Nygaard Abstract Objective To study the association between informant stress and appraisal of patients' cognitive functioning as reported by the Informant Questionnaire on Cognitive Decline in the Elderly,IQCODE. Methods Routinely collected data from a geriatric outpatient department (207 dyads) during the years 1995,1998 were analysed. Relative stress scale (RSS) has been categorised for possible low, intermediate and high risk of psychiatric morbidity and caregivers were combined to four groups (female and male spouses and female and male non-spouses, respectively). The relationship between IQCODE (dependent) and categorised RSS and informant groups and patient age was further studied by means of the general linear model (GLM,UNIANOVA). Results In general, spouses reported better cognitive functioning than non-spouses. There was a significant association between IQCODE and RSS (p,<,0.001), and the composite variable informant group and informant gender (p,<,0.001). The main effect of the interaction term RSS,×,informant group,+,informant gender was not significant. Post hoc test, however, revealed a significant effect of the interaction term RSS,×,female spouses (p,<,0.001) on IQCODE. Conclusion IQCODE is associated with informant stress. Categorisation of RSS score into groups of low, intermediate and high risk for psychiatric morbidity can be a valuable contribution to a more meaningful application of RSS in general practice. [source] Phonation threshold flow measurements in normal and pathological phonationTHE LARYNGOSCOPE, Issue 4 2009Peiyun Zhuang MD Abstract Objectives/Hypothesis: Phonation threshold flow (PTF) may provide a tool to assess laryngeal function and could differentiate between normal and pathological voices. Both polyps and nodules contribute to an increased PTF by creating an incomplete glottal closure and increased vocal fold mass and thickness. Study Design: Prospective study. Methods: The Kay Elemetrics Phonatory Aerodynamic System (PAS) (Kay Elemetrics Corp., Lincoln Park, NJ) was used to collect mean flow rate (MFR) and PTF measurements from 40 normal subjects, 21 patients with vocal fold nodules, and 23 patients with vocal fold polyps. Gender-based differences were assessed using a t test. The effect of vocal pathology on PTF and MFR was determined with an ANOVA. Diagnostic potential was evaluated using a receiver operation characteristics (ROC) analysis. Results: Both PTF (P = .047) and MFR (P = .008) were significantly affected by gender. Using a two-way ANOVA and correcting for gender differences, the influence of pathology on PTF was determined to be significant (P < .001). Post hoc tests found a significant difference between normal and polyp subjects (P < .001) but not normal and nodule subjects (P = .177) or nodule and polyp subjects (P = .246). ROC analysis found that PTF (area under the curve [AUC] = 0.691) and MFR (AUC = 0.684) had a similar diagnostic utility. Conclusions: PTF can be used to differentiate between normal and pathological voices. As a parameter that is experimentally sensitive to the biomechanical parameters providing its theoretical basis, it could be used clinically to analyze laryngeal functionality. Future research could focus on measuring PTF in other pathologies, such as paralysis or scarring, which would also affect the effort required to produce voice. CHEETHAM Summary Reasons for performing study: The success of laryngoplasty is limited by abduction loss in the early post operative period. Objective: To determine the efficacy of polymethylmethacrylate (PMMA) in stabilising the cricoarytenoid joint (CAJ) and reducing the force on the laryngoplasty suture. Hypothesis: Injection into the cricoarytenoid joint resists the forces produced by physiological laryngeal air flows and pressures thereby reducing the force experienced by the laryngoplasty suture. Methods: Ten cadaver larynges were collected at necropsy and PMMA was injected into one CAJ at selected random. Each larynx was subjected to physiological conditions with with constant (static) or cycling (dynamic) flow. The specimens were tested sequentially in each of 4 conditions: 1) bilateral full abduction (Control 1); 2) transection of the suture on the side without PMMA; 3) bilateral abduction achieved by replacing the suture (Control 2); and 4) cutting the suture on the PMMA side. Tracheal pressure and flow and pressure in the flow chamber were recorded using pressure and flow transducers. The strain experienced by each suture during bilateral abduction (Controls 1 and 2) was measured. Statistical comparison of the 4 conditions was performed using a mixed effect model with Tukey's post hoc test for multiple comparisons. The strain gauge data were analysed by paired comparison of the regression slopes. Results: In the static and dynamic states, tracheal pressure increased and tracheal flow decreased when the suture on the non-cement side was cut (P<0.05). There was no significant difference in any outcome measure between PMMA injected into the CAJ and bilaterally abducted specimens (Controls 1 and 2) for either condition. The rate of increase in strain with increasing translaryngeal pressure was significantly less on the suture with PMMA placed in the CAJ (P = 0.03). Conclusions: These data provide strong evidence that injecting PMMA into the CAJ resists the collapsing effect of physiological airflows and pressures in vitro and reduces the force experienced by the laryngoplasty suture during maximal abduction. Potential relevance: Augmentation of prosthetic laryngoplasty with this technique may reduce arytenoid abduction loss in the early post operative period. Cytotoxicity and genotoxicity of sodium percarbonate: a comparison with bleaching agents commonly used in discoloured pulpless teeth. International Endodontic Journal, 43, 102,108, 2010. Abstract Aim, To evaluate the cytotoxicity and genotoxicity of sodium percarbonate (SPC) in comparison with bleaching agents used on discoloured pulpless teeth. Methodology, The cytotoxicity and genotoxicity of bleaching agents were evaluated both in their pure form as well as at concentrations commonly used in clinical practice. Hydrogen peroxide (HP), carbamide peroxide (CP), sodium perborate (SP) and SPC were diluted in Dulbecco's modified Eagle's medium (DMEM) in