EXPLORING DISPARITIES BETWEEN GLOBAL HIV/AIDS FUNDING AND RECENT TSUNAMI RELIEF EFFORTS: AN ETHICAL ANALYSIS

ABSTRACT Objective:, To contrast relief efforts for the 26 December 2004 tsunami with current global HIV/AIDS relief efforts and analyse possible reasons for the disparity. Methods:, Literature review and ethical analysis. Results:, Just over 273,000 people died in the tsunami, resulting in relief efforts of more than US$10 bn, which is sufficient to achieve the United Nation's long-term recovery plan for South East Asia. In contrast, 14 times more people died from HIV/AIDS in 2004, with UNAIDS predicting a US$8 bn funding gap for HIV/AIDS in developing nations between now and 2007. This disparity raises two important ethical questions. First, what is it that motivates a more empathic response to the victims of the tsunami than to those affected by HIV/AIDS? Second, is there a morally relevant difference between the two tragedies that justifies the difference in the international response? The principle of justice requires that two cases similarly situated be treated similarly. For the difference in the international response to the tsunami and HIV/AIDS to be justified, the tragedies have to be shown to be dissimilar in some relevant respect. Are the tragedies of the tsunami disaster and the HIV/AIDS pandemic sufficiently different, in relevant respects, to justify the difference in scope of the response by the international community? Conclusion:, We detected no morally relevant distinction between the tsunami and the HIV/AIDS pandemic that justifies the disparity. Therefore, we must conclude that the international response to HIV/AIDS violates the fundamental principles of justice and fairness. [source]


Glechoma hederacea (Lamiaceae) in North America: invasion history and current distribution,

Glechoma hederacea L. (Ground-ivy, Lamiaceae), a perennial mat-forming herb, is native to the temperate regions of Eurasia and was introduced elsewhere (South East Asia, New Zealand and North America). Based on data obtained from herbaria, literature, online and other data bases and field studies, we documented the invasion history and current distribution of this plant in North America. At present, the plant is recorded from all but two continental states of the USA and all southern provinces of Canada. There are two main ranges: the larger one covers mainly the eastern part of the U.S.A. and a smaller one stretches along the West Coast. While published records of Glechoma hederacea date from 1814, the oldest specimen is from 1829. During the 19th century the species spread westwards at a rate of approximately 30 km/year. The spread and present range of G. hederacea can only be explained by climatic factors (degree of oceanicity) and considering human activity. Especially long distance propagation of vegetative parts of the plants and the change of the environment that accompanies human settlements may have had a major influence on these processes. Basierend auf Daten aus der Literatur und Datenbanken, Belegdaten aus Herbarien und Felduntersuchungen werden die Ausbreitungsgeschichte der Art in Nordamerika und ihr gegenwärtiges Areal dokumentiert. Gegenwärtig ist die Art aus allen kanadischen Provinzen und mit Ausnahme von zwei Bundesstaaten auch von allen kontinentalen Bundesstaaten der USA dokumentiert. Es gibt zwei Hauptareale: ein großes, welches einen Großteil der östlichen USA einnimmt und ein kleineres an der Westküste. Der älteste Nachweis von 1814 von Glechoma hederacea stammt aus der Literatur, der älteste Beleg von 1829. Im Laufe des 19. Jahrhunderts breitete sich die Art mit einer Geschwindigkeit von etwa 30 km/Jahr nach Westen aus. Ausbreitungsgeschwindigkeit und das gegenwärtige Areal können nur mit Hilfe klimatischer (Ozeanität) und anthropogener Faktoren erklärt werden. Der Mensch trägt vor allem zur Verbreitung vegetativer Pflanzenteile bei und schafft in Siedlungen günstige Wachstumsbedingungen. [source]


Monoterpene emissions from rubber trees (Hevea brasiliensis) in a changing landscape and climate: chemical speciation and environmental control

