producción de especies individuales.     Effect of Perilesional Injections of PEG-Interleukin-2 on Basal Cell Carcinoma

    Background: Multiple modalities are available for the treatment of basal cell carcinoma (BCC). The most commonly used modalities include simple excision, Mohs micrographic surgery, curettage and electrodessication, cryosurgery, and irradiation therapy. Interleukin-2 (IL-2) is a cytokine produced chiefly by activated T lymphocytes and has effects on various components of the immune system. Until now the primary clinical use of IL-2 has been in advanced stages of metastatic melanoma and renal cell carcinoma. Systemic administration of IL-2 is known to cause significant toxicity. Objective: The objective of this study was to evaluate the therapeutic efficacy and safety of perilesional PEG-IL-2 injections in patients with BCC in an open label, uncontrolled pilot study. Methods: Patients with histologically confirmed primary BCC over 18 years of age were included in the study. Lesions were treated by injecting a total volume of 0.5 cc of IL-2 in a radial fashion in the subcutaneous tissue. Injection dosages ranged from 3000 to 1,200,000 IU in one to four weekly dosages. A total of 12 tumors were treated in eight patients. Results: Overall response rates were as follows: complete response in 8 of 12 treated tumors (66.6% cure rate), partial response in 3 of 12 injected tumors (25% partial response rate), stable disease with no improvement in 1 treatment site (8.4%). Side effects included local pain, swelling, and erythema, and in one patient flulike symptoms. There were no significant changes of blood tests as compared to baseline levels. Conclusions: The therapeutic response induced by perilesional PEG-IL-2 injections was found to be an encouraging, safe, and well-tolerated treatment of BCC. Further studies including a larger patient population and long-term follow-up are necessary in order to substantiate these findings. [source]


    A rare landform: Yerköprü travertine bridges in the Taurids Karst Range, Turkey

    Abstract Two examples of travertine bridges are observed at 8 to 15 m above stream level in the Lower Zamanti Basin, Eastern Taurids, Turkey. Yerköprü-1 and Yerköprü-2 bridges are currently being deposited from cool karstic groundwaters with log PCO2 > 10,2 atm. The surface area and the total volume of travertine in Yerköprü-1 bridge are 4350 m2 and 40 000 m3, whereas the values for Yerköprü-2 are 2250 m2 and 20 000 m3, respectively. The interplay of hydrogeological structure, local topography, calcite-saturated hanging springs, algal activity and rapid downcutting in the streambed appear to have led to the formation of travertine bridges. Aeration through cascades and algal uptake causes efficient carbon dioxide evasion that enhances travertine formation. Algal curtains aid lateral development of travertine rims across the stream. Model calculations based on a hypothetical deposit in the form of a half-pyramid implied that lateral development should have occurred from both banks of the stream in the Yerköprü-1 bridge, whereas one-sided growth has been sufficient for Yerköprü-2. The height difference between travertine springs and the main stream appears to be a result of Pleistocene glaciation during which karstic base-level lowering was either stopped or slowed down while downcutting in the main stream continued.     Testing the energetic equivalence rule with helminth endoparasites of vertebrates

    Abstract As a general test of the energetic equivalence rule, we examined macroecological relationships among abundance, density and host body mass in a comparative analysis of the assemblages of trophically transmitted endoparasitic helminths of 131 species of vertebrate hosts. Both the numbers and total volume of parasites per gram of host decreased allometrically with host body mass, with slopes roughly consistent with those expected from the allometric relationship between host basal metabolic rate and body mass. From an evolutionary perspective, large body size may therefore allow hosts to escape from the deleterious effects of parasitism. [source]


    Alcohol and Russian mortality: a continuing crisis

    ABSTRACT Background Russia remains in the grip of a mortality crisis in which alcohol plays a central role. In 2007, male life expectancy at birth was 61 years, while for females it was 74 years. Alcohol is implicated particularly in deaths among working-age men. Aims To review the current state of knowledge about the contribution of alcohol to the continuing very high mortality seen among Russian adults Results Conservative estimates attribute 31,43% of deaths among working-age men to alcohol. This latter estimate would imply a minimum of 170 000 excess deaths due to hazardous alcohol consumption in Russia per year. Men drink appreciably more than women in Russia. Hazardous drinking is most prevalent among people with low levels of education and those who are economically disadvantaged, partly because some of the available sources of ethanol are very cheap and easy to obtain. The best estimates available suggest that per capita consumption among adults is 15,18 litres of pure ethanol per year. However, reliable estimation of the total volume of alcohol consumed per capita in Russia is very difficult because of the diversity of sources of ethanol that are available, for many of which data do not exist. These include both illegal spirits, as well as legal non-beverage alcohols (such as medicinal tinctures). In 2006 regulations were introduced aimed at reducing the production and sale of non-beverage alcohols that are commonly drunk. These appear to have been only partially successful. Conclusion There is convincing evidence that alcohol plays an important role in explaining high mortality in Russia, in particular among working age men. However, there remain important uncertainties about the precise scale of the problem and about the health effects of the distinctive pattern of alcohol consumption that is prevalent in Russia today. While there is a need for further research, enough is known to justify the development of a comprehensive inter-sectoral alcohol control strategy. The recent fall in life expectancy in Russia should give a renewed urgency to attempts to move the policy agenda forward. [source]


    A micropillar-integrated smart microfluidic device for specific capture and sorting of cells

