A Simple Method of Designing a Bilobed Flap Using a Triangle Template

Background. The bilobed flap is useful for the reconstruction of skin defects in which the primary closure is difficult. Proper design is paramount to achieve excellent cosmetic results, but flap design often appears unnecessarily complex and difficult. Objective. The objective was to discuss the geometry of the bilobed flap and describe a simple and practical method for constructing such flaps using a triangle template with emphasis on the pivot point. Methods. A detailed description with diagrams and an illustrative case are presented to demonstrate the technique. Results. A patient with a scalp defect was reconstructed with a triangle-template-designed bilobed flap with excellent postoperative results. Conclusion. This new method using a triangle template ensures proper placement of the pivot point and will enable practitioners to achieve superior outcomes. [source]


Transjugular intrahepatic portosystemic shunts: an update

Transjugular intrahepatic portosystemic shunts (TIPS) have been used in the treatment of complications of portal hypertension. TIPS is used for the control of acute variceal bleeding and for the prevention of vericeal rebleeding when pharmacologic therapy and endoscopic therapy have failed. Patients with refractory ascites with adequate hepatic reserve and renal function who fail to respond to large volume paracentesis may be reasonable candidates for TIPS. Promising indications for TIPS are Budd-Chiari syndrome uncontrolled by medical therapy, severe portal hypertensive gastropathy, refractory hepatic hydrothorax, and hepatorenal syndrome. TIPS cannot be recommended for preoperative portal decompression solely to facilitate liver transplantation. Special care should be taken to insure proper placement of the stent to avoid increasing the technical difficulty of the transplantation procedure. The major limiting factors for TIPS success are shunt dysfunction and hepatic encephalopathy. Because shunt stenosis is the most important cause of recurrent complications of portal hypertension, a surveillance program to monitor shunt patency is mandatory. The MELD score may be useful in predicting post-TIPS survival, and also in counseling patients and their families. [source]


Expediting sustainable brownfields redevelopment by applying Triad using the membrane interface probe

Redevelopment and reuse plans are often based upon an expedited delineation and remediation life cycle, since delayed reuse usually has economic consequences. It has also become increasingly important to utilize sustainable practices to achieve investigation and remediation goals. In this article, the Triad approach is used to expedite the delineation of a source area within a municipal landfill to complete the remedial effort prior to construction of an urban civic center. The Triad approach uses the three elements of systematic project planning, dynamic work strategy, and real-time measurement to expedite site characterization (Interstate Technology and Regulatory Council, 2003). In this article, the Triad sampling strategy consisted of two phases. The first phase included in situ screening of soil and groundwater using the membrane interface probe (MIP), and the second phase included confirmatory sampling via vertical profiles in the soil and groundwater. This study found that, using the MIP in a dynamic sampling strategy, a critical element of the Triad approach, combined with the proper placement of confirmatory samples, significantly reduced overall project cost and will expedite the site redevelopment. Crystallization and preliminary X-ray crystallographic analysis of MinE, the cell-division topological specificity factor from Helicobacter pylori

Cell division in Gram-negative bacteria is driven by the formation of an FtsZ ring at the division site. MinE regulates the proper placement of the FtsZ ring at mid-cell by blocking the inhibitory action of the MinCD complex. Diffraction data were collected at 2.8,Å resolution from a native crystal of full-length Helicobacter pylori MinE. The crystal belonged to space group P64. Assuming the presence of two molecules in the asymmetric unit, the calculated Matthews coefficient was 2.58,Å3,Da,1, which corresponds to a solvent content of 52.3%. For MAD phasing, a four-wavelength data set was collected at 3.0,Å resolution. [source]