Abstract Emissions of biogenic volatile organic compounds (VOCs) have important roles in ecophysiology and atmospheric chemistry at a wide range of spatial and temporal scales. Tropical regions are a major global source of VOC emissions and magnitude and chemical speciation of VOC emissions are highly plant-species specific. Therefore it is important to study emissions from dominant species in tropical regions undergoing large-scale land-use change, for example, rubber plantations in South East Asia. Rubber trees (Hevea brasiliensis) are strong emitters of light-dependent monoterpenes. Measurements of emissions from leaves were made in the dry season in February 2003 and at the beginning of the wet season in May 2005. Major emitted compounds were sabinene, , -pinene and , -pinene, but , -ocimene and linalool also contributed significantly at low temperature and light. Cis -ocimene was emitted with a circadian course independent of photosynthetic active radiation (PAR) and temperature changes with a maximum in the middle of the day. Total isoprenoid VOC emission potential at the beginning of the wet season (94 ,g gdw,1 h,1) was almost two orders of magnitude higher than measured in the dry season (2 ,g g dw,1 h,1). Composition of total emissions changed with increasing temperature or PAR ramps imposed throughout a day. As well as light and temperature, there was evidence that assimilation rate was also a factor contributing to seasonal regulating emission potential of monoterpenes from rubber trees. Results presented here contribute to a better understanding of an important source of biogenic VOC associated with land-use change in tropical South East Asia. [source]


Solving nursing shortages: a common priority

Aims and objectives., This paper provides a context for this special edition. It highlights the scale of the challenge of nursing shortages, but also makes the point that there is a policy agenda that provides workable solutions. Results., An overview of nurse:population ratios in different countries and regions of the world, highlighting considerable variations, with Africa and South East Asia having the lowest average ratios. The paper argues that the ,shortage' of nurses is not necessarily a shortage of individuals with nursing qualifications, it is a shortage of nurses willing to work in the present conditions. The causes of shortages are multi-faceted, and there is no single global measure of their extent and nature, there is growing evidence of the impact of relatively low staffing levels on health care delivery and outcomes. The main causes of nursing shortages are highlighted: inadequate workforce planning and allocation mechanisms, resource constrained undersupply of new staff, poor recruitment, retention and ,return' policies, and ineffective use of available nursing resources through inappropriate skill mix and utilisation, poor incentive structures and inadequate career support. Conclusions., What now faces policy makers in Japan, Europe and other developed countries is a policy agenda with a core of common themes. First, themes related to addressing supply side issues: getting, keeping and keeping in touch with relatively scarce nurses. Second, themes related to dealing with demand side challenges. The paper concludes that the main challenge for policy makers is to develop a co-ordinated package of policies that provide a long term and sustainable solution. Relevance to clinical practice., This paper highlights the impact that nursing shortages has on clinical practice and in health service delivery. It outlines scope for addressing shortage problems and therefore for providing a more positive staffing environment in which clinical practice can be delivered. [source]


Epidemiology of hepatitis B virus infection in the Asia,Pacific region

There is a wide variation of hepatitis B virus (HBV) infection in the Asia,Pacific region. The prevalence of chronic HBV infection is lowest (< 1%) in North America, Australia and New Zealand, 2,4% in Japan, 5,18% in China and highest (15,20%) in Taiwan as well as several other countries in South East Asia. Perinatal transmission is common in HBV-hyperendemic areas. Geographical clusters of horizontal HBV infection have been reported in both high- and low-risk countries. Common sources of infection, including iatrogenic and sexual transmission, have been implicated. Migrant studies indicate the importance of childhood environments in the determination of HBV infection. Rural,urban and ethnic differences in the prevalence of HBV infection have also been reported. There has been a decrease in the prevalence of HBV infection after mass HBV vaccination programmes in some Asia, Pacific countries, which may be due to the intervention of possible transmission routes through the use of disposable syringes and needles, screening of HBV infection markers in blood banks, and prevention of high-risk tattooing, acupuncture, ear-piercing and sexual contact. A striking decrease in the incidence of HBV infection and hepatocellular carcinoma has been observed among children in Taiwan and other areas where mass vaccination programmes have been implemented. Withania somnifera (Ashwagandha): a Novel Source of L-asparaginase

