Mechanisms of intercellular hypertonicity and isotonic fluid absorption in proximal tubules of mammalian kidneys

ABSTRACT The main purpose of this theoretical analysis (second of two articles) is to examine whether transjunctional diffusion of NaCl causes intercellular hypertonicity, which permits transcellular water transport across solute-impermeable lateral cell membranes until osmotic equilibration. In the S2 segment with tubular NaCl concentration 140 mM, the calculated apical intercellular NaCl concentration is c0 , 132 mM, which exceeds peritubular NaCl concentration by 12 mM or 22 mOsm kg,1. Variations in volume flow, junctional reflection coefficient (,NaCl=0.25,0.50), gap distance (g=6,8 Å), junctional depth (d=18,100 Å), intercellular diffusion coefficient (DLIS=500,1500 ,m2 s,1) and hypothetical active NaCl transport alter c0 only by a fraction of 1 mM. However, dilution and back-leakage of NaHCO3 lower apical intercellular hyperosmolality to ,18 mOsm kg,1. Water transport through solute-impermeable lateral cell membranes continues until intercellular and cellular osmolalities are equal. Transcellular and transjunctional volume flow are of similar magnitude (2 nL min,1 mm,1 tubule length) in the S2 segment. Thus, diffusion ensures isotonic absorption of NaCl. Two-thirds of NaHCO3 and other actively transported sodium salts are extruded into the last third of the exponentially widening intercellular space where the exposure time is only 0.9 s. Osmotic equilibration is dependent on aquaporins in the cell membranes. If permeability to water is low, transcellular water transport stops; tubular fluid becomes hypotonic; NaCl diffusion diminishes, but transjunctional water transport remains unaltered as long as transcellular transport of NaHCO3 and other solutes provides the osmotic force. Differential effects of short and long duration insulinotropic agents on meal-related glucose excursions

SUMMARY Aim Abnormal ,-cell function, characterized as the inability of the ,-cell to mount a rapid secretory response to glucose, is a well-established pathology of type 2 diabetes mellitus. These studies were designed to demonstrate the importance of early insulin release on the control of meal-induced glucose excursions by capitalizing on the significant pharmacodynamic differences between several oral insulin secreting agents. Methods Male Sprague Dawley fitted with indwelling jugular cannulas were used to compare the pharmacodynamic profiles of nateglinide (Nateg), glipizide (Glip) and repaglinide (Repag) through frequent blood samples following the administration of these compounds via oral gavage. In similar animals which were pretrained to consume their daily food intake in two discrete 45-min meals, the effects of compound induced changes in pre-meal, meal and post-meal insulin profiles on glycaemic control were assessed through frequent blood sampling following the administration of these compounds 10 min prior to a 30-min meal. Results There were significant pharmacodynamics differences between the three oral agents tested and the time to elicit peak insulin secretory responses increased from Nateg (4 min) to Repag (10 min) to Glip (45 min). During the meal tolerance test, glibenclamide did not increase pre-meal insulin levels and glucose excursions paralleled those in the control. Conversely, the other three agents, at doses that produced hypoglycaemic responses of similar magnitude, all increased early insulin release (,AUC(-15 to 3 min) = 0.5 ± 0.01, 1.6 ± 0.4, 3.6 ± 0.0, 1.2 ± 0.1 and 1.73 ± 0.4 nmol/min, for control, Nateg at 60 and 120 mg/kg, Glip and Repag, respectively) and curbed glucose excursions during the meal at varying rates and degrees (,AUC(0,30 min) = 39 ± 6, 8 ± 7, 5 ± 7, ,,1 ± 8 and ,,3 ± 8 mmol/min for control, Nateg at 60 and 120 mg/kg, Glip and Repag, respectively). However, unlike Nateg, the longer duration of action of Repag and Glip elicited sustained post-meal relative hypoglycaemia. Conclusion These data support the impact of early and rapid insulin release in the control of prandial and post-meal glycaemia and demonstrate that a short anticipatory burst of insulin, restricted to the beginning of a meal, provides a clear metabolic advantage and prevents post-meal hypoglycaemic episodes when compared to a greater but reactive insulin exposure that follows a meal-induced increase in glucose excursion. Gauging the societal impacts of natural disasters using a capability approach

There is a widely acknowledged need for a single composite index that provides a comprehensive picture of the societal impact of disasters. A composite index combines and logically organizes important information policy-makers need to allocate resources for the recovery from natural disasters; it can also inform hazard mitigation strategies. This paper develops a Disaster Impact Index (DII) to gauge the societal impact of disasters on the basis of the changes in individuals' capabilities. The DII can be interpreted as the disaster impact per capita. Capabilities are dimensions of individual well-being and refer to the genuine opportunities individuals have to achieve valuable states and activities (such as being adequately nourished or being mobile). After discussing the steps required to construct the DII, this article computes and compares the DIIs for two earthquakes of similar magnitude in two societies at different levels of development and of two disasters (earthquake and wind storm) in the same society. Nitric oxide synthase inhibition in Thoroughbred horses augments O2 extraction at rest and submaximal exercise, but not during short-term maximal exercise

