Osborne SummaryBackground Since the 1980s there have been dedicated pigmented lesion clinics (PLCs) in the U.K. Important considerations when comparing the efficacy of the PLC with other referral clinics include diagnostic accuracy. Objectives To compare the false-negative rate of clinical diagnosis (FNR) in the PLC with that in the other clinics of primary referral of malignant melanoma (MM) in the same geographical area. We have previously shown that certain clinical features are risk factors for diagnostic failure of MM. A further aim of this study was to correct for any differences in frequency of these factors in the melanoma populations between clinics and to estimate the false-positive diagnostic rate (FPR) in the PLC. Methods To compare the FNR between clinics, the case notes of all patients presenting with histologically proven cutaneous MM in Leicestershire between 1987 and 1997 were examined retrospectively. A false-negative diagnosis was defined as documentation of another diagnosis and/or evidence in the case notes that the diagnosis was not considered to be MM. The FNR was estimated as the number of false-negative clinical diagnoses/number of true-positive histological diagnoses. To estimate the diagnostic FPR, which was defined as the number of false-positive clinical diagnoses of MM/total number of positive clinical diagnoses, in the PLC, the outcome of 500 consecutive patients attending the PLC was surveyed. Results The case notes of 731 patients were available, of whom approximately two-thirds initially attended the PLC, one-fifth the General Dermatology clinics (D) and the remainder were divided approximately equally (one-twentieth each) between Plastic Surgery clinics (P), other clinics (O) and the surgery of the general practitioner (GP). The last was regarded as the primary referral clinic if the lesion were excised there prior to any referral. The FNR was lowest for the PLC, at 10%, compared with 29% (D), 19% (P), 55% (O) and 54% (GP) (P < 0·0001). Lesions with risk factors for diagnostic failure were under-represented in the PLC (P < 0·0001), the mean frequencies of the risk factors being 20% (PLC), 25% (D), 22% (P), 31% (O) and 30% (GP). Differences were not large but still could partially explain the lower FNR of the PLC. However, when the FNR was estimated for lesions exhibiting each of these risk factors, the PLC was found to have the lowest rate in every case (PLC vs. all clinics combined, P = 0·04 to P < 0·0001). The mean FNR for the risk factors combined was 18% (PLC), 45% (D), 50% (P), 68% (O) and 71% (GP). Also on logistic multivariable analysis of the PLC vs. all the other clinics on FNR and the above factors, the higher FNR of the other clinics retained significance (odds ratio 5·9, P < 0·0001). In the 500 patients surveyed separately in the PLC, the MM pick-up rate on biopsy was 32% and the diagnostic FPR was 41%. Conclusions The FNR of MM was lower in the PLC than in the other clinics, while the pick-up rate for MM on biopsy and the FPR were acceptably low. [source] Effect of Temperature on Fecundity, Life Span and Morphology of Long- and Short-Spined Clones of Brachionus caudatus f. apsteini (Rotifera)INTERNATIONAL REVIEW OF HYDROBIOLOGY, Issue 6 2008Sujiporn Athibai Abstract We investigated the effect of temperature (20, 25 and 30 °C) on fecundity, life span and morphology of the rotifer Brachionus caudatus f. apsteini. For each temperature, short posterior-spined and long posterior-spined clones of B. caudatus f. apsteini were individually cultured for up to six generations. The rotifers were fed Chlorella sp. at a density of 1 × 106 cells ml,1. Morphometric data (body size and spine length) were collected. Total number of offspring producing by a single female per life cycle at high temperature was higher than at low temperature. The duration of juvenile period, reproductive period, post-reproductive period and life span of both clones of B. caudatus f. apsteini decreased with increasing temperature. All offspring of short posterior-spined clone produce posterior spines at 20 and 25 °C, with an average length of 19.8 ± 6.6 and 11.9 ± 2.6 ,m, respectively. In contrast, they cannot develop posterior spines at 30 °C, at which the average length of the posterior spine remnant was 6.4 ± 1.3 ,m. On the other hand, all offspring of long posterior-spined clone have long posterior spines with average lengths of 36.8 ± 6.1, 36.3 ± 5.2 and 36.6 ± 6.2 ,m at 20, 25 and 30 °C, respectively. This study indicated that the production of posterior spines can be induced by low temperature and that short posterior-spined and long posterior-spined clones are genetically different. Horgas RN OBJECTIVES: To investigate the relationship between self-report and behavioral indicators of pain in cognitively impaired and intact older adults. DESIGN: Quasi-experimental, correlational study of older adults. SETTING: Data were collected from residents of nursing homes, assisted living, and retirement apartments in north-central Florida. PARTICIPANTS: One hundred twenty-six adults, mean age 83; 64 cognitively intact, 62 cognitively impaired. MEASUREMENTS: Pain interviews (pain presence, intensity, locations, duration), pain behavior measure, Mini-Mental State Examination, analgesic medications, and demographic characteristics. Participants completed an activity-based protocol to induce pain. RESULTS: Eighty-six percent self-reported regular pain. Controlling for analgesics, cognitively impaired participants reported less pain than cognitively intact participants after movement but not at rest. Behavioral pain indicators did not differ between cognitively intact and impaired participants. Total number of pain behaviors was significantly related to self-reported pain intensity (,=0.40, P=.000) in cognitively intact elderly people. CONCLUSION: Cognitively impaired elderly people self-report less pain than cognitively intact elderly people, independent of analgesics, but only when assessed after movement. Behavioral pain indicators do not differ between the groups. The relationship between self-report and pain behaviors supports the validity of behavioral assessments in this population. These findings support the use of multidimensional pain assessment in persons with dementia. Brandl Abstract Aims:, The dynamics of bioaerosol generation in specific occupational environments where mail is manually unpacked and sorted was investigated. Methods and Results:, Total number of