[source] Challenges confronting clinicians in acute careJOURNAL OF NURSING MANAGEMENT, Issue 6 2009VICKI PARKER RN Aim, To engage acute care clinicians in prioritizing professional issues of concern and to help them identify and design change projects. Background, In order to meet and respond to challenges and to ensure safety, efficiency and positive patient and staff outcomes, it is imperative to understand the nature of difficulties faced by health professionals and for clinicians to be included in decision making and change. Method, A three-phase mixed-method design utilizing descriptive and interpretive approaches. Data were collected via survey, focus groups and nominal group workshops. Results, Communication, skill mix and work environments were identified as issues of most concern. Participants were able to identify and prioritize a range of projects to help them better understand and alleviate workplace problems. Conclusion, This study highlights key directions for practice change and confirms previous findings identifying urgent need for research that aims to overcome poor communication and skill shortages. It differs from other studies by providing a platform for participants to design projects leading to solutions and participate in change. Implications for nursing management, Support must be provided for managers in rostering, staffing, and resource procurement and allocation. The results of the present study highlights a need to refocus management styles on staff empowerment, participation and team building. SIMPSON ba ma rmn When working with adult survivors of childhood sexual abuse, the pairing of client with practitioner has implications for both parties. Many therapeutic pairings are ad hoc or based on stereotypical assumptions made by referrers or practitioners themselves. This paper reports on a small-scale study of practitioners working in the mental health directorate of an NHS Trust in Wales. One aim of the study was to explore practitioners' views on appropriate therapeutic pairings. Fifty-three respondents completed a postal questionnaire and six face-to-face interviews were also conducted. Key themes emerged from the study. These were social and cultural stereotyping, gender specific issues, the therapeutic relationship, service and professional issues and special opinions and unique angles. The terms ,therapist' and ,practitioner' will be used interchangeably in this paper. [source] Doctors' professional values: results from a cohort study of United Kingdom medical graduatesMEDICAL EDUCATION, Issue 8 2001Lorelei Cooke Objectives To examine young doctors' views on a number of professional issues including professional regulation, multidisciplinary teamwork, priority setting, clinical autonomy and private practice. Method Postal survey of 545 doctors who graduated from United Kingdom medical schools in 1995. Results Questionnaires were returned by 95% of the cohort (515/545). On issues of professional regulation, teamwork and clinical autonomy, the majority of doctors held views consistent with current General Medical Council guidance. The majority supported the right of doctors working in the NHS to engage in private practice. Most respondents thought that public expectations of doctors, medicine and the NHS were too high, and that some form of rationing was inevitable. On many issues there was considerable variation in attitudes on the basis of sex and intended branch of medicine. Conclusions The results highlight the heterogeneity of the profession and the influence of specialty and gender on professional values. Doctors' attitudes had also been shaped by broader social changes, especially debates surrounding regulation of the profession, rising public expectations and the need for rationing of NHS care.