Summary Reason for performing study: Work is required to establish the role of endogenous nitric oxide (NO) in metabolism of resting and exercising horses. Objectives: To examine the effects of NO synthase inhibition on O2 extraction and anaerobic metabolism at rest, and during submaximal and maximal exertion. Methods: Placebo and NO synthase inhibition (with N,-nitro-L-arginine methyl ester [l -NAME] administered at 20 mg/kg bwt i.v.) studies were performed in random order, 7 days apart on 7 healthy, exercise-trained Thoroughbred horses at rest and during incremental exercise leading to 120 sec of maximal exertion at 14 m/sec on a 3.5% uphill grade. Results: At rest, NO synthase inhibition significantly augmented the arterial to mixed-venous blood O2 content gradient and O2 extraction as mixed-venous blood O2 tension and saturation decreased significantly. While NO synthase inhibition did not affect arterial blood-gas tensions in exercising horses, the exercise-induced increment in haemoglobin concentration and arterial O2 content was attenuated. In the l -NAME study, during submaximal exercise, mixed-venous blood O2 tension and haemoglobin-O2 saturation decreased to a greater extent causing O2 extraction to increase significantly. During maximal exertion, arterial hypoxaemia, desaturation of haemoglobin and hypercapnia of a similar magnitude developed in both treatments. Also, the changes in mixed-venous blood O2 tension and haemoglobin-O2 saturation, arterial to mixed-venous blood O2 content gradient, O2 extraction and markers of anaerobic metabolism (lactate and ammonia production, and metabolic acidosis) were not different from those in the placebo study. Conclusion: Endogenous NO production augments O2 extraction at rest and during submaximal exertion, but not the during short-term maximal exercise. Also, NO synthase inhibition does not affect anaerobic metabolism at rest or during exertion. Potential relevance: It is unlikely that endogenous NO release modifies aerobic or anaerobic metabolism in horses performing short-term maximal exertion. [source]


A cross-national meta-analysis of alcohol and injury: data from the Emergency Room Collaborative Alcohol Analysis Project (ERCAAP)

ABSTRACT Aims, To examine the relationship of acute alcohol consumption with an injury compared to a non-injury event in the emergency room across ERs in five countries. Design, Meta-analysis was used to evaluate the consistency and magnitude of the association of a positive blood alcohol concentration (BAC) at the time of arrival in the ER and self-reported consumption within 6 hours prior to the event with admission to the ER for an injury compared to a non-injury, and the extent to which contextual (socio-cultural and organizational) variables explain effect sizes. Findings, When controlling for age, gender and drinking five or more drinks on an occasion at least monthly, pooled effect size was significant and of a similar magnitude for both BAC and self-reported consumption, with those positive on either measure over half as likely again to be admitted to the ER with an injury compared to a medical problem. Effect sizes were found to be homogeneous across ERs for BAC, but not for self-report. Trauma center status and legal level of intoxication were positively predictive of self-reported consumption effect size on injury. Conclusions, These data suggest a moderate, but robust association of a positive BAC and self-report with admission to the ER with an injury, and that contextual variables also appear to play a role in the alcohol,injury nexus. Methylene blue attenuates lung injury after mesenteric artery clamping/unclamping

Abstract Background, This controlled, experimental study was designed to assess the effects of intratracheal and intravenous methylene blue on reperfusion lung injury following superior mesenteric artery clamping/unclamping. Materials and methods, Superior mesenteric arteries of 144 anaesthetized adult male Wistar rats (n = 12/group) were clamped for 1 h. Ten minutes before unclamping, methylene blue or its vehicle was administered intratracheally or intravenously, followed by a 3 h-respiratory assessment and postexperimental assessment of survival. Results, Intravenous 3 and 9 mg kg,1 but not higher methylene blue doses, and intratracheal 6-mg kg,1 but not lower doses, significantly (P < 0·05) reduced the 100% increase in plateau pressure, 30% reduction in PO2/FiO2, fourfold augmented bronchoalveolar lavage-retrieved volume and the increased polymorphonuclear leukocytes/bronchoalveolar cells' ratio associated with unclamping of the superior mesenteric artery. Lung tissue polymorphonuclear leukocytes count, total xanthine oxidase activity and wet-to-dry-weight ratio were also normal in these dose-treated groups. These effective regimens were also associated with longer animal survival. Conclusions, Intratracheal methylene blue mitigates lung reperfusion injury following superior mesenteric artery clamping/unclamping at a similar magnitude as an intravenous regimen. This finding is a novel potential use of methylene blue in vivo. [source]


U.S. Monetary Policy Surprises and Currency Futures Markets: A New Look

G14 Abstract Intraday currency futures prices react to both surprises in the federal funds target rate (the target factor) and surprises in the anticipated future direction of Federal Reserve monetary policy (the path factor) in similar magnitude, and the reaction is short-lived. Dollar-denominated currency futures prices drop significantly in response to positive surprises (i.e., unexpected increases) in the target and path factors, but have generally little response to negative surprises. A monetary policy tightening during expansionary periods leads to an appreciation of the domestic currency, while a monetary policy loosening during recessionary periods tends to have no significant impact. High-Strain Shape-Memory Polymers

