    Introducing market-like structures to public services is a key aspect of New Public Management. The restructuring of the NHS into an internal market of the 1990s is an example. Recent policies have further developed this notion. A new aspect of the restructuring is a focus on increasing the diversity of types of provider of healthcare organisations. The objectives of the restructuring policy entailing the increase in supply side diversity are examined, and the challenges raised by these changes are discussed. It is argued that the government is too optimistic about the benefits, and insufficiently concerned about possible undesirable consequences. [source]

    In-vitro characterisation of the nebulised dose during non-invasive ventilation

    Abstract Objectives, Non-invasive ventilation (NIV) with nebulised bronchodilators helps some patients to maintain effective ventilation. However, the position of the nebuliser in the ventilation circuit may affect lung delivery. Methods, We placed the nebuliser proximal (A) and distal (B) to a breathing simulator in a standard NIV circuit with inspiratory (I) and expiratory (E) pressures of 20 and 5 cm H2O, 1 : 3 I : E ratio, 15 breaths/min and a tidal volume of 500 ml. Five milligrams of terbutaline solution was nebulised using an Aeroneb Pro (AERO) and a Sidestream (SIDE) nebuliser. The fate of the nebulised dose was determined and the aerodynamic droplet characteristics were measured using a cooled Next Generation Impactor. Key findings, More terbutaline was entrained on the inhalation filter in position A than in position B (P < 0.001) for both nebulisers. These amounts were greater (P < 0.001) for AERO than SIDE due to a smaller (P < 0.001) residual volume. The mean (SD) fine particle doses for AEROA, AEROB, SIDEA and SIDEB were 1.31 (0.2), 1.13 (0.14), 0.56 (0.03) and 0.39 (0.13) mg. These amounts from AEROA were significantly greater (P < 0.001) than those of the other three methods. Conclusions, The results highlight the differences between nebulisers and the influence on the placement of the nebuliser in the NIV circuit. [source]

Chartered on August 20, 1991, the Native American Graves Protection and Repatriation Review Committee has held 42 meetings over the subsequent 19 years. The review committee's responsibilities include monitoring the summary, inventory, and repatriation process; facilitating the resolution of disputes; compiling an inventory of culturally unidentifiable human remains and recommending specific actions for their disposition; consulting with the Secretary of the Interior in the development of regulations; and submitting an annual report to Congress.     SEXUAL DIMORPHISM AND ADAPTIVE SPECIATION: TWO SIDES OF THE SAME ECOLOGICAL COIN

    Abstract Models of adaptive speciation are typically concerned with demonstrating that it is possible for ecologically driven disruptive selection to lead to the evolution of assortative mating and hence speciation. However, disruptive selection could also lead to other forms of evolutionary diversification, including ecological sexual dimorphisms. Using a model of frequency-dependent intraspecific competition, we show analytically that adaptive speciation and dimorphism require identical ecological conditions. Numerical simulations of individual-based models show that a single ecological model can produce either evolutionary outcome, depending on the genetic independence of male and female traits and the potential strength of assortative mating. Speciation is inhibited when the genetic basis of male and female ecological traits allows the sexes to diverge substantially. This is because sexual dimorphism, which can evolve quickly, can eliminate the frequency-dependent disruptive selection that would have provided the impetus for speciation. Conversely, populations with strong assortative mating based on ecological traits are less likely to evolve a sexual dimorphism because females cannot simultaneously prefer males more similar to themselves while still allowing the males to diverge. This conflict between speciation and dimorphism can be circumvented in two ways. First, we find a novel form of speciation via negative assortative mating, leading to two dimorphic daughter species. Second, if assortative mating is based on a neutral marker trait, trophic dimorphism and speciation by positive assortative mating can occur simultaneously. We conclude that while adaptive speciation and ecological sexual dimorphism may occur simultaneously, allowing for sexual dimorphism restricts the likelihood of adaptive speciation. Thus, it is important to recognize that disruptive selection due to frequency-dependent interactions can lead to more than one form of adaptive splitting. [source]


