P. van Wilgen PT Abstract Background. Quality of life has become a major issue in determining the outcome of treatment in head and neck surgery with curative intent. The aim of our study was to determine which factors in the postoperative care, especially shoulder and neck morbidity, are related to quality of life and how these outcomes compared between patients who had undergone surgery and a control group. Methods. We analyzed physical symptoms, psychological symptoms, and social and functional well-being at least 1 year after surgery and evaluated the differences in quality of life between patients who had undergone head and neck surgery and a control group. Results. Depression scores contributed significantly to all domains of quality of life. Reduced shoulder abduction, shoulder pain, and neck pain are related to several domains of quality of life. The patient group scored significantly worse for social functioning and limitations from physical problems but scored significantly better for bodily pain and health changes. Conclusion. Depression and shoulder and neck morbidity are important factors in quality of life for patients who have undergone surgery for head and neck cancer. © 2004 Wiley Periodicals, Inc. Head Neck26: 839,844, 2004 [source] Erosion modelling approach to simulate the effect of land management options on soil loss by considering catenary soil development and farmers perceptionLAND DEGRADATION AND DEVELOPMENT, Issue 6 2008A. C. Brunner Abstract The prevention of soil erosion is one of the most essential requirements for sustainable agriculture in developing countries. In recent years it is widely recognized that more site-specific approaches are needed to assess variations in erosion susceptibility in order to select the most suitable land management methods for individual hillslope sections. This study quantifies the influence of different land management methods on soil erosion by modelling soil loss for individual soil-landscape units on a hillslope in Southern Uganda. The research combines a soil erosion modelling approach using the physically based Water Erosion Prediction Project (WEPP)-model with catenary soil development along hillslopes. Additionally, farmers' perceptions of soil erosion and sedimentation are considered in a hillslope mapping approach. The detailed soil survey confirmed a well-developed catenary soil sequence along the hillslope and the participatory hillslope mapping exercise proved that farmers can distinguish natural soil property changes using their local knowledge. WEPP-model simulations show that differences in soil properties, related to the topography along the hillslope, have a significant impact on total soil loss. Shoulder and backslope positions with steeper slope gradients were most sensitive to changes in land management. Furthermore, soil conservation techniques such as residue management and contouring could reduce soil erosion by up to 70 percent on erosion-sensitive slope sections compared to that under tillage practices presently used at the study site. [source] Surgical treatment of "terrible triad of the elbow": technique and outcomeORTHOPAEDIC SURGERY, Issue 2 2010Yu-xing Wang MD Objective:, To describe the authors' surgical technique and to evaluate the final functional outcome of surgical treatment of the "terrible triad of the elbow". Methods:, Eight patients identified with "terrible triad" injury patterns, including posterior elbow dislocation, radial head fracture and coronoid fracture, were available for a minimum of 11 months follow-up. Evaluation of functional outcome was based on Mayo elbow performance, Broberg-Morrey scores, and the Disabilities of the Arm, Shoulder and Hand (DASH) questionnaire. Complications were also recorded. Results:, Five elbows redislocated while in a splint after manipulative reduction. Three had residual subluxation after operative treatment. The final mean extent of forearm movement was as follows: 21° of extension deficit (range, 5° to 45°), 126° of flexion (range, 110° to 140°), 75° of supination (range, 45° to 90°), and 71° of pronation (range, 30° to 90°). The mean Mayo, Broberg-Morrey, and DASH scores were 78.0 ± 13.4, 76.0 ± 14.0, and 28.0 ± 24.7, respectively. Conclusions:, When an elbow joint is affected by the terrible triad, it is very unstable and prone to numerous complications. With operative treatment, the surgeon should attempt to perform internal fixation of the coronoid fracture, to regain normal radiocapitellar contact (either by preserving the radial head with open reduction and internal fixation (ORIF) or by replacing it with a prosthesis), and to repair the lateral collateral ligament (LCL). Thus early functional recovery and a successful final functional outcome can be achieved. [source] Role of Biphosphonates and Lymphatic Drainage Type Leduc in the Complex Regional Pain Syndrome (Shoulder,Hand Syndrome)PAIN MEDICINE, Issue 1 2009Andrea Santamato MD ABSTRACT Background., Complex