    A Mixed Modelling Approach for Randomized Experiments with Repeated Measures

    Abstract Repeated measurements on the same experimental unit are common in plant research. Due to lack of randomization and the serial ordering of observations on the same unit, such data give rise to correlations, which need to be accounted for in statistical analysis. Mixed modelling provides a flexible framework for this task. The present paper proposes a general method to formulate mixed models for designed experiments with repeated measurements. The approach is exemplified by way of several examples. [source]


    Calibrated peer review for computer-assisted learning of biological research competencies

    Abstract Recently, both science and technology faculty have been recognizing biological research competencies that are valued but rarely assessed. Some of these valued learning outcomes include scientific methods and thinking, critical assessment of primary papers, quantitative reasoning, communication, and putting biological research into a historical and broader social context. This article presents examples of Calibrated Peer Review (CPR) assignments that illustrate a computer-assisted method to help students achieve biological research competencies. A new release of CPR is appropriate for engaging students online in reading and writing about investigations. A participant perception inventory was designed for use as a repeated measure to discriminate among beginning, middle, and ending student perceptions. Examples are provided to demonstrate how to assess student perceptions of what they gain from instruction related to science research competencies. Results suggest that students in a large enrollment class consider CPR to be useful for helping them learn about quantitative and categorical research variables; the use of the experimental method to test ideas; the use of controls; analysis, interpretation, and presentation of data; and how to critically read primary papers.     Considering effective divorce mediation: Three potential factors

    The purpose of this exploratory, qualitative stgdy was to examine mediator efect by employing a repeated measures research design in which we videotaped mediators working with actors and a scripted divorce case scenario. What factors distinpish higher-rated mediators from lower-rated ones? Our discourse analysys of four divorce mediations suggested three signijcant factors of injuence that finction interactively and afect both mediation outcome and process. Ethical implications regarding how mediators achieve success in these three domains are discussed. This article presents implications for researchers, trainers, and practitioners; it suggests important directions for fiture research with nonsimulated mediation. [source]


    New onsets of substance use disorders in borderline personality disorder over 7 years of follow-ups: findings from the Collaborative Longitudinal Personality Disorders Study

    ABSTRACT Aims The purpose of this study was to examine whether patients with borderline personality disorder (BPD) have a higher rate of new onsets of substance use disorders (SUD) than do patients with other personality disorders (OPD). Design This study uses data from the Collaborative Longitudinal Personality Disorder Study (CLPS), a prospective naturalistic study with reliable repeated measures over 7 years of follow-up. Setting Multiple clinical sites in four northeastern US cities. Participants A total of 175 patients with BPD and 396 patients with OPD (mean age 32.5 years) were assessed at baseline and at 6, 12, 24, 36, 48, 60, 72 and 84 months. Measurements The Structured Clinical Interview for DSM-IV Axis I Disorders and the Diagnostic Interview for DSM-IV Personality Disorders were used at baseline, the Follow-Along version of the DIPD-IV and the Longitudinal Interval Follow-up Evaluation at the follow-up evaluations. Kaplan,Meier analyses were calculated to generate the time to new onsets. Findings BPD patients showed a shorter time to new onsets of SUD. Thirteen per cent of BPD patients developed a new alcohol use disorder and 11% developed a new drug use disorder, compared to rates of 6% and 4%, respectively, for OPD. Non-remitted BPD and remitted BPD patients did not differ significantly in rates of new onsets of SUD. Conclusions BPD patients have a high vulnerability for new onsets of SUDs even when their psychopathology improves. These findings indicate some shared etiological factors between BPD and SUD and underscore the clinical significance of treating SUD when it co-occurs in BPD patients. [source]


    Effects of oral electrolyte supplementation on endurance horses competing in 80 km rides

    Summary Reasons for performing study: There is no evidence that use of oral electrolyte pastes enhances performance in competing endurance horses. Objective: To ascertain whether oral administration of a high dose (HD) of sodium chloride (NaCl) and potassium chloride (KCl) to endurance horses would differentially increase water intake, attenuate bodyweight (bwt) loss and improve performance when compared to a low dose (LD). Methods: A randomised, blinded, crossover study was conducted on 8 horses participating in two 80 km rides (same course, 28 days apart). Thirty minutes before and at 40 km of the first ride 4, horses received orally 0.2 g NaCl/kg bwt and 0.07 g KCl/kg bwt. The other 4 received 0.07 g NaCl/kg bwt and 0.02 g KCl/kg bwt. Horses received the alternate treatment in the second ride. Data were analysed with 2-way ANOVA for repeated measures (P<0.05). Results: Estimated water intake was significantly greater with HD both at the 40 km mark and as total water intake; however, differences in bwt loss and speed between HD and LD were not found. Treatment significantly affected serum Na+, Cl,, HCO3, pH and water intake, but not serum K+ or bwt. Serum Na+ and Cl, were significantly higher at 80 km when horses received HD, but no differences were found in early recovery. Venous HCO3 and pH were significantly lower throughout the ride and in early recovery when horses received HD. Conclusions and potential relevance: Other than enhancing water intake, supplementing endurance horses with high doses of NaCl and KCl did not provide any detectable competitive advantage in 80 km rides. Further, the elevated serum electrolyte concentrations induced with HD might not be appropriate for endurance horses. [source]


