Selected AbstractsGastric Stasis in Migraine: More Than Just a Paroxysmal Abnormality During a Migraine AttackHEADACHE, Issue 1 2006Objective.,The aim of this article is to evaluate gastric motility and emptying in the ictal and interictal period in migraine. Background.,Nausea is a predominant symptom of migraine and the basis of it is thought to be gastric stasis. Objective methods to establish this are however lacking. We utilized gastric scintigraphy studies to determine gastric motility in the ictal and interictal period of migraine. Methods.,Ten migraine subjects were compared to equal number of age and sex matched controls. Gastric scintigraphy using a standard meal was performed in all control subjects once and in all 10 migraine subjects in the interictal period and nine studies were performed in the ictal period migraine. Results.,The time to half emptying was delayed in migraine ictally (78%) and interictal period (80%) using normative data at this institution. Gastric stasis was less pronounced ictally (149.9 minutes) compared to interictal period (188.8 minutes). There was a significant delay compared to nonmigrainous controls (migraine 188.8 minutes vs normal controls 111.8 minutes; P < .05). These data were replicated in percentage of radioactive material remaining in the stomach at 2 hours. Conclusions.,Contrary to previous belief, this study has demonstrated that migraineurs suffer from gastric stasis both during and outside an acute migraine attack. This may suggest that migraineurs may have an abnormal autonomic function compared to nonmigrainous controls. The potential role of this in pathophysiology of migraine is discussed and avenues for further investigations are explored. [source] Measuring masticatory performance using a new device and ,-carotene in test gummy jellyJOURNAL OF ORAL REHABILITATION, Issue 11 2010T. NOKUBI Summary, Objective methods for evaluating masticatory performance are invaluable for quantitative comparisons between various dental treatments as well as diagnosing masticatory disorder. The purpose of this study was to develop an accurate method for automatically measuring masticatory performance using a new measuring device consisting of light-emitting diodes and a photodiode and test gummy jelly. First, the relationship between six known ,-carotene concentrations in aqueous solution and the voltage values exhibited on a photodiode was investigated. Mean voltage obtained by red light incident on the photodiode indicated a significantly high correlation (r = 0·999 in a cubic function, P < 0·01) with ,-carotene concentration in the aqueous solution. Second, to establish the optimal measuring conditions for evaluating masticatory performance, factors influencing mean voltage were investigated, such as water temperature and times for rinsing the gummy jelly and dissolving ,-carotene from the jelly. A stable mean voltage was obtained by measuring under the following conditions: rinsing water temperature, 35 °C; rinsing time, 30 s; water dissolving temperature, 35 °C; dissolving time, 10 s. Finally, the relationship between seven surface area values of divided gummy jelly and ,-carotene concentrations dissolved from the divided jellies was investigated. The increase in surface area of divided test gummy jelly particles was accurately calculated (r = 0·992, P < 0·001) from mean voltage on the photodiode to change with ,-carotene concentration dissolved from the jelly surface. This new method using test gummy jelly and an automatic measuring device appears useful for precisely evaluating masticatory performance. [source] Seasonal confounding and residual correlation in analyses of health effects of air pollutionENVIRONMETRICS, Issue 4 2007Isabella R. Ghement Abstract To investigate the health effects of air pollution via a partially linear model, one must choose an appropriate amount of smoothing for accurate estimation of the linear pollution effects. This choice is complicated by the dependencies between the various covariates and by the potential residual correlation. Most existing approaches to making this choice are inadequate, as they neither target accurate estimation of the linear pollutant effects nor handle residual correlation. In this paper, we illustrate two new adaptive and objective methods for determining an appropriate amount of smoothing. We construct valid confidence intervals for the linear pollutant effects, intervals that account for residual correlation. We use our inferential methods to investigate the same-day effects of PM10 on daily mortality in two data sets for the period 1994 to 1996: one collected in Mexico City, an urban area with high levels of air pollution, and the other collected in Vancouver, British Columbia, an urban area with low levels of air pollution. For Mexico City, our methodology does not detect a PM10 effect. In contrast, for Vancouver, a PM10 effect corresponding to an expected 2.4% increase (95% confidence interval ranging from 0.0% to 4.7%) in daily mortality for every 10,µg/m3 increase in PM10 is identified. The use of dental implants to improve the quality of life for edentulous patients Background:, Nowadays, there is some speculation among dental educators that the need