Fertility outcomes in women with hypopituitarism

Summary Background, Women with hypopituitarism are known to have a poor outcome once pregnancy has been achieved by ovulation induction. There are no data, however, recording the efficacy of ovulation induction and pregnancy rates in this group of subfertile women. Methods, The outcome of fertility treatments in all 19 women with hypopituitarism attending the fertility clinics of University College London Hospitals over the past 20 years was audited. Results, Ovulation was achieved in almost all women (95%) but occurred in only 60% of treatment cycles. Pregnancy was achieved in 47% of women or 11% of cycles resulting in a live birth rate of 6·7% per cycle. Seven of the 18 pregnancies (39%) miscarried. Only 42% of women treated achieved a live birth. Conclusion, Ovulation induction in women with hypopituitarism yields relatively low pregnancy rates in comparison to other causes of anovulation and a high miscarriage rate. Pituitary hormone deficiency beyond gonadotrophins has a major adverse effect on achieving pregnancy. [source]


First trimester screening for Down syndrome and assisted reproduction: no basis for concern

Abstract In pregnancies obtained after assisted reproduction the false-positive rate of second trimester Down syndrome (DS) screening is increased by 1.5,3-fold. This may cause an increase in the number of amniocenteses and the fetal loss rate. The present study for the first time examined whether assisted reproductive technologies affect the results of first trimester screening. The markers PAPP-A, free ,-hCG and the nuchal translucency (NT) thickness were examined at 12,14 weeks' gestation. Screening markers in 47 in vitro fertilisation (IVF), 63 ovulation induction (OI) and 3026 spontaneously conceived singleton pregnancies were compared. The MoM (multiples of the median) value in the IVF pregnancies was 1.02 (95% CI: 0.85,1.22) for PAPP-A, 1.14 (95% CI: 0.95,1.37) for ,-hCG and 0.97 (95% CI: 0.89,1.05) for NT; the MoM value in the OI pregnancies was 0.89 (95% CI: 0.76,1.05) for PAPP-A, 1.08 (95% CI: 0.93,1.25) for ,-hCG and 1.02 (95% CI: 0.95,1.11) for NT. The first trimester marker values in assisted reproductive pregnancies and spontaneously conceived pregnancies were not significantly different. Estimated false-positive rates for a risk cut-off of 1:400 varied from 4.7% in IVF pregnancies to 5.1% in OI pregnancies. Induced ovulation of yellow catfish (Pelteobagrus fulvidraco) using a combination of a gonadotrop-releasing hormone analogue and domperidone

Abstract The effects of an intraperitoneal hormone injection of gonadotropin-releasing hormone agonist (D-Ala6, Pro9 -NEt GnRHa) alone or in combination with a dopamine antagonist, domperidone (DOM), on ovulation induction in yellow catfish Pelteobagrus fulvidraco were tested. The hormone treatments were as follows: 6 mg kg,1 body weight (BW) of carp pituitary extract as a positive control, GnRHa 10, 20, 40 and 80 ,g kg,1 BW and a combination of GnRHa and DOM as follows: 10 ,g+5 mg, 20 ,g+10 mg, 40 ,g+20 mg and 80 ,g+40 mg kg,1 BW. Physiological saline (0.7% NaCl) was used as a negative control. Significant differences in the ovulation ratio, latency period and ovulation index (OI) were observed among treatments (P<0.05). The combination of GnRHa and DOM at doses of 40 ,g+20 mg kg,1 BW had higher values of the ovulation ratio and OI, and a shorter latency period compared with other treatments. The highest OI in GnRHa treatments was only 56.67%, suggesting a dopaminergic tone on gonadotropin secretion in this fish at the pre-ovulatory stage. Therefore, ovulation can be successfully induced in yellow catfish with 40 ,g kg,1 GnRHa+20 mg kg,1 DOM without affecting the egg quality. [source]


The influence of body weight on response to ovulation induction with gonadotrophins in 335 women with World Health Organization group II anovulatory infertility

Objective, To assess the influence of body weight on the outcome of ovulation induction in women with World Health Organization (WHO) group II anovulatory infertility. Design, The combined results of two studies in which either a highly purified urinary follicle-stimulating hormone or highly purified urinary menotrophin were compared with recombinant follicle-stimulating hormone. Setting, Thirty-six fertility clinics. Population, A total of 335 women with WHO group II anovulatory infertility failing to ovulate or conceive on clomifene citrate. Methods, Ovarian stimulation using a low-dose step-up protocol. Main outcome measures, The effects of body weight on ovarian response, ovulation rate and pregnancy rate after one treatment cycle. Results, With increasing body mass index (BMI), a higher threshold dose of gonadotrophins was required and there were more days of stimulation; yet, despite a greater concentration of antral follicles, there were fewer intermediate and large follicles. There was no difference in the rates of ovulation and clinical pregnancy in relation to body weight. Conclusions, Body weight affects gonadotrophin requirements but not overall outcome of ovulation induction in women with anovulatory polycystic ovary syndrome and a BMI of less than 35 kg/m2.