regional pain syndrome (CRPS) is a clinical entity that has been termed in numerous ways in the last years. Clinically, CRPS describes an array of painful conditions that are characterized by a continuing (spontaneous and/or evoked) regional pain that is seemingly disproportionate in time or degree to the usual course of any known trauma or other lesion. The pain is regional and usually has a distal predominance of abnormal sensory, motor, sudomotor, vasomotor, and/or trophic findings. Design., Case report. Setting., University Medical Center. Patients., In this report, we described the case of a 68-year-old hemiplegic female affected by cerebrovascular accident that presented a clinical case of CRPS shoulder,hand syndrome (CRPS-SHS) at the right hand after a hemorrhagic stroke. Interventions., This report evaluated the effects of biphosphonates and lymphatic drainage type Leduc in CRPS-SHS. Outcome Measures., The pain level of the patients was measured with the visual analog scale. A scoring system for the clinical severity of CRPS-SHS, laboratory tests, and X-ray films were also performed. Results., We reported in this patient a great improvement of pain and edema of the right hand, with a significant reduction of bone demineralization. Conclusions., This combined treatment may be a viable alternative for this syndrome; however, further investigation is needed to determine its reproducibility in large case series. Trinkoff ScD Abstract Background Though musculoskeletal disorders (MSDs) are highly prevalent among registered nurses (RNs), little is known about functional consequences of MSDs in nurses. Methods Data on neck, shoulder, and back MSD problems were analyzed in 1,163 working nurses (response rate,=,74%). Cases had relevant symptoms lasting at least 1 week or occurring at least monthly in the past year, with at least moderate pain intensity, on average. MSD problems with a frequency, duration, or pain intensity below the level needed to meet the case definition were defined as MSD symptoms. Those who did not meet symptom or case criteria at any body site were defined as asymptomatic. Odds of consequences (e.g., saw a doctor/provider, missed work, reduced/modified work, non-work activities, or recreation, medication use, inadequate sleep) were estimated for cases versus those with symptoms. Results We found 45.8, 35.1, and 47.0% of nurses had neck, shoulder, or back MSD problems (either at the case or symptom level), respectively, within the past year. Cases were far more likely to have seen a provider versus those with symptoms (adjusted odds ratio, aOR Neck: 4.33, 95% CI: 2.85,6.56; aOR Shoulder: 4.83, 95% CI: 3.00,7.77; aOR Back: 3.69, 95% CI: 2.47,5.49). Cases also were more likely to experience all other functional consequences. Conclusions MSD consequences are substantial and suggest opportunities for intervention. [source] Functional impairment after latissimus dorsi flapANZ JOURNAL OF SURGERY, Issue 1-2 2009Cherry E. Koh Abstract Background:, Available published reports suggest that loss of latissimus dorsi muscle following latissimus myocutaneous flap does not lead to functional impairment. However, significant functional impairment has been observed in clinical practice, prompting this study to evaluate the functional deficit in patients who had undergone reconstruction using latissimus dorsi muscle. Methods:, A retrospective review was undertaken between 1998 and 2003. Patients were identified from the audit of the plastic surgery unit at St Vincent's Hospital and recruited by direct telephone contact. They were assessed using the Disability of Arm, Shoulder and Hand questionnaire. Results:, There were 25 participants. Eighteen completed their questionnaires. Six of 18 patients reported scores of 30 or more, which corresponded to a global moderate functional deficit. These patients were severely limited in their ability to carry out housework. Major postoperative complications were associated with high Disability of Arm, Shoulder and Hand scores (P = 0.028). Three of five patients involved with sports reported impaired performance in sports. The subgroup of four patients who underwent bilateral latissimus dorsi flaps also experienced significant morbidity with all four reporting moderate to severe impairment in their ability to return to gardening and/or sports. Conclusion:, Patients undergoing this procedure should be fully informed of potential sequelae. Athletic patients and patients undergoing bilateral latissimus dorsi flaps should also be warned of possible reduced performance in sports. Alternatively, perforator cutaneous flaps, which preserve the latissimus muscle, should be chosen when feasible. [source] Overcoming Abnormal Joint Torque Patterns in Paretic Upper Extremities Using Triceps StimulationARTIFICIAL ORGANS, Issue 3 2005Thierry Keller Abstract:, The goal of this research project was to quantitatively assess whether transcutaneous triceps stimulation can overcome the expression