    Oral vitamin E supplementation on oxidative stress, vitamin and antioxidant status in intensely exercised horses

    Summary Reasons for performing study: Vitamin E is the most commonly supplemented antioxidant in horses; however, previous research is not conclusive as to the recommended level for exercising horses. Objective: To evaluate the effects of 3 levels of vitamin E supplementation on oxidative stress and vitamin/antioxidant status in intensely exercised horses to determine the optimal level of vitamin E supplementation. Methods: Twelve unfit Standardbreds were divided into 3 groups, supplemented orally with 0 (CON), 5000 (MOD), or 10,000 (HI) iu/day of DL-,-tocopheryl acetate. The 3 times 3 Latin square design consisted of three 4 week supplementation periods with 4 week wash out periods between. After each period, horses underwent a treadmill interval exercise test. Blood samples were collected and heart rate (HR) measured before, during and after exercise. Data were analysed using ANOVA with repeated measures in SAS. Results: The CON group had lower HR throughout the test compared to the MOD and HI groups (P<0.05). There was an increase in plasma retinol (RET), ,-carotene (BC), red blood cell total glutathione and glutathione peroxidase with exercise (P<0.05), but all groups returned to baseline after 24 h. Plasma ,-tocopherol (TOC) increased from baseline with exercise (P<0.0001) in all groups; treatment differences were observed at 24 h (P<0.05). The HI and CON groups had lower BC compared to the MOD group (P = 0.05). Conclusions: Horses supplemented with vitamin E, at nearly 10-times the 1989 NRC recommended level, did not experience lower oxidative stress compared to control horses. Additionally, lower plasma BC levels observed in the HI group, which may indicate that vitamin E has an inhibitory effect on BC metabolism. Potential relevance: Supplementation above control levels is not more beneficial to oxidative stress and antioxidant status in intensely exercising horses; indeed, levels 10 times in excess may be detrimental to BC and should be avoided.     Effects of an occlusal splint compared with cognitive-behavioral treatment on sleep bruxism activity

    The impact of an occlusal splint (OS) compared with cognitive-behavioral treatment (CBT) on the management of sleep bruxism (SB) has been poorly investigated. The aim of this study was to evaluate the efficacy of an OS with CBT in SB patients. Following a randomized assignment, the OS group consisted of 29, and the CBT group of 28, SB patients. The CBT comprised problem-solving, progressive muscle relaxation, nocturnal biofeedback, and training of recreation and enjoyment. The treatment took place over a period of 12 wk, and the OS group received an OS over the same time period. Both groups were examined pretreatment, post-treatment, and at 6 months of follow-up for SB activity, self-assessment of SB activity and associated symptoms, psychological impairment, and individual stress-coping strategies. The analyses demonstrated a significant reduction in SB activity, self-assessment of SB activity, and psychological impairment, as well as an increase of positive stress-coping strategies in both groups. However, the effects were small and no group-specific differences were seen in any dependent variable. This is an initial attempt to compare CBT and OS in SB patients, and the data collected substantiate the need for further controlled evaluations, using a three-group randomized design with repeated measures to verify treatment effects.     Retail Store Lighting for Elderly Consumers: An Experimental Approach

    The study presents an investigation of the influence of the color of light on readability by figure to background value contrast, color perception, and overall room-light estimation for elderly consumers in an experimental setting representing a retail store. A factorial design with repeated measures was used to identify the impact of three independent variables: (a) lamp color temperatures, (b) lamp color-rendering properties, and (c) age of the participants. The results show that older adults perceived the higher color temperature light source as less cool than did younger adults. Older adults rated their level of comfort and preference higher than the younger participants under all lighting conditions. Regarding readability, higher color-rendering light sources provide better readability, and older adults have more difficulty with warmer lighting when value contrasts were reduced. Implications from this study can be applied to retail lighting techniques to attract elderly consumers. [source]


    Relative association of treatment-emergent adverse events with quality of life of patients with schizophrenia: post hoc analysis from a 3-year observational study

