Selected AbstractsPolicy dependency and reform: economic gains versus political painsAGRICULTURAL ECONOMICS, Issue 2-3 2004David R. Harvey Abstract Economic analysis condemns market intervention in favour of farmers as inefficient and ineffective, and therefore worthy of radical reform. Practical experience, however, indicates that such lessons are hard to learn and implement. Economic analysis tends to ignore the path dependencies generated by the policy evolution process. Without reform strategies that take full account of these dependencies, policy reform will continue to be reluctant, slow and frequently counter-productive. This paper reconsiders the evolution of farm policies and the economic assessment of their costs and benefits. In so doing, it re-phrases conventional economic arguments in terms which seem to accord better with sensible intuition, which may prove more accessible and credible to policymakers and advisors. The difficulties of reconciling economic efficiency with political acceptability are identified. The paper concludes with a substantial challenge to the agricultural economics profession. Crishna Abstract Background There is a need to study methodologies for evaluating social development projects. Traditional methods of evaluation are often not able to capture or measure the ,spirit of change' in people, which is the very essence of human development. Using participatory methodologies is a positive way to ensure that evaluations encourage an understanding of the value of critical analysis among service providers and other stakeholders. Participatory evaluation provides a systematic process of learning through experiences. Methods Practical experiences of conducting a number of evaluation studies in social development projects have led the author to develop four basic principles of participatory evaluation strategies. This has been further conceptualized through an extensive literature search. The article develops and shares these principles through descriptions of field experiences in Asia. Results The article illustrates that the role of any evaluation remains a learning process, one which promotes a climate of reflection and self-assessment. It shows how using participatory methods can create this environment of learning. However, one needs to keep in mind that participatory evaluation takes time, and that the role and calibre of the facilitator are crucial. Conclusion Participatory evaluation methods have been recommended for social development projects to ensure that stakeholders remain in control of their own lives and decisions. [source] Enforcement of environmental charges: some economic aspects and evidence from the German Waste Water ChargeENVIRONMENTAL POLICY AND GOVERNANCE, Issue 5 2001Professor Dr Erik Gawel Enforcement problems are usually analysed with respect to command-and-control measures of environmental regulation. The recognition that any environmental policy instrument entails an enforcement problem in principle is basic to a comparative analysis of enforcement effects. This paper deals with the comparative enforcement effects of charges: How does enforcement of a charge function? Which problems occur particularly in the enforcement of charges? Could enforcement be facilitated by a specific construction of charge laws? Are economic concepts of charge enforceable at all, and if so, under what conditions? Are charges more readily enforceable than other instruments? Therefore, some economic theory assessments of enforcement processes are presented first. In a third part, the paper sheds light on the practical experience made with the enforcement of the German Waste Water Charge. It is argued that the well worn thesis of an enforcement-friendly ,self-control' of market instruments is based on unrealistic assumptions. Whether against this background enforcement of environmental policy can be facilitated by an increased application use of charges must be viewed sceptically in an overall assessment of the problematic. Moreover, the transition from allocative control tasks to fiscal environmental charges may well be a symptom of rather than a contribution to the solution of the political and administrative crisis of enforcement. Especially for charges, the crucial question seems to be the political implementation rather than concrete enforcement by local authorities. Canisso Summary Donkeys are bred throughout the world and often play an important role in agriculture. They are also bred to produce mules. Traditionally, jacks are considered challenging to breed in domestic conditions, whether for natural breeding or semen collection using either jennies or mares. The donkey's natural sexual behaviour significantly differs from that of other domestic animals. This presents challenges for in-hand donkey breeding, particularly on mule studs where normally only jacks and mares are kept for breeding. This article describes some of the authors' observations on sexual behaviour in donkeys and practical experience of some of the strategies employed to apply this knowledge to breeding management, in order to improve the success of using donkeys for both natural service and semen collection. [source] Description and evaluation of an education and communication skills training course in HIV and AIDS for dental consultantsEUROPEAN JOURNAL OF DENTAL EDUCATION, Issue 2 2000D. A. Lewis A 2-day course was organised for dental hospital consultants as part of a project on raising awareness of dental staff about HIV and AIDS. The course comprised an information update, practical experience in the diagnosis of oral conditions and a ,hands-on' exercise in infection control. The 2nd day of the course consisted of experiential