of abnormal torque patterns in the paretic upper limb of subjects with hemiparetic stroke. Abnormal torque patterns consist of strong coupling between shoulder abduction (SAB) and elbow flexion (EF) or between elbow extension (EE) and shoulder adduction (SAD) torques. Both patterns reduce the active range of motion during arm movements. Eight chronic stroke subjects with moderate to severe (Fugl-Meyer assessment scores of 21/66,36/66) upper limb motor impairment participated in this study. Shoulder and elbow joint torques were measured with a 6-degrees-of-freedom load cell under isometric conditions, while the triceps muscle was stimulated to generate EE torques. At the same time the subjects were asked to lift up their arm to generate different SAB torque levels. The obtained isometric results showed that electrical stimulation can overcome abnormal torque patterns in chronic stroke subjects while generating SAB. This is likely to have potential benefits to increase the, reaching, workspace, of, the, paretic, arm. [source] Intraarticular Lidocaine versus Intravenous Procedural Sedation with Narcotics and Benzodiazepines for Reduction of the Dislocated Shoulder: A Systematic ReviewACADEMIC EMERGENCY MEDICINE, Issue 8 2008Robert Warne Fitch MD Abstract Background:, Anterior shoulder dislocations commonly present to the emergency department (ED). The time associated with procedural sedation for the reduction of anterior shoulder dislocations can be lengthy and may require use of additional personnel. Complications associated with intravenous (IV) medications for procedural sedation are well documented. Objectives:, The aim was to determine if intraarticular lidocaine (IAL) injection is as effective as IV procedural sedation with narcotics and benzodiazepines for reduction of anterior shoulder dislocations. Methods:, This was a systematic review of randomized controlled trials (RCTs). The authors performed a PubMed, EMBASE, and Cochrane database search using key words: "shoulder dislocation" and "reduction" and retrieved every RCT published that compared the use of IV sedation to IAL as medication for reduction. Each manuscript was reviewed and the results of each was compared regarding medications used, success of reduction, complications, pain perceived, ease of reduction, and time spent in the ED. Results:, Six Level 1 RCTs were identified. No studies showed a statistically significant difference in success rate between IAL versus IV sedation. The complication rate was significantly higher in the IV sedation groups (p < 0.001), and the total time spent in the ED was longer for the IV sedation group. Conclusions:, The use of IAL for reduction of anterior shoulder dislocations should be strongly considered as a first line therapy because it is effective and safe and may potentially reduce time spent in the ED. [source] Exploring time-dependent symptom outcomes in office staffHUMAN FACTORS AND ERGONOMICS IN MANUFACTURING & SERVICE INDUSTRIES, Issue 3 2009Xiaoshu Lu This article illustrates the application of a new mathematical model developed for the study of time-dependent health outcomes for office staff during computer work. The model describes the time-dependent associations of computer usage with outcomes expressed as discomfort in multiple body regions. The association is explicitly presented with a functional relationship that is parameterized by body regions. The validation of the model demonstrated accuracy in reproducing the observed quantities for the study population. Therefore, we used this model to assess the impact of computer-related work exposure on discomfort in different body regions among office staff to better understand the behavior of musculoskeletal and other symptoms. The exposures and outcomes were recorded parallel in time as usage of keyboard and mouse and with diaries of discomfort. The body regions of neck/shoulders, eyes, head, shoulder joint/upper arm, and upper back were identified to have the highest discomfort levels and rates for the development of discomfort parallel with exposures. Most of our findings are consistent with the literature. The developed mathematical methodology may be used to understand how the human body reacts to computer work to further prevent potential musculoskeletal and other disorders. © 2009 Wiley Periodicals, Inc. [source] A Data-driven Segmentation for the Shoulder ComplexCOMPUTER GRAPHICS FORUM, Issue 2 2010Q Youn Hong Abstract The human shoulder complex is perhaps the most complicated joint in the human body being comprised of a set of three bones, muscles, tendons, and ligaments. Despite this anatomical complexity, computer graphics models for motion capture most often represent this joint as a simple ball and socket. In this paper, we present a method to determine a shoulder skeletal model that, when combined with standard skinning algorithms, generates a more visually pleasing animation that is a closer approximation to the actual skin deformations