    Abstract An integrated smart microfluidic device consisting of nickel micropillars, microvalves, and microchannels was developed for specific capture and sorting of cells. A regular hexagonal array of nickel micropillars was integrated on the bottom of a microchannel by standard photolithography, which can generate strong induced magnetic field gradients under an external magnetic field to efficiently trap superparamagnetic beads (SPMBs) in a flowing stream, forming a bed with sufficient magnetic beads as a capture zone. Fluids could be manipulated by programmed controlling the integrated air-pressure-actuated microvalves, based on which in situ bio-functionalization of SPMBs trapped in the capture zone was realized by covalent attachment of specific proteins directly to their surface on the integrated microfluidic device. In this case, only small volumes of protein solutions (62.5,nL in the capture zone; 375,nL in total volume needed to fill the device from inlet A to the intersection of outlet channels F and G) can meet the need for protein! The newly designed microfluidic device reduced greatly chemical and biological reagent consumption and simplified drastically tedious manual handling. Based on the specific interaction between wheat germ agglutinin (WGA) and N -acetylglucosamine on the cell membrane, A549 cancer cells were effectively captured and sorted on the microfluidic device. Capture efficiency ranged from 62 to 74%. The integrated microfluidic device provides a reliable technique for cell sorting. [source]


    Optimization of the simultaneous removal of nitrogen and organic matter from fishery wastewaters

    Abstract Anaerobic treatment of saline and protein-rich effluents reduces the organic concentration but forms ammonium that hinders nitrogen removal in a later aerobic treatment. The goal of this work was to optimize the design of a denitrifying,nitrifying system for the simultaneous removal of organic matter and nitrogenous compounds from fishery effluents to meet the Chilean legal standards and to compare pre- and postdenitrification processes in the biological treatment of high-strength effluents to minimize the total volume of biological reactors required. A predenitrifying system, that included three reactors,acidifying anaerobic filter, denitrifying (anoxic) filter, and aerobic-active sludge (nitrifying reactor) with recycle to the denitrifying reactor,reduced nitrogen to 0.33 g of total ammonia nitrogen (TAN) L,1, well above the allowed 0.05 g total nitrogen L,1. The predenitrifying system with a second denitrifying reactor, to which organic matter was added, met the legal organic matter and nitrogen emission concentrations (0.042 g TAN L,1).     Under-reporting of alcohol consumption in household surveys: a comparison of quantity,frequency, graduated,frequency and recent recall

    ABSTRACT Aim To compare alternative survey methods for estimating (a) levels of at risk alcohol consumption and (b) total volume of alcohol consumed per capita in comparison with estimates from sales data and to investigate reasons for under-reporting. Setting The homes of respondents who were eligible and willing to participate. Participants A total of 21 674 Australians aged 14 years and older. Design A 2001 national household survey of drug use, experiences and attitudes with weights applied for age, sex, geographic location and day of week of interview. Measures Self-completion questionnaire using quantity,frequency (QF) and graduated,frequency (GF) methods plus two questions about consumption ,yesterday': one in standard drinks, another with empirically based estimates of drink size and strength. Results The highest estimate of age 14 + per capita consumption of 7.00 l of alcohol derived from recall of consumption ,yesterday' or 76.8% of the official estimate. The lowest was QF with 49.8%. When amount consumed ,yesterday' was recalled in standard drinks this estimate was 5.27 l. GF questions yielded higher estimates than did QF questions both for total volume (5.25 versus 4.54 l) and also for the proportion of the population at risk of long-term alcohol-related harm (10.6%versus 8.1%). With the detailed ,yesterday' method 61% of all consumption was on high risk drinking days. Conclusions Questions about typical quantities of alcohol consumed can lead to underestimates, as do questions about drinking ,standard drinks' of alcohol. Recent recall methods encourage fuller reporting of volumes plus more accurate estimates of unrecorded consumption and the proportion of total alcohol consumption that places drinkers at risk of harm. However, they do not capture longer-term drinking patterns. It is recommended that both recent recall and measures of longer-term drinking patterns are included in national surveys. [source]


    IMAGING STUDY: Prefrontal cortex morphometry in abstinent adolescent marijuana users: subtle gender effects

    ABSTRACT Adult human studies suggest frontal dysfunction associated with chronic marijuana (MJ) use, but due to continued neuromaturation, adult studies may not generalize to adolescents. This study characterized prefrontal cortex (PFC) morphometry in chronic MJ-using adolescents following 1 month of monitored abstinence. Data were collected from MJ users (n = 16) and controls (n = 16) aged 16,18. Extensive exclusionary criteria included co-morbid psychiatric and neurologic disorders. Substance use and anatomical measures were collected after 28 days of monitored abstinence. PFC volumes were ascertained from manual tracing by reliable raters on high-resolution magnetic resonance images. After controlling for lifetime alcohol use, gender and intracranial volume, MJ users did not differ from controls in PFC volume. However, marginal group-by-gender interactions were observed (P < 0.09): female MJ users demonstrated comparatively larger PFC volumes while male MJ users had smaller volumes compared with same-gender controls. Further, group status and total PFC volume interacted in predicting executive functioning (P < 0.05). Among MJ users, smaller PFC total volume was associated with better executive functioning while the opposite pattern was seen among the controls. These preliminary results indicate that gender may moderate the relationship between MJ use and PFC morphometry. Given the relationship between larger PFC total volumes and poorer executive functioning among MJ users, female MJ users may be at increased risk for neurocognitive consequences. Future research will measure PFC gray and white matter separately and follow boys and girls over adolescence to examine the influence of MJ use on neurodevelopment. [source]


    Base-Induced Formation of Two Magnesium Metal-Organic Framework Compounds with a Bifunctional Tetratopic Ligand

