Errors in the Interpretation of Mohs Histopathology Sections Over a 1-Year Fellowship

BACKGROUND Errors can occur in the interpretation of Mohs histopathology sections. Errors in histology interpretation can lead to incomplete removal of cancer and cancer persistence or the unnecessary removal of uninvolved tissue. Extensive proctored training is necessary to reduce these errors to an absolute minimum level. OBJECTIVE To analyze and quantify the number of cases and the amount of time required to reach a satisfactory level of expertise in the reading and interpretation of Mohs histopathology. METHODS A single-institution pilot study was designed to track errors in the interpretation and mapping of Mohs histopathology sections. A Mohs surgery fellow independently preread Mohs cases and rendered his interpretation on the Mohs map. One of the Mohs program directors subsequently reviewed and corrected all cases. Errors were scored on a graded scale and tracked over the 1-year fellowship to determine the number of cases and amount of time necessary to reduce errors to a baseline minimal level. RESULTS One thousand four hundred ninety-one Mohs surgery cases were required to generate 1,347 pathology specimens for review and grading over 6 months of Mohs surgery fellowship before reducing errors to a minimum acceptable level of less than 1 critical error per 100 cases read. CONCLUSIONS The number of cases and time required to reduce errors in the interpretation of Mohs histology is substantial. Direct and immediate mentored correction of errors is essential for improvement. These results can act as a guide for Mohs surgery training programs to help determine the minimum number of directly proctored cases required to obtain expertise in this crucial component of Mohs surgery. Patients' satisfaction with outpatient psychiatric care

The purpose of this study was to describe patient satisfaction with outpatient psychiatric care in two community care clinics in Finland. Data were collected using a structured questionnaire from 300 outpatients between February and March 2000. A total of 171 patients returned completed questionnaires (response rate 57%). The data analysis was based on descriptive statistics, chi-square test, the Mann,Whitney U -test and the Kruskal,Wallis test. Item homogeneity was analysed using Cronbach alpha coefficient. The results showed that patient satisfaction was highest in areas pertaining to staff and care discussions, and lowest in areas pertaining to information. Patients who considered the wait for treatment to be too long were more dissatisfied with staff, their chance of influencing their own care, and the help they received, compared with patients who considered the wait to be reasonable in length. We can conclude that although patients are quite satisfied with their outpatient treatment in general, the patient information process is not at a satisfactory level at outpatient psychiatric clinics. In the future, more emphasis should be put on developing more innovative methods to increase psychiatric patients' knowledge level regarding their own illness and treatment. [source]


Water Resources Modeling of the Ganges-Brahmaputra-Meghna River Basins Using Satellite Remote Sensing Data,

Abstract:, Large-scale water resources modeling can provide useful insights on future water availability scenarios for downstream nations in anticipation of proposed upstream water resources projects in large international river basins (IRBs). However, model set up can be challenging due to the large amounts of data requirement on both static states (soils, vegetation, topography, drainage network, etc.) and dynamic variables (rainfall, streamflow, soil moisture, evapotranspiration, etc.) over the basin from multiple nations and data collection agencies. Under such circumstances, satellite remote sensing provides a more pragmatic and convenient alternative because of the vantage of space and easy availability from a single data platform. In this paper, we demonstrate a modeling effort to set up a water resources management model, MIKE BASIN, over the Ganges, Brahmaputra, and Meghna (GBM) river basins. The model is set up with the objective of providing Bangladesh, the lowermost riparian nation in the GBM basins, a framework for assessing proposed water diversion scenarios in the upstream transboundary regions of India and deriving quantitative impacts on water availability. Using an array of satellite remote sensing data on topography, vegetation, and rainfall from the transboundary regions, we demonstrate that it is possible to calibrate MIKE BASIN to a satisfactory level and predict streamflow in the Ganges and Brahmaputra rivers at the entry points of Bangladesh at relevant scales of water resources management. Simulated runoff for the Ganges and Brahmaputra rivers follow the trends in the rated discharge for the calibration period. However, monthly flow volume differs from the actual rated flow by (,) 8% to (+) 20% in the Ganges basin, by (,) 15 to (+) 12% in the Brahmaputra basin, and by (,) 15 to (+) 19% in the Meghna basin. Our large-scale modeling initiative is generic enough for other downstream nations in IRBs to adopt for their own modeling needs. [source]


Azimuthally symmetric theory of gravitation , I. On the perihelion precession of planetary orbits

ABSTRACT From a purely non-general relativistic standpoint, we solve the empty space Poisson equation (,2,= 0) for an azimuthally symmetric setting (i.e. for a spinning gravitational system like the Sun). We seek the general solution of the form ,=,(r, ,). This general solution is constrained such that in the zeroth-order approximation it reduces to Newton's well-known inverse square law of gravitation. For this general solution, it is seen that it has implications on the orbits of test bodies in the gravitational field of this spinning body. We show that to second-order approximation, this azimuthally symmetric gravitational field is capable of explaining at least two things: (i) the observed perihelion shift of solar planets; (ii) the fact that the mean Earth,Sun distance must be increasing (this resonates with the observations of two independent groups of astronomers, who have measured that the mean Earth,Sun distance must be increasing at a rate between about 7.0 ± 0.2 m century,1 and 15.0 ± 0.3 m cy,1). In principle, we are able to explain this result as a consequence of the loss of orbital angular momentum; this loss of orbital angular momentum is a direct prediction of the theory. Further, we show that the theory is able to explain at a satisfactory level the observed secular increase in the Earth year (1.70 ± 0.05 ms yr,1). Furthermore, we show that the theory makes a significant and testable prediction to the effect that the period of the solar spin must be decreasing at a rate of at least 8.00 ± 2.00 s cy,1. Persistence and efficacy of two diatomaceous earth formulations and a mixture of diatomaceous earth with natural pyrethrum against Tribolium confusum Jacquelin du Val (Coleoptera: Tenebrionidae) on wheat and maize