communication skills training using rôle-play with actors and video feedback. Evaluation of the course showed that the consultants perceived the course to be valuable. There was a general improvement in dentists' confidence in their knowledge, ability to communicate with HIV-positive patients and in talking to staff who are unwilling to provide treatment. These changes are statistically significant and these skills are still being utilized and maintained 2 years later. Information and training packs prepared by multidisciplinary groups using a variety of teaching methods should be made available to those involved in training dental staff. Today it is neither. Its study resides primarily in those countries that have both public lands, which hold fire, and scientific institutions, with which to study it. In particular, forestry has long claimed fire as a speciality and continues to harbour the most practical experience regarding it. In fact, fire may deserve better, and can give more. A case, not entirely whimsical, can be made for a programme of ,fire studies' that could span the many forms of scholarship that share an interest in humanity's species monopoly over fire's manipulation. [source] Distribution-based anomaly detection in 3G mobile networks: from theory to practiceINTERNATIONAL JOURNAL OF NETWORK MANAGEMENT, Issue 5 2010Alessandro D'Alconzo The design of anomaly detection (AD) methods for network traffic has been intensively investigated by the research community in recent years. However, less attention has been devoted to the issues which eventually arise when deploying such tools in a real operational context. We designed a statistical based change detection algorithm for identifying deviations in distribution time series. The proposed method has been applied to the analysis of a large dataset from an operational 3G mobile network, in the perspective of the adoption of such a tool in production. Our algorithm is designed to cope with the marked non-stationarity and daily/weekly seasonality that characterize the traffic mix in a large public network. Several practical issues emerged during the study, including the need to handle incompleteness of the collected data, the difficulty in drilling down the cause of certain alarms, and the need for human assistance in resetting the algorithm after a persistent change in network configuration (e.g. a capacity upgrade). We report on our practical experience, highlighting the key lessons learned and the hands-on experience gained from such an analysis. [source] Experience with Delay-Tolerant Networking from orbit,INTERNATIONAL JOURNAL OF SATELLITE COMMUNICATIONS AND NETWORKING, Issue 5-6 2010W. Ivancic Abstract We describe the first use from space of the Bundle Protocol for Delay-Tolerant Networking (DTN) and lessons learned from experiments made and experience gained with this protocol. The Disaster Monitoring Constellation (DMC), constructed by Surrey Satellite Technology Ltd (SSTL), is a multiple-satellite Earth-imaging low-Earth-orbit sensor network in which recorded image swaths are stored onboard each satellite and later downloaded from the satellite payloads to a ground station. Store-and-forward of images with capture and later download gives each satellite the characteristics of a node in a disruption-tolerant network. Originally developed for the ,Interplanetary Internet,' DTNs are now under investigation in an Internet Research Task Force (IRTF) DTN research group (RG), which has developed a ,bundle' architecture and protocol. The DMC is technically advanced in its adoption of the Internet Protocol (IP) for its imaging payloads and for satellite command and control, based around reuse of commercial networking and link protocols. These satellites' use of IP has enabled earlier experiments with the Cisco router in Low Earth Orbit (CLEO) onboard the constellation's UK-DMC satellite. Earth images are downloaded from the satellites using a custom IP-based high-speed transfer protocol developed by SSTL, Saratoga, which tolerates unusual link environments. Saratoga has been documented in the Internet Engineering Task Force (IETF) for wider adoption. We experiment with the use of DTNRG bundle concepts onboard the UK-DMC satellite, by examining how Saratoga can be used as a DTN ,convergence layer' to carry the DTNRG Bundle Protocol, so that sensor images can be delivered to ground stations and beyond as bundles. Our practical experience with the first successful use of the DTNRG Bundle Protocol in a space environment gives us insights into the design of the Bundle Protocol and enables us to identify issues that must be addressed before wider deployment of the Bundle Protocol. Abstract A psychometric evaluation of the Beck Depression Inventory (BDI) was carried out on contemporary studies to ascertain its suitability for use in an alcohol-dependent population. Three criteria were used for this: factor analysis, test,retest reliability and internal consistency reliability. Factor analysis revealed that its structure is consistent with either two or three factor models, depending on the population. Test,retest results concluded that the correlation coefficient remained above the recommended threshold and internal consistency reliability highlighted alpha coefficient results consistently above suggested scores, leading to the conclusion that the BDI is probably an effective screening tool in an alcohol-dependent population. [source] A Model for Mapping Linkages Between Health and Education Agencies to Improve School HealthJOURNAL OF SCHOOL HEALTH, Issue 2 2000Lawrence St. Leger ABSTRACT: Efforts to develop effective and sustainable school health programs evolved in sophistication the past 20 years through