    Abstract Two coordination polymers constructed from magnesium and the tetratopic organic linker 2,5-dihydroxyterephthalic acid are reported, denominated CPO-26-Mg and CPO-27-Mg. The organic component carries two different types of protic functional groups. The degree of deprotonation of the organic component can be regulated by the amount of sodium hydroxide employed in the synthesis, thus determining which of the compounds forms. In CPO-26-Mg, only the carboxylic acid groups of the linker are deprotonated and take part in the construction of the three-dimensional framework. The structure is non-porous, and its topology is based on the PtS net. In CPO-27-Mg, both the carboxylic acid and the hydroxy groups are deprotonated and involved in the construction of a microporous three-dimensional framework which is based on a honeycomb motif containing large solvent-filled channels. The metal atoms are arranged in chiral chains along the intersection of the honeycomb and contain one water molecule in their coordination sphere, which allows for the creation of coordinatively unsaturated metal sites upon dehydration. CPO-27-Mg is a potentially useful lightweight adsorbent with a pore volume of 60,% of the total volume of the structure and an apparent Langmuir surface area of up to 1030 m2,g,1. Its thermal stability was investigated by thermogravimetry and variable-temperature powder X-ray diffraction, which shows framework degradation to commence at 160 °C in air, at 235 °C under nitrogen, and at 430 °C in a dynamic vacuum. Thermogravimetric dehydration and re-hydration experiments at miscellaneous temperatures indicate that it is possible to obtain open metal sites in CPO-27-Mg, but the water is more tightly bound in this material than in the previously reported isostructural nickel compound.(© Wiley-VCH Verlag GmbH & Co.     Cadmium leaching from some New Zealand pasture soils

    Summary Cadmium (Cd) inputs and losses from agricultural soils are of great importance because of the potential adverse effects Cd can pose to food quality, soil health and the environment in general. One important pathway for Cd losses from soil systems is by leaching. We investigated loss of Cd from a range of contrasting New Zealand pasture soils that had received Cd predominantly from repeated applications of phosphate fertilizer. Annual leaching losses of Cd ranged between 0.27 and 0.86 g ha,l, which are less than most losses recorded elsewhere. These losses equate to between 5 and 15% of the Cd added to soil through a typical annual application of single superphosphate, which in New Zealand contains on average 280 mg Cd kg,1 P. It appears that Cd added to soil from phosphate fertilizer is fairly immobile and Cd tends to accumulate in the topsoil. The pH of the leachate and the total volume of drainage to some extent control the amount of Cd leached. Additional factors, such as the soil sorption capacity, are also important in controlling Cd movement in these pasture soils. The prediction of the amount of Cd leached using the measured concentrations of Cd in the soil solution and rainfall data resulted in an overestimation of Cd losses. Cadmium concentrations in drainage water are substantially less than the current maximum acceptable value of 3 µg l,1 for drinking water in New Zealand set by the Ministry of Health. [source]


    A new body mass estimation of Brachiosaurus brancai Janensch, 1914 mounted and exhibited at the Museum of Natural History (Berlin, Germany)

    Abstract Body mass and surface areas are important in several aspects for an organism living today. Therefore, mass and surface determinations for extinct dinosaurs could be important for paleo-biological aspects as well. Based on photogrammetrical measurement the body mass and body surface area of the Late Jurassic Brachiosaurus brancai Janensch, 1914 from Tendaguru (East Africa), a skeleton mounted and exhibited at the Museum of Natural History in Berlin (Germany), has been re-evaluated. We determined for a slim type of 3D reconstruction of Brachiosaurus brancai a total volume of 47.9 m3 which represents, assuming a mean tissue density of 0.8 kg per 1,000 cm3, a total body mass of 38,000 kg. The volume distributions from the head to the tail were as follows: 0.2 m3 for the head, neck 7.3 m3, fore limbs 2.9 m3, hind limbs 2.6 m3, thoracic-abdominal cavity 32.4 m3, tail 2.2 m3. The total body surface area was calculated to be 119.1 m2, specifically 1.5 m2 for the head, 26 m2 neck, fore limbs 18.8 m2, hind limbs 16.4 m2, 44.2 m2 thoracic-abdominal cavity, and finally the tail 12.2 m2. Finally, allometric equations were used to estimate presumable organ sizes of this extinct dinosaur and to test whether their dimensions really fit into the thoracic and abdominal cavity of Brachiosaurus brancai if a slim body shape of this sauropod is assumed.     Infiltrating Semiconducting Polymers into Self-Assembled Mesoporous Titania Films for Photovoltaic Applications,

In this paper we show that films of titania with a uniform distribution of pore sizes can be made using a block copolymer as a structure-directing agent, and that 33,% of the total volume of the film can be filled with a semiconducting polymer.     Hippocampal neuron and glial cell numbers in Parkinson's disease,A stereological study,

    Abstract Hippocampal atrophy and neuron loss are early and reproducible findings in Alzheimer's disease, and recent magnetic resonance imaging studies indicate that hippocampal atrophy may also be present in Parkinson's disease (PD). To determine whether or not cell loss occurs in PD, we estimated the total neuron and glial cell numbers as well as the total volume unilaterally in the hippocampi of eight demented PD patients and eight control subjects. Cell numbers were estimated in the neuron-containing layers of CA1, CA2-(3), CA4, the dentate gyrus, and subiculum using the optical-fractionator technique. The Cavalieri method was used to estimate the volume of the total hippocampus and its subregions. We did not find significant differences in cell numbers or volumes in PD brains when compared with control subjects. Our results thus indicate that hippocampal atrophy and cell loss are not necessarily involved in the memory impairment and dementia observed in PD. © 2006 Wiley-Liss, Inc. [source]