airborne particles was determined in four different size classes (0·3,0·5, 0·5,1, 1,5 and >5 ,m) by laser particle counting. Time dependent formation of bioaerosols was monitored by culturing methods and by specific staining followed by flow cytometry. Besides handling of regular mail, specially prepared letters (,spiked letters') were added to the mailbags to deliberately release powdered materials from letters and to simulate high impact loads. These letters contained various dry powdered biological and nonbiological materials such as milk powder, mushrooms, herbs and cat litter. Regarding the four size classes, particulate aerosol composition before mail handling was determined as 83·2 ± 1·0, 15·2 ± 0·7, 1·7 ± 0·4 and 0·04 ± 0·02%, respectively, whereas the composition changed during sorting to 66·8 ± 7·9, 22·3 ± 3·6, 10·4 ± 4·0 and 0·57 ± 0·27%, respectively. Mail processing resulted in an increase in culturable airborne bacteria and fungi. Maximum concentrations of bacteria reached 450 CFU m,3, whereas 270 CFU of fungi were detected. Conclusions:, Indoor particle concentrations steadily increased during mail handling mostly associated with particles of diameters >1 ,m. However, it was not possible to distinguish spiked letters from nonspiked by simple particle counting and CFU determinations. Significance and Impact of Study:, The dynamics of bioaerosol generation have to be addressed when monitoring specific occupational environments (such as mail sorting facilities) regarding the occurrence of biological particles. [source] Reduced reproductive success in small populations of the self-incompatible Primula vulgarisJOURNAL OF ECOLOGY, Issue 1 2004Rein Brys Summary 1Habitat fragmentation and the resulting decline in population size can affect biotic interactions and reproductive success of plant species. We investigated the impact of habitat type, population size, morph type and frequency, plant density, floral display and predation on different reproductive components in 16 populations of the distylous self-incompatible perennial herb, Primula vulgaris , a rare, declining species in Belgium. 2Although habitat type accounted for significant variation in population size, we did not find any relation between habitat type and either reproductive and vegetative characteristics. Population size, however, strongly affected reproductive success, such that plants in small populations produced significantly fewer fruits per plant and seeds per fruit, and therefore fewer seeds per plant. 3No significant difference was found between morph types for any reproductive characteristic, nor an interaction with population size. However, when morph frequency was strongly biased (, 1 : 3), the proportion of flowers setting fruit and the number of seeds per fruit were significantly lower in individuals of the common morph type. 4Within populations, individual plants varied tremendously in size and floral display. Total number of fruits per plant significantly increased with floral display, but the highest fruit set per flower was found at intermediate flower number. 5The proportion of fruit suffering pre-dispersal predation per plant significantly increased with floral display, but this did not offset the potential fitness gains of producing a large display. Furthermore, the absolute number of predated fruits per plant was significantly and positively affected by the interaction of the total number of fruits per plant and the density of the population. [source] Biogeographical determinants of lichen species diversity on islets in the West-Estonian ArchipelagoJOURNAL OF VEGETATION SCIENCE, Issue 2 2006Inga Jüriado Abstract Questions: Do islet area, number of biotopes and distance from the mainland shape lichen species richness on islets? Are there any species- or substrate group-specific trends associated with these factors? Location: Islets of the west Estonian Archipelago, Estonia. Methods: A species list was compiled for each of the 32 islets and the relative abundance of each species was estimated. The lichens were divided into seven groups according to their substrate preferences. Generalized linear model (GLIM) analysis was applied to test the effect of the islet traits on the number of lichen species on the islets and in the substrate groups. The probability of presence/absence and abundance of the most frequent species according to the islet traits were tested with GLIM and general linear mixed model. Results: The lichen flora of the islets consisted of 326 taxa, the number of lichen species per islet varied from 2 to 197. Total number of species per islet and within the substrate groups was positively correlated with islet area and with number of biotopes, and negatively correlated with distance from the mainland; however, these relationships varied among the substrate groups. Although individual lichen species showed variation in responses, general trends in island biogeography were evident. Conclusions: The distribution pattern of lichens on the studied islets follows the theory of island biogeography: the number of species per islet depends on isolation, area and biotope diversity. Species specific traits, such as dispersal strategy and growth form, as well as availability of a particular substrate are important for formation of the lichen flora on islets. NETTEL Abstract We describe 10 primers for amplification of microsatellite loci for the mangrove, Avicennia germinans. Eight loci were isolated from a DNA sample from the Pacific coast of Baja California, Mexico and two loci were isolated from a DNA sample from the Atlantic coast of Bermuda. Polymorphism was investigated in a population from the Mexican Pacific coast (n = 15) and in four samples scattered throughout the range of the species. Total number of alleles for the species ranged from two to 10. Observed heterozygosity in the Mexican Pacific coast population ranged from 0.27 to 0.60, with two loci having fixed alleles. [source] Low Back Pain in Older Adults: Are We Utilizing Healthcare Resources Wisely?PAIN MEDICINE, Issue 2 2006Debra K. Weiner MD ABSTRACT Objectives., 1) To examine recent change in prevalence and Medicare-associated charges for non-invasive/minimally invasive evaluation and treatment of nonspecific low back pain (LBP); and 2) to examine magnetic resonance imaging (MRI) utilization appropriateness in older adults with chronic low back pain (CLBP). Design., Two cross-sectional surveys of 1) national (1991,2002) and