research and practical experience. This paper reviews these developments, arguing they were significantly driven by public health priorities, and have not adequately accounted for educational perspectives and priorities. To better understand the differences in perspective, a model is presented which illustrates linkages between different school-based inputs and strategies, and long-term health and educational outcomes. The model describes similarities and differences between the two perspectives. A significant coincidence exists in factors that determine educational attainment and improved health outcomes for students. A more holistic and integrated approach to school health is emerging, and at these interfaces our implementation and research efforts for the 21st century should be concentrated. [source] Surgical skills training: simulation and multimedia combinedMEDICAL EDUCATION, Issue 9 2001Roger Kneebone Context Basic surgical skills are needed throughout the medical profession, but current training is haphazard and unpredictable. There is increasing pressure to provide transparency about training and performance standards. There is a clear need for inexperienced learners to build a framework of basic skills before carrying out surgical procedures on patients. Effective learning of a skill requires sustained deliberate practice within a cognitive framework, and simulation offers an opportunity for safe preparation. Objectives This paper presents a new approach to basic surgical skills training, where tuition using a specially designed computer program is combined with structured practice using simulated tissue models. This approach to teaching has evolved from practical experience with surgical skills training in workshops. Methods Pilot studies with 72 first-year medical students highlighted the need for separate programs for teaching and for self-directed learning. The authors developed a training approach in the light of this experience. Subsequent in-depth observational and interview studies examined (a) individual teaching sessions between surgical teachers and learners (five consultant surgeons and five senior house officers) and (b) group teaching sessions with general practitioners (14 participants in three group interviews). Further work has resulted in a self-directed learning program. Conclusions Qualitative analysis of observational and interview data provides strong preliminary support for the effectiveness of this approach. The response of teachers and learners was extremely positive. The combination of information (presented by computer) and practice of psychomotor skill (using simulated tissue models) could be extended to other surgical and practical skills. Dayteg Abstract In modern plant breeding, DNA marker analyses are of increasing importance and, as the methods become more widely adopted, the capacity for high-throughput analyses at low cost is crucial for its practical use. Automation of the analysis processes is a way to meet these requirements. In order to achieve this, while keeping adequate flexibility in the analysis processes, Svalöf Weibull AB (SW) has developed a fully automated polymerase chain reaction system. It has been evaluated on barley and canola lines and is capable of analysing up to 2200 samples per day at a cost of 0,24 , per analysis for marker-assisted selection and quality control of genetically modified organisms. A detailed description of this system is given, and improvements to the throughput and applications are discussed. Methods: Information on patients' experience was gathered from non-professional caregivers by an interview. A specific question was asked about the caregivers' emotional and practical experiences while assisting a terminally ill relative. A content analysis of the open question on caregivers' perceptions was performed on transcribed answers. Three researchers independently generated categories. Subsequently, areas where they differed were reconsidered and an interpretation was agreed upon. Results: Valid interviews were obtained from 1231 non-professional caregivers. Answers were classified according to the perception of the experience as positive (33.1%), negative (65.1%) or neutral (1.8%). Conclusion: Assisting a family member with cancer in his/her last three months of life is a very strong physical and mental stress for the caregiver. In some cases, this experience is nevertheless perceived as an evolution chance. Byrd R.N., Ph.D. Abstract Nurses have the potential to promote the public's health through active involvement in shaping health and social policy. Preparing nursing students to influence public policy is a major component of the curriculum in public/community health nursing. A series of learning experiences was designed to engage students in this process. First, students participate in information sessions at the State Health Department and the State House. This provides them with opportunities to engage in dialogue with public health leaders as well as advocates from both professional organizations and community groups. Next, students identify the legislators who represent them in the community and write a narrative that describes the legislators' interest and commitment to health-related legislation. Lastly, students work in clinical groups to analyze a public health problem that can be addressed through public policy interventions. This has led to the students testifying at legislative hearings and working with community groups involved with the issue. The students present their findings to their peers and to the wider college community. Through these learning experiences, students gain practical experience in understanding the political process that leads