    Abstract Because the traditional Soil Conservation Service curve-number (SCS-CN) approach continues to be used ubiquitously in water quality models, new application methods are needed that are consistent with variable source area (VSA) hydrological processes in the landscape. We developed and tested a distributed approach for applying the traditional SCS-CN equation to watersheds where VSA hydrology is a dominant process. Predicting the location of source areas is important for watershed planning because restricting potentially polluting activities from runoff source areas is fundamental to controlling non-point-source pollution. The method presented here used the traditional SCS-CN approach to predict runoff volume and spatial extent of saturated areas and a topographic index, like that used in TOPMODEL, to distribute runoff source areas through watersheds. The resulting distributed CN,VSA method was applied to two subwatersheds of the Delaware basin in the Catskill Mountains region of New York State and one watershed in south-eastern Australia to produce runoff-probability maps. Observed saturated area locations in the watersheds agreed with the distributed CN,VSA method. Results showed good agreement with those obtained from the previously validated soil moisture routing (SMR) model. When compared with the traditional SCS-CN method, the distributed CN,VSA method predicted a similar total volume of runoff, but vastly different locations of runoff generation. Thus, the distributed CN,VSA approach provides a physically based method that is simple enough to be incorporated into water quality models, and other tools that currently use the traditional SCS,CN method, while still adhering to the principles of VSA hydrology.     Effects of feeding frequency on nestling begging and digestion

    Nestling begging has the potential to provide parents with honest information about both short- and long-term nutritional needs, yet the importance of previous feeding experience remains largely untested in empirical studies. We examined the effect of two experimental feeding rates on nestling begging in Southern Grey Shrikes Lanius meridionalis using differences in load size to equalize the total volume of food received. There was variation in the pattern of begging behaviour between six pairs of siblings during a hand-feeding trial, although individuals maintained a similar begging intensity throughout a 9-h feeding period. Both treatment groups showed elevated begging responses during a terminal deprivation period, but nestlings fed small food items at frequent intervals demonstrated higher begging responses after a period of deprivation than did siblings fed large food items infrequently. As nestlings fed frequently with small food items had greater levels of undigested protein present in their faeces than birds fed large items infrequently, we suggest experimentally induced variation in digestive efficiency may account for the observed differences in begging behaviour. The possible role of learning, the adaptive significance of trade-offs between feeding rate and digestive efficiency, and a possible conflict of interests between parents and offspring are discussed.     Thermodynamic optimization of internal structure in a fuel cell

    Abstract This paper shows that the internal structure (relative sizes, spacings) of a fuel cell can be optimized so that performance is maximized at the global level. The optimization of flow geometry begins at the smallest (elemental) level, where the fuel cell is modelled as a unidirectional flow system. The polarization curve, power and efficiency are obtained as functions of temperature, pressure, geometry and operating parameters. Although the model is illustrated for an alkaline fuel cell, it may be applied to other fuel cell types by changing the reaction equations and accounting for the appropriate energy interactions. The optimization of the internal structure is subjected to fixed total volume. There are four degrees of freedom in the optimization, which account for the relative thicknesses of the two (anode and cathode) diffusion layers, two reaction layers and the space occupied by the electrolyte solution. The available volume is distributed optimally through the system so that the total power is maximized. Numerical results show that the optima are sharp, and must be identified accurately. Temperature and pressure gradients play important roles, especially as the fuel and oxidant flow paths increase. The optimized internal structure is reported in dimensionless form.     Efficacy and Feasibility of a Novel Tri-Modal Robust Exercise Prescription in a Retirement Community: A Randomized, Controlled Trial

    OBJECTIVES: To test the feasibility and efficacy of current guidelines for multimodal exercise programs in older adults. DESIGN: Randomized, controlled trial. SETTING: Retirement village. PARTICIPANTS: Thirty-eight subjects (14 men and 24 women) aged 76.6 ± 6.1. INTERVENTION: A wait list control or 10 weeks of supervised exercise consisting of high-intensity (80% of one-repetition maximum (1RM)) progressive resistance training (PRT) 3 days per week, moderate-intensity (rating of perceived exertion 11 to 14/20) aerobic training 2 days per week, and progressive balance training 1 day per week. MEASUREMENTS: Blinded assessments of dynamic muscle strength (1RM), balance, 6-minute walk, gait velocity, chair stand, stair climb, depressive symptoms, self-efficacy, and habitual physical activity level. RESULTS: Higher baseline strength and psychological well-being were associated with better functional performance. Strength gains over 10 weeks averaged 39±31% in exercise, versus 21±24% in controls (P=.10), with greater improvements in hip flexion (P=.01), hip abduction (P=.02), and chest press (P=.04) in the exercise group. Strength adaptations were greatest in exercises in which the intended continuous progressive overload was achieved. Stair climb power (12.3±15%, P=.002) and chair stand time (,7.1±15%, P=.006) improved significantly and similarly in both groups. Reduction in depressive symptoms was significantly related to compliance (attendance rate r=,0.568, P=.009, PRT progression in loading r=,0.587, P=.02, and total volume of aerobic training r=,0.541, P=.01), as well as improvements in muscle strength (r=,0.498, P=.002). CONCLUSION: Robust physical and psychological adaptations to exercise are linked, although volumes and intensities of multiple exercise modalities sufficient to cause significant adaptation appear difficult to prescribe and adhere to simultaneously in older adults.     Estimation of number of follicles, volume of colloid and inner follicular surface area in the thyroid gland of rats