Pennsylvania (2000,2002) Medicare data; and 2) patients aged ,,65 years with CLBP. Setting., Outpatient data. Participants., Patients aged , 65 years with LBP. Measurements., Study 1: Outpatient national and Pennsylvania Part A Medicare data were examined for number of patients and charges for all patients, and for those with nonspecific LBP. Total number of visits and charges for imaging studies, physical therapy (PT), and spinal injections was also examined for Pennsylvania. Study 2: 111 older adults with CLBP were interviewed regarding presence of red flags necessitating imaging and history of having a lumbar MRI, neurogenic claudication (NC), and back surgery. Results., Study 1: Between 1991 and 2002, there was a 42.5% increase in total Medicare patients, 131.7% increase in LBP patients, 310% increase in total charges, and 387.2% increase in LBP charges. In Pennsylvania (2000,2002), there was a 5.5% increase in LBP patients and 33.2% increase in charges (0.2% for PT, 59.4% for injections, 41.9% for MRI/CT, and 19.3% for X rays). Study 2: None of the 111 participants had red flags and 61% had undergone MRIs (29% with NC, 24% with failed back surgery syndrome). Conclusion., LBP documentation and diagnostic studies are increasing in Medicare beneficiaries, and evidence suggests that MRIs may often be ordered unnecessarily. Injection procedures appear to account for a significant proportion of LBP-associated costs. More studies are needed to examine the appropriateness with which imaging procedures and non-invasive/minimally invasive treatments are utilized, and their effect on patient outcomes. [source] Effect of Maternal Protein Restriction During Pregnancy and Lactation on the Number of Cardiomyocytes in the Postproliferative Weanling Rat HeartTHE ANATOMICAL RECORD : ADVANCES IN INTEGRATIVE ANATOMY AND EVOLUTIONARY BIOLOGY, Issue 3 2010Kyungjoon Lim Abstract Maternal protein restriction leads to a reduction in the number of cardiomyocytes in the rat heart at birth. However, in rats, cardiomyocytes continue to proliferate until about 2 weeks after birth. Hence, this study aimed to examine the effect of maternal protein restriction, on the number of cardiomyocytes in the young rat heart at a time point when the cardiomyocytes have ceased proliferating and are terminally differentiated. Female Wistar Kyoto rats were fed either a normal protein diet (NPD; 20% casein) or a low protein diet (LPD; 8.7% casein) during pregnancy and lactation. Offspring (seven males and seven females per group) were perfusion fixed at 4 weeks of age. Heart volume and total cardiomyocyte number were determined using stereological techniques. At 4 weeks of age, body weights in both male and female LPD offspring were significantly reduced compared with NPD controls whereas relative heart volumes were significantly increased in LPD offspring. Total number of cardiomyocytes was not significantly different between groups. In both groups, there was a significant linear correlation between cardiomyocyte number and heart volume. In conclusion, total cardiomyocyte number in the postproliferative rat heart does not appear to be affected by maternal protein restriction per se but is directly related to heart size. [source] Effectiveness of Chemotherapy and Radiotherapy in Sterilizing Cervical Nodal Disease in Squamous Cell Carcinoma of the Head and Neck,THE LARYNGOSCOPE, Issue 4 2005Michael G. Moore MD Abstract Objective: Determine effects of chemoradiotherapy on nodal disease in head and neck squamous cell carcinoma (SCCA). Study Design: Matched case-control study. Methods: A series of neck dissections (ND) performed for SCCA of the head and neck was retrospectively reviewed. Three groups were identified: 1) planned ND after chemoradiotherapy, 2) ND after radiotherapy alone, and 3) ND before adjuvant therapy (control group). Demographic data, tumor-node-metastasis stage, and pathology were reviewed. Total number of nodes recovered, number of positive nodes, and extracapsular spread (ECS) were recorded. To each patient in the chemoradiotherapy group, a randomly matched dissection in the control group was identified, matching for preoperative N stage and ND type (comprehensive, supraomohyoid, or selective). Comparisons were conducted for total nodes, presence of positive nodes, and ECS. Similar matched comparisons were conducted for ND after radiotherapy alone versus the control group. Results: Ninety-seven NDs (N0 = 10 cases, N1 = 9, N2 = 69, and N3 = 9) were matched to control NDs without previous therapy. Total nodal yield was not statistically different between chemoradiotherapy and control groups (23.5 vs. 23.0 nodes, respectively, P = .77). Positive nodal yield was significantly lower for chemoradiotherapy versus control dissections (0.76 vs. 3.0, P < .001). The percent of chemoradiotherapy dissections recovered as positive nodes was 24.7% versus 68.0% of the control group (P < .001). ECS was identified in 13.4% of the chemoradiotherapy group versus 32.0% of the control group (P = .002). Radiotherapy alone produced less pronounced nodal reductions. Conclusions: Chemoradiotherapy substantially decreases nodal disease and ECS in SCCA. However, a significant percentage of necks contain positive nodes after therapy, meriting consideration for ND. [source] Periconceptional nutrient intakes and risks of conotruncal heart defects,BIRTH DEFECTS RESEARCH, Issue 3 2010Gary M. Shaw Abstract BACKGROUND Few inquiries into periconceptional nutrition, other than folate, and risk of heart defects exist. We investigated the observed association between conotruncal heart defects and periconceptional vitamin use, as well as potential associations with other dietary nutrients. METHODS Data derived from a population-based, case-control study of fetuses and liveborn infants among California births between July 1999 and June 2004; 76% of eligible case mothers and 77% of eligible control mothers were interviewed. Cases included 140 with d-transposition of great arteries (dTGA), and 163 with tetralogy of Fallot (TOF). Total number of controls was 698. Use of vitamins was elicited by questionnaire for the periconceptional period. Dietary nutrient intake was elicited by a well-known food frequency questionnaire. RESULTS The odds ratio for dTGA associated with supplemental vitamin use was 1.0 (95% confidence interval [CI], 0.7,1.5) and for TOF was 0.9 (95% CI, 0.6,1.3). We observed increased risks associated with lower dietary intakes