Abstract Shape-memory polymers (SMPs) are self-adjusting, smart materials in which shape changes can be accurately controlled at specific, tailored temperatures. In this study, the glass transition temperature (Tg) is adjusted between 28 and 55,°C through synthesis of copolymers of methyl acrylate (MA), methyl methacrylate (MMA), and isobornyl acrylate (IBoA). Acrylate compositions with both crosslinker densities and photoinitiator concentrations optimized at fractions of a mole percent demonstrate fully recoverable strains at 807% for a Tg of 28,°C, at 663% for a Tg of 37,°C, and at 553% for a Tg of 55,°C. A new compound, 4,4,-di(acryloyloxy)benzil (referred to hereafter as Xini) in which both polymerizable and initiating functionalities are incorporated in the same molecule, was synthesized and polymerized into acrylate shape-memory polymers, which were thermomechanically characterized yielding fully recoverable strains above 500%. The materials synthesized in this work were compared to an industry standard thermoplastic SMP, Mitsubishi's MM5510, which showed failure strains of similar magnitude, but without full shape recovery: residual strain after a single shape-memory cycle caused large-scale disfiguration. The materials in this study are intended to enable future applications where both recoverable high-strain capacity and the ability to accurately and independently position Tg are required. [source]


Embryo growth rates in birds and mammals

Summary 1.,Embryo mass growth curves of 36 species of bird and 18 species of mammal were fitted by sigmoidal Gompertz functions, in which k (days,1) describes the rate at which the embryo approaches an asymptotic mass A (g). 2.,The parameters of the Gompertz function were uncorrelated with parameters of power functions fitted to the same growth data, indicating that the two models describe different aspects of growth. 3.,Asymptotes of the Gompertz functions for embryonic growth averaged 2·5 times neonate size, but were well below adult mass. Thus, the pre- and postnatal phases of growth are distinct and have different target sizes. 4.,Embryo growth rate (k) decreases as the ,1/4 power of neonate size; values for mammals were 63% of those of birds, on average. 5.,Embryo growth rate can be predicted solely by the length of the incubation or gestation period, regardless of the size of the neonate, with the same relationship for birds and mammals. 6.,Postnatal growth rate (kG) scales linearly with embryonic growth rate (k), but is nearly five- times more rapid, on average, in birds than in mammals. kG and k have similar magnitude in birds, but kG is relatively much slower than k in mammals. 7.,Rate of actuarial senescence , (year,1), measured by the increase in mortality rate with age, is positively related to the rate of embryo growth in both birds and mammals, but is higher in the latter. Moreover, rate of ageing in birds is uniquely related to embryo growth while that in mammals is uniquely related to postnatal growth rate. Thus, development and ageing appear to be differently connected in birds and mammals, although the basis for these relationships is not known. [source]


Sex-specific physiological and growth responses to elevated atmospheric CO2 in Silene latifolia Poiret

Abstract Dioecy is found in nearly half of the angiosperm families, but little is known about how rising atmospheric CO2 concentration will affect male and female individuals of dioecious species. We examined gender-specific physiological and growth responses of Silene latifolia Poiret, a widespread dioecious species, to a doubled atmospheric CO2 concentration in environmentally controlled growth chambers. Elevated CO2 significantly increased photosynthesis in both male and female plants and by a similar magnitude. Males and females did not differ in net photosynthetic rate, but females had significantly greater biomass production than males, regardless of CO2 concentrations. Vegetative mass increased by 39% in males and in females, whereas reproductive mass increased by 82% in males and 97% in females at elevated CO2. As a result, proportionately more carbon was allocated to reproduction in male and female plants at elevated CO2. Higher CO2 increased individual seed mass significantly, but had no effect on the number or mass of seeds per female plant. Our results demonstrated that rising atmospheric CO2 will alter the allocation patterns in both male and female S. latifolia Poiret plants by shifting proportionally more photosynthate to reproduction. The pathology of hypertrophic cardiomyopathy

Sudden cardiac death (SCD) is devastating at any age, but even more so when the individual affected is young and asymptomatic, and the death is entirely unexpected. SCD is a catastrophic complication of hypertrophic cardiomyopathy (HCM) and may be the first manifestation of this disease. HCM is an inherited intrinsic disease of the myocardium characterized by left ventricular hypertophy without chamber dilatation, in the absence of either a systemic or other cardiac disease, which may cause a similar magnitude of hypertrophy. HCM may be a clinically silent disease. Indeed, the pathologist may be the first to encounter a case of HCM at autopsy. HCM has wide-ranging implications for affected families, who will require cardiac screening and genetic counselling even if mutations are not known. Therefore, prompt and accurate diagnosis of HCM is vital. This review article will focus on the pathological diagnosis of HCM, recent advances in the genetics of this disease, and common pitfalls which may arise, leading to diagnostic uncertainty. Above-stream microclimate and stream surface energy exchanges in a wildfire-disturbed riparian zone

