Selected AbstractsBarriers in initiating insulin therapy in a South Asian Muslim communityDIABETIC MEDICINE, Issue 2 2010U. S. Ahmed Diabet. Med. 27, 169,174 (2010) Abstract Aims, Insulin therapy is often required for optimal glycaemic control. Pakistani patients display reluctance to use insulin. We aimed to determine the reasons for this and to assess impressions after initiation of insulin in our patients. Methods, Patients with Type 2 diabetes attending Aga Khan Hospital were surveyed using a questionnaire detailing opinions on insulin use. This was a cross-sectional study of two groups, one with no experience with insulin use and the other who were insulin users. Results, Three hundred and seventeen patients were interviewed, 55.8% male, mean age 53.6 years. Of 210 patients who had never used insulin, 72.9% felt insulin was a measure of last resort and 45.2% thought that tolerance developed to insulin. Only 45.7% felt insulin would reduce complications, while 24% thought that insulin use would interfere with religious obligations. Thirty-four percent thought that it was difficult or very difficult to learn insulin administration, 41% felt that they could not self-inject even if absolutely necessary and 25% stated they would not use insulin in any circumstances. There was an association of lack of education with negative image of insulin usage. Among 107 patients using insulin, 52.3% were hesitant before initiation. However, 78.5% noted an improvement in glucose control and 86% said they would recommend insulin to others. Conclusions, Reluctance to use insulin prior to initiation is high, but views improve considerably after insulin initiation. Further awareness of the benefits of insulin use needs to be highlighted and the concerns of our population addressed. [source] Implementing a pre-operative checklist to increase patient safety: a 1-year follow-up of personnel attitudesACTA ANAESTHESIOLOGICA SCANDINAVICA, Issue 2 2010L. NILSSON Background: The operating room is a complex work environment with a high potential for adverse events. Protocols for perioperative verification processes have increasingly been recommended by professional organizations during the last few years. We assessed personnel attitudes to a pre-operative checklist (,time out') immediately before start of the operative procedure. Methods: ,Time out' was implemented in December 2007 as an additional safety barrier in two Swedish hospitals. One year later, in order to assess how the checklist was perceived, a questionnaire was sent by e-mail to 704 persons in the operating departments, including surgeons, anesthesiologists, operation and anesthetic nurses and nurse assistants. In order to identify differences in response between professions, each alternative in the questionnaire was assigned a numerical value. Results: The questionnaire was answered by 331 (47%) persons and 93% responded that ,time out' contributes to increased patient safety. Eighty-six percent thought that ,time out' gave an opportunity to identify and solve problems. Confirmation of patient identity, correct procedure, correct side and checking of allergies or contagious diseases were considered ,very important' by 78,84% of the responders. Attitudes to checking of patient positioning, allergies and review of potential critical moments were positive but differed significantly between the professions. Attitudes to a similar checklist at the end of surgery were positive and 72,99% agreed to the different elements. Conclusion: Staff attitudes toward a surgical checklist were mostly positive 1 year after their introduction in two large hospitals in central Sweden. [source] Parents' Beliefs About Condoms and Oral Contraceptives: Are They Medically Accurate?PERSPECTIVES ON SEXUAL AND REPRODUCTIVE HEALTH, Issue 2 2004Marla E. Eisenberg CONTEXT: Parents are encouraged to be the primary sex educators for their children; however, little is known about the accuracy of parents' views about condoms and oral contraceptives. METHODS: Telephone surveys using validated measures provided data on beliefs about the effectiveness, safety and usability of condoms and the pill among 1,069 parents of 13,17-year-olds in Minnesota and Wisconsin in 2002. Pearson chi-square tests and multivariate logistic regression models were used to compare beliefs according to sex, age, race, religion, education, income and political orientation. RESULTS: Substantial proportions of parents underestimated the effectiveness of condoms for preventing pregnancy and sexually transmitted diseases (STDs). Only 47% believed that condoms are very effective for STD prevention, and 40% for pregnancy prevention. Fifty-two percent thought that pill use prevents pregnancy almost all the time; 39% thought that the pill is very safe. Approximately one-quarter of parents thought that most teenagers are capable of using condoms correctly; almost four in 10 thought that most teenagers can use the pill correctly. Fathers tended to have more accurate views about condoms than mothers did; mothers' views of the pill were generally more accurate than fathers'. Whites were more likely than nonwhites to hold accurate beliefs about the pill's safety and effectiveness; conservatives were less likely than liberals to hold accurate views about the effectiveness of condoms. CONCLUSION: Campaigns encouraging parents to talk with their