series. To evaluate the cytotoxicity, the survival of 3T3/NIH mouse fibroblasts was measured photometrically using an 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide assay after a 24 h-exposure period. Genotoxicity was indicated by micronuclei (MN) formation, and modification of the normal cell was analysed by light microscopy (400×). Statistical analysis was performed by one-way anova, followed by a multiple-comparison Tukey post hoc test (P < 0.05). Results, All groups exhibited a dose-dependent cytotoxicity. However, CP showed a similar cytotoxic effect when compared with DMEM-untreated control (UC) group. HP and SPC were significantly more cytotoxic than SP. The genotoxicity test showed that SPC and SP had an intermediate rate of MN frequency when compared with the UC group. The mean rate of MN frequency for HP was higher and statistically more significant than for the other groups tested. No difference was observed when CP and UC groups were compared. Conclusions, Sodium percarbonate showed cytotoxicity and genotoxicity similar to those of the other products tested. However, before SPC is used clinically, studies should be conducted to confirm its safety in vivo. [source] Evaluation of canal filling after using two warm vertical gutta-percha compaction techniques in vivo: a preliminary studyINTERNATIONAL ENDODONTIC JOURNAL, Issue 7 2006M. Venturi Abstract Aim, To evaluate the quality of root canal filling when comparing two warm gutta-percha filling techniques in vivo. Methodology, Human teeth were randomly divided into two equal groups, with 30 canals each. The root canals were shaped by hand and ProFile 0.04 rotary instruments to size 20,40 at the end-point and then filled with gutta-percha cones and AH-Plus. In group A, a traditional warm vertical compaction technique was performed using the Touch'n Heat, and back-filling with the Obtura II. In group B, a modified warm vertical compaction technique was used: small amounts of gutta-percha were removed, and the remaining most apical 3 mm were compacted with a 1 mm movement; then thermomechanical back-filling was performed. The teeth were extracted, stored in dye, cleared, and the distance between the apex and apical limit of the filling, linear dye penetration, and voids were measured from the buccal, lingual, mesial and distal perspective. The homogeneity of variance and means was verified using Levene's test and t -test. anova and Dunnett post hoc test were used to establish the significance and to analyse the effects through multiple comparisons. Results, Compared with the specimens of group A, the specimens of group B exhibited less mean linear dye penetration (P < 0.05), smaller void length (P , 0.05) and maximal width (P , 0.05) when examined in all four views, and a more precise filling when viewed from the buccal aspect (P < 0.05). Conclusions, The modified warm vertical compaction technique with apical back-filling produced a more effective and precise three-dimensional filling. [source] Evaluation of root-end cavity preparation using ultrasonic retrotipsINTERNATIONAL ENDODONTIC JOURNAL, Issue 9 2003H. Ishikawa Abstract Aim, To evaluate and compare the efficiency of root-end preparations using ultrasonic retrotips coated with diamond and zirconium nitride. Methodology, Eighty-five extracted single-rooted teeth were root filled, and then resected 3 mm from their apices. Root-end cavities were prepared with KiS (zirconium nitride-coated retrotip), CT-5 (stainless steel tip) or diamond-coated (DC) ultrasonic retrotips, and 10 teeth served as controls. Thirty teeth were used for evaluation of the time required to prepare the root-end cavity, the number of microcracks produced on the resected surface and the number of dentinal tubule openings on the root-canal wall using scanning electron microscope (SEM) images. A further 55 teeth were used for evaluation of dye penetration following filling of the root-end cavities with Super EBA. The degree of dye penetration in millimetres was measured under the microscope after 7 days of immersion in India ink. Statistical analyses were performed using the one-way anova and Scheffe's F -test as the post hoc test. Results, There was no significant difference in the number of microcracks and dentinal tubule openings present in the root apices prepared by the three retrotips. The time required for root-end cavity preparation using the DC retrotip was significantly less than that using the other groups (P < 0.01). Positive controls showed dye penetration throughout the length of the root-end cavity, and negative controls showed no dye penetration. There was no significant difference between the three experimental groups in dye penetration. Conclusions, In this laboratory study, the time required to prepare root-end cavities using KiS retrotips was the same as that using CT-5 retrotips, and longer than that using DC retrotips. There was no significant difference in the number of microcracks or dye penetration between the three kinds of retrotips. Assenza Summary During long-distance exercise, branched-chain amino acid (BCAA) catabolism could lead to an increase in the blood tryptophan/BCAA ratio and an early onset of ,central fatigue'. Based on these considerations, we studied the modifications of blood serum BCAA and tryptophan (Try) levels in 30 endurance horses competing in rides varying in distance from 20 to 72 km. From all horses, blood samples were drawn just before and just after the end of the ride. Samples were analysed for their leucine (Leu), valine (Val), isoleucine (Iso) and Try levels. Data were processed by anova, using sampling moment and ride as factors, and by LSD post hoc test. Significant differences were recorded among the different distance rides for Leu, Val, Iso, Try, Try/BCAA ratio; the same trend was recorded between samples taken at the start and the end of the race for Val and Leu. The main effect observed was an increase of BCAA levels for all rides, except the 72-km ride; for Try, a significant increase was present in all races, except the 50-km ride. The Try/BCAA ratio decreased in 20- and 50-km races and increased in the others. These data confirm that long-distance exercise involves a mobilization of BCAA. The utilization of BCAA seems to be important in prolonged exercise: in the 72-km