Abstract Stream temperature and riparian microclimate were characterized for a 1·5 km wildfire-disturbed reach of Fishtrap Creek, located north of Kamloops, British Columbia. A deterministic net radiation model was developed using hemispherical canopy images coupled with on-site microclimate measurements. Modelled net radiation agreed reasonably with measured net radiation. Air temperature and humidity measured at two locations above the stream, separated by 900 m, were generally similar, whereas wind speed was poorly correlated between the two sites. Modelled net radiation varied considerably along the reach, and measurements at a single location did not provide a reliable estimate of the modelled reach average. During summer, net radiation dominated the surface heat exchanges, particularly because the sensible and latent heat fluxes were normally of opposite sign and thus tended to cancel each other. All surface heat fluxes shifted to negative values in autumn and were of similar magnitude through winter. In March, net radiation became positive, but heat gains were cancelled by sensible and latent heat fluxes, which remained negative. A modelling exercise using three canopy cover scenarios (current, simulated pre-wildfire and simulated complete vegetation removal) showed that net radiation under the standing dead trees was double that modelled for the pre-fire canopy cover. However, post-disturbance standing dead trees reduce daytime net radiation reaching the stream surface by one-third compared with complete vegetation removal. The results of this study have highlighted the need to account for reach-scale spatial variability of energy exchange processes, especially net radiation, when modelling stream energy budgets. CYP1A1 variants and smoking-related lung cancer in San Francisco bay area Latinos and African Americans

Abstract We examined CYP1A1 T6235C (M1) and A4889G (M2) polymorphisms in San Francisco Bay Area African Americans and Latinos who were newly diagnosed with primary lung cancer from September 1998 to November 2002 and in age-gender-ethnicity frequency-matched controls. Owing mainly to rapid mortality of cases, overall percentages of cases genotyped were 26% and 32% for Latinos and African Americans, respectively. CYP1A1 variants were genotyped for Latinos (104 cases, 278 controls) and African Americans (226 cases, 551 controls). M1 and M2 frequencies in controls were 0.23 and 0.02 for African Americans and 0.38 and 0.29 for Latinos. In Latinos, the overall inverse odds ratio (OR) of 0.51 (95% CI = 0.32,0.81) for M1 variant genotype resulted from an inverse interaction with smoking. Nonsmokers with M1 genotype had a slight elevated OR (1.5; 0.59,3.7), but those with less than 30 or 30 or more pack-year history had 0.20 (0.06,0.70) and 0.21 (0.06,0.81) times (about 1/5) the odds expected if smoking and genotype were independent lung cancer risk factors. African Americans had interactions of similar magnitude that were not statistically significant. Results for M2 were very similar. Inverse interactions of CYP1A1 variants and smoking-associated lung cancer risk in Latinos might be causal, due to undetected bias or confounding, or represent a unique linkage disequilibrium between a new lung cancer locus and CYP1A1 in this highly admixed population. Quality of Care of Nursing Home Residents Hospitalized With Heart Failure

OBJECTIVES: To determine whether the quality of heart failure (HF) care of hospitalized nursing home (NH) residents is different from that of patients admitted from other locations. DESIGN: Retrospective chart review. SETTING: Nursing home residents discharged from hospitals. PARTICIPANTS: Medicare beneficiaries aged 65 and older. MEASUREMENTS: Subjects were discharged with a primary discharge diagnosis of HF in Alabama in 1994. They were categorized as having been admitted from a NH or other locations. Bivariate logistic regression analysis was used to estimate crude odds ratios (ORs) and 95% confidence intervals (CIs) for left ventricular function (LVF) evaluation and angiotensin-converting enzyme (ACE) inhibitor use for NH residents relative to nonresidents. Multivariate generalized linear models were developed to determine independence of associations. RESULTS: Subjects (N = 1,067 years) had a mean age ± standard deviation of 79 ± 7.4, 60% were female, and 18% were African Americans. Fewer NH residents (n = 95) received LVF evaluation (39% vs 60%, P < .001) and ACE inhibitors (50% vs 72%, P = .111). NH residents had lower odds for LVF evaluation (OR = 0.42, 95% CI = 0.27,0.64). The odds for ACE inhibitor use, although of similar magnitude, did not reach statistical significance (OR = 0.40, 95% CI = 0.12,1.28). After adjustment of patient and care characteristics, admission from a NH was significantly associated with lower LVF evaluation (adjusted OR = 0.64, 95% CI = 0.49,0.82) but not with ACE inhibitor use (adjusted OR = 0.59, 95% CI = 0.16,2.14). CONCLUSIONS: Quality of HF care received by hospitalized NH residents was lower than that received by others. Further studies are needed to determine reasons for the lack of appropriate evaluation and treatment of NH patients with HF who are admitted to hospitals. [source]


Post-dispersal fate of seeds in the Monte desert of Argentina: patterns of germination in successive wet and dry years

