Relationship of Light to Moderate Alcohol Consumption and Risk of Hypertension in Japanese Male Office Workers

Background A close relationship between alcohol consumption and hypertension has been established, but the effect of light to moderate alcohol consumption on blood pressure (BP) remains unclear. Methods A total of 5275 Japanese male office workers aged 23 to 59 years partook in a survey. Subjects were classified as nondrinkers or current drinkers who averaged less than 12, 12 to 22, 23 to 45, or ,46 g/day of ethanol. BPs were measured at annual health examinations from May 1996 through May 2000. A total of 3784 hypertension-free (systolic BP <140 mm Hg, diastolic BP <90 mm Hg, no medication for hypertension, and no history of hypertension) men were observed for 4 years. Men in whom hypertension (systolic BP ,140 mm Hg, diastolic BP ,90 mm Hg, or both or receipt of antihypertensive medication) was found during repeated surveys were defined as incident cases of hypertension. Results After controlling for potential predictors of hypertension, systolic and diastolic BP levels and the incidence of hypertension were lowest in nondrinkers in all three age ranges (23,35, 36,47, and 45,59 years) and increased in a dose-dependent manner as alcohol consumption increased. For individuals aged 23 to 35 years, systolic and diastolic BP levels and the incidence of hypertension were significantly higher among those who drank an average of ,23 g/day of ethanol than among nondrinkers. For those aged 36 to 59, the diastolic BP level was significantly higher among those who drank any alcohol at all than among nondrinkers, and the systolic BP level and the incidence of hypertension were significantly higher among those who drank an average of ,12 g/day of ethanol than among nondrinkers. Conclusions Light to moderate alcohol consumption seems to have an important influence on BP in both young and middle-aged Japanese men. [source]

Contact allergy to farnesol in 2021 consecutively patch tested patients.

Farnesol is one of the fragrances considered to be a significant contact allergen. Therefore, it was decided by the European Union to label products containing farnesol. Farnesol was tested [5% petrolatum (pet.)] together with the standard series between 1 January 2003 and 30 June 2003 in 2021 consecutive patients, 1243 females and 778 males. Of these, 22 [1.1%, 95% confidence interval (CI): 0.7,1.6%] had a positive reaction to farnesol. 147 (8.1%) of those 1825 tested to Myroxylon pereirae resin (balsam of Peru, 25% pet.) at the same time reacted positively, 143 (7.8%) of those 1823 tested to the fragrance mix (FM) (8% pet.) and 34 (1.9%) of 1831 tested to propolis (10% pet.). With regard to concomitant reactions in farnesol-positive patients, 5 of 22 reacted additionally to the FM [odds ratio (OR): 4.3; CI: 1.53,12.15] and 2 (of these 5) additionally to M. pereirae resin (OR: 1.27; CI: 0.29,5.54). The strongest association was seen to propolis (OR: 6.2; 95% CI: 1.4,27.7). Compared to those with negative reactions to farnesol, the group of patients allergic to farnesol was characterized by a higher proportion of young females and office workers, and the hand and the face were more often affected. In conclusion, farnesol is an important allergen. We recommend that farnesol should be included in a fragrance patch-test preparation and that its use should be regulated for consumer safety reasons. Furthermore, the extent of exposure to farnesol should be further studied. [source]


Sources and migration of volatile organic compounds in mixed-use buildings

Abstract, We examined concentrations and migration of volatile organic compounds (VOCs) in ten mixed-use buildings in southeast Michigan, USA. In an office and work zone in each building, air exchange rates (AERs) were measured using perfluorocarbon tracers, and over 96 VOC species were measured by GC/MS over a 7-day period. VOCs were then apportioned to sources in offices, work zones, and outdoors using a two-zone mass balance model. AERs averaged 3.9 h (0.2,14.2 h) in offices and 1.9 h (0.4,3.5 h) in work zones. The dominant VOCs included aromatics, terpenes and alkanes. VOC concentrations were uniform in the smaller spaces, and more variable in some of the very large spaces. Apportionments depended on the VOC and building, but emissions in industrial zones of buildings often migrated to office areas where they frequently accounted for the bulk of VOC concentrations. Outdoor sources accounted for most benzene and carbon tetrachloride, and a small fraction of aromatic and aliphatic compounds. This study shows that pollutant migration can be a significant and not uncommon problem in mixed-use buildings, and it demonstrates the need for better control of emissions and pollutant migration. Practical Implications Pollutant exposures in industrial, commercial, and institutional buildings arise from indoor and outdoor sources that can be identified, apportioned, and controlled with knowledge of emission sources and building airflows. We show that multi-tracer techniques are an effective and practical means of determining airflows and exchange rates in large buildings. In examining a set of mixed-use buildings, a substantial fraction of VOC exposures in otherwise relatively ,clean' offices is due to pollutant migration from ,dirty' zones of the building. This indicates the need for corrective actions to minimize exposures of office workers that are unwanted and probably unknown to building managers. These actions should include better control of emissions, isolation or control of air and pollutant flows between building zones, and documentation of the effectiveness of such measures when strong emission sources are present. Synergy and Movement within Suburban Mixed-Use Centers: The Toronto Experience

The article measures the intensity of interaction between activities within suburban mixed-use centers (also referred to as suburban downtowns or edge cities), and the reliance on walking for intra-center journeys. It examines whether or not these centers are achieving, as intended in planning documents, an inner dynamic that distinguishes them from typical suburban commercial developments. A survey of office workers within the Greater Toronto Area's three most developed suburban mixed-use centers reveals a moderate level of inner synergy (interaction between activities beneficial to all those involved) and of walking for intra-center journeys. The article highlights the advantages of such an inner dynamic for activities located within suburban mixed-use centers. It also points at the surveyed centers' car orientation and the inhospitality of their walking environment to explain why synergy and pedestrian movements are not any higher. This leads to an examination of the advantages that would accrue from a more intense pedestrian-based synergy and of the means of achieving such an outcome. [source]