    Abstract Objective To explore the relative association of adverse events with health-related quality of life (HRQL) in patients (N,=,16,091) with schizophrenia, treated with antipsychotic medication. Methods In this post hoc analysis of data from two 3-year observational studies, a mixed effects model with repeated measures was used to evaluate the association between HRQL (EuroQoL visual analogue scale (EQ-VAS)) and pre-specified covariates including: severity of illness, extrapyramidal symptoms, tardive dyskinesia, sexual dysfunction, and clinically significant weight gain (>,7% increase from baseline after ,,3 months of treatment). Results Mean EQ-VAS increased from 47.8,±,21.7 at baseline to 72.4,±,18.4 after 36 months. The rank order of the negative association of adverse events with HRQL was: sexual dysfunction (effect estimate ,4.04; 95% CI ,4.30 to ,3.79), extrapyramidal symptoms (effect estimate ,2.09; 95% CI ,2.43 to ,1.75), and tardive dyskinesia (effect estimate ,0.89; 95% CI ,1.46 to ,0.32). Conclusions Differences were observed in the direction and magnitude of the association between each adverse event and HRQL. Recognition of the relative association of adverse events with HRQL may contribute to improved adherence of patients with schizophrenia to antipsychotic therapy.     A high sugar content, low caffeine drink does not alleviate sleepiness but may worsen it

    Abstract Although the ingestion of high levels of glucose might have a short acting alerting effect, there is evidence of an ensuing enhancement of sleepiness in people already sleepy. Some ,energy drinks' contain large quantity of sugars. We compared 250 ml of a well known ,energy drink' (42 g sugars, containing a low [30 mg] level of caffeine for ,flavouring') with a nil sugar nil caffeine, similar tasting control. These were given a week apart, in a repeated measures, double blind, balanced design, to 10 participants sleep restricted to 5 h the prior night. They had a light lunch, consumed a drink at 13:50 h, and 10 min later underwent 3,×,30 min consecutive periods at a reaction time (RT) task (the Psychomotor Vigilance Test), separated by 3 min breaks when self-ratings of sleepiness were made. The energy drink did not counteract sleepiness, and led to slower RTs and more lapses during the final 30 min session, around 80 min after consumption. Copyright © 2006 John Wiley & Sons, Ltd.     Evaluation of the cognitive, psychomotor and pharmacokinetic profiles of rupatadine, hydroxyzine and cetirizine, in combination with alcohol, in healthy volunteers

    Abstract Introduction The Central Nervous System (CNS) impairment induced by moderate alcohol (ALC) ingestion may be enhanced if other drugs are taken simultaneously. Rupatadine (RUP) is a new H1 -antihistamine which also inhibits platelet activating factor (PAF) release in inflammatory reactions. Objective The main aim of the study was to assess the effects of ALC 0.8,g/Kg on RUP (10,mg and 20,mg) CNS effects. An evaluation of alcohol and RUP pharmacokinetics was also attained. Methods Eighteen healthy young volunteers of both sexes participated in a phase I, randomised, crossover, double-blind, placebo-controlled study. At 2-week intervals they received six treatments: (a) placebo (PLA), (b) ALC alone and ALC in combination with: (c) hydroxyzine 25,mg (HYD), (d) cetirizine 10,mg (CET), (e) RUP 10,mg or (f) RUP 20,mg. At baseline and several times thereafter, seven psychomotor performance tests (finger tapping, fine motoric skills, nystagmus, temporal estimation, critical-flicker-fusion frequency, ,d2' cancellation, simple reaction) and eleven subjective self-reports (drunkenness, sleepiness, alertness, clumsiness, anger, inattentiveness, efficiency, happiness, hostility, interest and extraversion) were carried out. Two-way (treatment, time) ANOVAs for repeated measures to each variable together with a multivariate non-parametric approach were applied. Plasma concentrations of alcohol, and of RUP and its metabolites, were quantified by validated immunofluorescence and LC/MS/MS methods, respectively. Plasma-time curves for all compounds were analysed by means of model-independent methods. Results The combination of alcohol with HYD, CET and RUP 20,mg produced more cognitive and psychomotor impairment as compared to alcohol alone, being the combination of alcohol and HYD the one which induced the greatest deterioration. The combination of alcohol and RUP 10,mg could not be differentiated from ALC alone. Subjective self-reports reflect effects on metacognition after the combination of alcohol with HYD and CET i.e. the increased objective impairment observed was not subjectively perceived by the subjects. No significant differences were obtained when comparing alcohol plasma concentrations assessed after the treatments evaluated. RUP showed a lineal kinetic relationship after 10 and 20,mg with a higher exposition to both metabolites assayed. Conclusions Present results showed that single oral doses of rupatadine 10,mg in combination with alcohol do not produce more cognitive and psychomotor impairment than alcohol alone. Higher doses of rupatadine, in combination with alcohol, may induce cognitive and psychomotor deterioration as hydroxyzine and cetirizine at therapeutic doses.     Determinants of health status and the influence of primary health care services in Latin America, 1990,98