    DIET OF RINGED SEALS (PHOCA HISPIDA) ON THE EAST AND WEST SIDES OF THE NORTH WATER POLYNYA, NORTHERN BAFFIN BAY

    Abstract In conjunction with the International North Water Polynya Study (NOW) in northern Baffin Bay, we examined the diets of ringed seals (Phoca hispida)1 on the west (Grise Fiord, Nunavut) and east (Qaanaaq, Greenland) sides of the polynya, using conventional stomach content analysis, as well as inferences from stable isotope ratios in seal muscle. Between May and July 1998, stomach and muscle tissue samples were collected from 99 ringed seals taken near Grise Fiord and 100 taken near Qaanaaq. The amphipod Themisto libellula was the dominant prey type in the diet of immature ringed seals from Grise Fiord, whereas arctic cod (Boreogadus saida) and polar cod (Arctogadus glacialis) predominated in the diet of adults. Both immature and adult seals collected near Qaanaaq fed predominantly on arctic cod. Overall, seals collected near Grise Fiord had significantly higher ,13C values than those collected near Qaanaaq (P < 0.001), but there was no statistical separation in ,15N values between the two samples (P= 0.06). Differences in diets of ringed seals from the east and west sides of the North Water Polynya may be due to differences in prey distribution and/or differences in biological productivity and fish biomass within the polynya.     An Improved Algorithm for Satellite-derived UV Radiation,

    ABSTRACT The improved algorithm surface irradiance derived from a range of satellite-based sensors (SIDES) is presented in this article. It calculates various types of surface UV intensities, such as biologically weighted or unweighted UV spectra, integrated doses or irradiance at specific wavelengths, using data from satellite instruments. These surface UV data are mainly useful for environmental impact or process studies where high accuracy or a high temporal resolution is required. In contrast to several previous studies, SIDES has been validated with spectral measurements. By this method an averaging of positive or negative deviations over the complete wavelength range is avoided. This is especially important for UV wavelengths around 300 nm where biological effectiveness is highest. The results of SIDES deviate less than 7% from ground-based observations for wavelengths between 295 and 400 nm. In contrast, the corresponding deviations of the joint research center algorithm escalate for shorter wavelengths, reaching 35% at 295 nm. This large deviation is due to an inaccurate interpolation procedure that has been detected by spectral analysis. Thus, spectral validation is demonstrated to be an appropriate tool to detect weaknesses in such an algorithm and provides information essential for improvement. [source]


    Tackling the Down Side: Social Capital, Women's Empowerment and Micro-Finance in Cameroon

    Micro-finance programmes are currently dominated by the ,financial self-sustainability paradigm' where women's participation in groups is promoted as a key means of increasing financial sustainability while at the same time assumed to automatically empower them. This article examines the experience of seven micro-finance programmes in Cameroon. The evidence indicates that micro-finance programmes which build social capital can indeed make a significant contribution to women's empowerment. However, serious questions need to be asked about what sorts of norms, networks and associations are to be promoted, in whose interests, and how they can best contribute to empowerment, particularly for the poorest women. Where the complexities of power relations and inequality are ignored, reliance on social capital as a mechanism for reducing programme costs may undermine programme aims not only of empowerment but also of financial sustainability and poverty targeting.     The Macroeconomics of Doubling Aid to Africa and the Centrality of the Supply Side

    The proposed doubling of aid to Africa by 2010 is a less simple proposition, from a recipient point of view, than is commonly supposed. This article argues that it is difficult to manage large and rapidly increasing aid inflows in ways which do not disadvantage producers of tradeable goods, and the private sector generally. This difficulty can be averted if conscious efforts are made to offset it and to stimulate positive responses from the supply side. Whether such responses prevail over the shorter-term management difficulties depends on the efficacy of state actions , and of aid , to bolster the supply side. The outcome is likely to be mixed, depending on country circumstances. [source]


    Evidence of Robust Coupling of Atrioventricular Mechanical Function of the Right Side of the Heart: Insights from M-Mode Analysis of Annular Motion

