The effect of roasting on the nutritional and antioxidant properties of yellow and white maize varieties

Summary Maize varieties (yellow and white) were roasted for 17 min; and allowed to cool, and later milled into powder. The nutritional evaluation (proximate composition, mineral and antinutrient content determination) and antioxidant properties investigation (reducing power, free radicals scavenging ability and Fe2+ chelating ability) of the product was subsequently carried out. The result of the study revealed that roasting caused a significant increase (P < 0.05) in the crude fat, carbohydrate, Ca, Na, Mg and Zn content. Conversely, a significant decrease (P < 0.05) was observed in crude protein, crude fibre, Fe and K content. A significant decrease in the phytate content was also observed. However, the reduced phytate content did not have sparing effect on Zn bioavailability. Roasting significantly (P < 0.05) reduced the extractible phenol and flavonoid content of the maize varieties. The antioxidant properties (1,1-diphenyl-2-picrylhydracyl free radical scavenging ability and Fe2+ chelating ability) followed the phenolic content pattern. However, roasting caused a significant increase in the ferric reducing antioxidant power of the maize varieties. Thus, roasting reduced the protein content of maize but also increased the energy value and antioxidant capacity as exemplified by high reducing power. [source]


Intrathecal sufentanil decreases the median effective dose (ED50) of intrathecal hyperbaric ropivacaine for caesarean delivery

Background: The addition of opioid to local anaesthetics has become a well-accepted practice of spinal anaesthesia for caesarean delivery. Successful caesarean delivery anaesthesia has been reported with the use of a low dose of intrathecal hyperbaric ropivacaine coadministered with sufentanil. This prospective, double-blinded study determined the median effective dose (ED50) of intrathecal hyperbaric ropivacaine with and without sufentanil for caesarean delivery, to quantify the sparing effect of sufentanil on the ED50 of intrathecal hyperbaric ropivacaine. Methods: Sixty-four parturients undergoing elective caesarean delivery with combined spinal,epidural anaesthesia were randomized into two groups: Group R (ropivacaine) and Group RS (ropivacaine plus sufentanil 5 ,g). The initial dose of ropivacaine was 13 mg in Group R and 10 mg in Group RS. The effective dose was defined as a T6 level attained within 10 min and no supplemental epidural anaesthetic required during surgery. Effective or ineffective responses determined, respectively, a 0.3 mg decrease or increase of the dose of ropivacaine for the next patient using an up,down sequential allocation. Results: The ED50 of intrathecal ropivacaine was 11.2 mg [confidence interval (CI) 95%: 11.0,11.6] in Group R vs. 8.1 mg (CI 95%: 7.8,8.3) in Group RS. Motor block was markedly more intense in Group R than in Group RS, and the incidence of shivering was lower in Group RS than in Group R. There were no differences in the onset time of sensory block or motor block, in the incidence of hypotension, nausea and vomiting. Conclusion: Intrathecal sufentanil 5 ,g produced a 28% reduction of ED50 of intrathecal hyperbaric ropivacaine for caesarean delivery. [source]


Individual response of growing pigs to sulphur amino acid intake

Summary Two N balance experiments were conducted to study the individual response of growing pigs to limiting amino acid (AA) intake. Series of fifteen diets with increasing concentration of sulphur amino acids (SAA, Expt 1) or methionine in the presence of excess cystine (Expt 2) were fed sequentially to nine pigs during a 15-day experimental period. The concentration of the AA under test ranged from 50% to 140% of the requirement while other essential AA were given in a 25% excess relative to the limiting AA. N retention was related to the limiting AA intake using rectilinear and curvilinear models. In Expt 1, the quadratic-plateau model fitted the individual data significantly better (p = 0.01) than the linear-plateau model. No difference was found between the two models in Expt. 2, presumably due to the sparing effect of excess cystine on methionine utilization. Exponential, saturation kinetics or four-parameter logistic models fitted to data for all pigs showed that their goodness of fit was similar to those of quadratic-plateau or linear-plateau models. Significant differences (p < 0.05) were found between individual plateau values for N retention within each experiment while the slopes of the regression lines did not significantly differ either in Expt 1 (p = 0.07) or Expt 2 (p = 0.45). There was a positive correlation between the slope and plateau values of the linear-plateau model in Expt 1 (r = 0.74, p = 0.02) but no significant correlation was found in Expt 2 (r = ,0.48, p = 0.13). Marginal efficiencies of SAA and methionine utilization derived from the linear-plateau model were 0.43 and 0.65 respectively. Based on linear-plateau and quadratic-plateau models, daily requirements of SAA and methionine for a 50 kg pig were estimated to be 13.0 and 5.9 g and 14.3 and 6.1 g respectively. [source]