    Abstract Volume is an important variable in assessing the growth and involution of the thyroid gland. The functional unit in the thyroid is the follicle, which consists of thyrocytes surrounding colloid. The size of a follicle depends on the number of cells and the amount of colloid. These are interchangeable and vary according to biological activity. Direct measurements of these variables provide information on structures involved in thyroid hormone synthesis, storage and secretion, and also on changes at the morphological and functional levels. Stereological methods are developed to obtain information on three-dimensional structures from two-dimensional sections and to achieve information on an entire organ by examining a minor part of it. Full-grown male Sprague,Dawley rats were used to develop a set of methods relying on unbiased stereological principles to determine the number of follicles, the total volume of colloid and the inner follicular surface area in the thyroid gland. The total volume of colloid was positively correlated (P < 0.021) with the number of follicles and the inner follicular surface area (P < 0.002) but not to the mean volume of colloid in each follicle. Thus under physiological conditions an increase in the total volume of colloid is associated with an increased number of follicles with a constant size distribution rather than a larger volume of colloid in each follicle. This implies that under physiological conditions there is equilibrium in the size distribution of the volume of colloid in each follicle. [source]


    Enhanced Chondrogenesis and Wnt Signaling in PTH-Treated Fractures,

    Abstract Studies have shown that systemic PTH treatment enhanced the rate of bone repair in rodent models. However, the mechanisms through which PTH affects bone repair have not been elucidated. In these studies we show that PTH primarily enhanced the earliest stages of endochondral bone repair by increasing chondrocyte recruitment and rate of differentiation. In coordination with these cellular events, we observed an increased level of canonical Wnt-signaling in PTH-treated bones at multiple time-points across the time-course of fracture repair, supporting the conclusion that PTH responses are at least in part mediated through Wnt signaling. Introduction: Since FDA approval of PTH [PTH(1,34); Forteo] as a treatment for osteoporosis, there has been interest in its use in other musculoskeletal conditions. Fracture repair is one area in which PTH may have a significant clinical impact. Multiple animal studies have shown that systemic PTH treatment of healing fractures increased both callus volume and return of mechanical competence in models of fracture healing. Whereas the potential for PTH has been established, the mechanism(s) by which PTH produces these effects remain elusive. Materials and Methods: Closed femoral fractures were generated in 8-wk-old male C57Bl/6 mice followed by daily systemic injections of either saline (control) or 30 ,g/kg PTH(1,34) for 14 days after fracture. Bones were harvested at days 2, 3, 5, 7, 10, 14, 21, and 28 after fracture and analyzed at the tissue level by radiography and histomorphometry and at the molecular and biochemical levels level by RNase protection assay (RPA), real-time PCR, and Western blot analysis. Results: Quantitative ,CT analysis showed that PTH treatment induced a larger callus cross-sectional area, length, and total volume compared with controls. Molecular analysis of the expression of extracellular matrix genes associated with chondrogenesis and osteogenesis showed that PTH treated fractures displayed a 3-fold greater increase in chondrogenesis relative to osteogenesis over the course of the repair process. In addition, chondrocyte hypertrophy occurred earlier in the PTH-treated callus tissues. Analysis of the expression of potential mediators of PTH actions showed that PTH treatment significantly induced the expression of Wnts 4, 5a, 5b, and 10b and increased levels of unphosphorylated, nuclear localized ,-catenin protein, a central feature of canonical Wnt signaling. Conclusions: These results showed that the PTH-mediated enhancement of fracture repair is primarily associated with an amplification of chondrocyte recruitment and maturation in the early fracture callus. Associated with these cellular effects, we observed an increase in canonical Wnt signaling supporting the conclusion that PTH effects on bone repair are mediated at least in part through the activation of Wnt-signaling pathways.     QTL Analysis of Trabecular Bone in BXD F2 and RI Mice,

    Abstract A sample of 693 mice was used to identify regions of the mouse genome associated with trabecular bone architecture as measured using ,CT. QTLs for bone in the proximal tibial metaphysis were identified on several chromosomes indicating regions containing genes that regulate properties of trabecular bone. Introduction: Age-related osteoporosis is a condition of major concern because of the morbidity and mortality associated with osteoporotic fractures in humans. Osteoporosis is characterized by reduced bone density, strength, and altered trabecular architecture, all of which are quantitative traits resulting from the actions of many genes working in concert with each other and the environment over the lifespan. ,CT gives accurate measures of trabecular bone architecture providing phenotypic data related to bone volume and trabecular morphology. The primary objective of this research was to identify chromosomal regions called quantitative trait loci (QTLs) that contain genes influencing trabecular architecture as measured by ,CT. Materials and Methods: The study used crosses between C57BL/6J (B6) and DBA/2J (D2) as progenitor strains of a second filial (F2) generation (n = 141 males and 148 females) and 23 BXD recombinant inbred (RI) strains (n , 9 of each sex per strain). The proximal tibial metaphyses of the 200-day-old mice were analyzed by ,CT to assess phenotypic traits characterizing trabecular bone, including bone volume fraction, trabecular connectivity, and quantitative measures of trabecular orientation and anisotropy. Heritabilities were calculated and QTLs were identified using composite interval mapping. Results: A number of phenotypes were found to be highly heritable. Heritability values for measured phenotypes using RI strains ranged from 0.15 for degree of anisotropy in females to 0.51 for connectivity density in females and total volume in males. Significant and confirmed QTLs, with LOD scores ,4.3 in the F2 cohort and ,1.5 in the corresponding RI cohort were found on chromosomes 1 (43 cM), 5 (44 cM), 6 (20 cM), and 8 (49 cM). Other QTLs with LOD scores ranging from 2.8 to 6.9 in the F2 analyses were found on chromosomes 1, 5, 6, 8, 9, and 12. QTLs were identified using data sets comprised of both male and female quantitative traits, suggesting similar genetic action in both sexes, whereas others seemed to be associated exclusively with one sex or the other, suggesting the possibility of sex-dependent effects. Conclusions: Identification of the genes underlying these QTLs may lead to improvements in recognizing individuals most at risk for developing osteoporosis and in the design of new therapeutic interventions.     Mapping Quantitative Trait Loci for Vertebral Trabecular Bone Volume Fraction and Microarchitecture in Mice,