    Abstract Primary health care (PHC) services have been advocated as a means by which less developed countries may improve the health of their populations even in the face of poverty, low levels of literacy, poor nutrition and other factors that negatively influence health status. Using aggregated data from the World Bank and UNICEF this study examined which factors, both within the health care system and outside of it, are associated with under-5 mortality rates in 22 countries of Latin America and the Caribbean during the 1990s. In a multivariate analysis using generalized estimating equations for repeated measures, five factors were found to be independent predictors of lower under-5 mortality rates (U5MRs). These were vaccination levels, female literacy, the use of oral rehydration therapy, access to safe water and GNP per capita. When the magnitude of these associations were assessed, higher levels of GNP per capita was found to be very weakly associated with lower U5MRs, compared with female literacy and vaccination rates. These findings suggest that government policies which focus only on promoting economic growth, while not making important investments in PHC services, female education and access to safe water are unlikely to see large improvements in health status.     The PFA-100Ô system for the assessment of platelet function in normotensive and hypertensive pregnancies

    Platelet function was studied in 30 pregnant women: 14 normotensive (C), and 16 affected by pregnancy-induced hypertension (PIH). Platelet aggregometry (PA) on platelet-rich plasma according to Born was compared with the new PFA-100Ô System (Dade International Inc, Miami, USA). This device evaluates platelet function (expressed in seconds as closure time, CT) in anticoagulated whole blood ex vivo at high shear rates. PA (expressed as percentage of light transmission) and CT were measured at baseline and after incubation with L-Arginine (L-Arg). MANOVA for repeated measures showed that L-Arg incubation significantly decreased PA (F=7.2, P < 0.05) and increased CT (F=6.05, P < 0.05) in the whole population of pregnant women. Moreover, we analysed separately both parameters in C and in PIH subjects. No differences in PA were found in both groups, neither at baseline nor after L-Arginine incubation. In contrast, CT was significantly longer in PIH in comparison to C before (95.9 s vs. 84 s, P < 0.05) as well after (115 s vs. 92 s, P < 0.05) L-Arginine incubation. Data from PFA-100Ô confirm our previous reports that during pregnancy the L-Arginine: Nitric Oxide pathway regulates platelet function. In hypertensive patients a significant decrease in platelet function was found by using the PFA-100Ô system. [source]


    Nutritional status and health outcomes for older people with dementia living in institutions

    Abstract Title.,Nutritional status and health outcomes for older people with dementia living in institutions Aim., This paper is a report of a study to determine changes over a 3-month period among older people with dementia living in long-term care settings, related to: (1) changes in body mass index, and (2) health outcomes and associated factors. Background., Nutritional deficiencies are common problems among older people, but frequently unrecognized, both in long-term care settings and in the community. Method., A cross-sectional design with repeated measures of body weights and medical record reviews was adopted. The study was conducted in 2003 in two long-term care facilities for older people with dementia in Taiwan. Fifty-five residents participated in the study. Results., Eighteen percent of the residents were under-nourished (body mass index <18·5). There was a trend toward decreasing body mass index over the 3-month study period. Residents with low body mass index tended to need assistance at mealtimes. Nineteen residents, many receiving naso-gastric tube-feeding, experienced adverse health events during the study period. Dependency in eating was the major factor differentiating residents with normal or low body mass index values, and also in distinguishing those who experienced adverse health outcomes. Conclusion., Assessment of eating ability, mode of feeding and measurement of body weight can be used by nurses in long-term care settings for early identification of the nutritional status of older people with dementia.     Daily pattern of some fatty acids in the athletic horse

    Summary In the sport field, non-esterified fatty acids (NEFA) are important for the physical performance during the aerobic exercise of short intensity and long duration. In man, rat, goat and in the sedentary horse studies on the chronometabolism showed the presence of a circadian rhythm of the plasmatic concentration of NEFA while data for the athletic horse are lacking. To define a chronogram helpful for a specific planning and the differentiation of the training programmme in the athletic horse, the circadian pattern of some fatty acids (NEFA, palmitic, stearic, oleic, linoleic and linolenic acids) was studied in five Sella Italiana horses. These horses trained following a daily model of activity consisting of walk, trot, gallop and jump of obstacles of different heights. Blood samples were collected from the jugular vein every 4 h, starting at 08:00 hours, for 2 days to assess the concentrations of total NEFA (by spectrophotometry), palmitic, stearic, oleic, linoleic and linolenic acids (by gas chromatography). anova for repeated measures showed a statistical significant effect of the time of the day in NEFA, oleic and linolenic acids. The application of the periodic model showed the periodic pattern of NEFA, oleic, linoleic and linolenic acids. Acrophases were in the afternoon for all parameters. The results obtained showed a different trend of the circadian pattern of the studied parameters in the athletic horse than in the sedentary one because the physical activity and the post-prandial metabolism acted as zeitgebers.     Gingival changes during pregnancy: I. Influence of hormonal variations on clinical and immunological parameters

