West Coast study of childhood brain tumours and maternal use of hair-colouring products

Summary The immature nervous system of the fetus is characterised by rapid cell growth and division and is particularly vulnerable to carcinogens and mutagens. Several epidemiological studies have reported an increased risk for childhood brain tumours (CBT) associated with exposure to N-nitroso compounds (NOC). Hair-colouring products (hair ,dyes') that contain NOC-related aromatic amines have shown mutagenicity in vitro and carcinogenic properties in vivo. The potential public health impact of the relationship between hair dye use and carcinogenesis has prompted epidemiological research, given that a large proportion of American women have used hair dyes. A large population-based case,control study was conducted on the west coast of the USA to investigate risk factors for CBT including exposure to NOC. Eligible CBT patients (<20 years of age and diagnosed between 1984 and 1991) were identified from cancer registries in Los Angeles County, the San Francisco Bay Area in California and the Seattle area in Washington state. A total of 540 biological mothers of these children were interviewed, and 801 control subjects who were frequency matched to the CBT patients on birth year and sex were obtained using random digit dialling. Mothers were asked details about personal use of hair dyes during the index pregnancy including frequency of use, trimester of use and type of dye used. Results from age- and sex-adjusted unconditional logistic regression analyses showed no association between risk for CBT and use of hair dyes 1 month before and/or during pregnancy nor during specific trimesters. A nearly twofold increased risk for CBT was associated with single-interval use during the 1 month before pregnancy, but the confidence interval (CI) was imprecise and the estimate was not different from unity (OR = 1.9, 95% CI [0.5, 7.0]). Exclusive use of permanent dye, temporary dye or hair darkeners was not associated with risk for CBT. A twofold increased risk (OR = 2.0, 95% CI [0.83, 4.7]) was observed with exclusive use of semi-permanent dye during the month before or during pregnancy. Exclusive use of semi-permanent dye during the month before pregnancy and/or first trimester also was associated with an elevated risk for CBT, again not different from unity and with an imprecise CI (OR = 2.5, 95% CI = [0.58, 10.3]). There was no evidence of an association between risk for CBT by histological subtypes and use of hair dyes during the index pregnancy or the month before conception. Together with results from previous studies, these results provide no consistent evidence of an association between risk for CBT and use of hair dyes during pregnancy. [source]


Actual asthma control in a paediatric outpatient clinic population: Do patients perceive their actual level of control?

Several epidemiological studies described poor asthma control in children. However, the diagnosis of childhood asthma in these studies is uncertain, and asthma control in children of an outpatient clinic population during treatment by a paediatrician is unknown. (1) to investigate the hypothesis that asthma control in a paediatric outpatient clinic population is better than epidemiological surveys suggest; (2) to find possible explanations for suboptimal asthma control. Asthmatic children aged 6,16 years, known for at least 6 months by a paediatrician at the outpatient clinic, were selected. During a normal visit, both the responsible physicians and parent/children completed a standardised questionnaire about asthma symptoms, limitation of daily activities, treatment, asthma attacks and emergency visits. Overall, excellent asthma control of 8.0% in this study was not significantly better than of 5.8% in the European AIR study (Chi-square, p = 0.24). Separate GINA goals like minimal chronic symptoms and no limitation of activities were better met in our study. Good to excellent controlled asthma was perceived by most children/parents (83%), but was less frequently indicated by the paediatrician (73%), or by objective criteria of control (45%) (chi-square, p = 0.0001). The agreement between patient-perceived and doctor assessed control was low, but improved in poorly controlled children. Patients were not able to perceive the difference between ,excellent asthma control' and ,good control' (p = 0.881). Too little children with uncontrolled disease got step-up of their asthma treatment. Although separate GINA goals like ,minimal chronic symptoms' and ,no limitation of activities' were significantly better in our study, overall, asthma control in this outpatient clinic population, treated by a paediatrician, was not significantly better than in the European AIR study. Poorly controlled disease was related to several aspects of asthma management, which are potentially accessible for improvements. [source]


Exposure to respirable dust and crystalline silica in bricklaying education at Dutch vocational training centers

