Unlocking the opportunity of tight glycaemic control

Inhaled insulin: clinical efficacy
Numerous attempts have been made to develop novel routes of insulin delivery that are both effective and tolerable. Of all the potential non-invasive delivery options, pulmonary delivery is the most clinically viable. Early studies demonstrate that the inhaled insulin is rapidly absorbed and is closer to biological insulin than standard subcutaneous insulin (SC). To date, inhaled insulin (Exubera®) has been clinically assessed in more than 3500 patients with type 1 or type 2 diabetes, some treated for more than 7 years. Several phase 3 studies of 24-week duration have demonstrated comparable glycosylated haemoglobin (HbA1c) control in patients with type 1 diabetes treated with Exubera® vs. SC insulin. Similar results have also been recorded in patients with type 2 diabetes. Furthermore, Exubera® has shown clinical superiority to oral agent regimens in patients with type 2 diabetes who failed to achieve their target HbA1c using lifestyle modification and oral agents. Exubera® was well tolerated and treatment satisfaction was high, with Exubera® being the preferred insulin therapy in all studies. The results of these trials, and others, suggest that Exubera® may be a valuable tool to help a wide variety of patients with type 1 or type 2 diabetes reach their recommended goals for glycaemic control, irrespective of their current therapy. Mexico's Community-Managed Forests as a Global Model for Sustainable Landscapes

On the other hand, conservationists have declared that the only way to stem the tide of deforestation is to place as many tracts as possible under strict protection. In this context, Mexico presents a national laboratory for studying the social and ecological benefits of delivering forests to local people. As a little-noticed result of the Mexican Revolution in the second decade of the twentieth century, well over half of the forests of Mexico were placed in community-held lands. In historic struggles that passed through several phases, most of these communities have now gained substantial control over the use of their forests. Because of the substantial degree of social capital in rural forms of organization in Mexico, this control of forest resources has led to an estimated 290,479 community forest enterprises ( CFEs ), through which communities are producing timber on their own lands. New studies are beginning to suggest that important gains in both social and economic justice, good forest management, and biodiversity protection are resulting from the actions of these CFEs. As more forests globally are being devolved to local communities, it is important to carry out more research on the Mexican model of community forest management for timber production. Resumen: Investigadores preocupados por el manejo sostenible de bosques en los trópicos han argumentado que el camino para una custodia mas efectiva de los recursos forestales es la transferencia de la responsabilidad a las comunidades locales que obtienen sustento de ellos. Por otro lado, conservacionistas han declarado que la única manera de detener la ola de deforestación es colocar bajo protección estricta tantas regiones como sea posible. En este contexto, México representa un laboratorio para el estudio de los beneficios sociales y ecológicos de entregar los bosques a los habitantes locales. Como un resultado poco conocido de la Revolución Mexicana, en la segunda década del siglo veinte, más de la mitad de los bosques de México se ubicaban en tierras que estaban en manos de las comunidades. Las comunidades forestales han atravesado por distintas etapas de conflicto, tras la cuáles han obtendio un control sustancial de los usos de sus bosques. Debido al nivel considerable de capital social en formas de organización rural en México, este control de los recursos forestales ha conducido a la integración de entre 290,479 empresas forestales comunitarias ( EFC ), en las que las comunidades están produciendo madera en sus propias tierras. Nuevos estudios están comenzando a sugerir que se están produciendo ganancias importantes en las acciones de estas EFC están generando beneficios importantes, tanto en lo que se refiere a justicia social como económica, la administración correcta de bosques y la protección de la biodiversidad. A medida en que se deleguen mayores extensiones de bosques a comunidades locales, es importante llevar a cabo más investigación sobre el modelo mexicano de manejo comunitario de bosques para la producción de forestal maderable. Civil Society and the State: Turkey After the Earthquake

On 17 August 1999 Turkey was hit by a massive earthquake. Over 17,000 lives were lost and there was extensive damage to Turkey's heartland. This paper examines how various public and private institutions, including state and civil society institutions such as NGOs and the media responded to the needs of earthquake survivors. It documents the extensive involvement of NGOs in the relief efforts immediately after the disaster and examines the impact of such participation on state-civil society relations in the country. The data show that state response to the disaster went through several phases from a period of ineptitude to effective management. The paper credits the media and the NGOs for acting as advocates for survivors and forcing changes at the state level. The paper argues that an ideal response system, which fully addresses the needs of victims, can only be based on state-civil society relations that are both collaborative and adversarial. [source]


Silica as a shock index in shergottites: A cathodoluminescence study

Determining its structural state as either silica glass, quartz, cristobalite, tridymite, coesite, stishovite, or post-stishovite could provide informations about their shock history. The purpose of this work is to assess the shock intensity in shergottites using two spectroscopic methods. On a conventional polished section, a scanning electron microscope (SEM) enables us to study the cathodoluminescence (CL) of silica at variable magnification. The results were crosschecked by systematic Raman spectroscopy of the selected areas. CL spectra differ substantially from one another and enable separating stishovite, high and low pressure silica glass, quartz, and cristobalite. We studied a set of five shergottites: Northwest Africa (NWA) 480, NWA 856, Zagami, Shergotty, and Los Angeles. Stishovite is common in Shergotty, Zagami, NWA 856, and NWA 480 and absent in the studied section of Los Angeles. High-pressure glass is very common, particularly in close association with stishovite. According to the textural relationship, it may be a product of the retromorphosis (amorphization during decompression) of stishovite. Large stishovite areas result from the transformation of preexisting low-pressure silica crystals, while needles result from the high-pressure transformation of pyroxene to glass (melt) and silica. In the latter case, they are found in melt pockets and represent a small fraction of areas of overall pyroxene composition. Needles exhibit square sections of about 1 ,m. Silica spots identical to those described previously as post-stishovite are found in Shergotty, Zagami, NWA 480, and NWA 856. At present, the spectroscopic distinction of post-stishovite from stishovite is difficult. Post-stishovite is destroyed under the Raman beam, and CL spectra are possible mixtures of several phases (e.g., glass and post-stishovite). It is concluded that the shock intensity is highly heterogeneous, and the pressure probably exceeded 60 GPa in all shergottites studied here. [source]