of linoleic acid, total carbohydrate, and fructose for dTGA, whereas decreased risks were observed for lower intakes of total protein and methionine for TOF. Lower dietary intake of several micronutrients,namely folate, niacin, riboflavin, and vitamins B12, A, and E, even after simultaneous adjustment for other studied nutrients,was associated with increased risk of dTGA but not TOF. These associations were observed among women who did not use vitamin supplements periconceptionally. Analytic consideration of several potential confounders did not reveal alternative interpretations of the results. CONCLUSION Evidence continues to accumulate to show that nutrients, particularly folate, influence risks of structural birth defects. [source] Quantification of left ventricular volumes and ejection fraction from gated 99mTc-MIBI SPECT: MRI validation of the exini heart software packageCLINICAL PHYSIOLOGY AND FUNCTIONAL IMAGING, Issue 2 2009Oliver H. Winz Summary The aim of the study was to validate the accuracy of the exini heart software (exini) package in assessing left ventricular end-diastolic/systolic volumes (EDV, ESV) and ejection fraction (LVEF) from gated 99mTc-MIBI single-photon emission tomography (SPECT). Cardiac magnetic resonance imaging (cMRI) was used as reference. Furthermore, effects of perfusion defects and image quality in SPECT on correlation between gated SPECT and magnetic resonance imaging were investigated. Methods:, Seventy patients were examined using gated SPECT (rest study, eight gates per cardiac cycle). EDV, ESV and LVEF were calculated from gated SPECT using exini. Directly before or after SPECT, cMRI (20 gates cardiac per cycle) was performed. EDV, ESV and LVEF were calculated using Simpson's rule. Perfusion defects were quantified using the summed-rest-score (SRS). Total number of myocardial counts were used to rate image quality. Results:, Correlation between results of gated SPECT and cMRI was high for EDV (R = 0·89) and ESV (R = 0·94) and good for LVEF (R = 0·78). ESV (exini 54 ± 31 ml versus cMRI 57 ± 34 ml) and LVEF (exini 62·9 ± 11·7% versus cMRI 60·6 ± 13·9%) did not differ significantly whereas exini overestimated EDV significantly compared with cMRI (exini 144 ± 41 ml versus cMRI 137 ± 36 ml; P<0·005). No correlation was found between absolute differences of the results given by gated SPECT and cMRI and SRS or total myocardial counts (R < 0·18). Conclusion:, End-diastolic volume, ESV and LVEF calculated from gated SPECT using exini agree with cMRI over a wide range of values. Correlation between both the methods was good for EDV and ESV, and acceptable for LVEF. No relevant influence of image quality or SRS on the accuracy of exini results was found. Irwin BA RGN MSc Abstract Background, The results of three rounds of National Stroke Audit in England,, Wales and Northern, Ireland are, compared. Methods, Audit of the organization of stroke services and retrospective case-note audit of up to 40 consecutive cases admitted per hospital over a 3-month period was conducted in each of 1998, 1999 and 2001/02. The changes in the organizational, case-mix and process results of the hospitals that had participated in all three rounds were analysed. Results, 60% of all eligible trusts from England, Wales and Northern Ireland took part in all three audits in 1998, 1999 and 2001/02. Total numbers of cases were 4996, 4841 and 5152, respectively. Case-mix variables were similar over the three rounds. Mortality at 7 and 30 days fell by 3% and 5%, respectively. The proportion of hospitals with a stroke unit rose from 48% to 77%. The proportion of patients spending most of their stay in a stroke unit rose from 17% in 1998 to 26% in 1999 and 29% in 2001/02. Improvements achieved in process standards of care between 1998 and 1999 (median change was a gain of 9%) failed to improve further by 2001/02 (median change was 0%). In all three rounds process standards ,of ,care ,tended ,to ,be ,better ,in ,stroke units. Conclusions, Three rounds of national audit of stroke care have shown standards of care on stroke units were notably higher than on general wards. Slowing in the rise of the proportion managed on stroke units mirrors the slow down in improvement to overall national standards of care. To further improve outcomes and national standards of stroke care a much higher proportion of patients needs to be managed in stroke units. [source] Involvement of breast cancer resistance protein expression on rheumatoid arthritis synovial tissue macrophages in resistance to methotrexate and leflunomideARTHRITIS & RHEUMATISM, Issue 3 2009Joost W. van der Heijden Objective To determine whether multidrug-resistance efflux transporters are expressed on immune effector cells in synovial tissue from patients with rheumatoid arthritis (RA) and compromise the efficacy of methotrexate (MTX) and leflunomide (LEF). Methods Synovial tissue biopsy samples obtained from RA patients before treatment and 4 months after starting treatment with MTX (n = 17) or LEF (n = 13) were examined by immunohistochemical staining and digital image analysis for the expression of the drug efflux transporters P-glycoprotein, multidrug resistance,associated protein 1 (MRP-1) through MRP-5, MRP-8, MRP-9, and breast cancer resistance protein (BCRP), and the relationship to clinical efficacy of MTX and LEF was assessed. Results BCRP expression was observed in all RA synovial biopsy samples, both pretreatment and posttreatment, but not in control noninflammatory synovial tissue samples from orthopedic patients. BCRP expression was found both in the intimal lining layer and on macrophages and endothelial cells in the synovial sublining. Total numbers of macrophages in RA patients decreased upon treatment; in biopsy samples with persistently high macrophage counts, 2-fold higher BCRP expression was observed. Furthermore, median BCRP expression was significantly increased (3-fold) in nonresponders to disease-modifying antirheumatic drugs (DMARDs) compared with responders to DMARDs (P = 0.048). Low expression of MRP-1 was found on synovial macrophages, along with moderate expression in T cell areas of synovial biopsy specimens from one-third of the RA patients. Conclusion These findings show that the drug resistance,related proteins BCRP and MRP-1 are expressed on inflammatory cells in RA synovial tissue. Since MTX is a substrate for both BCRP and MRP-1, and LEF is a high-affinity substrate for BCRP, these transporters