    Background: Extensive data exist regarding annular descent and ventricular function. We have already demonstrated significant differences in amplitude and timing of events between maximal mitral (MAPSE) and tricuspid (TAPSE) annular plane systolic excursion as well as described quantitative temporal differences in annular ascent (AA) between the right and left sides of the heart. However, whether any relationship exists between annular ascent and descent components remains uninvestigated. Methods: Left ventricular ejection fraction (LVEF), right ventricular fractional area change (RVFAC), MAPSE, TAPSE, MV, and TV AA as well as pulsed tissue Doppler of the lateral MV and TV annuli were recorded from 53 patients. Results: In this population (age 55 ± 17 years) mean LVEF was 55 ± 19%, mean RVFAC was 47 ± 20%, mean MAPSE was 2.11 ± 0.72 cm, mean TAPSE was 1.48 ± 0.44 cm, mean MV AA was 0.52 ± 0.17 cm, TV AA was 0.96 ± 0.47, MV A-wave 0.10 ± 0.04 cm/s, and TV A-wave was 0.13 ± 0.05 cm/s. A more robust correlation was seen between TV AA and RVFAC than between MV AA and LVEF and also between TV AA and pulsed TDI TV A-wave velocity than between MV AA and pulsed TDI MV A-wave. Conclusion: Our data reveal that mechanical systolic functions of the atria and the ventricles are more closely coupled on the right than on the left side of the heart. Whether this is a result of anatomic linking or chamber geometry will require further study. [source]


    Multivariate Neuropsychological Prediction of Seizure Lateralization in Temporal Epilepsy Surgical Cases

    Summary:,Purpose: Neuropsychological assessment can be of assistance in determining seizure lateralization in cases where EEG and MRI findings do not provide clear lateralizing data. While several studies have examined the lateralizing value of individual neuropsychological measures, clinicians are still in need of a statistically sound method that permits the incorporation of multiple neuropsychological variables to predict seizure lateralization in the individual patient. Method: The present study investigated the lateralizing value of several commonly used neuropsychological measures in a large sample of patients (n = 217) who eventually underwent surgical resection to treat their epilepsy. Side of surgery was used to operationally define seizure lateralization. A comparison of the relative utility of a multivariate versus univariate approach to predict seizure lateralization was conducted in temporal epilepsy cases. Results: The results provide evidence for the incremental validity of neuropsychological measures, other than memory and IQ tests, in the prediction of seizure lateralization in patients with medically intractable epilepsy. These data indicate that a multivariate approach increases the accuracy of prediction of seizure lateralization for temporal lobe epilepsy cases. Conclusion: This study supports the use of a multivariate approach using neuropsychological measures to predict seizure lateralization in temporal epilepsy surgical candidates. Regression formulas are provided to enhance the clinical utility of these findings. [source]


    Off the Back or on the Side: Comparison of meso and 2-Substituted Donor-Acceptor Difluoroborondipyrromethene (Bodipy) Dyads

    Abstract The preparation of several difluoroborondipyrromethene (Bodipy) dyads is described incorporating covalently attached hydroquinone/quinone groups at the 2-position (BD-SHQ, BD-SQ, BD-SPHQ, BD-SPQ). The compounds, currently under investigation as chemical sensors for reactive oxygen species, show various levels of fluorescence depending on the oxidation state of the appended group. The 19F NMR spectrum for BD-SHQ in CDCl3 at room temperature reveals the two fluorines are inequivalent on the NMR timescale. In contrast, the 19F NMR spectrum for the counterpart quinone compound, BD-SQ, is consistent with two equivalent fluorine atoms. The two results are interpreted as the quinone is free to rotate around the connector bond, whereas this motion is restricted for the hydroquinone group and makes the fluorines chemically inequivalent. Cyclic voltammograms recorded for all derivatives in CH2Cl2 electrolyte solution are consistent with typical Bodipy-based redox chemistry; the potentials of which depend on factors such as presence of the phenylene spacer and oxidation state of the appended group. A comparison of the electrochemical behaviour with the counterpart meso derivatives reveals some interesting trends which are associated with the location of the HOMO/LUMOs. The absorption profiles for the compounds in CH3CN are again consistent with Bodipy-based derivatives, though there are some subtle differences in the band-shapes of the closely-coupled systems. In particular, the absorption spectra for the dyad, BD-SQ, in a wide range of solvents are appreciably broader than for BD-SHQ. Femtosecond transient absorption spectroscopy performed on the hydroquinone derivatives, BD-SHQ and its meso analogue is interpreted as electron transfer occurs from the hydroquinone unit to the first-excited singlet (S1) state of the Bodipy center, followed by ultrafast charge recombination to reinstate the ground state. The coupling of OH vibrations to the return electron transfer process is invoked to explain the lack of clear identification of the charge-separated state in the transient records. [source]


