Screening for Asymptomatic Left Ventricular Dysfunction Using B-Type Natriuretic Peptide

Asymptomatic left ventricular dysfunction (ASLVD), a known precursor phase of heart failure, fulfills the essential criteria that should be met before screening for a disease. It is common and associated with reduced longevity and quality of life. Left untreated, it progresses to heart failure, which incurs a mortality greater than most cancers as well as significant morbidity rates. In addition, we now have several population-based studies that demonstrate that both B-type natriuretic peptide (BNP) and N-terminal prohormone brain natriuretic peptide (NTproBNP) can accurately exclude left ventricular systolic dysfunction. More recent work shows that this can be done cost-effectively. There is also a wealth of evidence from randomized controlled trials indicating that the treatment of ASLVD can reduce both morbidity and mortality and slow progression to the heart failure state. The main stumbling block to implementation of screening, in addition to the perceived cost, may well be the lack of a randomized study showing that screening the population for ASLVD really does alter the natural history of the condition, something that other screening strategies have so far failed to do. Does cost,benefit analysis or self-control predict involvement in bullying behavior by male prisoners?

Abstract The main aim of this study is to assess whether lack of self-control or the perceived costs and benefits of aggression provide the better predictors of bullying behavior and victimization, and direct aggression perpetration, in a sample of 122 male British prisoners. We also assessed whether bullying was associated with height and weight. Zero-order correlations showed that perceived benefits, self-control, and perceived costs were most closely associated with perpetration of bullying, and that lack of self-control was weakly associated with victimization. Height and weight were unrelated to bullying or victimization. In a standard regression analyses, perceived benefits was the strongest predictor of bullying perpetration, with lack of self-control contributing further; all three variables made a significant contribution when direct aggression was the criterion. Mediation analysis showed that a combined cost,benefit measure partially mediated the association between self-control and both bullying and direct aggression. Toward a Psychosocial Theory of Military and Economic Violence in the Era of Globalization

A theory of the roots of violent conflict in the global era focuses upon a pattern of intervention by the United States, its allies, and proxy forces. It emphasizes a dominant set of beliefs and powerful networks in a position to apply them. The networks protect and extend their concentrations of wealth using violence or the threat of violence to produce compliant governments, to identify enemies, to mobilize consent, and to minimize the perceived costs of such activity. U.S. government agencies and large global corporations are central to this effort. Illustrations are provided by descriptions of military actions in Venezuela, East Timor, and Iraq. Implications for research include the value of using network analysis to identify centers of combined corporate and governmental power and the value of combining the study of belief systems with studies identifying such centers of power. Research Note: The Influence of the Press in Shaping Public Opinion towards the European Union in Britain

Existing research finds that European citizens evaluate the EU according to the perceived costs and benefits of integration. Instead of assuming that cue-givers provide an informational role in this process, we investigate the direct effects of positive and negative EU messages from prominent cue-givers, including political parties and the media. Using the 2001 British Election Study, we examine the impact of the main political parties and newspapers on public attitudes towards membership of the EU and the prospect of joining the single European currency. During the 2001 British General Election campaign, the media and the main political parties had small independent effects on attitudes towards EU membership and the potential adoption of the single European currency. When voters receive the same messages from both their party and their newspaper, these effects are considerable. [source]