may contribute to reduced therapeutic efficacy of these DMARDs. Clark Problem, Spontaneous abortions in the CBA × DBA/2 model are normally reported as number of resorptions/total number of implantations (R/T), pooling data from individual mice. The significance of differences between groups has been determined using non-parametric statistics (e.g. chi-square or Fisher's Exact test) based on a priori predictions. Recently, it has been argued that medians with box plots should replace the accepted standard, but this deprives readers of data needed to verify P -values, and leads to inferences incompatible with biological and statistical reality. Method of study, Raw data on 173 individual CBA × DBA/2 matings were analyzed by median and mean, along with R/T data from 18 independent experiments containing 5,10 mice per group. Raw data from 19 CBA × BALB/c matings were similarly analyzed. Results, Individual CBA × DBA/2 mouse resorption rates showed a non-Gaussian distribution, but the mean and median differed by <0.5%. Resorption data from 6 and 12 independent pools of mice were normally distributed. Only the mean enabled a between-group P -value calculation. CBA × BALB/c matings gave a median of 0 and mean of 5.1%; the data were not normally distributed, but that was because of a bimodal distribution. One group of mice had 0 abortions, and the second a mean of 13.9% abortions, and the data from the latter group were normally distributed. Conclusion, Although it is possible to compare individual mice, and even individual implantation sites, in resorption (abortion) studies, as the relevant question is the significance of differences between treatment groups of mice, and reproducibility, the established classical method of reporting R/T should continue to be provided. In CBA × BALB/c matings, where abortion rates are low, using the median is misleading and may obscure the existence of two distinct populations. Crescenzi Abstract We study the connectivity properties of a family of random graphs that closely model the Bluetooth's device discovery process, where each device tries to connect to other devices within its visibility range in order to establish reliable communication channels yielding a connected topology. Specifically, we provide both analytical and experimental evidence that when the visibility range of each node (i.e. device) is limited to a vanishing function of n, the total number of nodes in the system, full connectivity can still be achieved with high probability by letting each node connect only to a ,small' number of visible neighbors. Our results extend previous studies, where connectivity properties were analyzed only for the case of a constant visibility range, and provide evidence that Bluetooth can indeed be used for establishing large ad hoc networks. [source] A quantitative study of the optic nerve in diabetic mutant, Otsuka Long-Evans Tokushima Fatty (OLETF) ratsCONGENITAL ANOMALIES, Issue 4 2001Kazuhiko Sawada ABSTRACT, Optic nerves of the Otsuka Long-Evans Tokushima Fatty (OLETF) rat, an animal model of non-insulin dependent diabetes mellitus, were examined using quantitative stereological procedures. At 67 weeks of age, OLETF rats showed a mild hyperglycemia: their blood glucose level was 196 ± 93 mg/dl, significantly higher than that of non-diabetic control Long-Evans Tokushima Otsuka (LETO) rats (110 ± 24 mg/dl). However, there were no differences in the cross sectional area of optic nerves (the mean minimum diameter), the total number and mean diameter of both myelinated and non-myelinated fibers, or the thickness of the myelin sheath between OLETF and LETO rats. The results suggested that a mild hyperglycemia in OLETF rats could not cause any morphological changes in the optic nerve. Hughes To explore the feasibility of basing conservation action on community-level biogeography, we sampled a montane insect community. We addressed three issues: (1) the appropriate scale for sampling insect communities; (2) the association of habitat specialization,perhaps a measure of extinction vulnerability,with other ecological or physical traits; and (3) the correlation of diversity across major insect groups. Using malaise traps in Gunnison County, Colorado, we captured 8847 Diptera (identified to family and morphospecies), 1822 Hymenoptera (identified to morphospecies), and 2107 other insects (identified to order). We sampled in three habitat types,meadow, aspen, and conifer,defined on the basis of the dominant vegetation at the scale of hundreds of meters. Dipteran communities were clearly differentiated by habitat type rather than geographic proximity. This result also holds true for hymenopteran communities. Body size and feeding habits were associated with habitat specialization at the family level. In particular, habitat generalists at the family level,taxa perhaps more likely to survive anthropogenic habitat alteration,tended to be trophic generalists. Dipteran species richness was marginally correlated with hymenopteran species richness and was significantly correlated with the total number of insect orders sampled by site. Because these correlations result from differences in richness among habitat types, insect taxa may be reasonable surrogates for one another when sampling is done across habitat types. In sum, community-wide studies appear to offer a practical way to gather information about the diversity and distribution of little-known taxa. Resumen:No existe ni el tiempo ni los recursos para diseñar planes de conservación para cada especie, particularmente para los taxones poco estudiados, no carismáticas, pero ecológicamente importantes que componen la mayoría de la biodiversidad. Para explorar la factibilidad de basar acciones de conservación en biogegrafía a nivel comunitario, muestreamos una comunidad de insectos de montaña. Evaluamos tres aspectos: (1) la escala adecuada para el muestreo de comunidades de insectos; (2) la asociación de especialización de hábitat,quizá una medida de vulnerabilidad de extinción,con otras características ecológicas o físicas; y (3) la correlación de la diversidad a lo largo de los grupos principales de insectos. Mediante el uso de trampas en el condado Gunnison, en Colorado, capturamos 8847 dípteros (identificados a nivel de familia y morfoespecies), 1822 himenópteros (identificadas hasta morfoespecies) y 2107 otros insectos (identificados a nivel de orden). [source] Characterizing Core and Corridor Use by Nubian Ibex