for complete dentures will significantly decrease in the future and that training in their provision should be removed from the dental curriculum. Objective:, To sensitise the reader to the functional shortcomings of complete denture therapy in the edentulous patient and present restorative options including implants to improve edentulous quality of life in these patients. Methods:, Information retrieval followed a systematic approach using PubMed. English articles published from 1964 to 2008, in which the masticatory performance of patients with implant-supported dentures was assessed by objective methods and compared with performance with conventional dentures, were included. Results:, National epidemiological survey data suggested that the adult population in need of one or two complete dentures will increase from 35.4 million adults in 2000 to 37.9 million adults in 2020. Clinical studies have showed that the ratings of general satisfaction were significantly better in the patients treated with implant overdentures post-delivery compared with the complete denture users. In addition, the implant group gave significantly higher ratings on comfort, stability and ability to chew. Furthermore, patients who received mandibular implant overdentures had significantly fewer oral health-related quality of life problems than did the conventional group. Conclusion:, Implant-supported dentures including either complete overdentures or a hybrid prosthesis significantly improve the quality of life for edentulous patients compared with conventional removable complete dentures. Therefore, the contemporary dental practitioner should consider other options as well as conventional removable complete dentures to restore edentulous patients. Claydon Abstract Objectives: Plaque scoring usually employs subjective indices. The aim was to compare plaque inhibition of three toothpastes using two objective and one subjective measures of plaque. Material and Methods: Formulations were: (1) an experimental anti-plaque paste (test); (2) an experimental paste (minus active or negative control); and (3) a proprietary anti-plaque toothpaste product (positive control). The study was a blind, randomised crossover design using a 4-day, no tooth brushing, plaque regrowth model and involving 22 healthy subjects. After baseline plaque removal, subjects rinsed twice a day with slurries of the allocated paste. On day 5, plaque was scored by index, wet weight and optical density of extracted disclosing solution from the plaque (stain intensity). Results: All data showed the same pattern. There were highly significant subject and treatment effects but not period effects. The positive control was highly significantly more effective in plaque control than the test and minus active experimental formulations, which in turn were not significantly different from each other. There were strong and significant correlations between pairs of scoring methods particularly wet weight and stain intensity. Conclusion: The use of objective methods of plaque alongside conventional subjective indices provided convincing evidence for increased discriminatory power in a study comparing plaque inhibition by toothpastes. [source] Ghanaian Cocoa Bean Fermentation Characterized by Spectroscopic and Chromatographic Methods and ChemometricsJOURNAL OF FOOD SCIENCE, Issue 6 2010Patrick C. Aculey Abstract:, Export of cocoa beans is of great economic importance in Ghana and several other tropical countries. Raw cocoa has an astringent, unpleasant taste, and flavor, and has to be fermented, dried, and roasted to obtain the characteristic cocoa flavor and taste. In an attempt to obtain a deeper understanding of the changes in the cocoa beans during fermentation and investigate the possibility of future development of objective methods for assessing the degree of fermentation, a novel combination of methods including cut test, colorimetry, fluorescence spectroscopy, NIR spectroscopy, and GC-MS evaluated by chemometric methods was used to examine cocoa beans sampled at different durations of fermentation and samples representing fully fermented and dried beans from all cocoa growing regions of Ghana. Using colorimetry it was found that samples moved towards higher,a* and,b* values as fermentation progressed. Furthermore, the degree of fermentation could, in general, be well described by the spectroscopic methods used. In addition, it was possible to link analysis of volatile compounds with predictions of fermentation time. Fermented and dried cocoa beans from the Volta and the Western regions clustered separately in the score plots based on colorimetric, fluorescence, NIR, and GC-MS indicating regional differences in the composition of Ghanaian cocoa beans. The study demonstrates the potential of colorimetry and spectroscopic methods as valuable tools for determining the fermentation degree of cocoa beans. Using GC-MS it was possible to demonstrate the formation of several important aroma compounds such 2-phenylethyl acetate, propionic acid, and acetoin and the breakdown of others like diacetyl during fermentation. Practical Application:, The present study demonstrates the potential of using colorimetry and spectroscopic methods