Clinical trial: a dose,response study of fospropofol disodium for moderate sedation during colonoscopy

Summary Background, An effective agent is needed that provides rapid onset of sedation and quick recovery for patients undergoing colonoscopy. Aim, To assess the efficacy and safety of fospropofol disodium in providing sedation in patients undergoing colonoscopy. Methods, A randomized, double-blind, multicentre trial evaluated 127 adult patients who received fospropofol (2, 5, 6.5 or 8 mg/kg) or midazolam 0.02 mg/kg following pre-treatment with fentanyl. Supplemental doses of study medication were allowed to reach a Modified Observer's Assessment of Alertness/Sedation scale score ,4. Efficacy end points included sedation success, measures of clinical benefit, sedation, and recovery as well as patient- and doctor-rated satisfaction. Results, Fospropofol produced a significant dose-dependent increase in sedation success from 24% (2 mg/kg), 35% (5 mg/kg) and 69% (6.5 mg/kg) to 96% (8 mg/kg; P < 0.001). There were also dose-dependent trends for time to sedation, requirements for alternative sedative medication, supplemental doses of sedative and fentanyl, time to ready for discharge and doctor-rated satisfaction scores. Fospropofol was well tolerated, with most adverse events mild-to-moderate in severity. Conclusion, The 6.5 mg/kg dose of fospropofol provides the ideal balance of efficacy and safety for patients undergoing colonoscopy and has been selected for phase 3 clinical development. [source]


Ethylene insensitivity impedes a subset of responses to phosphorus deficiency in tomato and petunia

ABSTRACT The role of ethylene in growth and developmental responses to low phosphorus stress was evaluated using ethylene-insensitive ,Never-ripe' (Nr) tomato and etr1 petunia plants. Low phosphorus increased adventitious root formation in ,Pearson' (wild-type) tomato plants, but not in Nr, supporting a role for ethylene in adventitious root development and showing that ethylene is important for this aspect of phosphorus response. Low phosphorus reduced ethylene production by adventitious roots of both genotypes, suggesting that ethylene perception , not production , regulates carbon allocation to adventitious roots at the expense of other roots under low phosphorus stress. With the exception of its effect on adventitious rooting, Nr had positive effects on growth and biomass accumulation in tomato whereas etr1 tended to have negative effects on petunia. This was particularly evident during the recovery from transplanting, when the effective quantum yield of photosystem II of etr1 petunia grown with low phosphorus was significantly lower than ,Mitchell Diploid', suggesting that etr1 petunia plants may undergo more severe post-transplant stress at low phosphorus availability. Our results demonstrate that ethylene mediates adventitious root formation in response to phosphorus stress and plays an important role for quick recovery of plants exposed to multiple environmental stresses, i.e. transplanting and low phosphorus. Expression of water and ion transporters in tracheal aspirates from neonates with respiratory distress

Abstract Aim:, The aim of the study was to determine whether neonatal respiratory distress is related to changes in water and ion transporter expression in lung epithelium. Methods:, The study included 32 neonates on mechanical ventilation: 6 patients with normal lung X-rays (control group), eight with respiratory distress syndrome (RDS), eight with transient tachypnea of the newborn (TTN), 10 with abnormal lung X-rays (mixed group). The protein abundance of water channel AQP5, epithelial sodium channel (ENaC; ,-, ,- and ,-ENaC) and Na+, K+ -ATPase ,1 were examined in tracheal aspirates using semiquantitative immunoblotting. Results:, ,-ENaC level was significantly lower in RDS group compared with infants with TTN and infants in the control group. AQP5 expression was significantly higher in TTN compared with the infants with RDS and all other infants with abnormal lung X-rays. Conclusion:, Neonatal respiratory distress is associated with changes in ,-ENaC and AQP5 expression. The lower ,-ENaC expression may be one of the factors that predispose to the development of RDS. The higher AQP5 expression may provide the possibility for reabsorption of postnatal lung liquid, which contributes to quick recovery of infants with TTN. [source]


Large-Scale Synthesis of Long Crystalline Cu2-xSe Nanowire Bundles by Water-Evaporation-Induced Self-Assembly and Their Application in Gas Sensing

Abstract By a facile water evaporation process without adding any directing agent, Cu2-xSe nanowire bundles with diameters of 100,300,nm and lengths up to hundreds of micrometers, which comprise crystalline nanowires with diameters of 5,8,nm, are obtained. Experiments reveal the initial formation/stacking of CuSe nanoplates and the subsequent transformation to the Cu2-xSe nanowire bundles. A water-evaporation-induced self-assembly (WEISA) mechanism is proposed, which highlights the driving force of evaporation in promoting the nanoplate stacking, CuSe-to-Cu2-xSe transformation and the growth/bundling of the Cu2-xSe nanowires. The simplicity, benignancy, scalability, and high-yield of the synthesis of this important nanowire material herald its numerous applications. As one example, the use of the Cu2-xSe nanowire bundles as a photoluminescence-type sensor of humidity is demonstrated, which shows good sensitivity, ideal linearity, quick response/recovery and long lifetime in a very wide humidity range at room temperature.