Abstract Aim: To test whether exacerbated gingival inflammation in pregnancy is associated with increased salivary hormone levels and changes in gingival crevicular fluid (GCF) interleukin-1, (IL-1,) and prostaglandin-E2 (PGE2) levels. Material and methods: In this cohort study, 48 pregnant women without periodontitis were evaluated in the first, second, and third trimesters and at 3 months postpartum. Twenty-eight non-periodontitis non-pregnant women were evaluated twice, with a 6-month interval. Plaque and gingival indices (PlI, GI), salivary progesterone and estradiol and GCF IL-1, and PGE2 levels were determined. anova for repeated measures or Friedman's test were used for intragroup analyses. Inter-group comparisons were analysed with t -test or Mann,Whitney U -test. Correlations were evaluated with Pearson's and Spearman's test. Results: Pregnant women showed an increase in GI (p<0.05) despite maintaining low PlI values. No changes in IL-1, and PGE2 levels were observed during pregnancy. No significant correlation was found between the GI increase and salivary hormone levels. GI (p<0.05) and IL-1, levels (p<0.001) were lower in non-pregnant than in pregnant women. Conclusions: This study confirms the presence of an exacerbated gingival inflammation during pregnancy, but this phenomenon could not be associated with an increase in progesterone or estradiol or with changes in PGE2 or IL-1,.     Periodontal health improves systemic inflammatory and haemostatic status in subjects with coronary heart disease

    Abstract Objectives: A relationship between poor oral health and coronary heart disease (CHD) and systemic inflammatory and haemostatic factors has been recently documented in an Italian population. The present study was performed to assess whether intensive dental care may produce a periodontal improvement along with a change in systemic inflammatory and haemostatic factors. Material and Methods: The study population consisted of 18 males aged 40,65 years with proven CHD and elevated values of systemic inflammatory and haemostatic factors. A detailed description of their oral status was given by using two different dental indices (clinical periodontal sum score and clinical and radiographic sum score). Blood samples were taken for measurement of the following systemic markers of inflammation [(C-reactive protein (CRP), leucocytes, fibrinogen)] and haemostatic factors [(von Willebrand factor, fibrin D-dimer and oxidized-low density lipoprotein (Ox-LDL)]. All parameters were determined in each subject at baseline, after 4 months as a control and 3 months after an intensive protocol of scaling and root planing. anova for repeated measures was used for the statistical analysis. Results: No statistical difference was found between values at baseline and at the 4-month-control. All oral indexes showed a significant decrease (p<.01) 3 months after periodontal treatment. All systemic inflammatory indexes decreased but only the decrease in CRP reached statistical significance (p<.05). A significant decrease (p<.01) was also found as regards Ox-LDL among haemostatic factors. Conclusions: Preliminary results from the present study suggest an association between poor oral status and CHD, and provide evidence that the improvement of periodontal status may influence the systemic inflammatory and haemostatic situation.     The Effects of Lighting on Consumers' Emotions and Behavioral Intentions in a Retail Environment: A Cross-Cultural Comparison

    ABSTRACT As an important component of a retail store's atmospherics, lighting can affect the emotional responses that influence consumer shopping behavior. The purpose of this study is to examine, through cross-cultural comparison, the effect of the color quality of light in a retail environment on consumers' emotional states, behavioral intentions, and perceptions. The experimental research followed a 2 times 2 × 2 factorial design with repeated measures to identify the impact of culture group, color rendering index, and color temperature. The results of this study indicate that consumers are aroused and pleased by certain lighting effects and that cultural differences influence perceptions as well as the behavioral intentions of "approach-avoid" in a retail environment. Practical implications of this study could include application of store lighting techniques that enhance visual perceptions of consumers, induce emotional states of arousal and pleasure, and appeal to consumers from different cultures.     Acupuncture and arthroscopic acromioplasty

    Background. Acupuncture alleviates acute and chronic shoulder pain. Yet it has not been determined whether acupuncture is useful following musculoskeletal surgery. Hypothesis: Compared to sham acupuncture, arthroscopic acromioplasty subjects who received real acupuncture would manifest significantly better recovery as demonstrated by: UCLA shoulder scale, improved range of motion, diminished pain, decreased need and duration of analgesic use, and enhanced patient satisfaction. Methods. Forty arthroscopic acromioplasty patients were randomized to real or sham acupuncture. UCLA shoulder scale scores, pain intensity, analgesic use, range of motion, and quality of life were monitored for four months. Data were analyzed with the general linear model ANOVA for repeated measures. Results. Thirty-five subjects completed the study. Real acupuncture subjects scored significantly better on UCLA shoulder scale (p < 0.000); pain intensity (p < 0.022); self-reported analgesic use (p < 0.008); angles of abduction (p < 0.046); and in six of eight health status questionnaire components. Conclusions. Following arthroscopic acromioplasty, real acupuncture compared to sham acupuncture offered significantly greater improvement via: (1) lower pain level, (2) less analgesic use, (3) range of motion, and (4) patient satisfaction. © 2003 Orthopaedic Research Society.     Comparison of Repair Methods for Ceramic-Fused-to-Metal Crowns