Targeting epileptogenesis-associated induction of neurogenesis by enzymatic depolysialylation of NCAM counteracts spatial learning dysfunction but fails to impact epilepsy development

Abstract Polysialylation is a post-translational modification of the neural cell adhesion molecule (NCAM), which in the adult brain promotes structural changes in regions of neurogenesis and neuroplasticity. Because a variety of plastic changes including neurogenesis have been suggested to be functionally involved in the pathophysiology of epilepsies, it is of specific interest to define the impact of the polysialic acid (PSA)-NCAM system on development of this disease and associated comorbidities. Therefore, we studied the impact of transient enzymatic depolysialylation of NCAM on the pathophysiology in an electrically induced rat post-status epilepticus (SE) model. Loss of PSA counteracted the SE-induced increase in neurogenesis in a significant manner. This effect of endoneuraminidase (endoN) treatment on hippocampal neurogenesis did not impact the subsequent development of spontaneous seizures. In contrast, transient lack of PSA during SE and in the early phase of epileptogenesis exhibited a cognition sparing effect as revealed in the Morris water maze paradigm. In conclusion, our data do not support a central role of neurogenesis in the development of a hyperexcitable epileptic network. However, in view of the cognition-sparing effect, the transient modulation of the PSA-NCAM system seems to allow beneficial long-term disease modification, which might be mediated by the partial normalization of neurogenesis. Dietary Lipid Utilization by Juvenile Summer Flounder Paralichthys dentatus

The ability of juvenile summer flounder Paralichthys dentatus to utilize dietary lipid as energy, and the effect of dietary lipid on weight gain and body composition was investigated in a 12-week feeding trial. Diets were formulated to provide 55% crude protein from herring meal and casein. Menhaden oil was added to produce diets with 8, 12, 16 or 20% total lipid while providing 16.0 kJ available energy/g dry diet. The diet containing 20% total lipid supplied 16.7 kJ available energy/g dry diet due to the high levels of protein and lipid. An additional diet was included to reproduce currently available commercial diet formulations for flounder, providing 55% crude protein supplied solely from herring meal and 16% total dietary lipid. Juvenile summer flounder (initial weight 23 g) were stocked into triplicate aquaria in a closed, recirculating system maintained at 20 C. Fish were fed 2% of body weight each day divided into two equal feedings. Upon termination of the study, effects of dietary lipid on weight gain, body condition indices, and proximate composition were determined. Weight gain (96,149% of initial weight), feed efficiency ratio values (0.43,0.48). fillet yield, and whole-body composition all were unaffected by dietary lipid level. High levels of dietary lipid did increase the lipid content in the finray muscle, as fish fed diets containing 16 and 20% dietary lipid had significantly higher lipid levels than fish fed the diet containing 8% lipid. No apparent protein sparing effect of lipid was observed. These data indicate that currently available commercial feeds for summer flounder may be over-formulated and show a need for further research to determine specific and accurate nutritional information for this species. Ketorolac in the Era of Cyclo-Oxygenase-2 Selective Nonsteroidal Anti-Inflammatory Drugs: A Systematic Review of Efficacy, Side Effects, and Regulatory Issues

