Heights of occupied patient beds: a possible risk factor for inpatient falls

Aims., The aim of this study was to ascertain the average height of occupied patient beds in a general medical ward and to investigate the relationship between staff working-height for patient beds, time and whether the patient was on fall precaution. Background., The height of occupied patient beds can be an overlooked contributor to inpatient falls. Better physical design of hospital equipment such as patient beds may reduce patient falls and injuries. Methods., This study took place in an acute medical ward of a Michigan medical center. One researcher collected all the data and used the same metric for all the measurements. Univariate analyses were performed. Results., The average staff working-height measurement taken at the weekend was significantly higher than that taken on weekdays. The average height of patient beds on fall precaution was significantly higher than of those not on fall precaution. Conclusions., A higher patient/nurse ratio at weekends than on weekdays may result in fewer bedside nursing hours and nurses being less conscientious about keeping beds in the low position after treatments. In an effort to prevent high-fall-risk patients from falling, nurses may have consciously or unconsciously kept their beds in higher positions. Relevance to clinical practice., If the patient bed can be manually or automatically adjusted, nurses must lower the height of the bed to the lowest position after completing treatments or tasks. This after-procedure activity should be enforced and monitored regularly as part of a hospital's patient fall prevention programme. Low beds should be used for patients at high risk of falling. Future research should investigate patients' and staff's views on hospital equipment to provide evidence-based information for policy-makers determining the design-regulation standard for hospital bedframes. The impact of lianas on 10 years of tree growth and mortality on Barro Colorado Island, Panama

Summary 1. Lianas compete intensely with trees, but few studies have examined long-term effects of liana infestation on tree growth and mortality. We quantified the effects of lianas in tree crowns (n = 2907) and rooted within 2 m of trees (n = 1086) on growth and mortality of 30 tree species from 1995 to 2005 on Barro Colorado Island (BCI), Panama, documented liana infestation in tree crowns in 1996 and 2007 to determine the dynamics of liana infestation, and quantified liana infestation in the crowns of 3231 additional canopy trees (d.b.h. ,20 cm) in 2007 to compare with the same metric determined by previous studies in 1967 and 1980. 2. Severe liana infestation increased tree mortality: 21% of liana-free trees in 1996 had died by 2007, whereas 42% of trees with more than 75% of the crown infested by lianas in 1996 had died by 2007. 3. Liana infestation of tree crowns significantly reduced tree growth, particularly on sun-exposed trees. The proximity of rooted lianas significantly reduced the growth of shaded trees. 4. Liana infestation was dynamic: 10.9% of trees with severe liana infestation in their crowns in 1996 had shed all of their lianas by 2007 and 5.3% of trees with no lianas in their crown in 1996 had severe liana infestation in 2007. 5. Liana infestation was common: lianas were present in 53% of trees of the 30 focal species. Including lianas rooted within 2 m of the tree increased this percentage to 78%. Using both above- and below-ground measures may provide a better estimate of liana competition than either measure alone. 6. Liana infestation is increasing on BCI. Lianas were present in the crowns of 73.6% of canopy trees (d.b.h. ,20 cm). Liana canopy infestation was 57% higher than in 1980 and 65% higher than in 1967, which is consistent with reported increases in liana abundance, biomass, and leaf and flower production. 7.,Synthesis. We used one of the largest studies ever conducted on lianas to confirm the negative effects of lianas on tree growth and survival over 10 years. Biotic affinities of rocky reef fishes, invertebrates and macroalgae in different zones of the Port Davey marine protected area, south-western Tasmania

Abstract 1.Assemblages of fishes, invertebrates, and macroalgae showed strong and predictable distributional patterns within the newly declared Port Davey marine protected area (MPA) in south-western Tasmania. Biotic assemblages in the eastern estuarine section of the MPA within Bathurst Channel were extremely anomalous, both in relation to biota elsewhere in the Port Davey region and also to those present along the wider Tasmanian and Australian coasts. Much of this variation was due to the phenomenon of deepwater emergence, with species in 5,m water depth in eastern Bathurst Channel possessing a mean maximum recorded depth of 200,m, compared with<80,m for the same metric when calculated for sites studied elsewhere around Australia. Deepwater emergence in Bathurst Channel was particularly notable for sessile organisms, although also evident among fishes and mobile macro-invertebrates. 2.Quantitative baseline surveys of reef-associated species were undertaken at sites interspersed among MPA management zone types and biotic community types, thereby providing an appropriate benchmark for assessing ecological changes in different management zones within the Port Davey region through the long term. Distinctive biota present in eastern and western Bathurst Channel, and eastern Port Davey, are well protected within ,no-take' sanctuary zones; however, a bias in location of sanctuary zones towards areas with little fishery resources resulted in less protection for the western Port Davey biota, which also extends along the open coast. Although the lack of high level protection for sites with fishery resources detracts from conservation goals, the Port Davey MPA nevertheless represents a major advance in environmental protection because the ecologically unique, fully protected locations are a necessary inclusion within any comprehensive Australian MPA network. Factorial Invariance Within Longitudinal Structural Equation Models: Measuring the Same Construct Across Time

Abstract, Charting change in behavior as a function of age and investigating longitudinal relations among constructs are primary goals of developmental research. Traditionally, researchers rely on a single measure (e.g., scale score) for a given construct for each person at each occasion of measurement, assuming that measure reflects the same construct at each occasion. With multiple indicators of a latent construct at each time of measurement, the researcher can evaluate whether factorial invariance holds. If factorial invariance constraints are satisfied, latent variable scores at each time of measurement are on the same metric and stronger conclusions are warranted. This article discusses factorial invariance in longitudinal studies, contrasting analytic approaches and highlighting strengths of the multiple-indicator approach to modeling developmental processes.
