Revisiting two local constraints of the Galactic chemical evolution

ABSTRACT I review the uncertainties in two observational local constraints of the Galactic disc chemical evolution: the metallicity distribution of long-lived dwarfs and the age,metallicity relation. Analysing most recent data, it is shown first that the observed metallicity distribution at solar galactocentric radius, designed with standard methods, is more fit to a closed-box model than to the infall metallicity distribution. We argue that this is due to the specific contribution of the thick-disc population, which has been overlooked both in the derivation of the observed metallicity distribution and in the standard chemical evolution models. Although this agreement disqualifies the metallicity distribution as the best supportive (indirect) evidence for infall, we argue that the evolution must be more complex than described by either the closed-box or the standard infall models. It is then shown that recent determinations of the age,metallicity distribution (AMD) from large Strömgren photometric surveys are dominated by noise resulting from systematic biases in metallicities and effective temperatures. These biases are evaluated and a new AMD is obtained, where particularities of the previous determinations are phased out. The new age,metallicity relation shows a mean increase limited to about a factor of 2 in Z over the disc age. It is shown that below 3 Gyr, the dispersion in metallicity is about 0.1 dex, which, given the observational uncertainties in the derived metallicities, is compatible with the small cosmic dispersion measured on the interstellar medium and meteoritic pre-solar dust grains. A population that is progressively older and more metal rich arises at a metallicity greater than that of the Hyades, to reach [Fe/H],+0.5 dex at ages greater than 5 Gyr. We suggest that this is best explained by radial migration. A symmetrical widening of the metallicity interval towards lower values is seen at about the same age, which is attributed to a similar cause. Finally, the new derived ages are sufficiently consistent that an age,metallicity relation within the thick disc is confirmed. These new features altogether draw a picture of the chemical evolution in the solar neighbourhood where dynamical effects and complexity in the AMD dominate, rather than a generalized high dispersion at all ages. Adverse Event Protocol for Interventional Pain Medicine: The Importance of an Organized Response

ABSTRACT Although a significant number of interventional pain therapies are performed in office and fluoroscopy suite settings, the incidence of adverse events associated with these procedures is unknown. To minimize patient morbidity and physician liability, the preparation for and response to such events should follow a standardized protocol. This article provides a detailed protocol for responding to adverse events associated with interventional pain procedures performed in private office or fluoroscopy suite settings. The purposes of this protocol are to ensure quality patient care during and after an adverse event, to promote a better understanding of staff responsibilities at those times, to decrease the likelihood that an adverse event will become life-threatening, to suggest an appropriate format for the documentation of such events, and to reduce the likelihood of the recurrence of adverse events from a similar cause. Adherence to this protocol may also mitigate professional liability. [source]


From Great Depression to Great Credit Crisis: similarities, differences and lessons

Summary The Great Depression of the 1930s and the Great Credit Crisis of the 2000s had similar causes but elicited strikingly different policy responses. While it remains too early to assess the effectiveness of current policy, it is possible to analyse monetary and fiscal responses in the 1930s as a natural experiment or counterfactual capable of shedding light on the impact of current policies. We employ vector autoregressions, instrumental variables, and qualitative evidence for 27 countries in the period 1925,39. The results suggest that monetary and fiscal stimulus was effective -- that where it did not make a difference it was not tried. They shed light on the debate over fiscal multipliers in episodes of financial crisis. They are consistent with multipliers at the higher end of those estimated in the recent literature, and with the argument that the impact of fiscal stimulus will be greater when banking systems are dysfunctional and monetary policy is constrained by the zero bound. --- Miguel Almunia, Agustín Bénétrix, Barry Eichengreen, Kevin H. Factors predicting mortality in midlife adults with and without Down syndrome living with family

Abstract Background Little is known about the mortality of individuals with Down syndrome who have lived at home with their families throughout their lives. The current study evaluates the predictors, causes and patterns of mortality among co-residing individuals in midlife with Down syndrome as compared with co-residing individuals with ID owing to other causes. Method This paper examines mortality in 169 individuals with and 292 individuals without Down syndrome from 1988 to 2007. Dates and causes of death were obtained from maternal report, the Social Security Death Index and the National Death Index. Risk factors predicting mortality, including demographic variables, transition variables, and initial and change measures of health, functional abilities and behaviour problems, were obtained from maternal report. Results Having Down syndrome is a risk factor of mortality, net of other risk factors including older age, poorer functional abilities, worsening behaviour problems, residential relocation and parental death. The causes of death among individuals with and without Down syndrome who are in midlife and co-residing with their families are similar, and are most commonly due to cardiovascular or respiratory problems. Conclusions The findings indicate that midlife adults with Down syndrome who co-reside with their families generally exhibit similar causes of mortality as do midlife adults with intellectual disability owing to other causes, but show an elevated risk of mortality in midlife net of other variables, such as age and changes in functional abilities and behaviour problems.