of the human body. We use a data-driven approach and collect ground truth skin deformation data with an optical motion capture system with a large number of markers (200 markers on the shoulder complex alone). We cluster these markers during movement sequences and discover that adding one extra joint around the shoulder improves the resulting animation qualitatively and quantitatively yielding a marker set of approximately 70 markers for the complete skeleton. We demonstrate the effectiveness of our skeletal model by comparing it with ground truth data as well as with recorded video. We show its practicality by integrating it with the conventional rendering/animation pipeline. [source] Preparation, structure and photoluminescence properties of SiO2,coated ZnS nanowiresCRYSTAL RESEARCH AND TECHNOLOGY, Issue 10 2010Changhyun Jin Abstract It is essential to passivate one-dimensional (1D) nanostructures with insulating materials to avoid crosstalking as well as to protect them from contamination and oxidation. The structure and influence of thermal annealing on the photoluminescence properties of ZnS-core/SiO2 -shell nanowires synthesized by the thermal evaporation of ZnS powders followed by the sputter deposition of SiO2 were investigated. Transmission electron microscopy and X-ray diffraction analyses revealed that the cores and shells of the core-shell nanowires were single crystal zinc blende-type ZnO and amorphous SiO2, respectively. Photoluminescence (PL) measurement showed that the core-shell nanowires had a green emission band centered at around 525 nm with a shoulder at around 385 nm. The PL emission of the core-shell nanowires was enhanced in intensity by annealing in an oxidative atmosphere and further enhanced by subsequently annealing in a reducing atmosphere. KGaA, Weinheim) [source] Long-term follow-up of children with obstetric brachial plexus palsy I: functional aspectsDEVELOPMENTAL MEDICINE & CHILD NEUROLOGY, Issue 3 2007Christina Strömbeck MD The aims of this study were to describe the development of sequelae in obstetric brachial plexus palsy (OBPP) and to identify possible differences in functional outcome from 5 years of age to follow-up, 2 to 15 years later. A cohort of 70 participants (35 males, 35 females; age range 7-20y, mean 13y 6mo [SD 4y 3mo], median 13y) with OBPP of varying degrees of severity were monitored. Differences in status between 5 years of age and follow-up were studied. Active joint motion in the shoulder and hand function, especially grip strength, generally remained unchanged or improved, whereas a slight but significant deterioration occurred in elbow function. Shoulder surgery resulted in considerable improvement of shoulder function. Participants with nerve reconstruction had a similar profile of change as the non-operated group. It was concluded that ongoing follow-up of children with OBPP, beyond the preschool years, is required due to decreases in elbow function, a commonly occurring restriction in external rotation of the shoulder, together with individual variations in long-term outcomes. [source] A new articulated hybodontoid from Late Permian of northwestern ChinaACTA ZOOLOGICA, Issue 2009N.-Z. Wang Abstract A new genus and species of hybodontoid elasmobranchs, Gansuselache tungshengi, is described on the basis of an articulated skeleton from the Fangshankou Formation (Late Permian) of the Mazongshan Mountain of Gansu Province, northwestern China. All dermal skeletons including dorsal fin-spines, cephalic spines, dermal denticles and one tooth are well preserved in their natural position. It is a shark of about 490 mm total length with a fusiform body. The braincase has large, downturned postorbital processes and otic capsules, no postorbital articulation, long palatoquadrate and large triangular Meckel's cartilage. It bears two dorsal fins and two pairs of heteromorphic cephalic spines. The tooth is of low-crowned, multicuspid type; the elongate tooth crown has stout crown shoulder and well-developed, rounded labial peg; the cusp and cusplets are moderately tumid, and have a few strong and curved folds; the tooth root is higher on the labial face than the lingual face. The dermal denticle is placoid scale-like, with its crown surface ornamented with some parallel ridges. Gansuselache, the first articulated hybodontiform from Asia, also represents the first complete hybodontiform from the Permian. [source] Multi-year tracking of sediment sources in a small agricultural watershed using rare earth elementsEARTH SURFACE PROCESSES AND LANDFORMS, Issue 14 2006A. Kimoto Abstract Rare earth elements (REEs) have been successfully used as a sediment tracer, but the REE technique has never been used for studying sediment sources for a multi-year period. A nearly four-year field experiment was conducted on a small agricultural watershed near Coshocton, OH, USA, to assess the applicability of the REE technique for a multi-year period and to evaluate the relative contributions