Summary 1,Patterns of seed germination of grass and forb species were studied in open Prosopis woodland of the central Monte desert (Argentina) during several years, to test the hypotheses that (i) seed germination is positively affected by both rainfall and protection afforded by vegetation cover (a facilitative effect), (ii) the number of surviving plants is positively influenced by rainfall but negatively affected by established vegetation (a competitive effect), and (iii) seed loss from soil banks owing to germination is lower than that caused by granivorous animals. 2,Forb species germinated during restricted periods, either in early autumn or in spring. Grasses, however, germinated throughout the growing season, but because seedlings could not be identified to species level, it was impossible to discern whether different species germinated in particular seasons, or if all grasses germinated in all seasons. Grass and forb germination were generally of similar magnitude, but grass germination increased by an order of magnitude during a summer of unusually abundant rainfall related to an El Niño Southern Oscillation (ENSO) event. 3,Overall, the spatial distribution of neither germinating seeds nor surviving plants could be explained by interactions with established vegetation (facilitation and competition effects, respectively). An alternative explanation may be provided by the distribution of forb and grass seeds in the soil. 4,Seed loss owing to germination was low in both dry and rainy years. For forbs, such loss totalled <,1% of soil-seed reserves, and no forb species suffered losses >,4%. Total grass-seed loss to germination was usually <,0.5%, and the 5% reached in 1997,98 corresponded to an interruption of a prolonged drought by unusually abundant rainfall associated with a reduced seed bank. 5,Grass-seed loss caused by germination was one to two orders of magnitude lower than that reported due to autumn-winter granivory in the central Monte desert. Effects of rapid response systems on clinical outcomes: Systematic review and meta-analysis

Abstract BACKGROUND A rapid response system (RRS) consists of providers who immediately assess and treat unstable hospitalized patients. Examples include medical emergency teams and rapid response teams. Early reports of major improvements in patient outcomes led to widespread utilization of RRSs, despite the negative results of a subsequent cluster-randomized trial. PURPOSE To evaluate the effects of RRSs on clinical outcomes through a systematic literature review. DATA SOURCES MEDLINE, BIOSIS, and CINAHL searches through August 2006, review of conference proceedings and article bibliographies. STUDY SELECTION Randomized and nonrandomized controlled trials, interrupted time series, and before-after studies reporting effects of an RRS on inpatient mortality, cardiopulmonary arrests, or unscheduled ICU admissions. DATA EXTRACTION Two authors independently determined study eligibility, abstracted data, and classified study quality. DATA SYNTHESIS Thirteen studies met inclusion criteria: 1 cluster-randomized controlled trial (RCT), 1 interrupted time series, and 11 before-after studies. The RCT showed no effects on any clinical outcome. Before-after studies showed reductions in inpatient mortality (RR = 0.82, 95% CI: 0.74-0.91) and cardiac arrest (RR = 0.73, 95% CI: 0.65-0.83). However, these studies were of poor methodological quality, and control hospitals in the RCT reported reductions in mortality and cardiac arrest rates comparable to those in the before-after studies. CONCLUSIONS Published studies of RRSs have not found consistent improvement in clinical outcomes and have been of poor methodological quality. The positive results of before-after trials likely reflects secular trends and biased outcome ascertainment, as the improved outcomes they reported were of similar magnitude to those of the control group in the RCT. The anorexigenic 20 amino acid neuropeptide S (NPS) has not been studied in an animal model of hypo- or hyperphagia. The present study aimed to elucidate whether central NPS appetite-related effects are different in lines of chickens that had undergone long-term divergent selection for low (LWS) or high (HWS) body weight and that were hypo- and hyperphagic, respectively. It took a longer time for food intake to be reduced in LWS than HWS chicks administered the lowest dose of NPS tested (0.14 nmol) and, at the highest dose tested (0.56 nmol), they had a greater reduction in food intake than did HWS chicks. HWS chicks responded with a similar magnitude of food intake reduction that was independent of NPS dose. Although water intake was reduced concurrently with food intake after central NPS in both lines, blood glucose concentrations were not affected. Hypothalamic signalling was different between the lines. Although both lines respond to central NPS with decreased c-Fos immunoreactivity in the lateral hypothalamus, the periventricular nucleus had increased c-Fos immunoreactivity in LWS but not HWS chicks. After central NPS treatment, there was increased c-Fos immunoreactivity in the paraventricular nucleus in HWS but not LWS chicks. These data support the notion of differences in the central NPS system between the LWS and HWS lines and infer that central NPS may differentially affect appetite-related processes in other species that contain hypo- and hyperphagic individuals. Depression and diffuse physical symptoms in southern Chinese with temporomandibular disorders

Summary, The study investigated the experience of depressive symptoms and the relationship with diffuse physical symptoms reporting in southern Chinese seeking professional care for temporomandibular disorders (TMD) in Hong Kong. Eighty-seven new patients [77 females/10 males; mean age 39·3 years (SD 12·7)] referred to the specialist TMD clinic at the Prince Philip Dental Hospital, Hong Kong participated in this study. The Research Diagnostic Criteria (RDC)/TMD history questionnaire was used to derive Axis II psychological data. Psychological status was assessed through depression and non-specific physical symptoms (NPS) scores (pain items included and excluded) measured with RDC/TMD Axis II instruments; 42·5% of patients experienced moderate/severe depression symptoms; 59·8% and 57·5% had moderate/severe NPS scores when pain items were included and excluded, respectively. Strong, positive and statistically significant correlations were noted between depression scores and the NPS scores that included pain items (r = 0·80) and those that did not (r = 0·80). The correlations remained consistent and were of similar magnitude when male patients were excluded from the computation and also when the possible effect of patient age was controlled. While taking into account the modest patient sample which was related to a low rate of treatment seeking, depressive symptoms were common and similar to other western and Chinese patient groups. NPS reports were higher than in Singapore Chinese patients. There appeared to be a clear association between depression and diffuse physical symptoms. The findings should be considered in the holistic care of Chinese people with TMD. [source]