ride, we observed a decrease in BCAA blood serum levels, while a major role of Try was indicated by its increase, resulting in a rise of the Try/BCAA ratio. Carey Background:, Cardiovascular disease (CVD) is the main cause of premature death in the UK and accounts for 36% of all premature male deaths and 27% of female deaths every year (British Heart Foundation, 2006). Although many risk factors for CVD are known, family history has been identified as being of particular importance in premature CVD (Lloyd-Jones et al., 2004). Recently, it was suggested that an elevated homocysteine (tHcy) may be associated with premature CVD (Homocystiene Studies Collaboration, 2002). The main genetic determinant of tHcy is the common 677 C,T polymorphism, in the enzyme methylenetetrahydrofolate reductase (MTHFR), which is prevalent in approximately 10% of the UK population. Relatively few studies have examined the association between tHcy and premature CVD and hardly any have considered the role of this polymorphism. The aim of this study therefore was to examine the relationships between the MTHFR 677 C,T polymorphism, tHcy and a family history of CVD in patients with established premature CVD. Methods:, An analysis was conducted on medical, lifestyle and family history data collected from patients and age-sex matched controls, recruited through the GENOVIT study in 2003. This case,control study involved n = 404 premature CVD patients and a similar number of age-sex matched controls, all of whom were screened for the TT genotype. A subset of patients (n = 196) and controls (n = 167) provided a blood sample, from which the tHcy concentration was established. Independent sample t -tests were used to determine differences between patients and controls and differences among genotype groups were examined using a one-way analysis of variance, followed by a Tukey's post hoc test. Results:, Plasma tHcy was significantly elevated in patients with a family history of CVD (compared to those without) (P = 0.013). A nonsignificant trend towards higher tHcy (compared to those without) was observed in patients with the TT genotype (P = 0.419). Furthermore, specifically in those with the TT genotype, those with a family history of CVD (compared to those without) showed significantly higher tHcy concentrations (P < 0.005). Those with the TT genotype who smoked had significantly higher tHcy (P < 0.05) than the CC and CT genotypes. Discussion:, The findings presented provide evidence to support an association between the MTHFR 677C,T polymorphism, elevated homocysteine and family history of premature CVD. Given that dietary levels of riboflavin have been shown to lower homocysteine specifically in individuals with the TT genotype (McNulty et al., 2006), these results have implications for the dietary management of premature CVD in those individuals with a genetic predisposition for elevated tHcy. In conclusion, further research in larger cohort numbers, regarding the correlation between family history, tHcy and the MTHFR polymorphism, would be beneficial for establishing their cause and effect relationship. Manfredini summary, Psychological factors play an important role in the aetiopathogenesis of temporomandibular disorders (TMD), as demonstrated by an increase in stress, anxiety, depression and somatization in TMD patients. The aim of this work was to investigate the presence of mood and panic-agoraphobic symptoms in different groups of TMD patients by means of a spectrum approach to psychopathology. A total of 131 subjects were included in this study and TMD signs and symptoms were investigated by means of a standardized clinical examination. Two self-report questionnaires were used to evaluate mood (MOODS-SR) and panic-agoraphobic (PAS-SR) spectrum. anova and Bonferroni's post hoc test for multiple comparisons were used to compare mean scores of all TMD groups for MOODS-SR, PAS-SR and all their domains. Results revealed a significantly higher prevalence of both mood (P < 0·001) and panic-agoraphobic (P < 0·01) symptoms in myofascial pain patients than in all other diagnostic groups (TMD-free, disc displacement and joint disorders). With regard to mood spectrum, strong differences emerged for all domains evaluating depressive symptoms. As for the panic-agoraphobic spectrum, myofascial pain patients differed from the other groups for the presence of stress sensitivity, panic, separation anxiety, hypochondriac and agoraphobic symptoms. It was concluded that myofascial pain patients differed from those with disc displacement, joint disorders and no TMD in relation to some psychopathological symptoms, while the last three groups presented very similar profiles. Yap summary, This study evaluated the corrosion behaviour of two high copper dental amalgam alloys [Dispersalloy (Dentsply-Caulk) and Tytin (Kerr)] in different electrolytes. Amalgam specimens were prepared, coupled to a copper wire, cemented into glass tubes and polished to a 600-grit finish. A corrosion cell was prepared using a carbon counter-electrode, a standard calomel electrode as the reference and amalgam as the working electrode. The alloys were tested in the following mediums at 37 °C: (i) artificial saliva based on Fusayama's solution (FS), (ii) artificial saliva with citric acid adjusted to pH 4·0 (FC) and (iii) 1% sodium chloride solution (SC). Corrosion potentials (Ecorr) and corrosion rates (Icorr) were determined using potentiostatic and impedance spectroscopy methods. Data was subjected to anova/Scheffe's post hoc test at 0·05 significance level. For both alloys, the corrosion potential in FS was significantly greater than in SC. Corrosion potential of Tytin in FS and SC was also significantly greater than in FC. The corrosion rate of Dispersalloy in FC was significantly greater than in FS and SC. For Tytin, corrosion rate in SC was significantly greater than in FS and FC. Although no significant difference in corrosion potential/rate was observed between the alloys when tested in FS, significant differences were observed when electrochemical testing was carried out in FC and SC. The corrosion behaviour of high copper amalgam alloys are both material and environment dependent. Certain food substances may increase the corrosion of high copper amalgams. [source] Marginal gap formation