    Enantioselective Recognition of Aliphatic Amino Acids by ,-Cyclodextrin Derivatives Bearing Aromatic Organoselenium Moieties on the Primary or Secondary Side

    Abstract Spectrophotometric titrations have been performed in order to determine the stability constants of inclusion complexation of some aliphatic amino acids with four structurally related organoselenium-modified ,-cyclodextrins: mono(6-phenylseleno-6-deoxy)-,-cyclodextrin (1a), mono[6-(p -methoxyphenylseleno)-6-deoxy]-,-cyclodextrin (1b), mono(2-phenylseleno-2-deoxy)-,-cyclodextrin (2a), and mono[2-(p -methoxyphenylseleno)-2-deoxy]-,-cyclodextrin (2b). Conformation analysis by circular dichroism and 2D NMR spectroscopic studies revealed that the aryl-substituted ,-cyclodextrins gave self-inclusion intramolecular complexes in aqueous solution, while the extent of penetration depended both on the positions and on the structures of substituents. Quantitative investigation on the binding ability of the hosts with amino acids showed that they were able to recognize the size and the shape of guests, affording supramolecular complexes with quite small stability constants ranging from 24 to 355 M,1. The molecular recognition abilities are discussed from the viewpoints of induced-fitting mechanisms, geometric complementary, and cooperative binding processes. Furthermore, these ,-cyclodextrin derivatives displayed considerable enantioselectivity towards L/D -amino acid isomers, giving the highest L -enantioselectivity (up to 8.4) for inclusion complexation between leucine and 2a. The binding modes of L/D -leucine with 1b were elucidated from NOESY studies and the chiral recognition phenomena were interpreted accordingly. (© Wiley-VCH Verlag GmbH & Co. Findings indicate that the heterogeneity of interests represented on the board, members' possession of relevant knowledge, and the presence of ex-ante conflict resolution mechanisms combine in shaping if and how board members engage in strategy-related activities and how strategic decisions are taken. Findings extend current understandings about the strategic functions of the board (monitoring, advice, and resource-dependence), suggesting how, under certain conditions, boards may act as negotiation forums where directors search for a reconciliation between diverging shareholders' interests and views.     /r/ and the construction of place identity on New York City's Lower East Side1

    This paper argues that a group of white residents on the Lower East Side of Manhattan use a New York City English (NYCE) feature , non-rhoticity in the syllable coda , in the construction of a place identity, one aspect of identity tied to localness and authenticity. A quantitative analysis confirms that the change in progress towards rhoticity in NYCE (Labov 1966) continues to advance slowly, so that non-rhoticity remains a resource for New Yorkers, imbued with local social meaning. Ethnographic observation of the Lower East Side reveals conflict among residents, which motivates one group to highlight their place identity by using non-rhoticity. These Lower East Siders utilize micro-variation of /r/ in stretches of interview talk, increasing non-rhoticity when discussing neighborhood topics. Results support a social practice approach to stylistic and sociolinguistic variation, where Lower East Siders use /r/ in constructing a place identity in order to present themselves as authentic neighborhood residents.     This article uses U.S. EPA's National Air Toxics Assessment (NATA) for 1996 to examine environmental inequality in California, a state that has been a recent innovator in environmental justice policy. We first estimate potential lifetime cancer risks from mobile and stationary sources. We then consider the distribution of these risks using both simple comparisons and a multivariate model in which we control for income, land use, and other explanatory factors, as well as spatial correlation. We find large racial disparities in California's "riskscape" as well as inequalities by other factors and suggest several implications for environmental and land use policy. [source]

