Examination stress in Singapore primary schoolchildren: how compliance by subjects can impact on study results

Objective: Examinations are anecdotally viewed as extremely stressful to Singapore schoolchildren. We test this postulate by obtaining parental ratings of children's emotional stress levels longitudinally in a large representative sample of sixth (P6) and fifth (P5) class primary schoolchildren, respectively, exposed and unexposed to a streaming examination. Method: Children's stress levels were rated monthly by a parent for 10 months. Results: Analyses failed to find evidence of any differential stress impact across P6 and P5 comparison groups, apart from a subset of P6 children whose parents complied with every monthly survey. Conclusion: The streaming examination in the final year of primary school did not emerge as a general stressor to children, but achieved salience within a defined subset of children whose parents were highly study compliant. Study compliance may be a proxy variable of some import, and have wider relevance to other cohort studies and to intervention trials. [source]


A sociodental approach to assessing dental needs of children: concept and models

Summary. Objectives., Traditional normative methods of assessing dental needs do not correspond to current concepts of ,health' and ,need'. Although there is dental research on quality of life, evidence-based practice, and oral behaviours, those concepts are rarely applied to dental needs estimation. Dental needs are usually calculated mainly from clinical data and are likely to be inaccurate. A structured comprehensive method for assessing dental needs is required. The objectives of this study are to develop and test a new sociodental system of needs assessment for overall dental needs of primary schoolchildren. Furthermore, normative and sociodental estimates of need are compared. Design., The study developed a theoretical framework and pathway algorithms of sociodental needs assessment and applied them to assessing overall dental needs. Normative dental needs were assessed using standard normative criteria. The child oral impacts on daily performances (Child-OIDP) was used to assess oral impacts, and a self-administered questionnaire was used to obtain information on demographic variables and oral behaviours. Data were analysed according to the developed algorithms. Setting., A cross-sectional survey in Suphanburi Province, Thailand. Participants., All 1126 children aged 11,12 years in a town. Main results., The sociodental approach was acceptable and not costly. In all, 54·4% had normative need under the dental needs model for life-threatening and progressive conditions, but only 16·6% had high propensity-related need; the remaining 37·8% would require dental health education or oral health promotion (DHE/OHP) or both and appropriately adjusted clinical interventions. Under the basic model of dental needs, 45·1% had normative need. Two-thirds of them (30·9%) had impact-related need and the remaining 14·2% did not have oral impacts and therefore should only receive dental health education. Only one-third of those with impact-related need had high propensity and were suitable for evidence-based conventional treatments; the remaining two-thirds should receive DHE/OHP and alternative clinical interventions. Conclusions., A sociodental system of dental needs assessment was developed and tested on school children. It decreased the estimates of conventionally assessed dental treatment needs and introduced a broader approach to care. [source]


Weight status, energy-balance behaviours and intentions in 9,12-year-old inner-city children

Abstract Background:, Dutch youth health care promotes four so-called energy-balance behaviours for the prevention of obesity: increasing physical activity, reducing sedentary behaviour and sugar-containing drinks, and eating breakfast. However, data on the prevalence of these behaviours and intentions to engage in them among primary schoolchildren is limited, especially for multi-ethnic, inner-city populations. The present study aimed to provide these data and explore differences according to socio-demographic characteristics and weight status. Methods:, Data on behaviours and accompanying intentions were collected using classroom questionnaires. Stature and body weight were measured by trained staff. Twenty primary schools in Rotterdam participated. Data on 1095 9,12 year olds (81.7% response rate) were available for analysis. Multiple logistic regression analyses were conducted to determine associations between behaviours (favourable or unfavourable), intentions (positive or not), gender, age, ethnicity, neighbourhood income level and weight status. Results:, The prevalence of being overweight was 30.4%, including 9.0% obesity. Engagement in energy-balance behaviours varied from 58.6% for outdoor play (>1 h previous day) to 85.9% for active transportation to school (day of survey). The highest positive intentions were reported for taking part in sports (83.9%), and lowest for reducing computer time (41.3%). Small differences in behaviours and intentions according to socio-demographic characteristics were found, most notably a lower engagement in physical activity by girls. Skipping breakfast and total number of energy-balance behaviours were associated with being overweight. Conclusions:, The prevalence of being overweight among Dutch inner-city schoolchildren is high. A general rather than a differentiated approach is needed to improve engagement in energy-balance behaviours among inner-city schoolchildren. [source]