Using the polymer partitioning method to probe the thermodynamic activity of poorly water-soluble drugs solubilized in model lipid digestion products

Abstract The thermodynamic activity of solubilized drug is an important determinant of the extent of absorption of lipophilic drugs from the gastrointestinal tract. In this study, the polymer partitioning method was evaluated for its use in the determination of the thermodynamic activity of lipophilic drugs when solubilized in colloidal digestion products, using drug in dilute solution as a reference ideal solution. The lipophilic drugs griseofulvin, diazepam, and danazol partitioned into a polymeric receiver phase from non-micellar solution as a function of drug lipophilicity. The concentration of drug that partitioned into the polymer was linearly proportional to the concentration of free drug in solution, and this allowed the measured partition coefficient to be utilized as an indicator of the drug activity coefficient. The addition of a solubilizing species such as bile salt micelles caused a reduction in drug activity of a similar magnitude to that predicted from micelle equilibrium solubility data in the identical micellar solutions. The addition of micelle swelling lipids such as lecithin and fatty acids resulted in further reductions in activity coefficient. The kinetics of enhanced spin capturing polymerization: Influence of the nitrone structure

Abstract Several nitrones and one nitroso compound have been evaluated for their ability to control the molecular weight of polystyrene via the recently introduced radical polymerization method of enhanced spin capturing polymerization (ESCP). In this technique, molecular weight control is achieved (at ambient or slightly elevated temperatures) via the reaction of a growing radical chain with a nitrone forming a macronitroxide. These nitroxides subsequently react rapidly and irreversibly with propagating macroradicals forming polymer of a certain chain length, which depends on the nitrone concentration in the system. Via evaluation of the resulting number-average molecular weight, Mn, at low conversions, the addition rate coefficient of the growing radicals onto the different nitrones is determined and activation energies are obtained. For the nitrones N - tert -butyl-,-phenylnitrone (PBN), N -methyl-,-phenylnitrone (PMN), and N -methyl-,-(4-bromo-phenyl) nitrone (pB-PMN), addition rate coefficients, kad,macro, in a similar magnitude to the styrene propagation rate coefficient, kp, are found with spin capturing constants CSC (with CSC = kad,macro/kp) ranging from 1 to 13 depending on the nitrone and on temperature. Activation energies between 23.6 and 27.7 kJ mol,1 were deduced for kad,macro, congruent with a decreasing CSC with increasing temperature. Almost constant Mn over up to high monomer to polymer conversions is found when CSC is close to unity, while increasing molecular weights can be observed when the CSC is large. From temperatures of 100 °C onward, reversible cleavage of the alkoxyamine group can occur, superimposing a reversible activation/deactivation mechanism onto the ESCP system. © 2009 Wiley Periodicals, Inc. Novel fluorescent (p -phenylene ethynylene)-calix[4]arene-based polymer: Design, synthesis, and properties

Abstract A novel fluorescent (p -phenylene ethynylene)-calix[4]arene-based polymer (CALIX-PPE) has been successfully synthesized by cross-coupling polymerization of bis-calix[4]arene 1 with 1,4-diethynylbenzene. The polycondensation was carried out in toluene/NEt3 at 35 °C for 24 h, using PdCl2(PPh3)2/CuI as the catalytic system, furnishing CALIX-PPE in excellent isolated yields (higher than 95%, several runs). The yellow polymer is freely soluble in several nonprotic organic solvents. The GPC trace of the isolated polymer showed a monomodal distribution and a number-average molecular weight of 23,300 g mol,1 (Mw/Mn = 2.05). No evidence was found in the structural analysis (FTIR and 1H/13C NMR) regarding the formation of alkyne homocoupled segments along the polymer chain. For comparative purposes, the synthesis of an analogous poly(p -phenylene ethynylene) containing p - t -butyl-phenoxymethyl side chains (TBP-PPE) was also undertaken. A great similarity was found between the photophysical properties of CALIX-PPE and TBP-PPE in solution (UV,vis and laser induced luminescence), clearly demonstrating their unique dependence on the structure and conformation of the conjugated PPE backbone. The fluorescence spectra of polymers are of nearly identical shape, displaying their maximum emission around 420 nm. Translating Effective Web-Based Self-Help for Problem Drinking Into the Real World