    Purpose: The objective of this study was to evaluate the effect of four repair methods on the fracture load of repaired ceramic-fused-to-metal crowns. Materials and Methods: Metal-ceramic crowns were fractured, and the failure load was measured. The fractured metal-ceramic crowns (n = 9) were assigned randomly to the following treatment groups: (1) hydrofluoric acid (9.5%) etching, (2) air-particle abrasion (50 ,m Al2O3), (3) silica coating (30 ,m SiOx), and (4) the application of a layer of glass fiber-reinforced composite (FRC) (thickness: 0.12 mm) on the repair surface. The crowns were repaired with a highly filled resin composite and subjected to 3 repair cycles (n = 27). All specimens were stored in water at 37°C for 24 hours and then thermocycled (6000 cycles, 5°C to 55°C). The fracture load values for final failure of intact and repaired crowns were measured with a universal testing machine, and failure types were recorded. Results: No significant differences ( p > 0.05) were found between the final failure values for the groups treated with 9.5% hydrofluoric acid (376 N) and airborne particle abrasion with either Al2O3 (432 N) or SiOx (582 N) followed by silanization, respectively. Significantly, higher ( p < 0.0001) final failure values (885 N) were obtained with the use of the FRC layer when compared with the other repaired groups. There was no significant difference ( p > 0.05) between the final fracture load of intact crowns (872 N) and those repaired with FRC (885 N) (One-way ANOVA with repeated measures, Bonferroni test). No significant difference in fracture loads was found between the 1st, 2nd, and 3rd repair cycles (558 N, 433 N, 485 N, respectively). Failure sites were predominantly at the alloy/veneering resin interface in Group 1; Groups 2 and 3 both showed more cohesive failures than Group 1. In the case of FRC, the failure pattern was exclusively cohesive between the two laminates of FRC layer. Conclusions: The conditioning methods (Groups 1 to 3) of the repair surfaces did not show differences between each other; each resulted in mean fracture loads at lower levels than that of the intact crowns. Addition of an FRC layer increased the fracture load to the level of intact crowns. This suggests that the use of FRC in repairs of metal-ceramic crowns might be a viable option. [source]


    Fostering a culture of engagement: an evaluation of a 2-day training in solution-focused brief therapy for mental health workers

    The focus of mental health care has changed considerably in recent years, from an almost exclusive inpatient system of care to one where the majority of care is being delivered within the community. Arguably this has contributed to a reduction in the length of inpatient admissions. Therefore, there is a need to understand the ramifications that shorter admissions have on inpatient care and nursing practice. This paper reports on a study designed to test the knowledge and skill acquisition and self-reported application of solution-focused brief therapy by staff following a 2-day training. The study adopted a repeated measures design where participants' baseline knowledge was measured prior to the 2-day training and then at 3 and 6 months post-training. This study has demonstrated that the 2-day training was effective in increasing participants' reported knowledge and understanding of solution-focused brief therapy and their self-reported use of the techniques in routine clinical practice. In conclusion, this study has established that staff from a variety of professional and non-professional backgrounds can make good skill acquisition from a fairly modest training. Additionally, the current study has also highlighted the need for well-conducted large-scale trials of this potentially important technology. [source]


    The effect of psychological and educational counselling in reducing anxiety in nursing students

    A quasi-experimental pre-, post-test, follow-up and control group design was used to investigate the effect of psychological and educational counselling in reducing anxiety in nursing students. The research study used methodological triangulation, involving the use of structured data collection techniques such as standardized questionnaires and semi-structured focus groups. Focus groups were used to provide greater insight regarding the student's opinions. The sample consisted of 100 second- and fourth-year baccalaureate nursing students from the Faculty of Nursing at Shiraz University of Medical Sciences, Iran. They were randomly assigned to either an experimental group (n = 50) or a control group (n = 50). The experimental group received a 12-week intervention programme. Quantitative analysis of data was undertaken using t -test and analysis of variance for repeated measures to test differences between and within groups. The results indicated that from pre- to post-test there was no statistically significant reduction in anxiety between groups, but there was a statistically significant reduction in anxiety after one semester (in the follow-up). Student self-esteem was increased significantly from pre- to post-test. This increase was statistically significant and remained the same in follow-up. A statistically significant difference was seen in the student grade point average from pre-test to follow-up in the experimental group but not for the control group. The implementation of an intervention programme reduced their anxiety, increased their self-esteem and improved their grade point average over time. [source]


