McGill Pain Questionnaire: A multi-dimensional verbal scale assessing postoperative changes in pain symptoms associated with severe endometriosis

Abstract Background:, Objective evaluation of pelvic pain symptoms using a standard pain questionnaire is essential to assessing the treatment of endometriosis and related pain. Aim:, To evaluate the McGill Pain Questionnaire (MPQ) as a multi-dimensional verbal scale in providing information about chronic pelvic pain associated with endometriosis, before and after laparoscopic surgery. Methods:, Fifty-five women undergoing laparoscopy for severe endometriosis were asked to complete the MPQ before surgery and at the 6-month follow up. All patients presented with preoperative pain symptoms of variable severity. We obtained the pain indexes and studied their relation with: patients' characteristics (age, body mass index, parity, qualification, occupation); operative findings (number, site and size of endometriotic lesions and presence of pelvic adhesions); and postoperative evolution of variable MPQ pain indexes at the 6-month follow up. Results:, Median present pain index (PPI) (index of pain intensity), before surgical treatment was 3 (2,4): preoperative PPI was <2 in 25% of patients while 25% of patients had PPI > 4. Overall median PPI after surgical treatment was 1 (0,2): postoperative index of pain intensity was <1 in 50% of patients, >2 in 25% of patients while 25% of patients did not experience postoperative pain. Overall pain intensity significantly decreased after laparoscopic treatment of endometriosis (Wilcoxon test P < 0.0005). None of the patients' characteristics were found to be significantly correlated with the severity or improvement of preoperative pain at postoperative follow up (P > 0.05), and the intensity of preoperative pain was not correlated to any of the operative variables. There was a significant reduction in all individual MPQ pain indexes; however 18.2% of women did not show improvement of pain symptoms after laparoscopic surgery. An increasing endometrioma diameter was associated with a significant decrease in the difference in evaluative rank score of pain rating index between pain indexes at the 6-month follow up and preoperatively (P = 0.04, Spearman's rank correlation Rho = ,0.277). Conclusions:, MPQ appears to be useful as a multi-dimensional scale in describing patients' pain semiology and evaluating pain evolution after surgical treatment. However, due to the extreme variability of pain experience, MPQ results don't clarify the relationship between pain intensity and the severity of endometriosis. [source]


Application of the behavioral investigative support system for profiling perpetrators of serial sexual assaults,

The authors developed a data-based profiling system in order to support offender profiling. The system stored incident records of prior offenders. Inputting offence details of an unsolved incident, a probability score was assigned to each prior offender in the system; the score represented the behavioral similarity with the unsolved incident. The system then ranked all offenders in the system according to the probability scores, and prioritized the high-ranked offenders as possible suspects. Moreover, the system inferred the characteristics of unknown offenders by accumulating characteristics of the high-ranked offenders. The system achieved promising accuracy, especially for linking crimes to perpetrators. In 45 out of 81 simulation trials, the target offenders were retrieved as a rank score of 1 from among 868 sex offenders. Copyright © 2007 John Wiley & Sons, Ltd. [source]


Conservation Status as a Biodiversity Trend Indicator: Recommendations from a Decade of Listing Species at Risk in British Columbia

To learn about its value for tracking trends, we examined provincial lists of terrestrial vertebrate species and subspecies at risk in British Columbia, Canada, for 1992 and 2002 to see whether changes in these lists reflected changes in the status of the taxa they represent. Examination of the case histories of individual species and subspecies showed that 65% of additions and deletions to the British Columbia Red List were the result of improvement in knowledge of species status, changes in assessment procedures, and refinements in taxonomy rather than actual changes in a species' status. Comparison to an alternate set of rank scores provided by NatureServe for taxa that appeared on both 1992 and 2002 British Columbia Red Lists revealed changes in status that were not reflected by movement from the list. Estimates of historical conservation status for species on the 1992 British Columbia Red List demonstrated ambiguity around the natural baseline with regard to tracking changes in list composition over time. We discourage the continued use of indicators based solely on conservation status as a means of tracking biodiversity. Instead we recommend advancing strategic indicators around species at risk based on long-term monitoring data, deliberate and explicitly stated baselines, and consistent methods of conservation ranking. Informal social status among coworkers and risk of work-related injury among nurse aides in long-term care

Abstract Background A social network measure was used to explore whether one's rank in an informal social hierarchy of nurse aides employed in a single long-term care facility was associated with risk of work-related injury. Methods Six months of administrative staff schedule data and self-reported injury records were examined. Using survey data, social status rank in the informal hierarchy for each aide was operationalized as the number of coworkers who would approach the aide for advice about work-related matters. Conditional logistic regression was used to model the effect of social status on injury risk; cases were matched to controls consisting of coworkers present on the floor, shift, and date of the injury event. This allowed for a comparison of social status rank within social groups among workers with the same job title. Results Injury incidence rates decreased across tertiles of social status rank scores. A non-significant drop in injury risk in the highest tertile of social status was observed (adjusted OR,=,0.24 95% CI [0.05, 1.32]). Conclusion Findings of this exploratory study were internally consistent and support a theoretical framework suggesting that patterns of social relations between individuals based on informal social status in the workplace may contribute to differences in work-related injury risk among individuals with the same job title.