    Abstract BMD, which reflects both cortical and cancellous bone, has been shown to be highly heritable; however, little is known about the specific genetic factors regulating trabecular bone. Genome-wide linkage analysis of vertebral trabecular bone traits in 914 adult female mice from the F2 intercross of C57BL/6J and C3H/HeJ inbred strains revealed a pattern of genetic regulation derived from 13 autosomes, with 5,13 QTLs associated with each of the traits. Ultimately, identification of genes that regulate trabecular bone traits may yield important information regarding mechanisms that regulate mechanical integrity of the skeleton. Introduction: Both cortical and cancellous bone influence the mechanical integrity of the skeleton, with the relative contribution of each varying with skeletal site. Whereas areal BMD, which reflects both cortical and cancellous bone, has been shown to be highly heritable, little is known about the genetic determinants of trabecular bone density and architecture. Materials and Methods: To identify heritable determinants of vertebral trabecular bone traits, we evaluated the fifth lumbar vertebra from 914 adult female mice from the F2 intercross of C57BL/6J (B6) and C3H/HeJ (C3H) progenitor strains. High-resolution ,CT was used to assess total volume (TV), bone volume (BV), bone volume fraction (BV/TV), trabecular thickness (Tb.Th), separation (Tb.Sp), and number (Tb.N) of the trabecular bone in the vertebral body in the progenitors (n = 8/strain) and female B6C3H-F2 progeny (n = 914). Genomic DNA from F2 progeny was screened for 118 PCR-based markers discriminating B6 and C3H alleles on all 19 autosomes. Results and Conclusions: Despite having a slightly larger trabecular bone compartment, C3H progenitors had dramatically lower vertebral trabecular BV/TV (,53%) and Tb.N (,40%) and higher Tb.Sp (71%) compared with B6 progenitors (p < 0.001 for all). Genome-wide quantitative trait analysis revealed a pattern of genetic regulation derived from 13 autosomes, with 5,13 quantitative trait loci (QTLs) associated with each of the vertebral trabecular bone traits, exhibiting adjusted LOD scores ranging from 3.1 to 14.4. The variance explained in the F2 population by each of the individual QTL after adjusting for contributions from other QTLs ranged from 0.8% to 5.9%. Taken together, the QTLs explained 22,33% of the variance of the vertebral traits in the F2 population. In conclusion, we observed a complex pattern of genetic regulation for vertebral trabecular bone volume fraction and microarchitecture using the F2 intercross of the C57BL/6J and C3H/HeJ inbred mouse strains and identified a number of QTLs, some of which are distinct from those that were previously identified for total femoral and vertebral BMD. Identification of genes that regulate trabecular bone traits may ultimately yield important information regarding the mechanisms that regulate the acquisition and maintenance of mechanical integrity of the skeleton.     Measurement of Midfemoral Shaft Geometry: Repeatability and Accuracy Using Magnetic Resonance Imaging and Dual-Energy X-ray Absorptiometry

    Abstract Although macroscopic geometric architecture is an important determinant of bone strength, there is limited published information relating to the validation of the techniques used in its measurement. This study describes new techniques for assessing geometry at the midfemur using magnetic resonance imaging (MRI) and dual-energy X-ray absorptiometry (DXA) and examines both the repeatability and the accuracy of these and previously described DXA methods. Contiguous transverse MRI (Philips 1.5T) scans of the middle one-third femur were made in 13 subjects, 3 subjects with osteoporosis. Midpoint values for total width (TW), cortical width (CW), total cross-sectional area (TCSA), cortical cross-sectional area (CCSA), and volumes from reconstructed three-dimensional (3D) images (total volume [TV] and cortical volume [CVol]) were derived. Midpoint TW and CW also were determined using DXA (Lunar V3.6, lumbar software) by visual and automated edge detection analysis. Repeatability was assessed on scans made on two occasions and then analyzed twice by two independent observers (blinded), with intra- and interobserver repeatability expressed as the CV (CV ± SD). Accuracy was examined by comparing MRI and DXA measurements of venison bone (and Perspex phantom for MRI), against "gold standard" measures made by vernier caliper (width), photographic image digitization (area) and water displacement (volume). Agreement between methods was analyzed using mean differences (MD ± SD%). MRI CVs ranged from 0.5 ± 0.5% (TV) to 3.1 ± 3.1% (CW) for intraobserver and 0.55 ± 0.5% (TV) to 3.6 ± 3.6% (CW) for interobserver repeatability. DXA results ranged from 1.6 ± 1.5% (TW) to 4.4 ± 4.5% (CW) for intraobserver and 3.8 ± 3.8% (TW) to 8.3 ± 8.1% (CW) for interobserver variation. MRI accuracy was excellent for TV (3.3 ± 6.4%), CVol (3.5 ± 4.0%), TCSA (1.8 ± 2.6%), and CCSA (1.6 ± 4.2%) but not TW (4.1 ± 1.4%) or CW (16.4 ± 14.9%). DXA results were TW (6.8 ± 2.7%) and CW (16.4 ± 17.0%). MRI measures of geometric parameters of the midfemur are highly accurate and repeatable, even in osteoporosis. Both MRI and DXA techniques have limited value in determining cortical width. MRI may prove valuable in the assessment of surface-specific bone accrual and resorption responses to disease, therapy, and variations in mechanical loading. [source]


    Anaerobic biodegradation of two-phase olive mill solid wastes and liquid effluents: kinetic studies and process performance