to important policy change. This in turn prepares them for their roles as professional nurses and involved citizens. [source] Prehistoric trepanation in the Cuzco region of Peru: A view into an ancient Andean practiceAMERICAN JOURNAL OF PHYSICAL ANTHROPOLOGY, Issue 1 2008Valerie A. Andrushko Abstract In this study, patterns of prehistoric trepanation in the southern highlands of Peru were examined through an analysis of 11 Cuzco-region burial sites. Trepanations were found in 66 individuals, with several individuals exhibiting more than one trepanation, for a total of 109 perforations observed. The predominant methods used were circular cutting and scraping,methods that proved highly successful with an overall 83% survival rate and little ensuing infection. Survival rates showed a significant increase over time, apparently reflecting improvements in trepanation technique through experimentation and practical experience. Practitioners avoided certain areas of the cranium and employed methods that reduced the likelihood of damage to the cerebral meninges and venous sinuses. In many cases, trepanation as a medical treatment appears to have been prompted by cranial trauma, a finding that corroborates other studies pointing to cranial trauma as a primary motivation for the surgical procedure. [source] Brief Report: Quality Improvement in Critical Access Hospitals: Addressing Immunizations Prior to DischargeTHE JOURNAL OF RURAL HEALTH, Issue 4 2003Edward F. Ellerbeck MD These hospitalizations may represent a missed opportunity to address immunizations. Addressing these missed immunizations could provide an opportunity for CAHs to gain practical experience in data-driven quality improvement. Purpose: To improve documentation and delivery of influenza and pneumococcal immunizations prior to hospital discharge and provide CAHs with quality improvement experience. Methods: We recruited 17 CAHs in Kansas to participate in a rapidcycle quality improvement project to address inpatient immunizations. Each hospital identified patient discharges on a monthly basis and abstracted medical records to see if the patient's immunization status had been assessed and if patients had been vaccinated prior to discharge. Findings: Documentation of influenza immunization status improved from 17% of admissions at baseline to 62% at follow-up (P<0.001). Documentation of pneumococcal immunization status increased from 36% at baseline to 51% at follow-up (P<0.001). Documentation of immunizations was significantly higher among the 8 hospitals that developed standard charting forms for recording immunization status (P<0.01). Despite improved documentation of immunization status, at remeasurement only 3.4% received an influenza vaccination and 1.3% received a pneumococcal vaccination prior to discharge. Conclusions: Critical access hospitals can effectively participate in quality improvement activities, but increased involvement of medical staff or standing immunization orders may be needed to improve actual vaccine administration prior to discharge. [source] Use of Simulation Technology in Forensic Medical EducationACADEMIC EMERGENCY MEDICINE, Issue 2009Heather Rozzi Although the emergency department often provides the first and only opportunity to collect forensic evidence, very few emergency medicine residencies have a forensic medicine curriculum in place. Most of the existing curricula are composed only of traditional didactics. However, as with any lecture-based education, there may be a significant delay between the didactic session and clinical application. In addition, traditional curricula lack the opportunity for residents to practice skills including evidence collection, documentation, and use of a colposcope. At York Hospital, we have developed a forensic curriculum which consists of both traditional lectures and practical experience in our Medical Simulation Center. As part of their educational conference series, residents receive presentations on domestic violence, child abuse, elder abuse, evidence collection, sexual assault, ballistics, pattern injuries, documentation, forensic photography, and court testimony. Following these presentations, residents have the opportunity to apply their knowledge of forensic medicine in the Simulation Center. First, they interview a standardized patient. They then utilize the mannequins in the Simulation Center to practice evidence collection, photo documentation, and use of our specialized forensic medicine charts. After evidence collection and documentation, the residents provide safety planning for the standardized patients. Each portion is videotaped, and each resident is debriefed by victim advocates, experienced sexual assault nurse examiners, and emergency department faculty. The use of simulation technology in resident education provides the opportunity to practice the skills of forensic medicine, ultimately benefiting patients, residents, and law enforcement, and permitting teaching and evaluation in all six core competency areas. However, sufficient placements are not always available. A new fieldwork program attempts to meet this need. Three groups of students conducted risk assessments throughout the academic year, with generally positive results. Students enjoyed the self-directed nature of the placement and the opportunity to learn generic and occupational health related skills. Although there were some limitations to the program, we believe that fieldwork placements met the learning needs of second-year students and offered them an excellent opportunity to gain practical experience related to occupational health practice in prevention. In this review we gather medical, surgical and psychological literature pertaining