of composites in dentine: effect of water storageJOURNAL OF ORAL REHABILITATION, Issue 3 2003A. U. J. Yap summary, This study investigated the effects of water storage on the marginal adaptation of two composite resins (Spectrum TPH and Ariston AT), two polyacid-modified composite resins (Dyract AP and F2000) and a new PRG composite (Reactmer) to dentine over time. Two cylindrical dentine cavities (1·5 mm diameter and 1·5 mm deep) were prepared on the horizontally sectioned surfaces of freshly extracted teeth. In each tooth, one cavity was restored using composites with their respective bonding system and the other without the bonding system. The sample size for each material with/without bonding system was 6. The composites were placed in one increment, bulk-polymerized, immediately finished/polished with the Sof-lex discs system and fine polished on 30 ,m silicon carbide/9 ,m aluminium oxide lapping film discs in a microgrinding system. The specimens were then stored in distilled water at 37 °C and the maximum marginal gap width between the material and the dentine wall was determined at 24 h, 1 week, 2 weeks, 3 weeks and 4 weeks using a measurescope at ×500 magnification. Results were subjected to statistical analysis using monva, anova/Scheffe's post hoc test and independent samples of t -test at significance level 0·05. At all time intervals, no significant difference in marginal gap formation was observed between materials. Where bonding systems were not used, a decrease in gap widths over time was observed with most materials but only Dyract AP exhibited a significant decrease. The latter was observed after 2 weeks storage in water. The use of bonding systems reduced dentine marginal gaps significantly and is therefore mandatory for all composites evaluated. Marginal gaps arising from polymerization contraction of conventional, polyacid-modified and PRG composites cannot be fully compensated by hydroscopic expansion. Clinical relevance Bonding systems are mandatory for all composites when bonding to dentin. In the event of a bond failure at placement, hygroscopic expansion of composites can reduce marginal gaps but not completely. Aboushelib DDS Abstract Purpose: Selective infiltration etching (SIE) is a newly developed surface treatment used to modify the surface of zirconia-based materials, rendering them ready for bonding to resin cements. The aim of this study was to evaluate the zirconia/resin bond strength and durability using the proposed technique. Materials and Methods: Fifty-four zirconia discs were fabricated and divided into three groups (n = 18) according to their surface treatment: as-sintered surface (control group), airborne-particle abrasion (50-,m aluminum oxide), and SIE group. The zirconia discs were bonded to preaged composite resin discs using a light-polymerized adhesive resin (Panavia F 2.0). The zirconia/resin bond strength was evaluated using microtensile bond strength test (MTBS), and the test was repeated after each of the following intervals of accelerated artificial aging (AA): thermocycling (10,000 cycles between 5 and 55°C), 4 weeks of water storage (37°C), and finally 26 weeks of water storage (37°C). Silver nitrate nanoleakage analysis was used to assess the quality of zirconia/resin interface. A repeated measures ANOVA and Bonferroni post hoc test were used to analyze the data (n = 18, ,= 0.05) Results: There were significant differences in the MTBS values between the three test groups at each of the test intervals (p < 0.001). AA resulted in reduction in the bond strength of the as-sintered and the particle-abraded groups (5.9 MPa and 27.4, MPa, respectively). Reduction in the bond strength of these groups was explained by the observed nanoleakage across the zirconia/resin interface. The bond strength of the SIE specimens was stable after completion of AA (51.9 MPa), which also demonstrated a good seal against silver nitrate penetration across the zirconia/resin interface. Conclusion: SIE established a strong, stable, and durable bond to zirconia substrates. Conservative resin-bonded zirconia restorations are now possible using this new technique. Hatamleh BSc Abstract Purpose: To evaluate shear bond strength of Molloplast-B soft liner attached to different acrylic surfaces (smooth, rough, and Sticktech net fiber-reinforced interfaces) after 3000 thermal cycles. Materials and Methods: Sixty-nine specimens were fabricated by attaching Molloplast-B soft liner to acrylic bases of three interfaces (n= 23); smooth (Group 1, control), rough (Group 2), and Sticktech net fiber-reinforced interface (Group 3). The specimens underwent 3000 thermocycles (5 and 55°C) before being subject to a shear bond test at 2 mm/min crosshead speed. Debonding sites were investigated using an optical microscope at 40× magnification. Bond failures were categorized as adhesive, cohesive, or mixed. Results: Mean (SD) bond strength values (MPa) were: 0.71 (0.15); 0.63 (0.07); and 0.83 (0.12) for smooth, rough, and fiber-reinforced acrylic interfaces, respectively. The mean values were analyzed using one-way ANOVA and Bonferroni post hoc test for pairwise comparisons (p, 0.05). The net fiber-reinforced acrylic interface exhibited a statistically significantly higher bond strength value when compared to smooth and rough acrylic interfaces (P= 0.003 and P= 0.000, respectively). Modes of failure were mainly cohesive (91%), followed by mixed failures (9%). Conclusions: Molloplast-B exhibited a stronger bond to StickTech Net fiber-reinforced surfaces when compared to smooth and rough acrylic interfaces after thermocycling. This may enhance prosthesis serviceability during clinical use. A durable repairing system for denture base fracture is desired to avoid recurrent fracture. The purpose of this study was to evaluate the fracture force, deflection, and toughness of a heat-polymerized denture base resin repaired with autopolymerized resin alone (C), visible light-polymerizing resin (VLC), or autopolymerizing resin reinforced with unidirectional (Stick) (MA-FS) and woven glass fibers (StickNet) (MA-SN). Another group was repaired with autopolymerized resin after wetting the repair site with methyl methacrylate (MA-MMA) for 180 seconds. A group of intact specimens was used as control. Materials and Methods: Heat-polymerizing