Regulation and Social Solidarity

Justifications for regulation are commonly based on the identification of market failures. This is however inadequate to account for much regulation, and sees regulation as inherently second best to market allocations. This article argues that, although some regulation will be based on market failure, other justifications can be found in the protection of rights and in the maintenance of social solidarity. Theoretical support for this last rationale can be found in the work of Durkheim and Duguit and the concept of public service. To accept this rationale for regulation has important implications both for regulatory policies and regulatory instruments. [source]


Antipsychotic combination therapy in schizophrenia.

A review of efficacy, risks of current combinations
Objective:, To review the literature on efficacy and risks of combining antipsychotics (atypical with atypical or conventional) and suggest a rationale and strategies for future clinical trials. Method:, A computerized Medline search supplemented by an examination of cross-references and reviews was performed. Results:, Empirical evidence for the efficacy of combining antipsychotics is too limited to draw firm conclusions. The practice of augmenting clozapine with more ,tightly bound' D2 receptor antagonists as exemplified by risperidone augmentation of clozapine has some empirical and theoretical support. The risks of augmentation strategies have not been studied systematically. No study has examined the economic impact of combination treatment. Conclusion:, Further trials of antipsychotic combination therapies are needed before this currently unsupported practice can be recommended. Rationales for combination treatment include a broadening of the range of receptor activity or an increase in D2 receptor occupancy with certain atypical agents. Trial methodology needs to take into account subject characteristics, duration of treatment, optimization of monotherapy comparators, and appropriate outcome measures. [source]


Factors associated with constructive staff,family relationships in the care of older adults in the institutional setting

Abstract Background, Modern healthcare philosophy espouses the virtues of holistic care and acknowledges that family involvement is appropriate and something to be encouraged due to the role it plays in physical and emotional healing. In the aged care sector, the involvement of families is a strong guarantee of a resident's well-being. The important role family plays in the support and care of the older adult in the residential aged care environment has been enshrined in the Australian Commonwealth Charter of Residents' Rights and Responsibilities and the Aged Care Standards of Practice. Despite wide acknowledgement of the importance of family involvement in the healthcare of the older adult, many barriers to the implementation of participatory family care have been identified in past research. For older adults in the healthcare environment to benefit from the involvement of their family members, healthcare professionals need an understanding of the issues surrounding family presence in the healthcare environment and the strategies to best support it. Objectives, The objectives of the systematic review were to present the best available evidence on the strategies, practices and organisational characteristics that promote constructive staff,family relationships in the care of older adults in the healthcare setting. Specifically this review sought to investigate how staff and family members perceive their relationships with each other; staff characteristics that promote constructive relationships with the family; and interventions that support staff,family relationships. Search strategy, A literature search was performed using the following databases for the years 1990,2005: Ageline, APAIS Health, Australian Family and Society Abstracts (FAMILY), CINAHL, Cochrane Library, Dare, Dissertation Abstracts, Embase, MEDLINE, PsycINFO and Social Science Index. Personal communication from expert panel members was also used to identify studies for inclusion. A second search stage was conducted through review of reference lists of studies retrieved during the first search stage. The search was limited to published and unpublished material in English language. Selection criteria, The review was limited to studies involving residents and patients within acute, subacute, rehabilitation and residential settings, aged over 65 years, their family and healthcare staff. Papers addressing family members and healthcare staff perceptions of their relationships with each other were considered for this review. Studies in this review also included those relating to interventions to promote constructive staff,family relationships including organisational strategies, staff,family meetings, case conferencing, environmental approaches, etc. The review considered both quantitative and qualitative research and opinion papers for inclusion. Data collection and analysis, All retrieved papers were critically appraised for eligibility for inclusion and methodological quality independently by two reviewers, and the same reviewers collected details of eligible research. Appraisal forms and data extraction forms designed by the Joanna Briggs Institute as part of the QARI and NOTARI systematic review software packages were used for this review. Findings, Family members' perceptions of their relationships with staff showed that a strong focus was placed on opportunities for the family to be involved in the patient's care. Staff members also expressed a theoretical support for the collaborative process, however, this belief often did not translate to the staff members' clinical practice. In the studies included in the review staff were frequently found to rely on traditional medical models of care in their clinical practice and maintaining control over the environment, rather than fully collaborating with families. Four factors were found to be essential to interventions designed to support a collaborative partnership between family members and healthcare staff: communication, information, education and administrative support. Based on the evidence analysed in this systematic review, staff and family education on relationship development, power and control issues, communication skills and negotiating techniques is essential to promoting constructive staff,family relationships. Managerial support, such as addressing workloads and staffing issues; introducing care models focused on collaboration with families; and providing practical support for staff education, is essential to gaining sustained benefits from interventions designed to promote constructive family,staff relationships. A joint study based on the electron localization function and catastrophe theory of the chameleonic and centauric models for the Cope rearrangement of 1,5-hexadiene and its cyano derivatives