Cohort study for the effect of chronic noise exposure on blood pressure among male workers in Busan, Korea

Abstract Objective It has not yet been established whether exposure to chronic noise induces an increase in blood pressure or an increase in the development of hypertension. Therefore, a cohort study was performed to identify the effects of chronic noise exposure on blood pressure. Methods Five hundred thirty male workers at a metal manufacturing factory in Busan, Korea, were enrolled in the study. They were monitored with an annual health check-ups for nine consecutive years from 1991 to 1999. The subjects were divided into four groups which were determined by noise level categories (NLCs) according to the exposure of noise intensity; NLC-I: office workers who were exposed to <60 dBA at work; NLC-II: worksite technical supporters or inspectors who were intermittently exposed to noise and were not using hearing protection devices; NLC-III: worksite workers exposed to a noise below 85 dBA (TWA) and used one type of hearing protection device, earplug or earmuff; NLC-IV: worksite workers who were exposed to a noise level of 85 dBA or higher in average and used both earplug and earmuff. Results After controlling the possible confounders, such as baseline age, smoking, alcohol intake, exercise, family history of hypertension, systolic blood pressure (SBP), or diastolic blood pressure (DBP), and changes in body mass index (BMI), we determined that the mean values for the SBP over the duration of this study were 3.8, 2.0, and 1.7 mmHg higher in groups NLC-IV, NLC-III, and NLC-II, respectively, in comparison to that of the NLC-I group. There was no significant difference in DBP among the groups. Conclusion This study suggests that chronic noise exposure increases SBP independently, among male workers. Am. J. Ind. Prevalence and impact of musculoskeletal disorders in New Zealand nurses, postal workers and office workers

Abstract Objectives: To describe the prevalence, characteristics and impact of musculoskeletal disorders (MSDs) in New Zealand nurses, postal workers and office workers. Methods: A postal survey asked participants about MSDs, (low back, neck, shoulder, elbow, wrist/hand or knee pain lasting longer than one day), and demographic, physical and psychosocial factors. Nurses were randomly selected from the Nursing Council database, postal workers from their employer's database and office workers from the 2005 electoral roll. Results: The response rate of potentially eligible participants was 58% (n=443). Participants were aged 20,59 years; 86% were female. Over the 12 months prior to the survey 88% of respondents had at least one MSD lasting longer than a day and 72% reported an MSD present for at least seven days. Of the 1,003 MSDs reported, 18% required time off work and 24% required modified work duties. In the month prior to the survey 17% of MSDs made functional tasks difficult or impossible. Low back, neck and shoulder pain prevalence did not differ by occupation. Postal workers had the highest prevalence of elbow and wrist/hand pain; nurses of knee pain. Conclusions: The high prevalence of MSDs among these workers indicates that they are indeed in ,at risk' occupations. In each occupational group MSDs encompass a range of anatomical sites, however the overall pattern of MSDs differs by occupation. MSDs have a significant impact on activities at work and home. Implications: Primary and secondary prevention strategies should encompass a range of anatomical sites and specifically target different occupational groups. [source]


Chromosomal aberrations in peripheral lymphocytes of train engine drivers

Abstract Studies of Swedish railway employees have indicated that railroad engine drivers have an increased cancer morbidity and incidence of chronic lymphatic leukemia. The drivers are exposed to relatively high magnetic fields (MF), ranging from a few to over a hundred ,T. Although the possible genotoxic potential of MF is unclear, some earlier studies have indicated that occupational exposure to MF may increase chromosome aberrations in blood lymphocytes. Since an increased level of chromosomal aberrations has been suggested to predict elevated cancer risk, we performed a cytogenetic analysis on cultured (48 h) peripheral lymphocytes of Swedish train engine drivers. A pilot study of 18 engine drivers indicated a significant difference in the frequency of cells with chromosomal aberrations (gaps included or excluded) in comparison with seven concurrent referents (train dispatchers) and a control group of 16 office workers. The engine drivers had about four times higher frequency of cells with chromosome-type aberrations (excluding gaps) than the office workers (P,<,0.01) and the dispatchers (P,<,0.05). Seventy-eight percent of the engine drivers showed at least one cell per 100 with chromosome-type aberrations compared with 29% among the dispatchers and 31% among the office workers. In a follow-up study, another 30 engine drivers showed an increase (P,<,0.05) in the frequency of cells with chromosome-type aberrations (gaps excluded) as compared with 30 referent policemen. Sixty percent of the engine drivers had one or more cells (per 100 cells) with chromosome-type aberrations compared with 30% among the policemen. In conclusion, the results of the two studies support the hypothesis that exposure to MF at mean intensities of 2,15,,T can induce chromosomal damage. Dermatitis caused by physical irritants

SummaryBackground Although physical irritant contact dermatitis (PICD) is a common occupational dermatosis, it is one of the least well understood because of its multiple types, lack of diagnostic test, and the many mechanisms involved in its production. Objectives To characterize the materials and mechanisms of physical irritation of the skin. Methods We did a retrospective analysis over the past 20 years of all patients with a diagnosis of PICD at St John's Institute of Dermatology Contact Dermatitis Clinic. Results Of the 29 000 patients who attended the clinic over the study period, 392 patients were diagnosed with PICD and of these, 335 files were analysed. Conclusions Our findings show that PICD accounted for 1·15% of all patients attending the contact clinic over the study period. Diverse occupations and materials were implicated. The most common cause of PICD was low humidity due to air-conditioning, which caused dermatitis of the face and neck in office workers due to drying out of the skin. [source]