acrylic resin was used to fabricate the specimens. The specimens (10 per group) were sectioned in half, reassembled with a 3-mm butt-joint gap, and repaired. A cavity was included when glass fibers were used. Three-point bending was used to test the repaired site, and data were analyzed with one-way ANOVA and the Tukey's post hoc test (,, 0.05). Results: Fracture force, deflection, and toughness for the repaired groups without reinforcement (MA: 46.7 ± 8.6 N, 2.6 ± 0.3 mm, 0.08 ± 0.001 J; MA-MMA: 41.0 ± 7.2 N, 2.7 ± 0.4 mm, 0.07 ± 0.002 J) were significantly lower (p < 0.05) than the control group (C: 78.6 ± 9.6 N, 5.9 ± 0.4 mm, 0.27 ± 0.003 J). Repair with visible light-polymerizing resin (VLC, 15.0 ± 4.0 N, 1.2 ± 0.4 mm, 0.02 ± 0.0001 J) resulted in significant reduction of mechanical properties (p < 0.05). Reinforcement with glass fibers restored (MA-SN: 75.8 ± 9.2 N) or increased (MA-FS: 124.4 ± 12.5 N) the original strength. Conclusion: The most effective repair method was the use of autopolymerized resin reinforced with unidirectional glass fibers. Coward PhD Abstract Purpose: The aim of this study was to investigate the effectiveness of spectrophotometry and a computerized color formulation system to predict pigment formulas for color mixing silicone elastomer to match the skin color of African-Canadian people. Materials and Methods: In a prospective study, reflectance spectrophotometery was used to measure the skin color of 19 African-Canadian subjects. The spectral data for each subject was used in a computerized color formulation system to predict colorants required to mix silicone elastomer to match each subject's skin color. Delta-E values were recorded for each silicone sample in comparison to the subject's skin measurement. An analysis of variance was used to determine significance among variables, and a Tukey HSD post hoc test was used to assess paired comparisons. Results: Delta-E decreased with iterative mixes of colored silicone for each subject, and pigment loading increased with iterative mixes. Delta-E values for the third iterative mix (fourth and final sample) ranged between 1.49 and 8.82. Conclusion: Spectrophotometry and computerized color formulation provide a foundation in the color matching procedure for facial prostheses that offers objectivity to an otherwise subjective task. Through further study of spectrophotometry and computerized color formulation, and with the development of pigment databases appropriate for the African-Canadian population, it may be possible to establish a precise and repeatable color matching system that predicts required colorants and controls metamerism. The present study examined the effects of two thiol compounds, cysteine (0.1, 0.5, and 1.0 mM) and cysteamine (50, 100, and 200 µM), on cytoplasmic and nuclear maturation of canine oocytes. Oocytes collected from different reproductive stages were cultured in TCM-199 supplemented with 10% fetal bovine serum, 2.2 mg/ml sodium carbonate, 2.0 µg/ml estrogen, 0.5 µg/ml FSH, 0.03 IU/ml hCG, and 1% penicillin,streptomycin solution for 72 h. Data were analyzed by two-way ANOVA after arcscine transformation and protected by Bonferroni post hoc test. The effects of cysteine and cysteamine on canine IVM were varied depending on the reproductive stage of oocyte donor bitches. In the follicular stage, significantly more oocytes reached the metaphase II (M II) stage when cultured with 0.5 or 1.0 mM cysteine (16.7% and 16.9%, respectively) compared to the control (6.2%). In the follicular stage, cysteamine increased oocyte maturation rate upto the M II stage (15.1% to 17.0%) compared to the control (4.4%). Both the 0.5 mM cysteine and 100 µM cysteamine, alone or together, increased the intracellular GSH level of canine oocytes compared to the control. Irrespective of reproductive stage, no further beneficial effects on nuclear or cytoplasmic maturation were observed when 0.5 mM cysteine and 100 µM cysteamine were supplemented together. Kraus Abstract Aims To present the methodology, standardization techniques, and results from post hoc test,retest reproducibility analyses for a large, placebo-controlled, multicenter trial, employing urodynamic studies (UDS) to assess the impact of daily tadalafil on men with lower urinary tract symptoms (LUTS) with or without benign prostatic obstruction (BPO). Methods UDS implemented International Continence Society (ICS) Good Urodynamic Practice guidelines and standardized urodynamic and LUTS terminology. Further standardization procedures included: equipment calibration; a detailed procedure manual and centralized training; and implementation of a central reader. Measures included: monitoring of invalid studies, comparison of actual versus expected standard deviation (SD) for primary outcome (detrusor pressure at maximum urinary flow rate [pdetQmax]), and test,retest reproducibility of the placebo arm at baseline and endpoint. Results Two hundred men with moderate to severe LUTS (baseline IPSS ,13) at 20 sites were randomized to receive either tadalafil 20,mg or placebo. All men underwent non-invasive uroflow and pressure-flow studies. Numbers of invalid studies at baseline and endpoint were 9.3% and 0.6%, respectively. Variability of pdetQmax was lower than anticipated based on actual versus expected SD of 15 and 30, respectively. Correlation coefficients were very good for pressure-flow parameters including pdetQmax (r,=,.83). Conclusions Multicenter clinical trials using urodynamic outcomes require additional standardized procedures to limit inter-site variability. By implementing centralized training with a detailed procedure manual and use of a central reader, we were able to limit common difficulties arising in multicenter clinical trials, as well as demonstrate good test,retest reproducibility of pressure flow measures. [source] Longer wavelengths of light improve the growth, intake and feed efficiency of individually reared juvenile pikeperch Sander lucioperca (L.)AQUACULTURE RESEARCH, Issue 8 2009Ana Carolina Luchiari Abstract We tested the effects of monochromatic light on the specific growth rate (SGR), feed intake and feed efficiency (FE) of juvenile pikeperch, Sander lucioperca (L.). Pikeperch were reared individually for 42 days in aquaria covered with blue, green, yellow or