Abstract A novel interpretation of the chameleonic and centauric models for the Cope rearrangements of 1,5-hexadiene (A) and different cyano derivatives (B: 2,5-dicyano, C: 1,3,4,6-tetracyano, and D: 1,3,5-tricyano) is presented by using the topological analysis of the electron localization function (ELF) and Thom's catastrophe theory (CT) on the reaction paths calculated at the B3LYP/6-31G(d,p) level. The progress of the reaction is monitorized by the changes of the ELF structural stability domains (SSD), each being change controlled by a turning point derived from CT. The reaction mechanism of the parent reaction A is characterized by nine ELF SSDs. All processes occur in the vicinity of the transition structure and corresponding to a concerted formation/breaking of C1C6 and C3C4 bonds, respectively, together with an accumulation of charge density onto C2 and C5 atoms. Reaction B presents the same number of ELF SSDs as A, but a different order appears; the presence of 2,5-dicyano substituents favors the formation of C1C6 bonds over the breaking of C3C4 bond process, changing the reaction mechanism from a concerted towards a stepwise, via a cyclohexane biradical intermediate. On the other side, reaction C presents the same type of turning points but two ELF SSD less than A or B; there is an enhancement of the C3C4 bond breaking process at an earlier stage of the reaction by delocalizing the electrons from the C3C4 bond among the cyano groups. In the case of competitive effects of cyano subsituents on each moiety, as it is for reaction D, seven different ELF SSDs have been identified separated by eight turning points (two of them occur simultaneously). Both processes, formation/breaking of C1C6 and C3C4 bonds, are slightly favored with respect to the parent reaction (A), and the TS presents mixed electronic features of both B and C. Chromosomal rearrangements and genetic structure at different evolutionary levels of the Sorex araneus group

Abstract Robertsonian (Rb) fusions received large theoretical support for their role in speciation, but empirical evidence is often lacking. Here, we address the role of Rb rearrangements on the genetic differentiation of the karyotypically diversified group of shrews, Sorex araneus. We compared genetic structure between ,rearranged' and ,common' chromosomes in pairwise comparisons of five karyotypic taxa of the group. Considering all possible comparisons, we found a significantly greater differentiation at rearranged chromosomes, supporting the role of chromosomal rearrangements in the general genetic diversification of this group. Intertaxa structure and distance were larger across rearranged chromosomes for most of the comparisons, although these differences were not significant. This last result could be explained by the large variance observed among microsatellite-based estimates. The differences observed among the pairs of taxa analysed support the role of both the hybrid karyotypic complexity and the level of evolutionary divergence. [source]


Peripheral social awareness information in collaborative work

People working in physical proximity have access to information about one another. Much of this information is unavailable when people collaborate remotely using groupware. Being aware of other members of a team in a collaborative environment involves knowing both what people are doing and what is happening to the shared information space or artifact. Increasing the amount of information about the group in a computer-mediated system may increase the group's ability to complete the task. This article reports on a study that examined group performance on a task that was computer mediated with and without awareness information. In particular, the study examined how an awareness tool impacts the quality of the work effort and the communications between group members in the completion of a collaborative authoring task. The study found that the use of an awareness tool decreased the quality of the work effort. The number of communications also decreased when the tool was used. Although the results contradict some of the theoretical predictions, an examination of the data suggests theoretical support for a more complex interaction. There was evidence that the awareness tool may have reduced the users' need to communicate and this reduction in communications may have caused the reduction in the quality of the work effort. There is also data to suggest that the existence of the awareness tool may have negatively influenced the effort of some participants, and it was that effort reduction that caused the reduction in the quality of the product. Problem-based learning: why curricula are likely to show little effect on knowledge and clinical skills

Objectives A recent review of problem-based learning's effect on knowledge and clinical skills updated findings reported in 1993. The author argues that effect sizes (ES) seen with PBL have not lived up to expectations (0.8,1.0) and the theoretical basis for PBL, contextual learning theory, is weak. The purposes of this study were to analyse what constitutes reasonable ES in terms of the impacts on individuals and published reports, and to elaborate upon various theories pertaining to PBL. Design Normal theory is used to demonstrate what various ESs would mean for individual change and a large meta-analysis of over 10 000 studies is referred to in identifying typical ESs. Additional theories bearing upon PBL are presented. Results Effect sizes of 0.8,1.0 would require some students to move from the bottom quartile to the top half of the class or more. The average ES reported in the literature was 0.50 and many commonly used and accepted medical procedures and therapies are based upon studies with ESs below 0.50. Conclusions Effect sizes of 0.8,1.0 are an unreasonable expectation from PBL because, firstly, the degree of changes that would be required of individuals would be excessive, secondly, leading up to medical school, students are groomed and selected for success in a traditional curriculum, expecting them to do better in a PBL curriculum than a traditional curriculum is an unreasonable expectation, and, thirdly, the average study reported in the literature and many commonly used and accepted medical procedures and therapies are based upon studies having lesser ESs. Information-processing theory, Cooperative learning, Self-determination theory and Control theory are suggested as providing better theoretical support for PBL than Contextual learning theory. Even if knowledge acquisition and clinical skills are not improved by PBL, the enhanced work environment for students and faculty that has been consistently found with PBL is a worthwhile goal. The Slingshot Argument: An Improved Version

In the paper I exploit Frege's notions of sense and synonymity in order to amend the slingshot argument. The main emendation is to replace the assumption about logical equivalence by an assumption about synonymity. While the replaced assumption begs the question about the reference of sentences, the replacing assumption has much more theoretical support from Frege's general conception of sense and reference and the relation between them. In the paper I use a specific notion of synonymity which I believe is faithful to Frege's discussion of the subject. I notice that if a stronger (and to my mind implausible) notion of synonymity is used, my version of the argument fails. The failure is explained by showing that this stronger notion of synonymity enables the assignment of facts, and not truth values, as the references of sentences. [source]