    An in vivo Study of the Relationship between Craving and Reaction Time during Alcohol Detoxification Using the Ecological Momentary Assessment

    Abstract: Background: To study cognitive interference associated with craving for alcohol, the Ecological Momentary Assessment (EMA) method was used to measure the relationship between craving and reaction time. A secondary aim was the study of the predictive factors for craving during alcohol detoxification. The EMA enables both repeated measures of craving in a natural setting and the recording of reaction time without the patient being aware of this. Methods: Craving for alcohol, reaction time, sadness and anxiety were recorded 8 to 12 times a day, over three weeks of detoxification in 14 alcoholics (n= 1767 measures), on an electronic diary issuing random prompts. Mixed models were used for statistical analysis (,= 5%, 1-,= 88%). Results: Reaction time was significantly increased in univariate analysis when a craving episode occurred but this difference did not persist after multivariate analysis. Craving episodes were more frequent and intense than previously reported. Predictive factors of craving during detoxification were: age, gender, sadness, anxiety and the number of previous detoxifications. Antidepressants, anticraving medications but not benzodiazepines were negatively associated to craving. [source]


    On the analysis of long-term experiments

    Summary., Long-term experiments are commonly used tools in agronomy, soil science and other disciplines for comparing the effects of different treatment regimes over an extended length of time. Periodic measurements, typically annual, are taken on experimental units and are often analysed by using customary tools and models for repeated measures. These models contain nothing that accounts for the random environmental variations that typically affect all experimental units simultaneously and can alter treatment effects. This added variability can dominate that from all other sources and can adversely influence the results of a statistical analysis and interfere with its interpretation. The effect that this has on the standard repeated measures analysis is quantified by using an alternative model that allows for random variations over time. This model, however, is not useful for analysis because the random effects are confounded with fixed effects that are already in the repeated measures model. Possible solutions are reviewed and recommendations are made for improving statistical analysis and interpretation in the presence of these extra random variations. [source]


    Estimating the variance of estimated trends in proportions when there is no unique subject identifier

    Summary., Longitudinal population-based surveys are widely used in the health sciences to study patterns of change over time. In many of these data sets unique patient identifiers are not publicly available, making it impossible to link the repeated measures from the same individual directly. This poses a statistical challenge for making inferences about time trends because repeated measures from the same individual are likely to be positively correlated, i.e., although the time trend that is estimated under the naïve assumption of independence is unbiased, an unbiased estimate of the variance cannot be obtained without knowledge of the subject identifiers linking repeated measures over time. We propose a simple method for obtaining a conservative estimate of variability for making inferences about trends in proportions over time, ensuring that the type I error is no greater than the specified level. The method proposed is illustrated by using longitudinal data on diabetes hospitalization proportions in South Carolina. [source]


    Maximum likelihood estimation in semiparametric regression models with censored data

    Summary., Semiparametric regression models play a central role in formulating the effects of covariates on potentially censored failure times and in the joint modelling of incomplete repeated measures and failure times in longitudinal studies. The presence of infinite dimensional parameters poses considerable theoretical and computational challenges in the statistical analysis of such models. We present several classes of semiparametric regression models, which extend the existing models in important directions. We construct appropriate likelihood functions involving both finite dimensional and infinite dimensional parameters. The maximum likelihood estimators are consistent and asymptotically normal with efficient variances. We develop simple and stable numerical techniques to implement the corresponding inference procedures. Extensive simulation experiments demonstrate that the inferential and computational methods proposed perform well in practical settings. Applications to three medical studies yield important new insights. We conclude that there is no reason, theoretical or numerical, not to use maximum likelihood estimation for semiparametric regression models. We discuss several areas that need further research.     Association of Season and Pasture Grazing with Blood Hormone and Metabolite Concentrations in Horses with Presumed Pituitary Pars Intermedia Dysfunction

    Background: Pituitary pars intermedia dysfunction (PPID) is a risk factor for pasture-associated laminitis, which follows a seasonal pattern. Hypothesis: Hormonal responses to season differ between PPID and unaffected horses. Animals: Seventeen horses aged 8,30 years (14 horses , 20 years of age). Methods: Longitudinal observational study. Blood was collected monthly from August 2007 until July 2008 after pasture grazing and again after overnight stall confinement. Blood hormone and metabolite concentrations were measured and pasture grass samples were analyzed to determine carbohydrate content. Analysis of variance analysis for repeated measures was performed. Results: Mean ACTH concentrations varied significantly over time (P < .001), with higher concentrations detected in August, September, and October compared with November,April. Pasture × time effects were detected for glucose and insulin concentrations, with peaks observed in September. Horses were retrospectively allocated to PPID (n = 8) and control (n = 9) groups on the basis of plasma ACTH concentrations. Changes in insulin concentrations over time differed in the PPID group when compared with the control group. Insulin concentrations were positively correlated with grass carbohydrate composition. Conclusions and Clinical Importance: PPID did not affect the timing or duration of the seasonal increase in ACTH concentrations, but higher values were detected in affected horses. Insulin concentrations differed between groups, but hyperinsulinemia was rarely detected. Glucose and insulin concentrations peaked in September when horses were grazing on pasture, which could be relevant to the seasonal pattern of laminitis. [source]