red gelatin filters or white paper (control; n=5). Linear regression analysis indicated a significant positive effect of longer wavelengths of light on the condition factor (CF), FE and SGR. The final weight, SGR and CF were significantly higher in fish reared under red than under white light, and FE was better under green, yellow and red light than under white light (Dunnett's post hoc test, P<0.05) while blue was comparable to white light in terms of the measured parameters. After the growth trial, the spectral sensitivity of photoreceptor cells in the retina was tested using microspectrophotometry, which revealed the presence of rods with ,max at ca. 530 nm and two cone classes, absorbing maximally at ca. 535 and 603 nm, all containing a porphyropsin-based pigment. These results suggest that the presence of mid and long wavelength-sensitive cones enhances visual sensitivity under mid-wavelength and long-wavelength environments, and thus supports the finding that longer wavelengths of incoming light can improve FE and SGR of the cultivated pikeperch. Methods:, Fourteen orthodontic patients were divided into two equal groups. They received brackets fitted to all first premolars, bonded with either Aegis Ortho® (The Bosworth Co.), an ACP-containing orthodontic composite (experimental group), or Concise® (3M Dental Products), a resin-based orthodontic composite (control group). After 30 days, the teeth were extracted and longitudinally sectioned, and evaluated by superficial-microhardness analysis. The determinations were made at the bracket edge cementing limits and at occlusal and cervical points 100 and 200 ,m away from the edge. In all of these positions, indentations were made at depths of 10, 20, 30, 50, 70, and 90 ,m from the enamel surface. Analysis of variance (ANOVA) and Tukey post hoc test was used. The statistical significance level was set at p <0.05. Results:, The ANOVA showed statistically significant differences for position, material, depth, and their interactions (p <0.001). The multiple comparison test showed that the ACP-containing orthodontic composite was significantly more efficient than the control composite, reducing enamel demineralization in almost all evaluations (p <0.001). Conclusions:, Present results indicated that ACP-containing orthodontic composite for bonding orthodontic brackets successfully inhibited demineralization in vivo. This effect was localized to the area around the brackets and was statistically significant after 30 days. [source] Prosthetic treatment of maxillary lateral incisor agenesis with osseointegrated implants: a 24,39-month prospective clinical studyCLINICAL ORAL IMPLANTS RESEARCH, Issue 1 2006Fernando Zarone Abstract Objectives: The present study aimed at evaluating the marginal bone resorption and the peri-implant tissue conditions around Narrow-Neck ITI implants in the implant,prosthetic treatment of the agenesis of maxillary lateral incisors. Material and methods: Thirty patients affected by monolateral or bilateral agenesis of the maxillary lateral incisors were selected. Thirty-four ITI-SLA Narrow Neck implants were inserted and loaded about 4 months after the surgical procedure. The final restorations were realized using Aureo Galvan Crowns veneered with feldspathic ceramics. The follow-up period ranged from 24 to 39 months. Both marginal bone resorption and soft tissue quality were evaluated. The data were statistically analysed using analysis of variance (ANOVA) for repeated measures, one-way ANOVA and Tukey's post hoc test (P=0.05). Results: During the 24,39-month follow-up period, no implant showed either pain and sensitivity or mobility. After 39 months of functional loading, a cumulative survival rate of 97.06% and a cumulative success rate of 94.12% were calculated. Conclusions: In case of maxillary lateral incisor agenesis, the implant,prosthetic approach has proved to be a reliable and predictable treatment for both re-establishment of function and aesthetics. Satisfactory values of marginal bone resorption over time and optimal conditions of peri-implant tissue around Narrow-Neck ITI implants were found. [source] Pergolide mesylate can improve sexual dysfunction in patients with Parkinson's disease: the results of an open, prospective, 6-month follow-upEUROPEAN JOURNAL OF NEUROLOGY, Issue 7 2004M. Pohanka One of the most disabling problems in males suffering from advanced Parkinson's disease (PD) is complex sexual dysfunction. The effect of dopamine replacement or dopaminergic stimulation on sexual dysfunction has been recently examined and described in patients treated by L-DOPA or apomorphine. Pergolide mesylate is another dopamine agonist with a known high affinity to hD(2S) subtype and a lower affinity to hD(2L) subtype of D2 dopaminergic receptors. It has been repeatedly shown to be a highly effective treatment of the complicated and advanced stages of PD. The current study has been designed to assess its efficacy in the treatment of sexual dysfunction, which frequently accompanies the complicated stage of PD in males. Fourteen male patients suffering from PD, each of whom had been treated with L-DOPA, and in whom additional treatment with peroral dopaminergic agonist (DA) was needed, were followed for a 6-month period. Pergolide mesylate (Permax) was given to each patient, and titrated to a total daily dose of 3 mg. All of the patients were taking L-DOPA. The assessments performed before the start of pergolide treatment consisted of a neurological examination, including Unified Parkinson's Disease Rating Scale (UPDRS) III and IV subscales scoring, Mini Mental State Examination (MMSE) scoring, the neuropsychological examination including Zung scale scoring to exclude depression, biochemical and haematological examinations including the examination of prolactine serum levels; and a sexological examination during which the patients filled-in the International Index of Erectile Function (IIEF) questionnaire. These examinations were repeated during the control assessments at months 1, 3 and 6. To compare the examination results, anova, Friedmann's anova (non-parametric) and Tukey post hoc tests were used. There were statistically significant differences between the values of UPDRS III motor subscale, UPDRS IV (complications of therapy) subscale and all