Selected AbstractsOVERCOMING BARRIERS TO PAIN RELIEF IN THE CARIBBEANDEVELOPING WORLD BIOETHICS, Issue 3 2009CHERYL MACPHERSON ABSTRACT This paper examines pain and pain relief in the Caribbean, where pain is widely perceived as an unavoidable part of life, and where unnecessary suffering results from untreated and under treated pain. Barriers to pain relief in the Caribbean include patient and family attitudes, inadequate knowledge among health professionals and unduly restrictive regulations on the medical use of opioids. Similar barriers exist all over the world. This paper urges medical, nursing and public health professionals, and educators to examine attitudes towards pain and pain relief and to work towards making effective pain relief and palliation more accessible. It recommends that i) health professionals and officials be better educated about pain, palliation and opioids, ii) regulatory restrictions be updated in light of clinical and scientific evidence, iii) opioid procurement policies be adjusted to facilitate increased medical use, iv) medical charts and records be modified to routinely elicit and document patients levels of pain, and v) educational campaigns be developed to inform the public that moderate and severe pain can be safely relieved at the end of life and other stages of life. The professional, respectful, and beneficent response to patients in pain is to provide rapid and aggressive pain relief or to urgently consult a pain or palliative specialist. When a health system hinders such efforts the ethical response is to identify, facilitate and advocate for overcoming barriers to improvement. [source] Thermochemistry for enthalpies and reaction paths of nitrous acid isomersINTERNATIONAL JOURNAL OF CHEMICAL KINETICS, Issue 7 2007Rubik Asatryan Recent studies show that nitrous acid, HONO, a significant precursor of the hydroxyl radical in the atmosphere, is formed during the photolysis of nitrogen dioxide in soils. The term nitrous acid is largely used interchangeably in the atmospheric literature, and the analytical methods employed do not often distinguish between the HONO structure (nitrous acid) and HNO2 (nitryl hydride or isonitrous acid). The objective of this study is to determine the thermochemistry of the HNO2 isomer, which has not been determined experimentally, and to evaluate its thermal and atmospheric stability relative to HONO. The thermochemistry of these isomers is also needed for reference and internal consistency in the calculation of larger nitrite and nitryl systems. We review, evaluate, and compare the thermochemical properties of several small nitric oxide and hydrogen nitrogen oxide molecules. The enthalpies of HONO and HNO2 are calculated using computational chemistry with the following methods of analysis for the atomization, isomerization, and work reactions using closed- and open-shell reference molecules. Three high-level composite methods G3, CBS-QB3, and CBS-APNO are used for the computation of enthalpy. The enthalpy of formation, ,Hof(298 K), for HONO is determined as ,18.90 ± 0.05 kcal mol,1 (,79.08 ± 0.2 kJ mol,1) and as ,10.90 ± 0.05 kcal mol,1 (,45.61 ± 0.2 kJ mol,1) for nitryl hydride (HNO2), which is significantly higher than values used in recent NOx combustion mechanisms. H-NO2 is the weakest bond in isonitrous acid; but HNO2 will isomerize to HONO with a similar barrier to the HONO bond energy; thus, it also serves as a source of OH in atmospheric chemistry. Kinetics of the isomerization is determined; a potential energy diagram of H/N/O2 system is presented, and an analysis of the triplet surface is initiated. © 2007 Wiley Periodicals, Inc. Int J Chem Kinet 39: 378,398, 2007 [source] The Care,tech Link: An Examination of Gender, Care and Technical Work in Healthcare LabourGENDER, WORK & ORGANISATION, Issue 4 2008Sally Lindsay Despite the dramatic increase of technology in the healthcare field, little is known of how care work and technical work are related. Examining substitute healthcare providers offers a useful illustration of the care,tech link because nursing (care) and medical (technical) models often merge. Forty-two interviews with men and women (nurse practitioners, nurse anaesthetists and physician assistants) were conducted in the USA. The results showed that the gendered nature of care,tech boundaries has shifted in small but important ways and that the gendering of work influenced the shape of these boundaries. Men often encountered barriers when moving too far into the care realm and attempted to overcome this by ,caring cautiously' and emphasizing problem-solving care. Women faced similar barriers from the ,old boys network' when they entered highly technical areas. There is also evidence that men and women challenged existing care,tech boundaries and moved beyond their traditional roles. [source] Mother-to-Child Transmission of HIV in China and the USA: A Comparative AnalysisASIAN SOCIAL WORK AND POLICY REVIEW, Issue 3 2010Susanna Arcella This paper analyzes and compares the existing policies and programs for the prevention of mother-to-child transmission of HIV existing in China and in the USA. The implementation of these is still imperfect and there needs to be improvements in the education (the spreading of information) and in the general health care system of China. Particular attention will be drawn to the existing barriers to the effective implementation of prevention of mother-to-child-transmission. The recommendations aim to solve similar barriers in both countries and some specifically dealing with problems in China. Since HIV infection in children is caused almost entirely by prenatal transmission, it is important that the governments, in partnership with civic society organizations, make all the necessary efforts to save the lives of their newborn citizens.