in the Negev Desert, IsraelCONSERVATION BIOLOGY, Issue 1 2000Yehoshua Shkedy To identify and characterize corridors used by Nubian ibex (Capra ibex nubiana), we analyzed sighting data recorded for the past 20 years in the Israel Nature Reserves Authority data banks. We categorized each recorded sighting as belonging to a corridor or a core zone based on the total number of sightings in its vicinity. We identified three main core populations, a natural corridor connecting two of them, and a corridor that was not evident connecting the third population. Ibex inside and outside core zones were sighted on steeper terrain than expected by chance; this was more pronounced inside the core zones than outside them. We hypothesize that because ibex outside core zones must move rapidly and directionally across unfamiliar habitats, they must use more moderate terrain. Although ibex sightings in core zones were mostly in the vicinity of water, we found no relationship between the location of the sightings and proximity to water sources in the corridor. Hence, water does not appear to be an important factor in movement through corridors. It was more common to observe ibex out of the core zones during the summer. Males, which can be twice the size of females, were found traveling alone in corridors more often than in core zones. There was no difference between males and females in the steepness of terrain in which they were sighted outside core zones. Our data show that protecting ibex habitat in core zones and corridors is important to ibex conservation in Israel's arid zones. In addition, protecting this habitat may benefit other rock-dwelling species in the area. Resumen: Los corredores son un factor clave en esfuerzos de conservación. Para identificar y caracterizar el uso de corredores por el íbice de Nubia (Capra ibex nubiana) analizamos datos de avistamiento de los últimos 20 años en las bases de datos de la Autoridad Israelí de Reservas Naturales. Categorizamos cada registro asignándolo a un corredor o a una zona núcleo basándonos en el total de registros cercanos. Identificamos tres poblaciones en zona núcleo, un corredor natural que conectaba a dos de ellas y un corredor, no muy evidente, conectando a la tercera población. Se registraron íbices en terrenos inclinados dentro y fuera de zonas núcleo más de lo esperado al azar; esto fue más marcado dentro de las zonas núcleo que afuera. Hipotetizamos que deben utilizar terrenos más moderados, debido a que los íbices fuera de las zonas núcleo deben moverse rápida y direccionalmente a través de hábitats no familiares. Aunque los registros de íbices en zonas núcleo estuvieron cercanos a agua, no encontramos relación entre la localización de los registros y la cercanía a fuentes de agua en el corredor. Por tanto, el agua no parece ser un factor importante en el movimiento en los corredores. Fue más común observar íbices afuera de las zonas núcleo en el verano. Los machos, que pueden ser dos veces más grandes que hembras, fueron registrados desplazándose solos en los corredores más a menudo que en las zonas núcleo. Thomas Abstract Aim:, Our laboratory has shown that oestrogen acts to augment myoblast (satellite cell) activation, proliferation and total number and that this may occur through an oestrogen receptor (OR)-mediated mechanism. The purpose of this study was to further investigate the mechanism of oestrogen influence on augmentation of post-exercise myoblast numbers through use of a specific OR-, agonist, propyl pyrazole triol (PPT). Methods:, Ovariectomized rats were used (n = 64) and separated into four groups: sham, oestrogen supplemented, agonist supplemented, and a combined oestrogen and agonist supplemented group. These groups were further subdivided into control (unexercised) and exercise groups. Surgical removal of white vastus and soleus muscles was performed 72 h post-exercise. Muscle samples were immunostained for the myoblast markers Pax7 and MyoD. Results:, A significant increase in total (Pax7-positive) and activated (MyoD-positive) myoblasts was found in all groups post-exercise. A further significant augmentation of total and activated myoblasts occurred in oestrogen supplemented, agonist supplemented and the combined oestrogen and agonist supplemented groups post-exercise in white vastus and soleus muscles relative to unsupplemented animals. Conclusion:, These results demonstrate that both oestrogen and the specific OR-, receptor agonist, PPT, can significantly and to similar degrees augment myoblast number and activation following exercise-induced muscle damage. This suggests that oestrogen acts through an OR-mediated mechanism to stimulate myoblast proliferation following exercise, with OR-, playing a primary role. [source] FS11.1 Primula obconica , a falling allergenCONTACT DERMATITIS, Issue 3 2004Maureen Connolly Objective:, We believe the incidence of primula contact allergic dermatitis has fallen since the introduction of primin-free primula onto the European market and thus our study aims were twofold. Firstly to see if the incidence of primula contact allergic dermatitis was truly on the decline and secondly to confirm the presence and document retailers' knowledge and awareness of primin-free primula in the UK. Methods:, A questionnaire was sent to 22 contact dermatitis departments throughout the UK and Ireland looking at the number of primin positive patch tests in the years 1995/96, 1998, 2000 and 2002 compared with the total number of patch tests. 10 seed suppliers and 12 plant retailers were asked to complete a telephone survey. Results:, We showed a significant fall in the yearly incidence of contact allergy to primin from 0.785% in 1995/96 to 0.457% in 2002. This downward trend was statistically significant (p = 0.001). The telephone survey showed 90% of seed suppliers were aware that the older varieties of P. obconica could cause an allergic reaction whereas only 60% of them were aware that new primin-free varieties were now being bred. 50% of suppliers were in fact selling these primin-free varieties with 60% of them stocking a primin-free variety exclusively. 