Background:, Drinking Less (DL) is a 24/7 free-access anonymous interactive web-based self-help intervention without therapeutic guidance for adult problem drinkers in the community. In a randomized controlled trial (referred to here as DL-RCT), DL has been shown effective in reducing risky alcohol consumption. Objective:, To assess whether the findings of DL-RCT are generalizable to a naturalistic setting (DL-RW) in terms of ability to reach the target group and alcohol treatment response. Methods:, Pretest,posttest study with 6-month follow-up. An online survey was conducted of 378 of the 1,625 people who used DL-RW from May to November 2007. Primary outcome measures were (1) problem drinking, defined as alcohol consumption in the previous 4 weeks averaging >21 or >14 standard units (male/female) per week or ,6 or ,4 units (m/f) on 1 or more days per week; and (2) mean weekly alcohol consumption. DL-RW and DL-RCT data were compared and pooled. Intention-to-treat (ITT) analysis was performed to analyze and compare changes in drinking from baseline to follow-up. Results:, In the DL-RW group, 18.8% (n = 71) were drinking successfully within the limits of the Dutch guideline for low-risk drinking (p < 0.001) 6 months after baseline (ITT). The DL-RW group also decreased its mean weekly alcohol intake by 7.4 units, t(377) = 6.67, p < 0.001, d = 0.29. Drinking reduction in DL-RW was of a similar magnitude to that in the DL-RCT condition in terms of drinking within the guideline [,2(1) = 1.83, CI: 0.82,3.00, p = 0.18, RD = 0.05, OR = 1.55] and mean weekly consumption (a negligible difference of d = 0.03 in favor of DL-RW group). Conclusion:, The results from DL-RCT and DL-RW were similar, and they demonstrate that web-based self-help without therapeutic guidance is feasible, well accepted, and effective for curbing adult problem drinking in the community. Helplessness in the Tail Suspension Test Is Associated with an Increase in Ethanol Intake and Its Rewarding Effect in Female Mice

Background: Depression is frequently observed in drug abusers. However, depression may be a primary factor of predisposition to drug abuse or a consequence of drug abuse. The aim of this study was to analyze the influence of a preexisting depressive-like state/helplessness on subsequent alcohol responsiveness in mice. Methods: Male and female CD1 mice were selected according to their immobility time in the tail suspension test, and only mice with "high immobility" and "low immobility" time were retained. Using a two-bottle free-choice paradigm, these mice were given continuous access to tap water or solutions of ethanol (3,20% v/v), quinine (12.5,50 mg/liter), or sucrose (1,4% w/v). In female mice, rewarding and aversive effects of ethanol (1.5 and 3 g/kg, intraperitoneally) were also investigated using the conditioned place preference and the conditioned taste aversion paradigms. Results: Female mice were more immobile and drank more ethanol than male mice. No striking sex difference was observed in quinine consumption. Sucrose intake was higher in female than in male mice, whatever the solution concentration. At the 4% concentrated solution, a sucrose-induced increase in daily fluid intake was observed only in female mice. Female mice with high immobility time (HI) consumed more ethanol at the highest concentration than female mice with low immobility time (LI), whereas no difference was observed between HI and LI male mice. Moreover, whereas LI female mice failed to express place conditioning induced by the 3-g/kg dose of ethanol, HI female mice were strongly responsive to the rewarding effect of this high ethanol dose. Ethanol dose-dependently induced a conditioned taste aversion with a similar magnitude in both LI and HI female mice. Conclusions: The findings indicate that female CD1 mice tend to drink greater amounts of ethanol or sucrose solutions than male CD1 mice, suggesting that female mice may be a better model of excessive alcohol intake. Furthermore, no relationship was found between immobility scores and ethanol consumption in male mice. On the contrary, within female mice, HI mice consumed higher amounts of ethanol than LI mice probably because they experienced greater rewarding effects of ethanol. The present results support the hypothesis that depressive-like responses may predispose to ethanol abuse in female mice. [source]

Our results suggest that there are some, but not substantial, job losses in reaction to minimum-wage hikes and that the impact on firm wages is rather large, implying that further increases of similar magnitude might very well have negative consequences for employment. Review article: smoking cessation as primary therapy to modify the course of Crohn's disease

Summary This article aims to offer an updated review of the effects of smoking on inflammatory bowel disease, and provide a review of the methods of achieving smoking cessation. A systematic review of Embase and Medline databases was conducted. Smoking causes opposing effects on ulcerative colitis and Crohn's disease. The odds ratio of developing ulcerative colitis for smokers compared with lifetime non-smokers is 0.41. Conversely, smokers with Crohn's disease have a more aggressive disease requiring more therapeutic intervention. Smoking cessation is associated with a 65% reduction in the risk of a relapse as compared with continued smokers, a similar magnitude to that obtained with immunosuppressive therapy. Although difficult to achieve smoking cessation can best be encouraged by accessing appropriate counselling services, nicotine replacement therapy and bupropion. Using a combination of these treatments there is an improved chance of success of up to 20% compared with an unassisted quit attempt. Smoking cessation unequivocally improves the course of Crohn's disease and should be a primary therapeutic aim in smokers with Crohn's disease. [source]


Trends in Diet, Nutritional Status, and Diet-related Noncommunicable Diseases in China and India: The Economic Costs of the Nutrition Transition

Undernutrition is being rapidly reduced in India and China. In both countries the diet is shifting toward higher fat and lower carbohydrate content. Distinct features are high intakes of foods from animal sources and edible oils in China, and high intakes of dairy and added sugar in India. The proportion of overweight is increasing very rapidly in China among all adults; in India the shift is most pronounced among urban residents and high-income rural residents. Hypertension and stroke are relatively higher in China and adult-onset diabetes is relatively higher in India. Established economic techniques were used to measure and project the costs of undernutrition and diet-related noncommunicable diseases in 1995 and 2025. Current WHO mortality projections of diet-related noncommunicable diseases, dietary and body composition survey data, and national data sets of hospital costs for healthcare, are used for the economic analyses. In 1995, China's costs of undernutrition and costs of diet-related noncommunicable diseases were of similar magnitude, but there will be a rapid increase in the costs and prevalence of diet-related noncommunicable diseases by 2025. By contrast with China, India's costs of undernutrition will continue to decline, but undernutrition costs did surpass overnutrition diet-related noncommunicable disease costs in 1995. India's rapid increase in diet-related noncommunicable diseases and their costs projects similar economic costs of undernutrition and overnutrition by 2025. [source]


