Selected AbstractsThe impact of personal characteristics on engagement in nursing home residents with dementiaINTERNATIONAL JOURNAL OF GERIATRIC PSYCHIATRY, Issue 7 2009Jiska Cohen-Mansfield Abstract Objective To examine the impact of personal attributes on engagement in persons with dementia. Methods Participants were 193 residents of seven Maryland nursing homes. All participants had a diagnosis of dementia. Cognitive functioning was assessed via the Mini-Mental State Examination, and engagement was assessed via the Observational Measure of Engagement. Data pertaining to activities of daily living were obtained from the Minimum Data Set. Results Women had longer mean engagement duration than men, and significant results were not seen with the other demographic variables. Significant, positive correlations were found between higher cognitive functioning and longer engagement duration, more attention, a more positive attitude, and a higher refusal rate. There was a positive and significant correlation between the comorbidity index and engagement duration, and between the number of medications and attention. All functional status variables yielded significance in a positive direction. Participants with poor hearing had a higher refusal rate. Cognitive status was the most consistent and potent predictor of engagement in this population. Conclusion Despite a higher refusal rate among those with higher cognitive levels, their overall engagement with stimuli is higher. Caregivers should anticipate higher refusal rates in those with poor hearing, and therefore compensatory methods should be used in presenting stimuli in this population. The potent role of cognitive and functional status on engagement of persons with dementia underscores the importance of tailoring activities to nursing home residents' needs, interests, and limitations. Rodnick Streamside measurements of critical thermal maxima (Tcrit), swimming performance (Ucrit), and routine (Rr) and maximum (Rmax) metabolic rates were performed on three populations of genetically distinct redband trout Oncorhynchus mykiss in the high-desert region of south-eastern Oregon. The Tcrit values (29·4 ± 0·1° C) for small (40,140 g) redband trout from the three streams, and large (400,1400 g) redband trout at Bridge Creek were not different, and were comparable to published values for other salmonids. At high water temperatures (24,28° C), large fish incurred higher metabolic costs and were more thermally sensitive than small fish. Ucrit(3·6 ± 0·1 LF s,1), Rr(200 ± 13 mg O2 kg,0·830 h,1) and metabolic power (533 ± 22 mg O2 kg,0·882 h,1) were not significantly different between populations of small redband trout at 24° C. Rmax and metabolic power, however, were higher than previous measurements for rainbow trout at these temperatures. Fish from Bridge Creek had a 30% lower minimum total cost of transport (Cmin), exhibited a lower refusal rate, and had smaller hearts than fish at 12-mile or Rock Creeks. In contrast, no differences in Ucrit or metabolism were observed between the two size classes of redband trout, although Cmin was significantly lower for large fish at all swimming speeds. Biochemical analyses revealed that fish from 12-mile Creek, which had the highest refusal rate (36%), were moderately hyperkalemic and had substantially lower circulating levels of free fatty acids, triglycerides and albumin. Aerobic and anaerobic enzyme activities in axial white muscle, however, were not different between populations, and morphological features were similar. Results of this study: 1) suggest that the physiological mechanisms that determine Tcrit in salmonids are highly conserved; 2) show that adult (large) redband trout are more susceptible to the negative affects of elevated temperatures than small redband trout; 3) demonstrate that swimming efficiency can vary considerably between redband trout populations; 4) suggest that metabolic energy stores correlate positively with swimming behaviour of redband trout at high water temperatures; 5) question the use of Tcrit for assessing physiological function and defining thermal habitat requirements of stream-dwelling salmonids like the redband trout. [source] Exception from Informed Consent Enrollment in Emergency Medical Research: Attitudes and AwarenessACADEMIC EMERGENCY MEDICINE, Issue 2 2007Wayne Triner DO Objectives To explore attitudes surrounding exception from informed consent enrollment into research studies. In addition, the authors sought to determine the level of awareness of such an ongoing study among potential subjects, as defined by their presence in an emergency department (ED). Methods A convenience sample of urban academic ED patients and visitors was surveyed during a visit regarding their attitudes and awareness of an emergency exception from informed consent, blood-substitute trial ongoing in the community. Results There was a 13% refusal rate, and 32% of those approached had characteristics that met exclusion criteria. There were 497 surveys analyzed. There was a predominance of women, Caucasians, and persons with at least some college education. Only 39 (8%) of respondents reported awareness of the ongoing blood substitute trial, and only 19 (4%) were able to list a risk or benefit of participation. Education, income, and age were not associated with reported awareness. Male gender, younger age, awareness of the existing exception from informed consent study, and being married were associated with greater acceptability for such enrollment practices. Conclusions The overall awareness of an ongoing exception from informed consent trial after community consultation and notification was low. A population with potential for enrollment in such a study did not demonstrate a high degree of acceptance of such practices. There were differences among certain demographic groups in the degree of acceptance. These differences may guide institutional review boards and investigators in community-consultation strategies for future waiver of or exception from informed consent studies.