subscales of IIEF when months 0 and 1 were compared with the results obtained at months 3 and 6. The differences between months 0 and 1 and months 3 and 6 (in these items) were virtually insignificant. In conclusion, pergolide substantially improved sexual function in the younger male patients who were still interested in sexual activities. In such cases, the introduction of pergolide might be a better choice than treatment with sildenafile, which usually meets several contraindications in common PD male population. Kishen Abstract Aim, To investigate the stress distribution pattern in the periapical region caused by biting forces and to study the role of cyclic biting loads on periapical fluid movement. Methodology, In the first part, a digital photoelastic experiment was conducted to study stress distribution in the periapical region. In the second, 20 maxillary central incisors were selected and divided into three main groups: normal intact teeth (group 1), tooth specimens in which the root canal was enlarged and maintained wet (group 2), and tooth specimens in which the root canal was enlarged and maintained dry (group 3). The tooth specimens were placed in a polycarbonate support with a cavity filled with a sponge soaked in methylene blue solution to simulate a periapical defect with exudate. During testing, the specimens were placed in a water bath at 37 °C, and were loaded cyclically with a load of 20 N, at a rate of 72 cycles min,1, to a maximum of 20 000 cycles. The specimens were then sectioned and evaluated for retrograde fluid movement using light microscopy. The data were analysed using one-way anova (post hoc tests). Results, Digital photoelastic experiments showed that the compression of teeth produced bending stresses in the periapical region. Testing with cyclic loads demonstrated retrograde fluid movement into the apical portion of the root canal and extraradicular region in all groups. There was a significant difference amongst the apical retrograde fluid movement displayed by different groups (<0.01). Group 2, in which the root canal was enlarged and maintained wet showed maximum retrograde fluid movement, whilst group 3, in which the root canal was enlarged and maintained dry showed the least retrograde fluid movement. Conclusions, Biting forces would cause bending of the periapical bone and cyclic biting forces would contribute to retrograde fluid movement into the root canal space and extraradicular region. [source] Root canal preparation with FlexMaster: asessment of torque and force in relation to canal anatomyINTERNATIONAL ENDODONTIC JOURNAL, Issue 12 2003W. Hübscher Abstract Aim, To investigate physical parameters of FlexMaster nickel-titanium instruments while preparing curved canals in maxillary molars in vitro. Methodology, A torque-testing platform was used to prepare root canals in 11 extracted human maxillary molars with FlexMaster rotary instruments. Peak torque and force was registered along with numbers of rotations required to shape the canals. Canals were divided into ,wide' and ,constricted' groups depending on canal volumes assessed by microcomputed tomography. Resistance to cyclic fatigue was also tested. Mean scores for each instrument type were calculated and statistically compared using anova and Scheffé post hoc tests. Results, Mean torque varied between 0.1 ± 0.1 and 0.8 ± 0.5 N cm while mean force ranged from 4.2 ± 2.0 to 7.3 ± 3.5 N. Mean numbers of rotations totalled up to 18. All three variables registered showed weak correlations to preoperative canal volumes (P < 0.01) and differed significantly between ,wide' and ,constricted' canals (P < 0.001). Numbers of rotations to fracture in a cyclic fatigue test were between 348 and 1362. Conclusion, FlexMaster instruments generated low torque scores and were highly resistant to cyclic fatigue, whilst three instruments fractured in extremely narrow canals. Consequently, more research is required to limit fracture incidence and to optimize instrumentation guidelines. [source] How undergraduate clinical learning climates differ: a multi-method case studyMEDICAL EDUCATION, Issue 10 2008Klarke Boor Context, The clinical learning climate affects undergraduate medical students' behaviour, satisfaction and success. Most studies predominantly describe aspects of the clinical learning climate using quantitative methodologies, such as questionnaires. This study aimed to illuminate medical students' perceptions of the clinical learning climate, and which factors and their interactions explain differences in clinical learning climates. Methods, We carried out a multi-method case study. Twelve departments of obstetrics and gynaecology distributed the Postgraduate Hospital Educational Environment Measure (PHEEM), a reliable questionnaire measuring the clinical learning environment, among medical students. After analysis (using anova and post hoc tests), 14 medical students from the highest- and lowest-scoring departments participated in semi-structured interviews. We analysed the transcribed recordings using a content analysis approach. Researchers agreed on coding and an expert group reached consensus on the themes of the analysis. Results, We found a significant difference between departments in PHEEM scores. The interviews indicated that department and medical student characteristics determine the clinical learning climate. For departments, ,legitimacy', ,clerkship arrangements' and ,focus on personal development' were the main themes. For medical students, ,initial initiatives', ,continuing development' and ,clerkship fatigue' were the principal themes. The amount and nature of participation played a central role in all themes. Conclusions, Differences between clinical learning climates appear to be related to differing approaches to participation among departments. Participation depends on characteristics of both departments and students, and the interactions among them. The outcomes give valuable clues to how a favourable clinical learning climate is shaped. [source] Blood Flow in Snake Infrared Organs: Response-Induced Changes in Individual VesselsMICROCIRCULATION, Issue 2 2007RICHARD C. GORIS ABSTRACT Objective: In the past the microkinetics of blood flow in the infrared pit organs of pit vipers has been studied with Doppler flowmetry