Abstract Background Construction workers are educated at vocational training centers before they begin their working lives. Future bricklayers and their instructors are exposed to respirable dust and possibly to hazardous respirable crystalline silica from trial mortar. Methods Thirty-six personal air samples were collected at six training centers to estimate exposure to respirable dust for both students and teachers. A selection of 22 samples was analyzed for crystalline silica. Results Average respirable dust exposures ranged from 0.59,mg/m3 for teachers to 1.45,mg/m3 for students performing recycling and cleaning tasks. In 45% of the analyzed samples, respirable crystalline silica was detected. Exposure to silica remained below the Dutch OEL (75,µg/m3). Exposure was significantly less for teachers than for students. This effect was found in both types of vocational training centers present in the Netherlands. Dry sweeping, as performed at all locations in this study, contributed considerably to the exposure to respirable dust. A first step in reducing exposure to dust and silica at training centers would therefore be to avoid dry sweeping. The presence of a dust extraction system, although not optimally designed, also significantly lowered exposure. Conclusions To assess a construction worker's lifetime exposure to respirable dust and crystalline silica, the vocational training period should also be taken into account. Physical activity reduces prostate carcinogenesis in a transgenic model

Abstract BACKGROUND Several epidemiological studies have reported an inverse association between physical activity and the risk of prostate cancer. To date, there are few animal studies looking at physical activity and cancer incidence, although the results are consistent with the epidemiological evidence. In general, as exercise intensity increased in the rats/mice, the likelihood that physical activity inhibited carcinogenesis increased. METHODS The present study used voluntary wheel running with C3(1)Tag mice that are predisposed to prostate cancer due to the directed expression of SV40 oncogenes. After 10 weeks, the prostates were collected from run and non-run mice and histopathology performed for the presence or absence of low grade or high grade PINS. RESULTS We found that for those mice that ran >5Km/group, 83% of the dorsolateral prostates were classified as within normal levels vs. 43% for the <5 km/day group (p,=,0.16). In addition, there was a relationship between average distance run and pathologic progression to high-grade PIN and local invasion, considered to be an early event in the formation of prostate adenocarcinoma. Forty three percent of dorsalateral prostates from mice that ran less than 5 km/day were classified with advanced pathology as compared to none (0) in mice that ran more than 5.0 km/day (p,=,0.05). CONCLUSIONS The results presented herein indicate that prostatic cancer progression is likely delayed or diminished by wheel running activity in a dose dependent manner in transgenic mice. These findings provide further evidence that exercise acts to decrease the progression of prostate cancer and they establish a transgenic animal model for future studies to define mechanism(s). Testicular dysgenesis syndrome: foetal origin of adult reproductive problems

Summary The evidence for the existence of testicular dysgenesis syndrome (TDS) is presented in this review. Several epidemiological studies have shown that conditions like cryptorchidism, impaired spermatogenesis, hypospadias and testicular cancer can be associated as risk factors for each other. Thus, the risk of testis cancer is significantly increased in men with cryptorchidism and/or infertility. Several recent studies point towards early dysgenesis of the foetal testis as the biological link between these disorders. Dysgenesis has been demonstrated in biopsies of the contralateral testis of men with testis cancer and in infertile men. The histological evidence includes immature seminiferous tubules with undifferentiated Sertoli cells, microliths and Sertoli-cell only tubules. Dysgenetic testes often have an irregular ultrasound pattern, where microliths may also be visible. Our current hypothesis is that maternal exposure to endocrine disrupting chemicals may contribute to the pathogenesis of TDS. Animal experiments have shown that all TDS symptoms, except testicular cancer, can be induced by foetal exposure to anti-androgenic chemicals. However, the cause of TDS in humans remains to be determined. [source]


Analysis of the effects of ultrafine particulate matter while accounting for human exposure