Abstract Laboratory tests were conducted to assess the insecticidal and residual effects of three diatomaceous earth (DE) formulations, Insecto®, PyriSec® and SilicoSec®, against Tribolium confusum Jacquelin du Val on wheat and maize. Quantities of wheat and maize were treated with the above formulations at 500, 1000 and 1500 mg kg,1 and stored at 25 °C and 55% relative humidity (RH). Samples were taken on the day of storage and every 30 days until completion of a 360 day period of storage. Adults of T. confusum were exposed to these samples at 25 °C and 55% RH and the mortality was measured after 24 and 48 h and 7 and 14 days of exposure. Adult mortality was higher on wheat than on maize. At the beginning of the storage period, mortalities after 14 day exposure on maize treated with the highest rate were 60, 63 and 81% for Insecto®, PyriSec® and SilicoSec® respectively, while on wheat the mortality was 100% for all DEs. On the same commodities 360 days after treatment the respective mortality figures for wheat were 99, 98 and 100%, while in the case of maize they did not exceed 7%. Generally, mortality at exposures ,48 h decreased with increasing storage time. Furthermore, although mortality on wheat increased with dose, the increase in DE dose from 1000 to 1500 mg kg,1 resulted in only a small increase in mortality. Thus a DE treatment of 1000 mg kg,1 was shown to provide long-term protection of wheat against T. confusum, although higher DE application rates and exposure intervals are needed for a satisfactory level of protection of maize against this pest. Making agricultural landscapes more sustainable for freshwater biodiversity: a case study from southern England

Abstract 1.Agriculture is known to have a range of deleterious impacts on freshwater habitats and biota and many countries have introduced measures to attempt to mitigate these impacts through agri-environment initiatives. Despite the protection they provide, water bodies (any discrete body of surface fresh water) in farmland landscapes commonly remain impaired by agriculture. In some areas of the UK there have been calls to halt farming completely, indicating that the measures offered for the widespread protection of aquatic systems, particularly the use of buffer strips, may not be extensive enough to provide sufficient protection for freshwater biota. 2.This study investigated whether existing agri-environment measures for the widespread protection of aquatic habitats could be better deployed to provide a higher level of protection for the aquatic macrophytes and macroinvertebrates of a study area in southern England. 3.Reserve selection procedures were used to reallocate the area of land that could be remunerated under the Environmental Stewardship scheme as buffer strips bordering water bodies, so that a higher level of protection was provided for both the richness and rarity of aquatic species in the study area. 4.Almost 395 ha were available for reallocation in the reserve selection process, which was found to provide a satisfactory level of protection for up to 90% of the surveyed species. 5.The results showed that the agri-environment scheme in England has a great deal of potential to provide more effective protection for the aquatic biodiversity of agricultural landscapes if measures are targeted. The Presence of a Ferrocenyl Unit on an Estrogenic Molecule is Not Always Sufficient to Generate in,vitro Cytotoxicity

Abstract We recently reported the dual (antihormonal and cytotoxic) functionality of ferrocifens, which are organometallic complexes derived from hydroxytamoxifen, the standard molecule in the treatment of hormone-dependent breast cancers. To test the hypothesis that the presence of a ferrocenyl substituent on molecules with an affinity for the estrogen receptor is sufficient to give them cytotoxic properties in,vitro, we prepared complexes derived from estradiol with a ferrocenyl substituent at positions 7, and 17,. The complexes thus obtained retain a satisfactory level of affinity for the estrogen receptor (RBA values higher than 12,%). At low concentrations (0.1,1,,M) the complexes show an estrogenic effect in,vitro equivalent to that of estradiol on hormone-dependent (MCF-7) breast cancer cells, and no cytotoxic effect on hormone-independent (MDA-MB-231) breast cancer cells. At high concentrations (up to 50,,M) the 17, -ethynylferrocenyl estradiol and 7, -ferrocenylmethylthio estradiol become cytotoxic (IC50=13.2,,M and 18.8,,m, respectively) while the 17, -ferrocenylestradiol remains non toxic. The low toxicity of these compounds support our hypothesis that electronic communication between the ferrocenyl and phenol moieties in the hydroxyferrocifens series is a key parameter in the generation of cytotoxic effects at submicromolar concentrations. Extraction of media and plaque boundaries in intravascular ultrasound images by level sets and min/max flow

Abstract: Estimation of the plaque area in intravascular ultrasound images after extraction of the media and plaque,lumen interfaces is an important application of computer-aided diagnosis in medical imaging. This paper presents a novel system for fully automatic and fast calculation of plaque quantity by capturing the surrounding ring called media. The system utilizes an algorithm that consists of an enhanced technique for noise removal and a method of detecting different iso levels by sinking the image gradually under zero level. Moreover, an important novelty with this technique is the simultaneous extraction of media and lumen,plaque interfaces at satisfactory levels. There are no higher dimensional surfaces and evolution of contours, stopping at high image gradients. Thus, the system runs really fast with curvature velocity only and has no complexity. Experiments also show that this shape-recovering curvature term not only removes the noisy behaviour of ultrasound images but also strengthens very weak boundaries and even completes the missing walls of the media. In addition, the lumen,plaque interface can be detected simultaneously. For validation, a new and very useful algorithm is developed for labelling of intravascular ultrasound images, taken from video sequences of 15 patients, and a comparison-based verification is done between manual contours by experts and the contours extracted by our system.