of sediment sources in the watershed. Tracer depletion and tracer enrichment ratio (ratio of the tracer concentrations in sediment to the concentrations in the soil in the areas of application) were evaluated to examine the applicability and accuracy of the technique. A minimum of 91 per cent of the mass of the applied elements was still available on any individual morphological element at the end of the experimental period. The tracer enrichment ratio varied from 0·4 to 2·3, and it was not significantly related to time. The relative contributions of six morphological elements within the watershed were evaluated as proportions to total sediment yield. The relative contribution of the lower channel was significantly increased as a function of the amount of sediment yield, while that of the lower backslope was significantly decreased. The relative contribution of the lower channel significantly decreased as a function of cumulative sediment yield, while the contributions of the shoulder and the upper backslope significantly increased. Our results showed that the REE technique can be used to track sediment sources for a relatively long period with two limitations or potential sources of error associated with a selective depletion of tracers and a contamination of downslope areas with tagged sediments from upslope areas. [source] Sonovoltammetric Elucidation of Electron Transfer Rates: The Oxidation of Dimethyl- p -phenylenediamine in Aqueous SolutionELECTROANALYSIS, Issue 4 2003Craig Abstract The electrochemical oxidation of dimethyl- p -phenylenediamine (DMPD) in aqueous solution (pH 7 phosphate buffer) has been studied under conventional hydrodynamic and microelectrode voltammetric conditions and found to undergo a two-electron electrochemically reversible oxidation. Upon the application of ultrasound to the system an observed shoulder emerges in the oxidation wave. This effect has been attributed to the resolution of the two-electron transfer processes occurring: the first a relatively fast electron transfer (0.1,cm s,1) followed by a second slower (10,3 cm s,1) electron transfer: under the very high mass transport rates induced by insonation an overpotential develops for the second electron transfer so leading to the observed voltammetric resolution. Redding Summary Diagnosis of lameness associated with the shoulder region requires a careful clinical examination, the use of specifically placed intra-articular analgesia and a combination of some common imaging techniques to accurately define the source of pain. Most equine practices performing lameness examinations in the horse have the radiographic and ultrasonographic equipment necessary to accurately image the shoulder. This article presents a description of the unique anatomy of the shoulder and the specific application of radiographic and ultrasonographic techniques to provide a complete set of diagnostic images of the shoulder region. A brief discussion of nuclear scintigraphy of this region is also included. [source] Longitudinal development of equine conformation from weanling to age 3 years in the ThoroughbredEQUINE VETERINARY JOURNAL, Issue 7 2004T. M. ANDERSON Summary Reasons for performing study: There is little information available to define conformational changes with age using an objective but practical method of recording specific body measurements. Objective: To analyse conformation objectively in a population of racing Thoroughbreds and describe the changes from weanling to age 3 years. Methods: Annual photographs were taken over 4 years and conformation measurements made from photographs using specific reference points marked on the horses. Results: Correlation analysis revealed highly significant, moderate to strong relationships between long bone lengths and wither height for all ages. All long bone lengths showed moderate to strong relationships with each other for all ages. The front and rear pastern angles were significantly correlated with the angle of the dorsal surface of the front and rear hooves, respectively, for all. Wither height, croup height and length of neck topline, neck bottomline, scapula, humerus, radius and femur increased significantly from age 0,1 year and age 1,2 years. Hoof lengths (medial and lateral, right and left) grew significantly between the ages of 0 and 1 and 1 and 2 years, but decreased in length between age 2 and 3 years. Horses became more offset in the right limb between weanling and age 3 years, but the offset ratios did not change with age on the left limb. The angle of the scapula (I), shoulder and radiometacarpus significantly increased between all age groups (became more upright). The angle of the dorsal surface of the hooves (both front and hind) decreased significantly from ages 0 to 1 and 1 to 2 years, but showed no significant difference between ages 2 and 3 years. Conclusions: A strong relationship between long bone lengths and wither height for all ages supports the theory that horses are