    Acute effects of escalating doses of amiodarone in isolated guinea pig hearts

Cardiac effects of escalating concentrations of amiodarone were determined on isolated perfused guinea pig hearts (Langendorff preparations). Spontaneously beating hearts were instrumented for the measurement of RR, PQ, QRS, QT and QTc durations (from a bipolar electrogram), and dP/dtmax and dP/dtmin from an isovolumetric left ventricular pressure curve. Ten hearts were exposed to escalating concentrations of amiodarone (10,7, 10,6, 10,5 and 10,4 M) in dimethyl sulfoxide (DMSO)/Krebs,Henseleit or to DMSO/Krebs,Henseleit (vehicle). Measurements were collected during the last minute of a 15-min concentration. Means of all parameters were compared by ANOVA with repeated measures design. When compared with vehicle, amiodarone prolonged QT and QTc durations at concentrations >10,6 M. The apparent lengthening of RR, PQ and QRS at concentrations >10,6 M did not achieve statistical significance. Similarly, the apparent decreases in dP/dtmax and dP/dtmin at concentrations >10,6 M did not achieve statistical significance. The putative therapeutic concentration of amiodarone is between 2 and 4 × 10,6 M. In this study, at a concentration of 10,6 M, only RR and dP/dtmin tended to change, but they were not different from vehicle. Thus, amiodarone in this preparation has little potential for cardiac toxicity at therapeutic concentrations. [source]


    Operational implications of varying ambient light levels and time-of-day effects on saccadic velocity and pupillary light reflex

    Abstract Changes in maximal saccadic velocity (SV), initial pupil diameter (IPD), constriction latency (CL) and constriction amplitude (CA) determined by the pupillary light reflex have been found to be sensitive indicators of impairment as a result of drugs, sleepiness, and/or fatigue. Ambient illuminance and time of day are controlled when these indices are applied as repeated measures in fitness-for-duty determinations. The application of oculometrics in unrestricted operational environments, where ambient illuminance and time-of-day testing are not constant, requires understanding of, and potential compensation for, the effects of, and interactions among, these multiple uncontrolled variables. SV, IPD, CL, and CA were evaluated in the morning and evening on two consecutive days following adequate nightly sleep under one baseline ambient illuminance and seven test ambient illuminances. Sixteen healthy volunteers (21,38 years, eight females/eight males) participated. Within and across days, SV was unaffected by decreasing ambient light or time-of-day effects. With the increase of ambient light from 670 to 3300 lx, CL decreased by 1%, while IPD and CA decreased by 17% and 20%, respectively. IPD increased with time of day by 1,10% (IPD was smaller in the morning). The results show that SV and CL are essentially resistant to changes in ambient light and time-of-day effects, simplifying their application in uncontrolled operational environments. [source]


    Clustering of Device-Related Concerns and Type D Personality Predicts Increased Distress in ICD Patients Independent of Shocks,

    Background: This study examined the impact of clustering of device-related concerns and Type D personality on anxiety and depressive symptoms during a six-month period and the clinical relevance of shocks, implantable cardioverter defibrillator (ICD) concerns, and Type D. Methods: Consecutively implanted ICD patients (n = 176) completed questionnaires at baseline and six months and were divided into four risk groups: (1) No risk factors (neither ICD concerns nor Type D); (2) ICD concerns only; (3) Type D only; (4) Clustering (both ICD concerns and Type D). Results: The prevalence of Type D and concerns were 21.6% and 34.7%. Analysis of variance for repeated measures showed a reduction in anxiety over time (P < 0.001), with the risk groups exerting a stable (P = 0.14) but differential effect (P < 0.001); the highest level was seen in the clustering group. Similar results were found for depression, although depressive symptoms did not decrease (P = 0.08) and the impact of clustering was less clear. These results were confirmed in adjusted analysis, with shocks (P = 0.024) also being associated with anxiety but not depression. The impact of ICD concerns and Type D personality on anxiety and depression at baseline and six months was large (,0.8) compared to negligible to moderate for shocks (0.0,0.6). Conclusions: ICD patients with psychosocial risk factor clustering had the highest level of anxiety, whereas the pattern for depression was less consistent. Shocks influenced outcomes, but the impact was smaller compared to ICD concerns and Type D personality. It may be timely to expand the focus beyond shocks when seeking to identify ICD patients at risk for adverse clinical outcome due to their psychological profile. [source]