to adults with CAH and consider this alongside practical experience gained from a dedicated adult CAH clinic. There is increasing awareness for the need for multidisciplinary teams who have knowledge of CAH particularly with respect to gynaecological surgery and clinical psychology for women and testicular function in men. Routine management of CAH comprises adjustment of glucocorticoid and mineralocorticoid treatment according to individual needs balancing biochemical markers, compliance and long term risks. Bone density is one such long term concern and is not greatly reduced in most individuals with CAH. More recently, attention has turned to cardiovascular risk factors and catecholamine deficiency in adults with CAH. Women with CAH require access to an experienced gynaecologist, specialised pregnancy care and psychosexual support. The very low fertility rates for women with CAH previously reported appear to be improving with time. Men with CAH are often lost to follow up and therefore miss out on surveillance for hypogonadism either through the effect of adrenal rests of from suppression of gonadotrophins resulting in a high prevalence of oligospermia. Johnson Objective: This paper describes the development of an instrument for description and classification of mental health services and for measurement of service use. Purposes to be served by the instrument include: (i) identification of gaps in the spectrum of services in a catchment area; (ii) obtaining background information which may be important to understanding why apparently similar interventions lead to different outcomes in different areas; (iii) investigating how introduction of a particular type of service influences use of other local services; and (iv) understanding the relationship between sociodemographic factors and service use. Method: The instrument was developed through meetings of an international expert panel and pilot stages in several European countries. Results: Use of the European Mapping Service Mapping Schedule (ESMS) appears feasible in several countries and allowed description and classification of the full range of services identified within each of the study catchment areas. Their isolation and purification from natural sources can sometimes be very difficult and time-consuming when traditional phytochemical techniques are used. Countercurrent chromatography (CCC), a support-free liquid,liquid partition chromatography technique, is very useful for the isolation of polar compounds and its use is increasing in the natural products field. In this paper, we propose strategies of solvent system selection for the isolation of flavonoids by CCC, based on data from the literature, plus incorporation of own practical experiences. The selected references report the isolation of over 300 different flavonoid compounds from more than 100 plant species, using 40 different solvent systems, showing the versatility of this technique. The solvent system hexane-ethylacetate-methanol-water is proposed as a starting point for the separation of samples containing free flavonoids, as it was cited in more than 60% of the papers. A "fine tuning" step is proposed at each level of this solvent family. Other modifications include exchanging the alcohol in the system as well as introducing a fifth solvent. The solvent system ethyl-acetate-butanol-water is proposed as the starting point for glycosylated flavonoids. Other solvent systems are also discussed. The use of gradients is proposed for samples containing both free and glycosylated flavonoids, as the polarity window is larger in these cases. High-speed countercurrent chromatography was used in 89% of the reviewed data. Digital signal processing (DSP) software is hard real-time software that is a part of the software system needed in a base station. This article reports practical experiences related to organizing the development of embedded software in the telecommunication industry, at Nokia Networks. The article introduces the main factors influencing the development of DSP software and also compares the evolutionary process under study with both selected organizational models for a software product line and a multistage model for the software life cycle. We believe it is vitally important to formulate the organization according to the software architecture, and it is essential to have a dedicated development organization with long-term responsibility for the software. History shows that without long-term responsibility, there is no software reuse. In this paper we introduce a new organizational model for product line development. This new hybrid model clarifies long-term responsibilities in large software organizations with hundreds of staff members and formulates the organization according to the software architecture. Our case needs a couple more constraints to keep it in the evolution stage of the software life cycle. Using qualitative inquiry methodology, this study investigates the three referential frameworks (i.e., theoretical knowledge, practical experiences, and team consciousness) of the team mental models employed in a nonprofit organization which provides therapy for hearing-impaired children in Taiwan. The results indicate that fostering a strong team mental model may be an effective way to ensure that the performance of value-laden practice meets a certain expectation. Moreover, I present arguments that because various cognitive properties of team mental models are formed, shared, and represented in different ways, they should be investigated and understood from a holistic perspective; and that it should also be cultivated by an integrative approach that utilizes theoretical, practical, and organizational knowledge.