using various infrared stimuli such as a human hand or soldering iron at various distances, lasers of various wavelengths, etc. Quick-acting variations in blood flow were recorded, and interpreted as a cooling mechanism for avoiding afterimage in the infrared receptors. However, the Doppler measurements provided only the summation of blood flow in a number of vessels covered by the sensing probe, but did not give data on flow in individual vessels. Methods: In the present work the authors introduced into the bloodstream of Gloydius and Trimeresurus pit vipers fluorescent microspheres labeled with fluorescein isothiocyanate (FITC) contained in a solution of FITC-dextran in physiological saline. They observed the passage of the microspheres through individual pit organ vessels with a fluorescent microscope to which was attached a high-speed video camera and image intensifier. Output of the camera was recorded before, during, and after stimulus with a 810-nm diode laser. Recording was done at 250 frames/s on high-speed video apparatus and downloaded to a hard disk. Disk files were loaded into proprietary software and particles were tracked and average velocities calculated. The data were then tested for significance by ANOVA with post hoc tests. Results: A significant (p < .05) increase in blood velocity was found at the focal point of the stimulus laser, but not anywhere removed from this point. Proximal severing of the pit sensory nerves caused degeneration of the pit receptor terminals and abolished stimulus-induced blood flow changes, but did not affect normal blood flow. Conclusions: The authors conclude that the receptors themselves are directly and locally controlling the smooth muscle elements of the blood vessels, in response to heating of the receptors by infrared radiation. They speculate that the heavy vascularization constitutes a cooling system for the radiation-encoding receptors, and further that the agent of control may be a volatile neuromediator such as nitric oxide. [source] Diet reconstruction in antebellum Baltimore: Insights from dental microwear analysisAMERICAN JOURNAL OF PHYSICAL ANTHROPOLOGY, Issue 4 2010Peter H. Ma Abstract Construction in the City of Baltimore during 1996 led to the recovery of human skeletal remains dating from 1792 to 1856. Historical research indicates that the skeletal remains come from two adjacent graveyards: Christ's Church Episcopalian Cemetery and the Potters Field East. The different socioeconomic status of the internees in each cemetery suggests the possibility of marked contrasts in lifestyle, health, and diet. To shed further light on these possibilities, analyses of microscopic wear patterns on teeth, or dental microwear analyses, were undertaken. A sample from Spanish Florida was used to help interpret the results. Epoxy casts of incisor and molar teeth were placed in an SEM and photomicrographs of clean wear facets were taken. The photomicrographs were digitized using the software package Microware 4.02. Statistical analyses of rank transformed data consisted of single-factor ANOVA, followed by post hoc tests. No significant differences were found between Christ's Church and Potters Field East samples for any of the variables examined in either molar or incisor teeth. However, differences between each Baltimore sample and the La Florida samples give suggestions of possible diet differences in antebellum Baltimore. The mosaic of differences between the Baltimore and La Florida samples probably reflects the wide variety of foods available to antebellum Baltimoreans as well as the relative lack of abrasives in their diet. Wear resistance, Knoop hardness (Kh), flexural (Fs) and compressive strength (Cs) were evaluated. The GICs used were Riva Self Cure (RVA), Fuji IX (FIX), Hi Dense (HD), Vitro Molar (VM), Maxxion R (MXR) and Ketac Molar Easymix (KME). Methods:, Wear was evaluated after 1, 4, 63 and 365 days. Two-way ANOVA and Tukey post hoc tests (P = 0.05) analysed differences in wear of the GICs and the time effect. Fs, Cs, and Kh were analysed with one-way ANOVA. Results:, The type of cement (p < 0.001) and the time (p < 0.001) had a significant effect on wear. In early-term wear and Kh, KME and FIX presented the best performance. In long-term wear, Fs and Cs, KME, FIX and HD had the best performance. Strong explanatory power between Fs and the Kh (r2 = 0.85), Cs and the Kh (r2 = 0.82), long-term wear and Fs of 24 h (r2 = 0.79) were observed. Conclusions:, The data suggested that KME and FIX presented the best in vitro performance. HD showed good results except for early-term wear. Young Summary Background, In addition to an established role in the repair of postreplicative DNA errors, DNA mismatch repair (MMR) proteins also contribute to cellular responses to exogenous DNA damage. Previously, we have shown that Msh2 -null mice display increased sensitivity to ultraviolet (UV) B-induced tumorigenesis, but squamous cell carcinomas (SCC) generated are microsatellite stable, suggesting a role for MMR other than postreplicative repair in UV-induced cutaneous tumour formation. Objectives, We questioned whether there was evidence of MMR dysfunction in human SCC, thus validating the mouse models of MMR-dependent UVB-induced skin cancer. Methods, Using tissue microarrays we examined both nuclear and cytoplasmic levels of MMR proteins MSH2, MSH6, MSH3, MLH1 and PMS2 in more than 200 cases of cutaneous SCC and basal cell carcinoma (BCC). Results, We found that subsets of these 10 MMR protein measures were increased in nonmelanoma skin cancer (NMSC) compared with normal epidermal samples; this was particularly true of SCC. In fact, based on post hoc tests and MMR protein distribution patterns, BCC was distinct from SCC. With the exception of nuclear MSH2, the BCC had lower levels of identified MMR protein measures than SCC. We believe this to be important because not only is SCC more aggressive than BCC, but evidence suggests that these two NMSC subtypes arise through different molecular pathways. Conclusions, In combination with previously established roles for MMR proteins in response to UVB-induced DNA damage, our data point towards an expanded perspective of the importance of MMR proteins in the suppression of UVB-induced tumorigenesis and, potentially, tumour behaviour.