90% of retailers were not currently selling any variety of P. obconica. Conclusion:, Our study shows that the incidence of primula contact dermatitis is falling. The overall trend is moving towards primin-free varieties provided they continue to be horticulturally viable long term. [source] The Proportion and Social Capital of Outside Directors and Their Impacts on Firm Value: evidence from KoreaCORPORATE GOVERNANCE, Issue 6 2007Yangmin Kim This study examines the effects of outside directors' proportion and their social capital on firm value using a sample of 473 large, publicly traded Korean companies from 1998 through 2003. Outside director proportion, which is defined as the ratio of outside directors to the total number of directors of a company, is regarded as a proxy of board independence. Outside director social capital, which is defined as the degree to which outside board members have outside contacts in the external environment, is regarded as a proxy of board's ability to extract valuable resources or information from the environment. It is hypothesised that both the proportion of outside directors to the total directors and outside director social capital will be positively associated with firm value. This study reports strong GLS evidence of the relationship between outside director social capital and firm value but no significant relationship between outside director proportion and firm value. [source] A European interlaboratory testing of three well-known procedures for immunocytochemical detection of epithelial cells in bone marrow.CYTOMETRY, Issue 6 2006Results from analysis of normal bone marrow Abstract Background: This investigation intended to study the unspecific background to be expected in normal bone marrow (BM), comparing three well recognized protocols for immunocytochemical detection of disseminated carcinoma cells. The interlaboratory variation in screening and evaluation of stained cells was analyzed and different screening methods were compared. Methods: BM mononuclear cells (BM MNC) from 48 normal BMs were immunostained in parallel by three participating laboratories. The protocols, based on three different anti-cytokeratin antibodies, have all been in common use for detection of disseminated carcinoma cells: the A45-B/B3 protocol (Hamburg), the CK2 protocol (Augsburg) and the AE1AE3 protocol (Oslo). For all protocols, the immunostained cells were visualized by the same alkaline-phosphatase (AP) detection system (APAAP) followed by detection of the cells by manual screening and by two different automated screening systems (ACIS from Chromavision and MDS1 from Applied Imaging). Detected AP-visualized cells were morphologically classified into unambiguous hematopoietic (Uhc) and questionable cells (Qc, potentially interpreted as tumor cells). Results: Seven of 48 BMs (15%) harbored ,1 AP-visualized cell(s) among 1 × 106 BM MNC, both for the A45-B/B3- and for the AE1AE3 protocol, while for CK2 a higher proportion of BMs (21 BMs; 44%) harbored AP-visualized cells (P < 0.01, McNemar's test). The number of Qc was, for all protocols, 1 log lower than the total number of AP-visualized cells. On average, the frequency of Qc was 0.04, 0.08, and 0.02 per 106 BM MNC with A45-B/B3, CK2 and AE1AE3, respectively, and the number of Qc-positive BMs 1, 4, and 1. The MDS1 screening sensitivity was similar to manual screening, while ACIS detected fewer cells (P < 0.001, McNemar's test). Conclusions: All protocols resulted in AP-visualization of occasional hematopoietic cells. However, morphological classification brings the specificity to a satisfactory high level. Approximately 10% of AP-visualized cells were categorized "questionable". Chamorro However, the mechanisms by which different storage conditions alter the functional status of PDL cells have not been determined. The purpose of the present study was to investigate, in vitro, the level of programed cell death or apoptosis in a population of PDL cells following storage under different conditions. Primary human PDL cells were plated into 24-well-culture plates and allowed to attach for 24 h. Cells were then exposed for 1 h to milk, Hank's balanced salt solution (HBSS), Soft Wear contact lens solution or Gatorade at room temperature or on ice. Culture medium was used as a negative control. Apoptosis was evaluated at 24, 48, and 72 h after treatment on quadruplicate samples by using the ST 160 ApopTag Fluorescein Direct In Situ Detection Kit. The total number of cells and the total number of apoptotic cells were counted. The results indicated that at 24 and 72 h, PDL treated with Gatorade and the contact lens solution displayed the highest percentages of apoptotic cells when compared with the other treatment groups at room temperature. Overall, cells treated on ice showed significantly lower levels of apoptosis when compared with treatments at room temperature. In conclusion, the results indicated that apoptosis plays a major role in cell death in cells treated with Gatorade and contact lens solutions in comparison to other storage solutions and that storage on ice can inhibit programed cell death. Skaare Abstract ,,,Prevention of diseases and injuries is a field of priority for the health authorities in Norway. In 1990, an interdepartmental project was established in order to reduce the number of accidents, including dental injuries. The purpose of the present study was to examine the causes of dental injuries, the place of event and the possibilities for prevention. During a 1-year period, all new trauma episodes affecting children aged 7,18 years were registered by calibrated dentists in the public dental health service in the county of Nord-Trøndelag and in the capital city of Oslo. A distinction was made between school and leisure time accidents. The severity of injuries was classified as mild, moderate or severe, and the dentists were asked to judge whether the severe injuries were preventable. Nearly half of the 1275 injured individuals (48%) were injured at school. Almost half of the leisure time injuries occurred during children's play. Sports and traffic accidents were less common. Sports accidents represented only 8% of the total number of injuries, the same as the number of individuals injured by violence. Statistically significant associations were observed between the cause of injury and county (P < 0.001) and the cause of injury and age group (P < 0.001). In total, 4% of all injuries were classified as severe, of which only one-third were assumed to be preventable. In summary, because of the complexity of etiological factors, it is neither easy to prevent dental injuries nor to make guidelines on prevention. Pakkenberg Objective:, To describe structural and biochemical evidence from postmortem brains that implicates the reciprocal connections