Abstract Different parts of plant species belonging to Solanaceae and Fabaceae families were screened for L-asparaginase enzyme (E.C.3.5.1.1.). Among 34 plant species screened for L-asparaginase enzyme, Withania somnifera L. was identified as a potential source of the enzyme on the basis of high specific activity of the enzyme. The enzyme was purified and characterized from W. somnifera, a popular medicinal plant in South East Asia and Southern Europe. Purification was carried out by a combination of protein precipitation with ammonium sulfate as well as Sephadex-gel filtration. The purified enzyme is a homodimer, with a molecular mass of 72 ± 0.5 kDa as estimated by sodium dodecyl sulfate-polyacrylamide gel electrophoresisand size exclusion chromatography. The enzyme has a pH optimum of 8.5 and an optimum temperature of 37 °C. The Km value for the enzyme is 6.1 × 10,2 mmol/L. This is the first report for L-asparaginase from W. somnifera, a traditionally used Indian medicinal plant. [source]


Comparison of the technical and clinical performance of the Elecsys® HBsAg II assay with the Architect®, AxSym®, and Advia® Centaur HBsAg screening assays

Abstract South East Asia has some of the highest prevalence rates of hepatitis B virus (HBV) infection (,8%) in the world, and the emergence of hepatitis B surface antigen (HBsAg) mutant strains is a growing problem. Assays with the highest levels of sensitivity, including mutant detection, should be used for routine HBsAg screening. In this large multicenter study, the clinical and technical performance of the fully automated Elecsys HBsAg II assay was compared with the Architect, AxSYM, and Advia Centaur HBsAg assays for HBsAg screening. Nine laboratories (three each from Thailand, Korea, and Singapore) compared the Elecsys HBsAg II assay with their routine HBsAg screening assay against a range of stored and routine clinical samples, including recombinant mutants. The Elecsys HBsAg II assay demonstrated equivalent sensitivity and specificity to the Architect HBsAg assay. However, the Elecsys HBsAg II assay recognized a native mutant sample (L94S, L97V, L98V, T123A) that the Architect HBsAg assay failed to detect. The AxSYM and Advia Centaur HBsAg assays appeared less sensitive for the detection of early HBV infection and also failed to detect some of the recombinant mutant strains. There was almost complete agreement between the Elecsys HBsAg II assay and comparator assays with respect to routine serum samples. The results of this study demonstrate that the Elecsys HBsAg II assay is a highly sensitive and specific screening assay for HBsAg and detects reliably the most important and clinically relevant HBV mutants and genotypes. Moniliophthora perniciosa, the causal agent of witches' broom disease of cacao: what's new from this old foe?

SUMMARY Moniliophthora perniciosa (=Crinipellis perniciosa) causes one of the three main fungal diseases of Theobroma cacao (cacao), the source of chocolate. This pathogen causes Witches' broom disease (WBD) and has brought about severe economic losses in all of the cacao-growing regions to which it has spread with yield reductions that range from 50 to 90%. Cacao production in South America reflects the severity of this pathogen, as the yields in most of the infected regions have not returned to pre-outbreak levels, even with the introduction of resistant varieties. In this review we give a brief historical account and summarize the current state of knowledge focusing on developments in the areas of systematics, fungal physiology, biochemistry, genomics and gene expression in an attempt to highlight this disease. Moniliophthora perniciosa is a hemibiotrophic fungus with two distinct growth phases. The ability to culture a biotrophic-like phase in vitro along with new findings derived from the nearly complete genome and expression studies clearly show that these different fungal growth phases function under distinct metabolic parameters. These new findings have greatly improved our understanding of this fungal/host interaction and we may be at the crossroads of understanding how hemibiotrophic fungal plant pathogens cause disease in other crops. Historical summary of WBD:, The first WDB symptoms appear to have been described in the diaries of Alexandre Rodrigues Ferreira (described as lagartão; meaning big lizard) from his observations of cacao trees in 1785 and 1787 in Amazonia, which is consistent with the generally accepted idea that M. perniciosa, like its main host T. cacao, evolved in this region. The disease subsequently arrived in Surinam in 1895. WBD moved rapidly, spreading to Guyana in 1906, Ecuador in 1918, Trinidad in 1928, Colombia in 1929 and Grenada in 1948. In each case, cacao production was catastrophically affected with yield reductions of 50,90%. After the arrival of M. perniciosa in Bahia in 1989, Brazil went from being the world's 3rd largest producer of cacao (347 000 tonnes in 1988,1990; c. 15% of the total world production at that time) to a net importer (141 000 tonnes in 1998,2000). Fortunately for chocolate lovers, other regions of the world such as West Africa and South East Asia have not yet been affected by this disease and have expanded production to meet growing world demand (predicted to reach 3 700 000 tonnes by 2010). THE ECONOMIC INSTITUTION OF INTERNATIONAL BARTER