Objective., The recent introduction of oral COX-2 selective NSAIDs with potential for perioperative use, and the ongoing development of intravenous formulations, stimulated a systemic review of efficacy, side effects, and regulatory issues related to ketorolac for management of postoperative analgesia. Design.,To examine the opioid dose sparing effect of ketorolac, we compiled published, randomized controlled trials of ketorolac versus placebo, with opioids given for breakthrough pain, published in English-language journals from 1986,2001. Odds ratios were computed to assess whether the use of ketorolac reduced the incidence of opioid side effects or improved the quality of analgesia. Results., Depending on the type of surgery, ketorolac reduced opioid dose by a mean of 36% (range 0% to 73%). Seventy percent of patients in control groups experienced moderate-severe pain 1 hour postoperatively, while 36% of the control patients had moderate to severe pain 24 hours postoperatively. Analgesia was improved in patients receiving ketorolac in combination with opioids. However, we did not find a concomitant reduction in opioid side effects (e.g., nausea, vomiting). This may be due to studies having inadequate (to small) sample sizes to detect differences in the incidence of opioid related side effects. The risk for adverse events with ketorolac increases with high doses, with prolonged therapy (>5 days), or invulnerable patients (e.g. the elderly). The incidence of serious adverse events has declined since dosage guidelines were revised. Conclusions., Ketorolac should be administered at the lowest dose necessary. Analgesics that provide effective analgesia with minimal adverse effects are needed. [source]


Oblique sub-costal transversus abdominis plane (TAP) catheters: an alternative to epidural analgesia after upper abdominal surgery

Summary The authors present three cases where catheters placed in the oblique sub-costal transversus abdominis plane provided prolonged analgesia after upper abdominal surgery. Patient 1 was admitted with severe sepsis following major hepatobiliary surgery. Bilateral catheters facilitated weaning from mechanical ventilation and provided adequate analgesia for 4 days. Patient 2 underwent emergency laparotomy for intestinal obstruction having refused consent for epidural analgesia. The transversus abdominis plane catheters provided a significant opioid sparing effect. A unilateral catheter offered rescue analgesia in patient 3 when the epidural catheter was displaced. We put forward a case for oblique sub-costal transversus abdominis plane catheters as an alternative to epidural analgesia after upper abdominal surgery. [source]


Clinical and pathological findings associated with congenital hypovitaminosis A in extensively grazed beef cattle

Objective To determine the cause of exceptionally high mortality (41.4%) in perinatal calves on a beef cattle property 50 km south-west of Julia Creek in north-western Queensland. Design Investigations were based on clinical assessment of affected calves and laboratory analysis of pre- and postmortem specimens taken from 12 calves aged from 6 to 36 h of age. Methods Associations between gross and histopathological findings and biochemical analyses conducted on serum and tissue samples were examined in relation to clinical observations. Results Clinical signs varied, but commonly included mild to severe ataxia, difficulty finding a teat and sucking, blindness (partial or complete, as judged by avoidance of obstacles) and depression with prominent drooping of the head. Gross and histopathological findings included herniation of the cerebellar vermis through the foramen magnum, squamous metaplasia of interlobular ducts in the parotid salivary glands and Wallerian degeneration of the optic nerves. Biochemical analysis of serum and liver samples available from four of the calves revealed low or undetectable levels of both vitamin A and vitamin E. Conclusion Although vitamin E is known to have a sparing effect on vitamin A, the role (if any) played by deficiency of this vitamin was uncertain. The combination of clinical signs, postmortem findings, histopathological features and biochemical findings indicate that gestational vitamin A deficiency was highly likely to have been an important contributor to perinatal calf mortalities in this herd. Randomised controlled trial of LigaSure versus conventional suture ligature for abdominal hysterectomy

Objective To compare the use of LigaSure with conventional suture ligature in abdominal hysterectomy. Design Pilot randomised controlled unblinded trial with block randomisation according to three operating surgeons. Setting One Norwegian teaching hospital, Department of Gynaecology and Obstetrics. Sample Thirty women who underwent total or subtotal abdominal hysterectomy. Methods Data, with regard to operation time expenditure and the occurrence of peri- and post-operative complications, were collected and compared between the two techniques. Main outcome measures Operation time, peri- and post-operative complications. Results Mean operation duration was 61.7 minutes with LigaSure and 54.5 minutes with conventional suture ligature. The corresponding operative blood loss was 303 and 298 mL, respectively. Occurrence of complications was few and not significantly different between the two techniques. Mean hospital stay was longer following LigaSure operations (10 vs 6 days), probably due to a certain age imbalance between the study groups. Conclusions Within the limitation of this pilot study, we did not uncover a time sparing effect from the use of LigaSure or any difference in the occurrence of blood loss and complications. Acute increase in femoral artery resistance in response to direct physical stimuli in the human fetus