Child sun protection: Sun-related attitudes mediate the association between children's knowledge and behaviours

Aim: To describe and investigate the relationship among the sun-related knowledge, attitudes and behaviours of New Zealand primary schoolchildren and consider the roles of sex and school year level. Methods: A randomly selected, two-stage cluster sample of 488 children from 27 primary schools in five regions of New Zealand was surveyed regarding their sun-related knowledge, attitudes and behaviours. A scoring system was used to assign a knowledge, attitude and behaviour score to each child. Results: Although knowledge increased with school year level, there was a decline in sun protective attitudes and behaviours. There was little variation in knowledge, attitudes and behaviour between boys and girls, but sex,year level interactions were found for knowledge and behaviour. When considering children's knowledge, attitudes and behaviours simultaneously, knowledge was only significantly associated with behaviours when mediated by attitudes. Conclusions: When targeting child sun protection and skin cancer prevention programmes, a focus on attitudes towards sun exposure and a suntan may prove beneficial in influencing sun-related behaviours. The EuroPrevall-INCO surveys on the prevalence of food allergies in children from China, India and Russia: the study methodology

Abstract Background:, Very little is known regarding the global variations in the prevalence of food allergies. The EuroPrevall-INCO project has been developed to evaluate the prevalence of food allergies in China, India and Russia using the standardized methodology of the EuroPrevall protocol used for studies in the European Union. The epidemiological surveys of the project were designed to estimate variations in the prevalence of food allergy and exposure to known or suspected risk factors for food allergy and to compare the data with different European countries. Methods:, Random samples of primary schoolchildren were recruited from urban and rural regions of China, Russia and India for screening to ascertain possible adverse reactions to foods. Cases and controls were then selected to answer a detailed questionnaire designed to evaluate the possible risk factors of food allergies. Objective evidence of sensitisation including skin-prick test and serum specific IgE measurement was also collected. Results:, More than 37 000 children from the three participating countries have been screened. The response rates for the screening phase ranged from 83% to 95%. More than 3000 cases and controls were studied in the second phase of the study. Furthur confirmation of food allergies by double blind food challenge was conducted. Conclusions:, This will be the first comparative study of the epidemiology of food allergies in China, India, and Russia using the same standardized methodology. The findings of these surveys will complement the data obtained from Europe and provide insights into the development of food allergy. [source]


Assessing the effectiveness of a school-based oral health promotion programme in Yichang City, China

Abstract,,, Objectives:, To assess the outcome of oral health promotion in schoolchildren over a 3-year period in Yichang City, Hubei, China. Methods:, In a cluster randomized controlled trial, the concept of the World Health Organization Health Promoting Schools Project was applied to primary schoolchildren. Seven intervention schools and eight control schools were randomly selected from one district by stratified cluster sampling. The study was conducted as a 3-year follow-up study. After 3 years, 661 children remained in the intervention group and 697 children in the control group. Data on dental caries, plaque accumulation, and sulcus bleeding were collected by clinical examination, while behavioural data were gathered by self-administered questionnaires. Results:, The 3-year net mean DMFS increment score was 0.22 in the intervention schools and 0.35 in the control schools (P < 0.013). A statistically significant difference in mean plaque (P < 0.013) and sulcus bleeding (P < 0.005) increment scores after 3 years was found between the two groups. Statistically significant higher scores were observed in restorations received and sealants placed, and a lower score in untreated dental caries, in children from the intervention group than the control group after 3 years (P < 0.01). In addition, more children in the intervention schools adopted regular oral health behavioural practices such as brushing their teeth at least twice a day, visiting the dentist within the past calendar year, and using fluoride toothpaste. Conclusion:, The study suggests that the school-based oral health promotion was an effective way to reduce new caries incidence, improve oral hygiene and establish positive oral health behavioural practices in the targeted schoolchildren.