    Abstract The new two-phase olive oil mills produce three identifiable and separate waste streams, namely (1) the wash waters from the initial cleansing of the fruit, (2) the aqueous solid residues from the primary centrifugation and (3) the wash waters from the secondary centrifugation. As well as offering process advantages, they also consume less water. Therefore the solid residue, two-phase olive mill solid waste (OMSW), has a high organic matter concentration, giving it an elevated polluting load, and cannot be easily handled by traditional technology which deals with the conventional three-phase olive cake. In addition, the new two-phase olive mill effluents (TPOME) are made up of a mixture of effluents (1) and (3), the total volume of TPOME generated being ,0.25 dm3 kg,1 olives processed. This review aims to report the main features and characteristics of two-phase OMSW and TPOME as compared with the classical olive cake and olive mill wastewater (OMW) derived from the three-phase manufacturing process. The advantages and disadvantages of the two-phase decanting process are summarised. The anaerobic digestibility of two-phase OMSW using different influent substrate concentrations is reported. Kinetic studies of anaerobic digestion of two-phase OMSW are also reviewed and summarised, as well as mass balances to predict the behaviour of the reactor and simplified kinetic models for studying the hydrolysis, acidogenic and methanogenic steps of one- and two-stage anaerobic digestion of OMSW. The review also includes the following: assays of anaerobic digestion of wastewaters from the washing of olives, of olive oil and the two together using fluidised beds and hybrid reactors; the kinetics, performance, stability, purification efficiencies and methane yield coefficients.     Comparison of CD34+ cell collection efficiency on the COBE Spectra and Fenwal CS-3000 plus

    Abstract Optimal collections of mobilized CD34+ cells are important in terms of both patient toxicity and cost. The factors that determine CD34+ collection efficiency (CD34eff) of cell separators have not been well studied. In addition, because several cell separators are available, the type of collection device may also be a significant variable. Previous studies comparing the Baxter-Fenwal CS3000 and the COBE Spectra have not yielded consistent conclusions. Therefore, we retrospectively analyzed the collection outcomes of 163 consecutive donors with a peripheral CD34+ cell concentration (pCD34) of ,5 cells/,l on the first collection that had been harvested on one or the other device. The CS3000 was found to yield a significantly higher CD34eff (50% vs. 39%, P = 0.006). However, donors were not balanced for several prognostic factors, which may contribute to CD34eff including mobilization with G-CSF vs. chemotherapy+G-CSF, average flow rate, and total volume of peripheral blood processed. When appropriate variables were included in a stepwise multiple variable analysis, cell separator type emerged as a significant independent predictive factor for CD34eff (P = 0.018). Our data indicates that the CS3000 will, on average, show a higher absolute CDeff of 8%. Furthermore, since the two devices differ in mechanism, prognostic factors may also differ.     Volumetric analysis of extraction sockets using cone beam computed tomography: a pilot study on ex vivo jaw bone

    Abstract Aim: The aim of this study was to determine the accuracy of volumetric analysis of extraction sockets using cone beam computed tomography (CBCT). Material and Methods: The volume of 40 dental alveoli in nine dry skull specimens (four mandibles and five maxillae) was determined by measuring the volume of the tooth socket impression using the water displacement technique. This was considered as the gold standard. Then, the tooth socket was scanned with CBCT and data were uploaded in the semi-automated Livewire® segmentation software. The software segments the tooth socket in consecutive 1 mm-thick two-dimensional slices. After segmentation, the total volume of the delineated socket was computed. The statistical difference between direct volumetric measurements and those obtained with CBCT imaging was assessed using the Student paired t -test. Result: The mean socket volume of the skull specimens was 227±91 mm3 when obtained by direct measurement and 225±90 mm3 when obtained by CBCT imaging. Student paired t -test showed no significant differences between both volume measurements (p>0.1). Conclusions: CBCT permits imaging of anatomical structures in three planes and allows for reliable volume estimates. The results should be verified in clinical circumstances and might have potential applicability for evaluation of extraction socket healing under different conditions. [source]


    Effect of non-surgical periodontal therapy on clinical and immunological response and glycaemic control in type 2 diabetic patients with moderate periodontitis

    Abstract Objetives: The purpose of this study was to compare the local efficacy of nonsurgical periodontal therapy between type 2 diabetic and non-diabetic patients and the effect of periodontal therapy on glycaemic control. Background: A complex two-way relationship exists between diabetes mellitus and periodontitis. Material and Methods: After selection, 20 subjects (10 diabetic and 10 non-diabetic) underwent baseline examination, periodontal clinical study and biochemical analysis of gingival crevicular fluid (GCF). After the pre-treatment phase, subgingival scaling and root planing were performed. Subsequently, all subjects continued the maintenance programme and were re-examined at 3 and 6 months. Results: Diabetic and non-diabetic subjects responded well after therapy, showing a very similar progression during the follow-up period. Both groups showed clinically and immunologically significant improvements. Significant reductions were also found in the total volume of GCF and levels of interleukin-1, and tumour necrosis factor- ,. Diabetic subjects showed an improvement in their metabolic control. The change in glycosylated haemoglobin (HbA1C) was statistically significant at 3 and 6 months. Conclusions: The clinical and immunological improvements obtained were accompanied by a significant reduction in HbA1C values in type 2 diabetic subjects. Larger studies are needed to confirm this finding and establish whether periodontal therapy has a significant effect on glycaemic control. [source]


    Deproteinized cancellous bovine bone (Bio-Oss®) as bone substitute for sinus floor elevation