proportional. Longitudinal bone growth in the distal limb increased only 5,7% from weanling to age 3 years and is presumably completed prior to the yearling year. Several growth measures increased from ages 0 to 1 and 1 to 2 years, but did not increase from age 2,3 years; indicating that growth rate either showed or reached a plateau at this time. Potential relevance: This study provides objective information regarding conformation and skeletal growth in the Thoroughbred which can be utilised for selection and recognition of significant conformational abnormalities. [source] Clenbuterol administration does not attenuate the exercise-induced pulmonary arterial, capillary or venous hypertension in strenuously exercising Thoroughbred horsesEQUINE VETERINARY JOURNAL, Issue 6 2000M. MANOHAR Summary The present study was carried out to ascertain whether ,2 -adrenergic receptor stimulation with clenbuterol would attenuate the pulmonary arterial, capillary and venous hypertension in horses performing high-intensity exercise and, in turn, modify the occurrence of exercise-induced pulmonary haemorrhage (EIPH). Experiments were carried out on 6 healthy, sound, exercise-trained Thoroughbred horses. All horses were studied in the control (no medications) and the clenbuterol (0.8 ,g/kg bwt, i.v.) treatments. The sequence of these treatments was randomised for every horse, and 7 days were allowed between them. Using catheter-tip-transducers whose in-vivo signals were referenced at the point of the left shoulder, right heart/pulmonary vascular pressures were determined at rest, sub-maximal exercise and during galloping at 14.2 m/s on a 3.5% uphill grade - a workload that elicited maximal heart rate and induced EIPH in all horses. In the control experiments, incremental exercise resulted in progressive significant increments in right atrial as well as pulmonary arterial, capillary and venous (wedge) pressures and all horses experienced EIPH. Clenbuterol administration to standing horses caused tachycardia, but significant changes in mean right atrial or pulmonary vascular pressures were not observed. During exercise performed after clenbuterol administration, heart rate as well as right atrial and pulmonary arterial, capillary and wedge pressures also increased progressively with increasing work intensity. However, these values were not found to be statistically significantly different from corresponding data in the control study and the incidence of EIPH remained unaffected. Since clenbuterol administration also does not affect the transpulmonary pressure during exercise, it is unlikely that the transmural force exerted onto the blood-gas barrier of exercising horses is altered following i.v. clenbuterol administration at the recommended dosage. [source] Highly Efficient Visible-Light-Induced Photocleavage of DNA by a Ruthenium-Substituted Fluorinated PorphyrinEUROPEAN JOURNAL OF INORGANIC CHEMISTRY, Issue 11 2009Michelle Cunningham Abstract A new porphyrin, meso -5-(pentafluorophenyl)-10,15,20-tris(4-pyridyl)porphyrin, has been synthesized. Coordination of two [Ru(bipy)2Cl]+ moieties (where bipy = 2,2,-bipyridine) to the pyridyl nitrogen atoms in the 10,15-positions gives the target complex. Electronic transitions associated with the ruthenium,porphyrin include an intense Soret band and four less intense Q-bands in the visible region of the spectrum. An intense ,,,* transition in the UV region associated with the bipyridyl groups and a metal-to-ligand charge-transfer (MLCT) band appearing as a shoulder to the Soret band are also observed. Electrochemical properties associated with the complex include a redox couple in the cathodic region with E1/2 = ,0.84 V vs. Ag/AgCl attributed to the porphyrin and a redox couple in the anodic region at E1/2 = 0.79 V vs. Ag/AgCl due to the RuIII/II couple. DNA titrations and ethidium bromide displacement experiments indicate the ruthenium porphyrin interacts with DNA potentially through a partial intercalation mechanism. Norwood The limb girdle muscular dystrophies (LGMD) are termed as such as they share the characteristic feature of muscle weakness predominantly affecting the shoulder and pelvic girdles; their classification has been completely revised in recent years because of elucidation of many of the underlying genetic and protein alterations in the various subtypes. An array of diagnostic measures is possible but with varying ease of use and availability. Several aspects of muscle cell function appear to be involved in the causation of muscle pathology. These cellular variations may confer some specific clinical features thus permitting recognition of the LGMD subtype and hence directing appropriate levels of monitoring and intervention. Despite an extensive literature on the individual limb girdle dystrophies, these publications may be impenetrable for the general neurologist in this increasingly complex field. Larson Despite the fact that the shoulder is one of the most extensively