Reliability of cranial base measurements on lateral skull radiographs

Material and Methods,,, From the Helsinki longitudinal growth study, 20 randomly selected lateral radiographs were analyzed and re-analyzed by two examiners. Both located seven cephalometric landmarks based on which five measurements were calculated. The differences of results were compared. With similar method three radiographs were analysed by 11 examiners and results were compared. Results,,, Some anatomic landmarks were easier to locate than others on lateral skull radiographs leading to differences in measurements based on them. We found the magnitude of the difference to be dependent on the landmark serving as reference. Inter- and intra-examiner errors were of similar magnitude, although intra-examiner error declined in the repeated landmark identification. Variation in a single landmark location had in general little effect on the measurement value. Conclusion,,, Variations in landmark location lead to differences in numeric evaluation of the anatomic relationships in the skull base area. These differences were, however, shown to have little clinical significance. Hence, the documented methods are applicable for screening of basilar pathology. [source]


Prenatal polychlorinated biphenyl exposures in eastern Slovakia modify effects of social factors on birthweight

Summary Polychlorinated biphenyls (PCB) were widely used for industrial purposes and consumer products, but because of their toxicity, production was banned by most industrialised countries in the late 1970s. In eastern Slovakia, they were produced until 1985. During 2002,04, a birth cohort of mothers (n = 1057) residing in two Slovak districts was enrolled at delivery, and their specimens and information were collected after birth. Congeners of PCBs were measured in maternal serum by high-resolution gas chromatography with electron capture detection. In this study, we used multiple linear regression to examine the effects of prenatal PCB exposure on birthweight adjusted for gestational age, controlling for inter-pregnancy interval, and maternal smoking, age, education, ethnicity, pre-pregnancy body mass index and height. The association between total maternal serum PCB levels and birthweight was not statistically significant. However, an interaction model indicated that maternal PCB concentrations were associated with lower birthweight in Romani boys. Based on the fitted regression model, the predicted birthweight of Romani boys at the 90th percentile of maternal PCBs (12.8 ng/mL) was 133 g lower than the predicted birthweight at the 10th percentile of maternal PCBs (1.6 ng/mL). This is a similar magnitude of effect to that observed for maternal smoking and birthweight. These results suggest that higher levels of PCBs in maternal blood sera may inhibit growth in boys, particularly in those already affected by social factors related to ethnicity. This study is consistent with previous findings that boys are more susceptible than girls to growth restriction induced by in utero organochlorine exposures, and further indicates that high PCBs may magnify the influence of social disadvantage in this vulnerable group of boys. The Heterotrimeric G-protein Complex Modulates Light Sensitivity in Arabidopsis thaliana Seed Germination

Release of dormancy and induction of seed germination are complex traits finely regulated by hormonal signals and environmental cues such as temperature and light. The Red (R):Far-Red (FR) phytochrome photoreceptors mediate light regulation of seed germination. We investigated the possible involvement of heterotrimeric G-protein complex in the phytochrome signaling pathways of Arabidopsis thaliana seed germination. Germination rates of null mutants of the alpha (G,) and beta (G,) subunits of the G-protein (Atgpa1-4 and agb1-2, respectively) and the double mutant (agb1-2/gpa1-4) are lower than the wildtype (WT) under continuous or pulsed light. The G, and G, subunits play a role in seed germination under hourly pulses of R lower than 0.1 ,mol m,2 s,1 whereas the G, subunit plays a role in higher R fluences. The germination of double mutants of G-protein subunits with phyA-211 and phyB-9 suggests that AtGPA1 seems to act as a positive regulator of phyA and probably phyB signaling pathways, while the role of AGB1 is ambiguous. The imbibition of seeds at 4°C and 35°C alters the R and FR light responsiveness of WT and G-protein mutants to a similar magnitude. Thus, G, and G, subunits of the heterotrimeric G-protein complex modulate light induction of seed germination by phytochromes and are dispensable for the control of dormancy by low and high temperatures prior to irradiation. We discuss the possible indirect role of the G-protein complex on the phytochrome-regulated germination through hormonal signaling pathways. [source]


Using Multimember District Elections to Estimate the Sources of the Incumbency Advantage

In this article we use a novel research design that exploits unique features of multimember districts to estimate and decompose the incumbency advantage in state legislative elections. Like some existing related studies we also use repeated observations on the same candidates to account for unobserved factors that remain constant across observations. Multimember districts have the additional feature of copartisans competing for multiple seats within the same district. This allows us to identify both the direct office-holder benefits and the incumbent quality advantage over nonincumbent candidates from the same party. We find that the overall incumbency advantage is of similar magnitude as that found in previous studies. We attribute approximately half of this advantage to incumbents' quality advantage over open-seat candidates and the remainder to direct office-holder benefits. However, we also find some evidence that direct office-holder benefits are larger in competitive districts than in safe districts and in states with relatively large legislative budgets per capita.
