Amyotrophic lateral sclerosis and sports: a case,control study

An increased incidence of amyotrophic lateral sclerosis (ALS) amongst soccer players in Italy has recently been reported. A case,control study (300 cases and 300 matched controls) wass conducted to explore the association between ALS and physical/sports activities, with specific reference to trauma-related risk. Neither the practice of competitive sports nor sports-related traumas were found to be associated with an increased risk of ALS. The practice of physical activities or sports is not per se a risk factor for ALS. Our results exclude sports-related microtraumas as etiopathogenic factors in the natural history of ALS. [source]


A retrospective study of traumatic dental injuries in a Brazilian dental trauma clinic

Abstract , The purpose of this retrospective study was to analyse data from the records of patients seen in the dental trauma emergency clinic in a general hospital in the city of Recife, Brazil, during the years 1997,1999, according to sex, age, cause, number of injured teeth, type of tooth and type of trauma. The records of all patients seen by dentists were collected. Altogether, 250 patients from 1 to 59 years of age presenting 403 dental injuries were examined and/or treated. The causes of dento-alveolar trauma were classified in five categories: home injuries, street injuries, school injuries, sports activities, violence. The type of trauma was classified by dentists working at the dental trauma clinic on the basis of Andreasen's classification. The gender difference in the number of cases of trauma was statistically significant (males 63.2% vs females 36.8) (P<0.0001). Fracture in enamel only (51.6%) and fractures in dentine (40.8%) were the most commonly occurring types of injury. Injuries were most frequently diagnosed as serious among the youngest patients (up to 15 years of age); 82.4% of intrusive luxation cases were diagnosed in the 1,5 years age group. The main causes of tooth injury were falls (72.4%), collisions with objects (9.2%), violence (8.0%), traffic accidents (6.8%) and sports (3.6%). Trauma caused by violence was found to be statistically significant in the 6,15 years age group (P<0.0005). [source]


Infliximab improves quality of life in patients with Crohn's disease

Abstract Objective The aim of this study was to assess the effect of infliximab on quality of life in patients with active Crohn's disease (CD) inadequately responsive to concomitant therapies. Methods We examined responses to the Inflammatory Bowel Disease Questionnaire (IBDQ) from patients enrolled in a previously reported, randomized, placebo-controlled study. Patients with active CD received a single intravenous infusion of either placebo or infliximab 5, 10, or 20 mg/kg. Most patients received stable doses of mesalamine, corticosteroids, azathioprine, or 6-mercaptopurine throughout the study. Changes from baseline in overall IBDQ score and individual dimensions at 4 weeks postinfusion were compared. Results Patients treated with infliximab had a significantly larger improvement in overall IBDQ score than those treated with placebo at 4 weeks (p < 0.001). Infliximab-treated patients also had larger improvements in all IBDQ dimensions: bowel (p = 0.007), social (p = 0.002), emotional (p < 0.001), and systemic (p < 0.001). A significantly larger proportion of infliximab-treated patients reported having normal or near-normal frequency of bowel movements in the past week (p < 0.001), full or a lot of energy (p = 0.019), and no or hardly any difficulty doing leisure or sports activities (p = 0.011), and being extremely or very satisfied with their personal life (p = 0.046). They also significantly differed in responses regarding fatigue, frustration, ability to work, general well-being, depression, anxiety, and anger resulting from bowel problems. Conclusions These results indicate that infliximab significantly improved quality of life in patients with active CD, increasing their ability to work and participate in leisure activities, and decreasing feelings of fatigue, depression, and anger. Participation in extracurricular activities and emotional and behavioral adjustment in middle childhood in Spanish Boys and Girls,

This study examined the concurrent relationship between participation in extracurricular activities and externalizing and internalizing problems and social school behavior in a Spanish community sample, separately for boys (n=439) and girls (n=428), enrolled in 2nd, 4th, or 6th grades. Weekly participation in extracurricular activities was related to better emotional and behavioral adjustment and social competence. Overall, the relations were low and different according to gender, type of activity, and informant. Better adjustment was more related to participation in sports activities in boys and to participation in nonsports activities in girls. Size effect was higher for girls and decreased for both boys and girls when parental education and parenting practices were considered. Soldiers With Musculoskeletal Injuries