studied regions in comparative primate and human anatomy, two recent fossil hominin discoveries have revealed quite unexpected morphology. The first is a humerus of the diminutive fossil hominin from the island of Flores, Homo floresiensis (LB1/50), which displays a very low degree of humeral torsion1, 2 (Fig. 1; see Box 1). Modern humans have a high degree of torsion and, since this is commonly viewed as a derived feature shared with hominoids,3,6 one would expect all fossil hominins to display high humeral torsion. The second is the recently discovered Australopithecus afarensis juvenile scapula DIK-1-1 from Dikika, Ethiopia, which seems to most closely resemble those of gorillas.7 This specimen is the first nearly complete scapula known for an early hominin and, given the close phylogenetic relationship between humans and chimpanzees suggested by molecular studies,8,13 one would have expected more similarity to chimpanzees among extant hominoids. Johnson The ancient Maya subsisted in an environment limited by shallow soils and unpredictable weather patterns until their collapse ,A.D. 800,900. Ancient subsistence can be a difficult subject, with little physical evidence of agricultural artifacts and structures. This study characterized soil profiles and utilized changes in stable carbon isotope ratios of soil organic matter (SOM) to locate and interpret areas of ancient C4 plant growth and maize (Zea mays) cultivation among the Maya. The investigation indicated some of the challenges the Maya faced, including shallow and sloped soils in some areas. The C4 plant signature was found in seasonal wetland soils on the opposite side of the Laguneta Aguateca from the ruins of Aguateca, but not in the perennial wetlands on the immediate side. No C4 plant signature was detected in the shoulder and backslope soils. Based on these findings, the ancient Maya of Aguateca probably adapted to their environment by farming rich toeslope soils. It is possible that maize was also grown in the seasonal wetlands adjacent to the site. Bauer In East Africa, the feedback between tectonic uplift, erosional denudation and associated possible climate changes is being studied by a multidisciplinary research group, ,Riftlink'. The group's focus is the Albertine Rift, the northern part of the western branch of the East African Rift System, and in particular the rising Rwenzori Mountains that stretch along the border of the D.R. Congo and Uganda. Major questions relate to the timing of the formation of the Rwenzori Mountains, and whether the height of these mountains (> 5000 m) relates to rift movements in Neogene times, or represents an old basement block that formed a topographic high long before. Though, at first, research concentrated on the eastern (Ugandan) part of the Albertine Rift and Rwenzori Mountains, it has now moved further to the west to the D.R. Congo. A first field-campaign, covering the area from northern Lake Edward along the rift shoulder up to the Blue Mountains at Lake Albert, was conducted in summer 2009, in cooperation with the Ruwenzori State University of Butembo. Here, we present a brief overview of the field-campaign, with impressions gathered on the morphology and geology of the study area. D'YOUNG Summary., A conservative, non-operative physiotherapeutic regime for the management of chronic haematomata and pseudotumours in patients suffering from haemophilia is described in this article. Two cases are described where physiotherapy treatment is applied to large masses at the shoulder and femur respectively, where therapy commenced within the first 6 months following onset. These are presented relative to a case that was managed over a much longer period without early physiotherapy input, and the relative outcomes are examined. While both the early physiotherapy-managed cases showed a complete resolution at follow-up examination, the more established chronic pseudotumour required surgical excision, with significant residual muscle contractility, length and strength issues noted on clinical and magnetic resonance imaging reviews. No adverse symptoms or haemostatic issues were reported in response to this less invasive treatment regime by either patient in the two conservative physiotherapy cases. Although the lower limbs are predominantly affected due to their load-bearing nature, upper limb disease is common. Surgical management, including total joint arthroplasty, can be undertaken if conservative management fails. A search of the literature showed that a number of studies describing the use of total knee arthroplasty (TKA) and total hip arthroplasty (THA) in haemophilia have been published, whereas shoulder, elbow and ankle arthroplasties are confined to case reports. This paper reviews the functional outcome of arthroplasty in the different joints, the postoperative and long-term complications, and the impact of HIV. Although complications are commonly described and the surgery is technically demanding, the