Is multidisciplinary learning effective among those caring for people with diabetes?

Abstract The role of multi-professional learning for those providing clinical services to people with diabetes has yet to be defined. Several assumptions are generally made about education in the context of multi-professional settings. It is argued that different professions learning together could potentially improve professional relationships, collaborative working practices and ultimately standards of care. Greater respect and honesty may emerge from a team approach to learning with a commensurate reduction in professional antagonism. Personal and professional confidence is reportedly enhanced through close contact with other professionals during team-based learning exercises. We have examined current evidence to support multidisciplinary learning in the context of medical education generally as well as in diabetes education. Previous investigation of available literature by Cochrane reviewers, aimed at identifying studies of interprofessional education interventions, yielded a total of 1042 articles, none of which met the stated inclusion criteria. Searches involving more recent publications failed to reveal more robust evidence. Despite a large body of literature on the evaluation of interprofessional education, studies generally lacked the methodological rigour needed to understand the impact of interprofessional education on professional practice and/or health care outcomes. Nevertheless, planners continue to advocate, and endorse, joint training between different groups of workers (including nurses, doctors and those in professions allied to medicine) with the objective of producing an integrated workforce of multidisciplinary teams. Whilst the concept of multi-professional learning has strong appeal, it is necessary for those responsible for educating health care professionals to demonstrate its superiority over separate learning experiences. Information technology supporting diabetes sel-care: a pilot study

Abstract Although diabetes is a lifelong, incurable disease, people can live a full and normal life, provided that they receive appropriate and well-planned care. The care of people with diabetes should be organised as flexibly as possible to suit individual lifestyles. Information technology has become a useful tool to support functional patient,professional relationships and improve care balance. The Self-Care System software tool set by ProWellness is one such tool. Users can enter blood glucose data by using a computer, modem and mobile phone and diabetes nurses can monitor the situation from their own computer and, if necessary, give instructions by sending a SMS (text) message to the patient's mobile phone. This pilot study investigated whether the Self-Care System application supports people with diabetes and can be used as a diabetes education method. The study was carried out in the municipal consortium for healthcare of Siilinjärvi and Maaninka. Nine individuals with diabetes and three diabetes nurses were selected to participate in the study. Data were collected by questionnaire and interview. People with diabetes were sent a questionnaire and the nurses were interviewed. Content analysis was carried out on the interview data. The results suggest that the Self-Care System software supports and motivates diabetes self-care. The nurses felt that the application was useful when changes, such as starting insulin treatment, were introduced. The application was further described as effective and motivating in short-term intensive diabetes education and monitoring; however, both nurses and patients disliked the mechanical nature of the software. Repeat dispensing of prescriptions in community pharmacies: a systematic review of the UK literature

Objective To identify, review and evaluate the published literature that focused on the impact of repeat dispensing in community pharmacies in the United Kingdom. Method Electronic databases (e.g. Medline, Embase and CINAHL) were searched from 1992 to May 2005. This was supplemented by searching PJ-online, IJPP online conference abstracts and the bibliographies of retrieved articles. Analysis of the findings explored the quality of the assessed papers, stakeholders' perceptions of repeat dispensing, the impact on professional relationships and workload, quality of care and prescription cost savings. Key findings Four randomised controlled trials (RCTs) and one before-and-after study were identified; most studies also incorporated a qualitative component. The findings indicated that patients' satisfaction with repeat dispensing was high, mainly as the service was seen as more convenient and time saving. While pharmacists considered that their relationship with patients had improved, one study found that patients did not necessarily agree and considered that pharmacists still remained in their dispensaries. Quality of care was considered in two RCTs, which indicated that more adverse reactions and compliance issues were identified in the intervention group. However, no direct comparisons were reported in differences in rates between intervention and control groups. Likewise, it was not possible to determine if any of the reported cost savings were solely attributable to repeat dispensing, as direct comparisons between groups were not reported. Conclusions Definitive conclusions about the effectiveness and impact of repeat dispensing are difficult to draw given a lack of transparency and systematicity when reporting these studies. Nevertheless, the findings suggest that there are high levels of patient satisfaction with the service. Likewise, it was not possible to draw conclusions about the possible savings on the NHS drug budget. Important policy decisions are being made about the implementation of repeat dispensing; however they are currently been made in a vacuum of adequate information. [source]


Review of small rural health services in Victoria: how does the nursing-medical division of labour affect access to emergency care?

Aims., This paper is based on a review of the Australian and International literature relating to the nursing-medical division of labour. It also explores how the division of labour affects patient access to emergency care in small rural health services in Victoria, Australia. Background., The paper describes the future Australian health workforce and the implications for rural Victoria. The concept of division of labour and how it relates to nursing and medicine is critically reviewed. Two forms of division of labour emerge , traditional and negotiated division of labour. Key themes are drawn from the literature that describes the impact of a traditional form of division of labour in a rural context. Methods., This paper is based on a review of the Australian and international literature, including grey literature, on the subject of rural emergency services, professional boundaries and roles, division of labour, professional relationships and power and the Australian health workforce. Results., In Australia, the contracting workforce means that traditional divisions of labour between health professionals cannot be sustained without reducing access to emergency care in rural Victoria. A traditional division of labour results in rural health services that are vulnerable to slight shifts in the medical workforce, unsafe services and recruitment and retention problems. A negotiated form of division of labour provides a practical alternative. Conclusion., A division of labour that is negotiated between doctors and nurses and supported by a legal and clinical governance framework, is needed to support rural emergency services. The published evidence suggests that this situation currently does not exist in Victoria. Strategies are offered for creating and supporting a negotiated division of labour. Relevance to clinical practice., This paper offers some strategies for establishing a negotiated division of labour between doctors and nurses in rural emergency care. [source]


Views on nurse prescribing: a survey of community mental health nurses in the Republic of Ireland

A nurse prescribing scheme has recently been implemented within the Republic of Ireland. This paper reports on the views of community mental health nurses on nurse prescribing just prior to the implementation of the scheme. Data were gathered through a 13-item questionnaire administered to 103 members of the Association of Community Mental Health Nurses in Ireland. Results indicated a distinct difference of view between male and female community mental health nurses, with female nurses having greater reservations towards the desirability of nurse prescribing in relation to educational preparation and impact on professional relationships. Overall, only 17% of respondents favoured being supervised in their prescribing practice by their consultant psychiatrist. The paper concludes that there is ambivalence towards prescribing in this important group of nurses which may need to be taken into account if nurse prescribing is to be successfully implemented within the Irish mental health service context. DUTCH GENRE PAINTING AS RELIGIOUS ART: GABRIEL METSU'S ROMAN CATHOLIC IMAGERY

Gabriel Metsu's reputation rests on his popular genre imagery; however, the Dutch artist also produced religious paintings with Roman Catholic symbolism. Archival documents, personal and professional relationships, and a small number of paintings confirm a close engagement with the Dutch Roman Catholic community from his youth in Leiden to his maturity in Amsterdam. Metsu's connections with Roman Catholic painters, poets, and priests in the Netherlands provided the cultural context for his religious art. Moreover, contemporary devotional publications and reproductive engravings after Italian art established iconographic and compositional solutions evident in Metsu's traditional religious scenes and in his genre imagery.