Purpose: To describe Soldiers' (e.g., U.S. Army personnel) perspectives of the effect of musculoskeletal injuries. Design: Data were collected in the summer of 2003 using a prospective survey design. The survey was mailed to active duty Soldiers on modified work plans because of musculoskeletal injuries. These Soldiers were assigned to one Army installation in the US. Methods: Responses to the survey questions were analyzed using descriptive statistics. The numerous handwritten comments were evaluated qualitatively. Findings: Injuries most often involved the back and knees (18% each). At least 47% of the injuries were work related. Injuries interfered with Soldiers' abilities to perform military tasks such as road marching (80%) and organized physical training (69%). Although many respondents indicated they were not experiencing pain, at least some Soldiers reported mild pain for each of 19 anatomic locations. Severe pain was reported most often for the lower back (21%). In their written comments, Soldiers expressed a sense of frustration with their injuries, the healthcare system and providers, and their unit leaders. Conclusions: Healthcare personnel are challenged to better manage Soldiers with musculoskeletal injuries and expedite their return to full duty. Unit leaders are challenged to create work environments that focus on injury prevention and allow injured Soldiers time to heal. Clinical Relevance: The Soldiers in this study were often engaged in physically challenging work or sports activities when injured. Because people outside the Army engage in similar activities (e.g., construction workers, endurance athletes), the findings from this study might be applicable to nonmilitary communities. Additionally, with the number of Reserve and National Guard Soldiers currently on active duty, civilian nurses might be caring for Soldiers with musculoskeletal injuries. [source]


Three-dimensional knee joint kinematics during golf swing and stationary cycling after total knee arthroplasty

Abstract The expectation of returning to sports activities after total knee arthroplasty (TKA) has become more important to patients than ever. To our knowledge, no studies have been published evaluating the three-dimensional knee joint kinematics during sports activity after TKA. Continuous X-ray images of the golf swing and stationary cycling were taken using a large flat panel detector for four and eight post-arthroplasty knees, respectively. The implant flexion and axial rotation angles were determined using a radiographic-based, image-matching technique. Both the golf swing from the set-up position to the top of the backswing, and the stationary cycling from the top position of the crank to the bottom position of the crank, produced progressive axial rotational motions (p,=,0.73). However, the golf swing from the top of the backswing to the end of the follow-through produced significantly larger magnitudes of rotational motions in comparison to stationary cycling (p,<,0.01). Excessive internal,external rotations generated from the top of the backswing to the end of the follow-through could contribute to accelerated polyethylene wear. However, gradual rotational movements were consistently demonstrated during the stationary cycling. Therefore, stationary cycling is recommended rather than playing golf for patients following a TKA who wish to remain physically active. © 2008 Orthopaedic Research Society. Sports Medicine and School Nurses: A Growing Need for Further Education and Appropriate Resources

The use of exercise as a prerequisite for conditioning and proper treatment of injuries was first documented in early Greek civilization with the establishment of the Olympics. Today, sports by their very nature invite injury. In 2000, 2.5 million students participated in varsity sports with 750,000 injuries recorded. These numbers do not account for sports activities outside school or leisure activities. Another area of potential injury is physical education class. These classes are large with limited supervision and encompass students of varying age and abilities. Nurses do not have an extensive knowledge of injury prevention or assessment in their basic nursing education. School nurses, as a subspecialty within nursing, are expected to keep up with the requirements of the adolescent and pediatric populations as well basic nursing skills. Due to work schedules and limited resources for continuing education, school nurses are not afforded much time or benefits to attend classes that would teach them skills needed to assess athletic-type injuries. School nurses need printed resources specific to their setting to help fill this void. Recognizing this need, Sports Medicine Techniques for the School-Based Nurse is a manual in process that will help fill this void. Being developed specifically for school nurses, the manual will provide information on prevention, evaluation, and management of athletic-type injuries commonly seen in the school nurse's office. Health-related fitness and trainability in children with cystic fibrosis