teenagers about sexuality should provide parents with medically accurate information on the effectiveness, safety and usability of condoms and the pill. [source] Representative marketing-oriented study on implants in the Austrian population.CLINICAL ORAL IMPLANTS RESEARCH, Issue 5 2003Abstract: The number of dental implants inserted annually worldwide has been estimated to come close to a million. But the level of information available to patients about realistic, evidence-based treatment options by implants is often enough more than fragmentary, and what is disseminated by the media and the industry does not always reflect evidence-based empirical data. This survey of 1000 adults presented with 18 questions was designed to shed light on several points. These were (1) level of subjective patient information, (2) sources of information and prejudices, (3) future demand for implant treatment and target groups for patient information campaigns, and (4) potential misinformation, information deficits, discrepancies of information and how these come about. Of those questioned, 20% said unprompted that implants were a possibility to replace missing teeth. When prompted, 72% said that they knew about dental implants. Most of those questioned felt poorly informed about the options for replacing missing teeth and many knew less about implants than about other alternatives. The dentist was said to be the desired source of information, but 77% of those questioned reported that their dentists did not practice implant dentistry. More than 79% of those questioned did not know whether their dentist worked with implants. Forty-four percent thought that implants should only be placed by specially trained doctors. Sixty-one percent were of the opinion that dentists who provide implant dentistry were better qualified than their nonimplanting colleagues. Half of those questioned attributed implant failures to allergies and incompatibilities, the other half to poor medical care. Only 29% incriminated poor oral hygiene as a cause of implant failure. Future strategies should be geared to more professional public relations and patient information. Internationally operating qualified implant institutions could contribute much to balance discrepant information. Résumé Le nombre d'implants dentaires insérés annuellement dans le monde se chiffrerait à environ un million. Mais le niveau d'information disponible du patient en ce qui concerne les options de traitement basées sur l'évidence est souvent fragmentaire, et l'information dispensée par les média et l'industrie ne reflète pas toujours ces options. Cette enquête réalisée sur mille adultes ayant reçu un questionnaire de 18 questions a été effectuée afin de mettre en évidence certains points. Ces derniers étaient 1) le niveau de l'information subjective du patient, 2) les sources d'information et préjudices, 3) la demande future pour le traitement implantaire et les groupes cibles pour les campagnes d'information des patients, 4) les mauvaises informations potentielles, les déficits d'information, les désaccords de l'information et les causes de ces derniers. De ces personnes questionnées, 20% ont répondu spontanément que les implants étaient un moyen de remplacer les dents manquantes. Lorsqu'ils étaient interrogés, 72% ont répondu connaître les implants dentaires. La plupart de ces personnes se sentaient cependant peu informées sur les options de remplacement des dents manquantes et beaucoup d'entre-elles possédaient très peu d'information sur les implants par rapport aux autres possibilités existantes. Le dentiste semblait être la source préférentielle d'information mais 77 % des personnes questionnées ont rapporté que leur dentiste ne plaçait pas d'implant. Teil I: Stand der Information, Informationsquellen und Bedürfnis bezüglich Patienteninformation Die Anzahl weltweit gesetzter Implantate pro Jahr wird auf fast eine Million geschätzt. Aber die den Patienten zur Verfügung stehende Information über realistische, auf Evidenz basierende Behandlungsmöglichkeiten mit Implantaten ist mehr als lückenhaft. Was durch die Medien und durch die Industrie verbreitet wird, wiederspiegelt nicht immer auf Evidenz basierende empirische Daten. Diese Untersuchung an 1000 Erwachsenen, welchen 18 Fragen gestellt wurden, wurde entwickelt, um Licht auf verschiedene Punkte zu werfen. Die Punkte betrafen (1) Stand der subjektiven Patienteninformation; (2) Informationsquellen und Vorurteile; (3) zukünftiger Bedarf an Implantatbehandlungen und Zielgruppen für Propaganda zur Patienteninformation; (4) potentielle Fehlinformation, Informationsdefizite, Diskrepanzen in der Information und wie diese zustande kommen. Von den gefragten Individuen sagten ohne Vorinformation 20%, dass Implantate eine Varainte für den Ersatz von fehlenden Zähnen darstellen. Mit Vorinformation sagten 72%, dass sie von dentalen Implantaten Kenntnis haben. Die meisten der Befragten fühlten sich über die Möglichkeiten bezüglich Ersatz fehlender Zähne schlecht informiert und viele wussten weniger über Implantate als über andere Behandlungsvarianten. Es wurde gesagt, der Zahnarzt sei die gewünschte Informationsquelle, aber 77% der Befragten berichteten, dass ihr Zahnarzt keine Behandlungen mit Implantaten anbietet. La futura estrategia debe ser encaminada a unas relaciones públicas más profesionales y a más información del paciente.
