    Serum markers of chronic dehydration are associated with saliva spinability

    Summary, Findings of a relationship between saliva and dehydration have been observed, but the precise nature of these relationships is unclear and no evidence of a direct link has been found. In particular, no study reports a relationship between chronic dehydration and saliva conditions in community-dwelling older adults. This study aimed to identify whether salivary conditions are sensitive to body hydration markers in an elderly population. A total of 403 subjects aged 76 years participated in the study. Stimulated saliva flow rate and spinability of saliva were measured. In addition, determinations of serum levels of uric acid, blood urea nitrogen (BUN), creatinine, sodium and potassium were made. Dehydration was defined as uric acid , 7 mg dL,1 according to the standard value. The salivary spinability were significantly associated with the concentration of uric acid (OR=2·06, P=0·044) according to multiple logistic regression analysis. In addition, after adjusting for gender, the uric acid concentration and the salivary spinability was significantly associated with BUN, potassium and creatinine levels. The subjects with high uric acid levels (,,7 mg dL,1) had the most elastic saliva. Both BUN and serum creatinine are the most commonly used indicators of renal function. Therefore, our findings might demonstrate that older adults who are dehydrated showed highly elastic saliva, which was associated with renal function. In conclusion, this study suggests that there is a significant relationship between chronic dehydration status and salivary spinability level. [source]


    Unstimulated whole saliva flow rate in relation to sicca symptoms in Hungary

    Objective:, To assess the prevalence of xerostomia and the related oral and extraoral dryness symptoms in Hungary, to evaluate the association of those symptoms with the unstimulated whole saliva (UWS) flow rate, and to find correlation between the level of UWS flow rate and the oral health status of the questioned patients. Subjects And Methods:, A total of 600 patients between the age of 18 and 92 years, 265 male, 335 female, were selected in accordance with the current regional age and residence distribution scheme of the Hungarian Statistical Office. A questionnaire was designed to determine the subjective presence or absence of the sicca symptoms. UWS flow rate and the dental and periodontal status were determined. Results:, The percentages of subjective symptoms in the questioned subjects were oral dryness, 34%; reduced salivation, 11%; mucous saliva, 15%; dysphagia, 13%; glossopyrosis, 7%; dysphonia, 31%; dysgeusia, 9%; nasal dryness, 32%; ocular dryness, 21%; itching, 40% and xeroderma, 60%. Vaginal dryness was 14%, vaginal itching was 16% in the interviewed women. The grade of xerostomia, dysphagia, tiredness, and additionally the gingival bleeding index showed a negative correlation with the UWS flow rate. After all decayed, missing and filled teeth (DMF-T) mean values, gingival bleeding index and plaque index were significantly higher in hyposalivators, compared with those who had normal flow rates Conclusion:, This cross sectional study, representative of the Hungarian population, clearly shows that one-third of the adult population suffers from xerostomia. The clinical severity of the xerostomia demonstrated a strong relationship with the lower levels of UWS flow rate. Reduced levels of UWS flow rate in this study were also shown to be associated with dysphagia, fatigue, and increased DMF-T numbers. The data show that oral dryness, its associated desiccation symptoms and its clinical manifestations are significant health problems in Hungary. [source]


    Stimulated whole salivary flow rate and composition in menopausal women with oral dryness feeling

    The aim of this study was to compare stimulated whole saliva flow rate and composition of menopausal women with/without oral dryness (OD) feeling. A case,control study was carried out in 42 selected menopausal women aged 52,73 years with or without OD feeling (21 as case and 21 as control) conducted at the Clinic of Oral Medicine, Tehran University of Medical Sciences. Paraffin-stimulated saliva samples were obtained by expectoration. The stimulated whole saliva composition was measured by a spectrophotometer [magnesium (Mg+2), calcium (Ca+2), chloride (Cl,), inorganic phosphate (Pi) and total protein], flame-photometry [sodium (Na+)] and ion selective electrode (ISE) [potassium (K+)] methods. No significant differences were found in stimulated whole saliva flow rate, Mg+2, Cl,, Pi, Na+, K+ and total protein concentrations between the two groups, but the mean calcium concentration was significantly higher in cases than in controls (P = 0.003). It seems that the level of salivary calcium concentration may be higher in menopausal women with OD feeling than in the control group. [source]