Effect of V addition on magnetic, transport, and microstructural properties of La0.7Sr0.3MnO3

Abstract The microstructural, magnetic, and magnetotransport properties of polycrystalline samples with nominal composition of La0.7Sr0.3Mn1,xVx O3 (0 , x , 0.25) sintered in air have been investigated. It is found that V could not substitute for Mn to form La0.7Sr0.3Mn1,xVx O3. The samples contain several phases such as (La, Sr)Mn1,,O3, LaSr3V3O12, Mn3O4 etc. and constitute multi-phase composites. The Sr,V-rich phase form a liquid phase during sintering process and the sintering be long to liquid-phase sintering, which may effectively promote grain growth of (La, Sr)Mn1,,O3 phase. The Curie temperature TC and saturation magnetization of the composites decrease with increasing the V content x. The addition of V may effectively improve the low-field MR response at low temperatures due to the variation in the microstructure of the composites. Secular change in heights of indigenous adults from a Zapotec-speaking community in Oaxaca, southern Mexico

Abstract Secular change in adult height of residents in a rural indigenous community in the Valley of Oaxaca was evaluated. Subjects were measured in 1971 (49 males, 26 females 19,70 years), 1978 (128 males, 124 females 19,82 years) and 2000 (155 males, 255 females 19,89 years). Heights were adjusted for estimated loss with age using two protocols; height at 21 years of age was also estimated. The effects of age and secular factors on measured and adjusted heights were evaluated through segmented linear regressions for three birth periods, <1930, 1930 through 1959 and ,1960 which approximate significant periods in Mexican history. Secular increase in height occurred but estimated rates varied over time and between sexes. Males born before 1930 showed a secular increase in height but females did not. Adults of both sexes born 1930,1959 showed secular gains and estimated rates did not differ. The secular gain in height continued among those born 1960 and later and estimated rates were similar in both sexes. Estimated height at 21 years of age increased in males (not significant) but not in females born before 1930, showed little or no change in those born between 1930-1959, and increased (not significant) in those born 1960 and later. Combining observations on adults with those for youth in the community indicated several phases of secular change in height that varied with years of birth. Dynamics of bidirectional transport of Arc mRNA in neuronal dendrites

Abstract The mRNA for Arc (activity-regulated cytoskeletal protein) is delivered into dendrites and localizes selectively at active synapses. Here we use a green fluorescent protein-based labeling system and confocal microscopy to define the transport kinetics of exogenously expressed mRNA from chimaeric Arc constructs (Arc/MS2 mRNA) in the dendrites of living rat neurons in culture. Arc/MS2 mRNA assembles into particles that move independently, bidirectionally, and intermittently in a fashion indicative of transport. Transport velocities range from below 6 to 65 ,m/minute, which is consistent with actin-based and microtubule-based transport, respectively. In general, orthograde translocations are longer than retrograde translocations. Rapidly translocating Arc/MS2 mRNA particles sometimes reverse direction and decrease velocity just before stopping, suggesting that local signals regulate Arc mRNA targeting movements. Phylogeography of spiny mice (genus Acomys, Rodentia: Muridae) from the south-western margin of the Sahara with taxonomic implications

The present study aims to depict the overall pattern of Acomys history in south-western Sahara. We tested the specific predictions that: (1) several mitochondrial clades can be identified and that they coincide with the described species; (2) successive phases of desert expansion and contraction during the last 3 Myr have resulted in several phases of demographic expansion and population fragmentation in Acomys; and (3) isolation-by-distance occurs. The extent of phylogeographic patterns and molecular genetic diversity (cytochrome b gene and D-loop) were addressed in a survey of 90 individuals of Acomys from 38 localities. Our phylogeographical analyses show a strong genetic structure within western Saharan Acomys, with several phylogroups displaying non-overlapping geographic distributions. Restricted gene flow with isolation-by-distance was recorded and a signal of population expansion was detected within several clades. We suggest that during arid or semi-arid paleoclimatic periods, when large sandy areas were present, Acomys was restricted to rocky massifs, whereas, in more humid periods, when savannah and/or steppe habitat prevailed, this species was able to disperse and to survive outside rocky areas because food resources were abundant. Based on a comprehensive sampling and the use of an integrative approach (i.e. combining cytogenetic, molecular and morphological data), we firmly propose that Acomys airensis should be considered as a junior synonym of Acomys chudeaui. © 2009 The Linnean Society of London, Biological Journal of the Linnean Society, 2009, 98, 29,46. Optimism, Pessimism, and Coalitional Presidentialism: Debating the Institutional Design of Brazilian Democracy

Research on Brazil's political institutions has gone through several phases since democratisation in 1985. In the early years of democracy, pessimism prevailed with regard to governability. This view gave way in the mid-1990s to a more optimistic view that stressed two innovations of the Constitution of 1988: enhanced presidential power and centralised legislative procedure. In recent years, a third phase of research has shifted attention to the crucial role of inter-party alliances. These analytical approaches have converged into an emerging research programme on ,coalitional presidentialism', which places executive-legislative relations at the centre stage of macropolitical analysis. This article reviews the three phases of the debate and reflects on future research agendas. [source]