Abstract Particulate matter (PM) has been associated with mortality in several epidemiological studies. The US EPA currently regulates PM10 and PM2.5 (mass concentration of particles with diameter less than 10 and 2.5 µm, respectively), but it is not clear which size of particles are most responsible for adverse heath outcomes. A current hypothesis is that ultrafine particles with diameter less than 0.1 µm are particularly harmful because their small size allows them to deeply penetrate the lungs. This paper investigates the association between exposure to particles of varying diameter and daily mortality. We propose a new dynamic factor analysis model to relate the ambient concentrations of several sizes of particles with diameters ranging from 0.01 to 0.40 µm with mortality. We introduce a Bayesian model that converts ambient concentrations into simulated personal exposure using the EPA's Stochastic Human Exposure and Dose Simulator, and relates simulated exposure with mortality. Using new data from Fresno, CA, we find that the 4-day lag of particles with diameter between 0.02 and 0.08 µm is associated with mortality. This is consistent with the small particles hypothesis. Flaxseed attenuates the tumor growth stimulating effect of soy protein in ovariectomized athymic mice with MCF-7 human breast cancer xenografts

Abstract In several epidemiological studies, a phytoestrogen-rich diet containing lignans and isoflavones is associated with reduced breast cancer risk, but experimental findings are controversial. In postmenopausal mammary cancer xenograft model, flaxseed (FS), a rich source of plant lignans, reduced breast cancer growth, while soy protein (SP), a rich source of isoflavones, enhanced it. The intake of phytoestrogens is increasing particularly among postmenopausal women, emphasizing the importance of elucidating their interactive effects on breast cancer. Our study determined the effect of FS and SP diets, alone and in combination, on the established human breast cancer MCF-7 tumor growth in ovariectomized athymic nude mice. Tumor bearing mice were divided into 4 groups and fed for 25 weeks either the basal diet (BD), or BD supplemented with 10% FS, 20% SP or 10% FS and 20% SP. After estrogen deprivation, FS regressed the tumor size similar to that of control. SP initially regressed the tumors but starting at week 13, the tumors regressed significantly less than in control and 43% of the tumors were regrowing until the end of the experiment and were significantly larger in size than in control. The combination of SP with FS reduced the tumor growth similar to that of control, as suggested also by the reduced tumor cell proliferation index. In conclusion, dietary FS did not stimulate the growth of estrogen responsive MCF-7 cancers in ovariectomized mice, while long-term consumption of SP did. Furthermore, FS reduced the tumor growth stimulating effect of SP to the same level as control, suggesting tumor growth attenuating effect of FS. © 2006 Wiley-Liss, Inc. [source]


PPAR: a therapeutic target in Parkinson's disease

Abstract Parkinson's disease (PD) is a progressive and chronic neurodegenerative disorder, characterized by progressive loss of dopaminergic neurons in substantia nigra. The etiology and pathogenesis of PD is still elusive, however, a large body of evidence suggests a prominent role of oxidative stress, inflammation, apoptosis, mitochondrial dysfunction and proteosomal dysfunction in the pathogenesis of PD. Due to multifactorial nature of the disease, currently available drug therapy cannot halt / slow down the disease progression, and only provides symptomatic relief. Peroxisome proliferator-activated receptor (PPAR), a member of nuclear receptor superfamily, regulates development, tissue differentiation, inflammation, mitochondrial function, wound healing, lipid metabolism and glucose metabolism. Recently, several PPAR agonists were shown to exert neuroprotective activity against oxidative damage, inflammation and apoptosis in several neurodegenerative disorders including Alzheimer's, Parkinson's, Huntington's, amyotrophic lateral sclerosis and multiple sclerosis. Similarly, regular intake of PPAR activating non-steroidal anti-inflammatory drugs such as indomethacin and ibuprofen was associated with reduced incidence and progression of neurodegenerative disorders in several epidemiological studies. In this article, we review studies relating to the neuroprotective effect of PPAR agonists in in vitro and in vivo models of PD. Similarly, the pharmacological mechanism in neuroprotective actions of PPAR agonists is also reviewed. In conclusion, PPAR agonists exert neuroprotective actions by regulating the expression of a set of genes involved in cell survival processes, and could be a therapeutic target in debilitating neurodegenerative illnesses such as PD.