Objective To determine whether fetal response to needling resembles the fetal ,brain sparing' response seen with hypoxaemia. Design Prospective observational study. Setting Centre for Fetal Care, Queen Charlotte's and Chelsea Hospital, London. Population Eighty-five pregnant women undergoing invasive procedures associated with fetal prenatal diagnosis and/or management. Methods The femoral artery and the middle cerebral artery pulsatility index were measured by Doppler ultrasonography before and after 89 invasive procedures (fetal blood sampling, transfusion, bladder or cyst aspiration, shunt insertion and amniocentesis, between 17 and 36 weeks). Cases in which the fetal body was transgressed were compared with ,control' fetuses undergoing invasive procedures which did not directly involve needling the fetus (amniocentesis and placental cord insertion procedures). Main outcome measures Femoral artery and middle cerebral artery pulsatility index. Results The femoral artery pulsatility index rose after transgression [median change (,) 0.73; 95% confidence interval (CI) 0.51 to 0.98]. In contrast, there was no significant change in femoral artery pulsatility index after non-transgression procedures (mean , 0.28; 95% CI ,0.20 to 0.76). Analysis confirmed the fall in middle cerebral artery pulsatility index after transgression procedures (median ,,0.19; 95% CI ,0.07 to ,0.35), but there was also a significant fall in middle cerebral artery pulsatility index after non-transgression procedures (mean ,,0.47; 95% CI ,0.23 to ,0.71). Conclusions The human fetus mounts a peripheral haemodynamic response to invasive procedures involving transgression of the fetal body, which is consistent with the brain sparing effect. However, the change in middle cerebral artery pulsatility index in both transgression and control procedures suggests that the changes and mechanisms may be more complex than previously thought. Antagonism of long-acting ,2 -adrenoceptor agonism

The established place of regular long-acting ,2 -adrenoceptor agonists at step 3 in asthma management guidelines has evolved as a consequence of evidence showing additive effects of salmeterol and formoterol on exacerbation rates, resulting in a putative inhaled corticosteroid sparing effect. There is however, evidence to show that although long-acting ,2 -adrenoceptor agonists facilitate using a lower dose of inhaled corticosteroid, this may occur at the expense of suboptimal anti-inflammatory control. This is likely to be the case especially with fixed dose combination inhalers where it is not possible to properly titrate anti-inflammatory therapy with inhaled corticosteroids without also inadvertently overtreating with unnecessarily high doses of long-acting ,2 -adrenoceptor agonists. Most patients with mild to moderate persistent asthma can be adequately controlled on monotherapy with inhaled corticosteroid in low or medium dosage, which is considerably cheaper than concomitant ,,use ,,of ,,a ,,long-acting ,,,2 -adrenoceptor ,,agonist. ,,Subsensitivity ,,to ,,long-acting ,2 -adrenoceptor ,agonists ,is ,a ,predictable ,pharmacological ,phenomenon, which occurs despite concomitant inhaled corticosteroid therapy and occurs more readily for bronchoprotective ,than ,bronchodilator ,effects. ,Subsensitivity ,of ,salbutamol, protection against bronchoconstrictor stimuli occurs in patients receiving concomitant long-acting ,2 -adrenoceptor agonists, which may be due to ,2 -adrenoceptor down-regulation or prolonged receptor occupancy. Prospective large scale long-term studies are required to further define the clinical relevance of ,2 -adrenoceptor polymorphisms, to look at clinical control outcomes as well as propensity for subsensitivity. It would therefore make more sense to first of all optimize the dose of anti-inflammatory therapy with inhaled corticosteroid and to then consider adding a long-acting ,2 -adrenoceptor agonist for patients who are poorly controlled.
