Association of tightly locked occlusion with temporomandibular disorders

summary, The association between teeth loss and temporomandibular disorders (TMD) is still inconclusive. A kind of secondary changes of the occlusion after teeth lose called the tightly locked occlusion (TLO), defined as the occluding contact that delivers angled occlusal force on the drifted neighbour and/or the tipped antagonists of the lost posterior teeth, was hypothesized to be association with TMD. The study aimed at investigating the association between the TLO and TMD. A total of 113 posterior-teeth losing patients, 64 with TMD symptoms (group of TMD) and 49 without (group of TMD-Free) were included. Study casts and joint radiographs were made to diagnose the TLO and joint morphological changes. The simultaneous contribution of the potential variables of gender, age, tooth losing number, the TLO, joint symmetry and signs of osteoarthrosis shown on radiographs were tested through binary logistic regression analysis. In women, the TLO entered into logistic model, and had an effect on the incidence of TMD (P = 0·008). The odds ratio of with-TLO versus without-TLO is 2·6 (95% CI: 1·2, 5·8) after controlling for the effect of gender. Age, tooth lose number, joint asymmetry or osseous changes had no effect on the incidence of TMD. The tightly locked occlusion is associated with some signs and symptoms of TMD. Randomized controlled trials will be needed in further studies to test the hypothesis that treatment of a TLO, as defined in the present study, will have a beneficial effect on the signs and symptoms of TMD. Long-term illness and patterns of medicine taking: are people with schizophrenia a unique group?

Patients with schizophrenia relapse because of inadequate levels of medicine taking. Therefore, it seems logical to learn about the factors underpinning their medicine-taking decisions. Further research is urgently needed to explore this area and thereby to refine models of practice, to promote therapeutic interactions with medicine. Existing research tends to share three common deficits. ,It is grounded in the worldview of the psychiatrist not the patient. ,It studies the influence of symptoms on compliance behaviour in isolation from other potential variables. ,It overlooks the potential influence of the chronicity experience in general by examining schizophrenia outwith the context of other enduring illnesses. Overall these deficits may combine to distort the influence of schizophrenic symptoms on medicine decision making and justify a coercive rather than an empowering approach. A pernicious self-fulfilling spiral may be contributing to the problem of relapse in schizophrenia. Potentially the mental health nurse has a role in addressing this problem. [source]


Impact of the Community Reinvestment Act on New Business Start-Ups and Economic Growth in Local Markets,

Economic growth in the United States has historically bypassed many minorities and low-income communities. Some researchers and community advocates assert that the deterioration of these communities is in part caused by financial institutions' redlining and neglect. To rectify the situation, the government introduced the Community Reinvestment Act (CRA) for the purpose of encouraging banks and saving institutions to become more socially responsible and help meet the credit needs of communities in which they are located. The CRA was the government's response to bank lending discrimination. However, when passing the Act, Congress was equally concerned with reversing or at least halting disinvestment from inner-city communities and in turn revitalizing local economies. Many believe that the availability of credit to establish, refinance, and improve small businesses is critical to the well-being of local communities. Therefore, through the provision of small business loans, the CRA could be envisioned as a catalyst toward achieving that goal. Thus the aim of this paper is to investigate potential relationship between banks' CRA lending activities, and new business start-ups and economic growth in local markets. The paper proposes that new start-ups will have spillover effects that will consequently contribute to community development. After controlling for several potential variables that could have an impact on business start-ups and community developments, the study found a strong positive effect. Beside its social and economic implications, the study also considered policy implications associated with the CRA regulation as a welfare improving initiative in low-income communities. It offers ground for certain government intervention in the loan market. [source]


Tilting methods for assessing the influence of components in a classifier

Summary., Many contemporary classifiers are constructed to provide good performance for very high dimensional data. However, an issue that is at least as important as good classification is determining which of the many potential variables provide key information for good decisions. Responding to this issue can help us to determine which aspects of the datagenerating mechanism (e.g. which genes in a genomic study) are of greatest importance in terms of distinguishing between populations. We introduce tilting methods for addressing this problem. We apply weights to the components of data vectors, rather than to the data vectors themselves (as is commonly the case in related work). In addition we tilt in a way that is governed by L2 -distance between weight vectors, rather than by the more commonly used Kullback,Leibler distance. It is shown that this approach, together with the added constraint that the weights should be non-negative, produces an algorithm which eliminates vector components that have little influence on the classification decision. In particular, use of the L2 -distance in this problem produces properties that are reminiscent of those that arise when L1 -penalties are employed to eliminate explanatory variables in very high dimensional prediction problems, e.g. those involving the lasso. We introduce techniques that can be implemented very rapidly, and we show how to use bootstrap methods to assess the accuracy of our variable ranking and variable elimination procedures. [source]


Fractional factorial designs for legal psychology

Researchers considering novel or exploratory psycholegal research are often able to easily generate a sizable list of independent variables (IVs) that might influence a measure of interest. Where the research question is novel and the literature is not developed, however, choosing from among a long list of potential variables those worthy of empirical investigation often presents a formidable task. Many researchers may feel compelled by legal psychology's heavy reliance on full-factorial designs to narrow the IVs under investigation to two or three in order to avoid an expensive and unwieldy design involving numerous high-order interactions. This article suggests that fractional factorial designs provide a reasonable alternative to full-factorial designs in such circumstances because they allow the psycholegal researcher to examine the main effects of a large number of factors while disregarding high-order interactions.