Starting with the international debt crisis in the 1980s, international barter increased substantially. More recently, barter has emerged in Russia and South East Asia. This paper examines how barter can help highly indebted countries to finance imports if they cannot use standard credit arrangements. We argue that payment in goods is easier to enforce than payment in money. However, debtors may pay with inferior quality products. We rank goods with respect to these incentive properties and derive the economic institution of commodity money which explains the trade pattern in barter. Our theoretical predictions are consistent with data on barter contracts. [source]


Sexually transmitted infections: impact on male fertility

Summary The impact of sexually transmitted diseases (STD) on male fertility is strongly dependent on the local prevalence of the STDs. In Western countries STD-infections are of minor relevance. In other regions, i.e. Africa or South East Asia, the situation appears to be different. Acute urethritis could not be associated with male infertility. Chronic infections (gonorrhoea) can cause urethral strictures and epididymo-orchitis. Chlamydia trachomatis and Neisseria gonorrhoea can be transmitted to the female partner and cause pelvic inflammatory disease with tubal obstruction. Ureaplasma urealyticum may impair spermatozoa (motility, DNA condensation). Trichomonas vaginalis has, if any, only minor influence on male fertility. The relevance of viral infections (HPV, HSV) for male infertility is not resolved. Any STD increases the chances of transmission of the human immunodeficiency virus (HIV). The HIV infection is associated with infectious semen and the risk of virus transmission. Semen quality deteriorates with the progression of immunodeficiency. Special counselling of serodiscordant couples is needed. STDs should be treated early and adequately to prevent late sequelae for both men and women. [source]


Origin of Hungarian indigenous chicken breeds inferred from mitochondrial DNA D-loop sequences

Summary In this study, we assessed the maternal origin of six Hungarian indigenous chicken breeds using mitochondrial DNA information. Sequences of Hungarian chickens were compared with the D-loop chicken sequences annotated in the GenBank and to nine previously described reference haplotypes representing the main haplogroups of chicken. The first 530 bases of the D-loop region were sequenced in 74 chickens of nine populations. Eleven haplotypes (HIC1 - HIC11) were observed from 17 variable sites. Three sequences (HIC3, HIC8 and HIC9) of our chickens were found as unique to Hungary when searched against the NCBI GenBank database. Hungarian domestic chicken mtDNA sequences could be assigned into three clades and probably two maternal lineages. Results indicated that 86% of the Hungarian haplotypes are related to the reference sequence that likely originated from the Indian subcontinent, while the minor part of our sequences presumably derive from South East Asia, China and Japan. Cattle deaths during sea transport from Australia

Objective To establish the death rate and the causes of death in cattle exported by sea from Australia. Procedure Cattle deaths on voyages from Australia to all destinations between 1995 and 2000 were analysed retrospectively. On four voyages to the Middle East between December 1998 and April 2001, cattle that died were examined to determine the cause of death. Results The death rate was 0.24% among 4 million cattle exported, and a greater proportion of deaths occurred on voyages to the Middle East (0.52%, P < 0.05) than to south east Asia (0.13%). The risk of death on voyages to the Middle East was three times greater for cattle exported from southern ports in Australia compared to northern ports. The main causes of death were heat stroke, trauma and respiratory disease. Conclusion Cattle have a low risk of death during sea transport from Australia. The risk of death can be reduced on voyages to the Middle East by preferentially exporting cattle from northern ports, and selecting those with a higher Bos indicus content whenever possible. [source]