    Abstract Summary: Vapor diffusion coefficients in polymeric membranes were evaluated from dynamic permeation experiments. A membrane separated the diffusion cell into two parts , upstream and downstream. At the start of the experiment the concentration change in the upstream part (feed side) was made by substituting the input stream of pure nitrogen by the stream of permeant vapors. The solution of the Fick's second law with the step input concentration function is used for the evaluation of diffusion coefficients. The realization of the step input function can be difficult and its imperfection can negatively influence the evaluation process. This contribution deals with the description of the experimentally obtained input function and the study of its influence on evaluated values of diffusion coefficients. The mathematical model, which includes the non-perfect step input concentration function and the transport through a polymer was developed. The results of this study enable the estimation of diffusion coefficient evaluation errors as dependence on the experimental arrangements and on the membrane transport properties.     Another Side of the Story: A Qualitative Case Study of Voting Behaviour in the European Parliament

    This study re-examines the conclusions drawn from existing research on legislative behaviour in the European Parliament (EP). Using written questionnaires and qualitative interviews with all 14 current Danish Members of the European Parliament (MEPs) and three former MEPs, existing academic findings on voting behaviour in the EP are compared and contrasted with ground-level experience from the MEPs' point of view. This study lends support to many of the conclusions drawn from previous quantitative research. However, it also shows that some of these findings cannot be fully understood without individual-level information. [source]


    Is There a Dark Side to Government Supportfor Nonprofits?

    The relationship between government social spending and private donations to the nonprofit sector is an issue that is relevant to both public administrators and nonprofit managers. Does government funding displace philanthropy, or encourage it? This article introduces the debate into the public administration literature. First, I survey and interpret the empirical work performed to date in this area by economists. Second, I retest this question across four nonprofit subsectors using data on both federal and state/local spending. My survey of the literature shows mixed results, although a broad pattern indicates that "crowding out" tends to dominate, particularly in the areas of social service provision and health. My empirical results are consistent with these findings, although they must be interpreted cautiously from a policy perspective: While results are statistically significant, the degree of crowding out is generally small. On the other hand, the claim that government funding stimulates giving seems to lack both statistical and policy significance. [source]

    This article examines the effect of plant entry and exit in a deregulated ,energy only' electricity market. A partial equilibrium framework is presented that determines the optimal portfolio of base, intermediate and peaking plant for a given electricity load curve. An optimal result for Queensland is compared against the actual plant stock. Analysis of the portfolio indicates that deregulation is failing a key objective, namely enhancing dynamic efficiency, because too much base plant has been delivered. The research presents scenarios of structural corrections, using the theory of the generalised war of attrition to develop the cases. Results from simulation experiments are clear,consumers will secure lower electricity prices in the short run. But oversupply of base plant may suppress prices to such an extent that they fail to signal timely entry of peaking plant,the consequence of this failure being eventual price shocks and, potentially, load shedding.     Is Laterality Associated With a Higher Rate of Hip Arthroplasty on the Dominant Side?

    Abstract:, The authors analyzed data collected by the Registry of the Orthopedic Prosthetic Implantology in Italy. They found a higher rate of total hip arthroplasty on the right side (58%) in comparison with the left side in patients affected by primary coxarthrosis. To test whether laterality was the cause of this, they checked the prevalence of the upper and lower limbs in 262 patients treated for monolateral total hip prosthesis. They found that the percentage of left-handed patients was very low (0.8,6.5%). The percentage of left-footed patients was, instead, 26.5% for power tasks. They observed that, while the rate of arthroplasties on the right side was similar to that of the left side (50.7 and 49.3%) in the right-footed patients, there was a clear-cut prevalence in the number of operations on the right hip (76.8%) in comparison to the left one (23.2%) in the left-footed patients.