between the mediodorsal thalamic nucleus and the prefrontal cortex in cognitive symptoms of schizophrenia. Method:, The estimation of the regional volumes and cell numbers was obtained using stereological methods. The biochemical analyses of molecular expression in postmortem brain involve quantitative measurement of transcripts and proteins by in-situ (RNA) or Western blot/autoradiography in brains from patients with schizophrenia and comparison subjects. Results:, Stereological studies in postmortem brain from patients with schizophrenia have reported divergent and often opposing findings in the total number of neurons and volume of the mediodorsal (MD) thalamic nucleus, and to a lesser degree in its reciprocally associated areas of the prefrontal cortex. Similarly, quantitative molecular postmortem studies have found large inter-subject and between-study variance at both the transcript and protein levels for receptors and their interacting molecules of several neurotransmitter systems in these interconnected anatomical regions. Combined, large variation in stereological and molecular studies indicates a complex and heterogeneous involvement of the MD thalamic-prefrontal loop in schizophrenia. Conclusion:, Based on a considerable heterogeneity in patients suffering from schizophrenia, large variation in postmortem studies, including stereological and molecular postmortem studies of the MD thalamus and frontal cortex, might be expected and may in fact partly help to explain the variable endophenotypic traits associated with this severe psychiatric illness. This is not done consistently because of lack of defined targets, and interlaboratory comparison is almost nonexistent. Between July 2004 and April 2006 (N = 176,185 cases), the overall performance metrics of the Minnesota program, limited to MS/MS testing, were as follows: detection rate 1:1,816, positive predictive value 37% (54% in 2006 till date), and false positive rate 0.09%. The repeat rate and the proportion of cases with abnormal findings actually been reported are new metrics proposed here as an objective mean to express the overall noise in a program, where noise is defined as the total number of abnormal results obtained using a given set of cut-off values. Konradsen Objective:, To measure the number of submissions to Acta Pyschiatrica Scandinavica from low- and middle-income countries (LIC/MIC) compared with submissions from high income countries (HIC), to compare the way through the peer review process for the three groups respectively, as well as how they do as published articles eventually. Method:, By help of the Acta Psychiatrica Scandinavica editorial office database all submissions of reviews of the literature, clinical overviews, original articles, brief communications, and case reports from 2002 through 2005 were analysed as to external/in-house review, acceptance/rejection, number of downloads from Blackwell-Synergy, and number of citations [Institute of Scientific Information (ISI)] in a comparison between HIC and LIC/MIC. Results:, About 14.6% of the total submissions in 2002/2003 came from LIC and MIC countries, 15.5% for the 2004/2005 period. In both periods, a larger portion of LIC/MIC manuscripts were reviewed exclusively in-house compared with HIC ones and among those papers reviewed by external experts a smaller proportion of the submissions from HIC countries were rejected than of those from LIC and MIC countries. From the first to the second period there is a significant increase of proportion of submitted LIC/MIC papers accepted for publication compared with HIC papers. Conclusion:, Low- and middle-income countries manuscripts do gradually better in the competition with HIC papers in Acta Psychiatrica Scandinavica. [source] Evaluation of a Pediatric-sedation Service for Common Diagnostic ProceduresACADEMIC EMERGENCY MEDICINE, Issue 6 2006Wendalyn K. King MD Abstract Background: Pediatric patients often require sedation for diagnostic procedures such as magnetic resonance imaging and computed tomography scanning. In October 2002, a dedicated sedation service was started at a tertiary care pediatric facility as a joint venture between pediatric emergency medicine and pediatric critical care medicine. Before this service, sedation was provided by the department of radiology by using a standard protocol, with high-risk patients and failed sedations referred for general anesthesia. Objectives: To describe the initial experience with a dedicated pediatric-sedation service. Methods: This was a retrospective analysis of quality-assurance data collected on all sedations in the radiology department for 23-month periods before and after sedation-service implementation. Study variables were number and reasons for canceled or incomplete procedures, rates of referral for general anesthesia, rates of hypoxia, prolonged sedation, need for assisted ventilation, apnea, emesis, and paradoxical reaction to medication. Results are reported in odds ratios (OR) with 95% confidence intervals (95% CI). Results: Data from 5,444 sedations were analyzed; 2,148 before and 3,296 after sedation-service activation. Incomplete studies secondary to inadequate sedation decreased, from 2.7% before the service was created to 0.8% in the post,sedation-service period (OR, 0.29; 95% CI = 0.18 to 0.47). There also were decreases in cancellations caused by patient illness (3.8% vs. 0.6%; OR, 0.16; 95% CI = 0.10 to 0.27) and rates of hypoxia (8.8% vs. 4.6%; OR, 0.50; 95% CI = 0.40 to 0.63). There were no significant differences between the groups in rates of apnea, need for assisted ventilation, emesis, or prolonged sedation. The implementation of the sedation service also was associated with a decrease in both the number of patients referred to general anesthesia without a trial of sedation (from 2.1% to 0.1%; OR, 0.33; 95% CI = 0.06 to 1.46) and the total number of general anesthesia cases in the radiology department (from 7.5% to 4.4% of all patients requiring either sedation or anesthesia; OR, 0.56; 95% CI = 0.45 to 0.71). Conclusions: The implementation of a dedicated pediatric-sedation service resulted in fewer incomplete studies related to inadequate sedation, in fewer canceled studies secondary to patient illness, in fewer referrals for general anesthesia, and in fewer recorded instances of sedation-associated hypoxia. These findings have important implications in terms of patient safety and resource utilization. [source] |