    A retrospective, histomorphometrical study of five cases
    Abstract Objectives: To study in detail the performance of deproteinized cancellous bovine bone (DPBB, Bio-Osso®) granules as a bone substitute, a histomorphometric was performed on five patients treated with DPBB for reconstruction of the severely atrophic maxilla. Material and Methods: DPBB was used as mixture with autogenous bone particles, in concentrations that increased from 20% to 100% DPBB, with the time of healing increasing accordingly from 5 to 8 months. A total of 20 vertical biopsies was taken at the time of fixture installation and used for histomorphometry as undecalcified Goldner stained sections. Results: The results show that in all cases, the DPBB granules had been interconnected by bridges of vital newly formed bone. The volume of bone in the grafted area correlated inversely with the concentration of DPBB grafted, and varied between 37% and 23%. However, the total volume of mineralized material (bone plus DPI3B granules) remained within the same range in all five patients (between 53% and 59%). The high values for osteoid and resorption surface, and the presence of tartrate-resistant acid phosphatase-positive multinucleated osteoclasts in resorption lacunae, indicated that bone remodeling was very active in all grafts. Osteoclasts were also observed in shallow resorption pits on DPBB surfaces. The percentage DPBB surface in contact with bone remained stable at about 35% and could not be related to the proportion of DPBB grafted. Conclusion: Although the number of patients examined was limited, the data suggest that deproteinized cancellous bovine bone, preferably combined with autogenous bone particles, is a suitable material for sinus floor elevation in the severely atrophic human maxilla. Zusammenfassung Ent-Proteinisierter spöngiöser boviner Knochen (Bio-Oss) als Knochenersatz zur Sinusboden-Elevation. Eine retrospektive histomorphometrische Studie an 5 Fällen. Ziele:Um das Verhalten Ent-Proteinisierten spöngiösen bovinen Knochengranulats (DPBB, Bio,Oss) als Knochenersatzmaterial detailliert zu studieren, wurde an 5 Patienten, die mit DPBB zur Rekonstruktion der stark atrophischen Maxilla behandelt wurden, eine histomorphometrische Studie durchgeführt. Material und Methoden:DPBB wurde als Mischung mit autogenen Knochenpartikeln in einer Konzentration von 20 , 100% DPBB verwendet. Die Heilungsdauer wurde entsprechend von 5 auf 8 Monate erhöht. Zum Zeitpunkt der Implantatsetzung wurden insgesamt 20 vertikale Biopsien entnommen und für die Histomorphometrie als nicht-entkalkte, Goldner-gefärbte Schnitte verwendet. Ergebnisse: Die Ergebnisse zeigten in allen Fällen, dass die DPBB-Granula über Brücken von vitalem neu gebildetem Knochen miteinander verbunden waren. Das Volumen des Knochens in dem transplantiertem Gebiet korrelierte umgekehrt mit der Konzentration der transpantierten DPBB und variierte zwischen 37% und 23%. Jedoch lag das Gesamtvolumen des mineralisierten Materials (Knochen+DPBB-Granula)bei allen 5 Patienten im selben Bereich (zwischen 53% und 59%). Die hohen Werte für Ostoid und Resorptionsflächen sowie die Anwesenheit von TRAP-positiven multinukleären Osteoklasten in Resorptionslakunen, zeigte, dass das Knochenremodelling in allen Transplantaten sehr aktiv war. Auch in flachen Resorpionsgrübchen auf der DPBB-Oberfläche wurden Osteoklasten beobachtet. Der Prozentsatz der DPBB-Oberfläche welche im Knochenkontakt war blieb mit etwa 35% stabil und korrelierte nicht mit dem Anteil des transplantierten DPBBs. Schlussfolgerung:Obwohl die untersuchte Patientenzahl sehr gering ist, lassen die Daten vermuten, dass Ent-Proteinisierter spöngiöser boviner Knochen, vorzugsweise mit autogenen Knochenpartikeln kombiniert, ein geeignetes Material für die Sinusboden-Elevation bei stark atrophischer Maxilla ist. Une étude histomorphométrique rétrospective de cinq cas Le but de cette étude a été d'analyser en détail les performances de l'os bovin spongieux déprotéiné (DPBB, Bio-Oss®) en granules comme substitut osseux par une analyse histomorphométrique effectuée chez cinq patients traités pour une reconstruction au niveau d'un maxillaire sévèrement atrophié. DPBB a été utilisé en mélange avec des particules osseuses autogènes en concentrations augmentaient de 20 à 100% de DPBB, avec un temps de guérison augmentant paralèllement de cinq à huit mois. Vingt biopsies verticales ont ainsi été prélevées au moment du placement des implants et utilisées pour l'histomorphométrie sur coupes colorées Goldner non-décalcifiées. Les résultats ont indiqué que dans tous les cas les granules DPBB ont été interconnectées par des bridges d'os néoformé vivant. Le volume d'os dans l'aire greffée était en corrélation inverse avec les concentrations de DPBB greffées et variait entre 37 et 23 %. Cependant le volume total de matériel minéralisé (os+granules DPBB) restait dans les mêmes moyennes chez les cinq patients (entre 53 et 59 %). Les valeurs importantes pour la surface de résorption et ostéides, et la présence d'ostéoclastes multinucléaires positifs au TRAP dans les lacunes de résorption indiquaient que le remodelage osseux était très actif dans tous les greffons. Les ostéoclastes étaient également observés dans des petites crevasses de résorption étroites sur les surfaces DPBB. Le pourcentage DPBB en contact avec l'os demeurait stable à environ 35% et ne pouvait pas être mis en relation avec la proportion de DPBB greffé. Bien que le nombre de patients examinés aie été très limité, les données suggèrent que l'os bovin spongieux déprotéiné de préférence en combinaison avec des particules d'os autogène est un bon matériel pour l'épaississement du plancher sinusal en présence de maxillaire humain sévèrement atrophié.