Abstract The objective of this study was to examine the effects of a supervised exercise training program performed during an in patient rehabilitation course on various attributes of health-related fitness, e.g., flexibility, balance and coordination. 286 patients with CF, age range 6,18 years (11.8,±,3.4 years), mean forced expiratory volume in 1 sec (FEV1 82.7,±,22.3% predicted) were included. Patients performed the modified Munich fitness test (mMFT) to assess flexibility, balance, strength and coordination. To assess aerobic capacity a 6-min walk-test (6MWT) was performed. In addition, some of the patients performed an incremental exercise test on a bicycle ergometer using the Godfrey protocol before and after the exercise training program. The supervised training program consisted of 4,6 weeks of different sports activities 5 times per week. After training, pulmonary function showed a significant (P,<,0.05) increase. All test items of the mMFT improved significantly (P,<,0.05). Compared to healthy children test scores achieved from children with CF in the mMFT were lower but within a normal range. Our children with CF had a lower walk distance in 6MWT (P,<,0.05) compared to healthy. Our findings clearly demonstrated benefits of a systematic exercise training program on components of physical fitness in patients with CF, with improvements of test-tasks to predicted normal in some cases. The results from our study suggested that an exercise training program in CF should be focused on several aspects of physical fitness including all components of physical fitness, e.g. aerobic endurance, flexibility, balance and motor skills. Obesity and associated modifiable environmental factors in Iranian adolescents: Isfahan Healthy Heart Program , Heart Health Promotion from Childhood

Abstract Objective:,To evaluate the prevalence of overweight and obesity among Iranian adolescents and their relationship with modifiable environmental factors. Methods:,The subjects of the present study were 1000 girls and 1000 boys, aged between 11 and 18 years selected by multistage random sampling, their parents (n = 2000) and their school staff (n = 500 subjects) in urban and rural areas of two provinces in Iran. Data concerning body mass index (BMI), nutrition and the physical activity of the subjects were analyzed by SPSSV10/Win software. Results:,The prevalence of 85th percentile , body mass index (BMI) < 95th percentile and BMI > 95th percentile in girls was significantly higher than boys (10.7 ± 1.1 and 2.9 ± 0.1%vs 7.4 ± 0.9 and 1.9 ± 0.1%, respectively; P < 0.05). The mean BMI value was significantly different between urban and rural areas (25.4 ± 5.2 vs 23.2 ± 7.1 kg/m2, respectively; P < 0.05). A BMI> 85th percentile was more prevalent in families with an average income than in high-income families (9.3 ± 1.7 vs 7.2 ± 1.4%, respectively; P < 0.05) and in those with lower-educated mothers (9.2 ± 2.1 vs 11.5 ± 2.4 years of mothers education, respectively). The mean total energy intake was not different between overweight or obese and normal-weight subjects (1825 ± 90 vs 1815 ± 85 kCal, respectively; P > 0.05), but the percentage of energy derived from carbo­hydrates was significantly higher in the former group compared with the latter (69.4 vs 63.2%, respectively; P < 0.05). Regular extracurricular sports activities were significantly lower and the time spent watching tele­vision was significantly higher in overweight or obese than non-obese subjects (time spent watching telelvision: 300 ± 20 vs 240 ± 30 min/day, P < 0.05). A significant linear association was shown between the frequency of consumption of rice, bread, pasta, fast foods and fat/salty snacks and BMI (, = 0.05,0.06; P < 0.05). A significant correlation was shown between BMI percentiles and serum triglyceride, high-density lipoprotein,cholesterol and systolic blood pressure (Pearson's r = 0.38, ,0.32 and 0.47, respectively). Conclusions:,Enhanced efforts to prevent and control overweight from childhood is a critical national priority, even in developing countries. To be successful, social, cultural and economic influences should be considered. [source]