    Pilocarpine for the treatment of salivary glands' impairment caused by radioiodine therapy for thyroid cancer

    Objectives:, To study the effect of single-dose pilocarpine hydrochloride 5 mg on the whole unstimulated and stimulated salivary flow in patients suffering from thyroid cancer treated with radioiodine therapy, and to investigate the changes in vital signs during the treatment. Subjects and methods:, Five such patients were referred with complaints of dry mouth, rampant caries, and pain in the parotid gland region or history of chronic recurrent suppurative sialodenitis. A single dose of 5 mg pilocarpine hydrochloride was administered to each patient and blood pressure, heart rate, body temperature and salivary secretion rate were monitored at 1, 2 and 3 h. Results:, A significant elevation of unstimulated and stimulated saliva flow rate was observed in four patients without significant alteration of the monitored vital signs. Conclusions:, Treatment with pilocarpine hydrochloride may be beneficial in the case of impaired salivary function in patients treated with radioiodine. [source]


    Effectiveness of rituximab treatment in primary Sjögren's syndrome: A randomized, double-blind, placebo-controlled trial,

    Objective To study the efficacy and safety of B cell depletion with rituximab, a chimeric murine/human anti-CD20 monoclonal antibody, in patients with primary Sjögren's syndrome (SS) in a double-blind, randomized, placebo-controlled trial. Methods Patients with active primary SS, as determined by the revised American,European Consensus Group criteria, and a rate of stimulated whole saliva secretion of ,0.15 ml/minute were treated with either rituximab (1,000 mg) or placebo infusions on days 1 and 15. Patients were assigned randomly to a treatment group in a ratio of 2:1 (rituximab:placebo). Followup was conducted at 5, 12, 24, 36, and 48 weeks. The primary end point was the stimulated whole saliva flow rate, while secondary end points included functional, laboratory, and subjective variables. Results Thirty patients with primary SS (29 female) were randomly allocated to a treatment group. The mean ± SD age of the patients receiving rituximab was 43 ± 11 years and the disease duration was 63 ± 50 months, while patients in the placebo group were age 43 ± 17 years and had a disease duration of 67 ± 63 months. In the rituximab group, significant improvements, in terms of the mean change from baseline compared with that in the placebo group, were found for the primary end point of the stimulated whole saliva flow rate (P = 0.038 versus placebo) and also for various laboratory parameters (B cell and rheumatoid factor [RF] levels), subjective parameters (Multidimensional Fatigue Inventory [MFI] scores and visual analog scale [VAS] scores for sicca symptoms), and extraglandular manifestations. Moreover, in comparison with baseline values, rituximab treatment significantly improved the stimulated whole saliva flow rate (P = 0.004) and several other variables (e.g., B cell and RF levels, unstimulated whole saliva flow rate, lacrimal gland function on the lissamine green test, MFI scores, Short Form 36 health survey scores, and VAS scores for sicca symptoms). One patient in the rituximab group developed mild serum sickness,like disease. Conclusion These results indicate that rituximab is an effective and safe treatment strategy for patients with primary SS. [source]


    Salivary mucin as related to oral Streptococcus mutans in elderly people

    MG1 (MUC5b and MUC4 ) and MG2 (MUC7), predominant mucins in human whole saliva, provide lubrication and antimicrobial protection for oral tissues. This study examines potential relationships between Streptococcus mutans titers in the oral cavity and the following: mucin concentrations; unstimulated and stimulated whole saliva flow rates; decayed, missing, and filled tooth surfaces; and age of 24 elderly patients. S. mutans titers were determined using Denticult SM. Mucin concentrations were determined using Stains-all, sodium dodecyl sulfate,polyacrylamide gel electrophoresis. Logistic regression was used to identify potential relationships between the above variables. S. mutans classification served as the dependent variable. The remaining variables were possible predictor variables. The best model for predicting S. mutans category contained log MG2 as a predictor variable for all of its parameter estimates. No other set of parameter estimates were statistically significant. These results suggest that elevated S. mutans titers are significantly associated with diminished concentrations of MG2 in unstimulated whole saliva, as quantified in mucin-dye binding units. [source]