results suggest that arthroplasty, particularly of the hip and knee, can be a valuable option in the management of severe haemophilic arthropathy. To identify and rate the importance of several psychosocial and physiologic influences on quality of life (QOL) among a cohort of 5-year head and neck cancer survivors, we conducted a cross-sectional study of a convenience sample that used both questionnaires and physiologic evaluations. Methods. QOL was assessed by the Functional Assessment of Cancer Therapy (FACT) and the FACT Head and Neck additional concerns (FACT-H&N) questionnaires. Psychosocial characteristics (or risk factors) potentially influencing QOL were measured by the Millon Behavioral Health Inventory (MBHI) and the Social Support Questionnaire (SSQSR). Physiologic risk factors were measured in examinations that included shoulder and neck range of motion, whole and stimulated saliva measurements, and oropharyngeal swallowing efficiency. We evaluated the association of selected QOL measures with three groups of potential risk factors: psychosocial factors, consisting of selected MBHI and SSQSR scales; physiologic factors, consisting of selected physical ability measures; and a combination of psychosocial/physiologic factors. Results. The entire study population of 105 subjects completed the FACT and FACT-H&N questionnaires; 86 of these completed the physiologic tests as well. Combined psychosocial/physiologic models best predicted all QOL measures considered. Psychosocial models alone, compared with physiologic models alone, better predicted FACT physical and social/family well-being measures. Physiologic models alone, compared with psychosocial models alone, better predicted FACT-H&N additional concerns measures. Premorbid pessimism (MBHI) was consistently the best predictor of QOL measures. Conclusions. Both psychosocial and physiologic factors influence QOL in patients with head and neck cancer, but many QOL measures are most strongly influenced by psychosocial considerations. The aim of our study was to determine which factors in the postoperative care, especially shoulder and neck morbidity, are related to quality of life and how these outcomes compared between patients who had undergone surgery and a control group. Methods. We analyzed physical symptoms, psychological symptoms, and social and functional well-being at least 1 year after surgery and evaluated the differences in quality of life between patients who had undergone head and neck surgery and a control group. Results. Depression scores contributed significantly to all domains of quality of life. Reduced shoulder abduction, shoulder pain, and neck pain are related to several domains of quality of life. The patient group scored significantly worse for social functioning and limitations from physical problems but scored significantly better for bodily pain and health changes. Conclusion. Dijkstra PhD Abstract Background It is the purpose of this study to determine the incidence of shoulder pain and restricted range of motion of the shoulder after neck dissection, and to identify risk factors for the development of shoulder pain and restricted range of motion. Methods Clinical patients who underwent a neck dissection completed a questionnaire assessing shoulder pain. The intensity of pain was assessed using a visual analog scale (100 mm). Range of motion of the shoulder was measured. Information about reconstructive surgery and side and type of neck dissection was retrieved from the medical records. Results Of the patients (n = 177, mean age 60.3 years [SD, 11.9]) 70% experienced pain in the shoulder. Forward flexion and abduction of the operated side was severely reduced compared to the non-operated side, 21° and 47°, respectively. Non-selective neck dissection was a risk factor for the development of shoulder pain (9.6 mm) and a restricted shoulder abduction (55°). Reconstruction was risk factor for a restricted forward flexion of the shoulder (24.5°). Conclusions Shoulder pain after neck dissection is clinically present in 70% of the patients. Non-selective neck dissection is a risk factor for shoulder pain and a restricted abduction. Stemberga Abstract A case of primary non-Hodgkin lymphoma of the right humerus which occurred in a 21-year-old male patient after an impact to the right shoulder in a car accident in July 1983 is described. Seventeen years after the injury, due to a civil lawsuit, the biopsy material was revised. Immunohistochemical analysis showed CD20 and CD79a positivity on large pleomorphic cells, while small reactive lymphocytes were CD3, Bcl-2 and CD20 positive. Molecular analysis carried out with PCR revealed a monoclonal B-lymphocyte population. The diagnosis of diffuse large peripheral B cell lymphoma of the bone was confirmed. The present case concurs with the literature on primary bone lymphoma, in which the diagnostic problem, trauma-related presentation and an excellent prognosis of malignant tumour are emphasized.