Detecting malingering of Ganser-like symptoms with tests: A case study

Abstract, A middle-aged man presented with Ganser symptoms. He had been involved in a car crash and was seeking disability insurance benefits. Extensive testing with malinger instruments revealed that he performed below chance on simple memory tests and endorsed a variety of nonexistent symptoms. With this in mind, the authors collected collateral information which showed that the patient was involved in high level sports activities that were difficult to reconcile with the severe cognitive dysfunctions that he claimed to suffer from. The case demonstrates that Ganser-like symptoms deserve close scrutiny, preferably with malinger tests. [source]

Bayesian longitudinal plateau model of adult grip strength

Objectives: This article illustrates the use of applied Bayesian statistical methods in modeling the trajectory of adult grip strength and in evaluating potential risk factors that may influence that trajectory. Methods: The data consist of from 1 to 11 repeated grip strength measurements from each of 498 men and 533 women age 18,96 years in the Fels Longitudinal Study (Roche AF. 1992. Growth, maturation and body composition: the Fels longitudinal study 1929,1991. Cambridge: Cambridge University Press). In this analysis, the Bayesian framework was particularly useful for fitting a nonlinear mixed effects plateau model with two unknown change points and for the joint modeling of a time-varying covariate. Multiple imputation (MI) was used to handle missing values with posterior inferences appropriately adjusted to account for between-imputation variability. Results: On average, men and women attain peak grip strength at the same age (36 years), women begin to decline in grip strength sooner (age 50 years for women and 56 years for men), and men lose grip strength at a faster rate relative to their peak; there is an increasing secular trend in peak grip strength that is not attributable to concurrent secular trends in body size, and the grip strength trajectory varies with birth weight (men only), smoking (men only), alcohol consumption (men and women), and sports activity (women only). Conclusions: Longitudinal data analysis requires handling not only serial correlation but often also time-varying covariates, missing data, and unknown change points. Prevalence of nonspecific low back pain in schoolchildren aged between 13 and 15 years

Abstract Aim: To investigate the annual (2005) prevalence of nonspecific low back pain (LBP), defined as lumbago not attributed to recognizable, known specific pathology, in a population of adolescents. Methods: We investigated 7542 school teenagers with a structured questionnaire consisting of demographic and anthropometric items, psychosocial factors and life style, presence and intensity of LBP, family history of LBP, and questions about whether the teenager had been referred to a physician for this problem. Univariate (t -test, Mann,Whitney U-test and chi-square test) and multivariate analyses (logistic regression) were performed. Results: A total of 1180 (20.5%) teenagers reported one or more episodes of LBP, of whom 900 (76.3%) had consulted a health provider. A significantly greater percentage of LBP was found in the students who practised aerobics and swimming compared to the other sports. The search for risk factors in the whole population revealed no associations between LBP and the investigated anthropometrics or lifestyle items, whereas a significant association was found for gender (to be female), positive family history of LBP and absence of sports activity. Conclusions: Our study shows that nonspecific LBP is a frequent event in teenagers, particularly in females, sedentary children and those with a family history of LBP. [source]


Parental safety concerns , a barrier to sport and physical activity in children?

Objective: To examine the extent to which parents and carers perceive injury and safety risks as serious enough to prevent or discourage their children, aged 5,12 years, from participating in sports/physical activity and to identify factors that influence these perceptions. Methods: An analysis of the 2001 New South Wales Child Health Survey. Results: More than one-quarter of parents/carers of active children aged 5,12 years reported discouraging or preventing children from playing a particular sport (34.7% for boys and 16.6% for girls) because of injury and safety concerns. In boys, the most frequently discouraged sport was rugby league (23.2%), followed by rugby union (7.5%) and Australian rules football (2.8%). Among girls, the most frequently discouraged activities were rollerblading (2.7%), rugby league (2.3%) and soccer (2.1%). Multivariate analysis shows that factors independently associated with parents' decision to prevent/discourage their child from engaging in sport/physical activity include their child's age and gender, language spoken at home, presence of disability, and the respondent's relation to the child. Conclusions and implications:Efforts need to be made to modify some sports/ activities, such as football codes, in order to minimise injury and to ensure that children continue enjoying their favourite activity well into adulthood. Guidelines designed to promote physical activity among children and young adolescents need to take into account parental concerns regarding the associated risk of injury. [source]