as objective methods for determining cocoa bean quality along the processing chain. Development of objective methods for determining cocoa bean quality will be of great importance for quality insurance within the fields of cocoa processing and raw material control in chocolate producing companies. Conclusions:, Physical activity is a lifestyle factor that is a key focus in chronic disease,related research, prevention, and interventions. Healthy People 2010 set goals of decreasing the prevalence of preventable diseases by encouraging healthier lifestyle patterns. Shifts toward more sedentary lifestyles have resulted in increases in life-limiting disease states, including obesity, diabetes, heart disease, cancer, and osteoporosis. Physical activity measurements have been used widely in research studies but are less commonly used in primary care. Measuring individuals' physical activity levels as part of the health assessment will enhance the provider's ability to engage in health promotion and suggest health protection interventions. The strengths, weaknesses, and potential applications to practice of physical activity measures are summarized in an effort to familiarize nurse practitioners (NPs) with commonly used tools and encourage integration of physical activity assessment into their current practice. Implications for practice:, NPs are in an ideal position to promote health by encouraging appropriate amounts of physical activity. Screening, health promotion, and disease prevention are part of the core competencies of NP practice established by the National Organization of Nurse Practitioner Faculties. Increased knowledge of physical activity measures will enhance the NP's ability to evaluate relevant physical activity research for use in evidence-based practice. Incorporation of simple yet appropriate physical activity measurements into practice will expand the NP's ability to identify and thus address sedentary lifestyles in their clientele. Many other non-immune theories have also been suggested for vitiligo's pathogenesis, but the role of oxidative stress has gained more importance in recent years. Objective To compare the effect of topical 0.05% betamethasone vs. catalase/dismutase superoxide (C/DSO). Study design Randomized, matched-paired, double-blind trial. Setting Dermatology Section, University of Antioquia, Medellín, Colombia. Subjects Patients (aged > 18 years or between 12 and 18 years) with parent's informed consent, with stable or active bilateral vitiligo. Intervention Topical 0.05% betamethasone or C/DSO. Methods Two lesions similar to each other in size were chosen. All assessments were made by two blinded investigators, and photographs were subjected to morphometry analysis. Main outcome Skin repigmentation by digital morphometry. Results Twenty-five patients were enrolled in the study (21 women and 4 men). Mean age of participants was 40 years (range: 12,74 years). One patient on C/DSO experienced a mild local erythematous papular rash that self-resolved. At 4 months of therapy, there was no statistical difference on the percentage of repigmentation between betamethasone and C/DSO (5.63% ± 27.9 vs. 3.22% ± 25.8, respectively, P = 0.758). After 10 months of therapy, the percentage of skin repigmentation increased to 18.5 ± 93.14% with betamethasone and to 12.4 ± 59% with C/DSO, but again, we found no statistical differences (P = 0.79). Discussion and conclusions Few studies have described objective methods to evaluate repigmentation among vitiligo patients. Digital morphometry provides an objective assessment of repigmentation in vitiligo. Objective vitiligo repigmentation with topical C/DSO at 10 months is similar to topical 0.05% betamethasone. Although a mild adverse effect was related to the use of C/DSO, such finding was not severe enough to discontinue treatment. [source] Vegetation-environment relationships in Atlantic European calcareous grasslandsJOURNAL OF VEGETATION SCIENCE, Issue 1 2000J.C. Duckworth Hill et al. (1994); Tutin et al. (1964,1980) Abstract. The relationship between vegetation and environment was investigated for calcareous grasslands in a region in the west of Spain, France, Britain and Ireland defined by climatic criteria. Vegetation was sampled using objective methods and data collected on soils, land cover, location and management. Climate data were obtained from an available database. Examination of the first axis of vegetation variation as defined by Detrended Correspondence Analysis (DCA) showed a gradient from the Irish and British samples to those from France. The Spanish samples formed a separate group on the second axis. The species composition along the gradients is discussed. Correlations between the vegetation gradients and environmental variables were determined. The strongest correlations with the first DCA axis were for temperature, latitude, soil organic matter, grazing and land cover. The second DCA axis was highly correlated with rainfall, altitude and land cover. The third and fourth DCA axes were more difficult to interpret but appeared to be related to land cover. The results indicate that climate factors are important at this scale, but should not be considered in isolation and that factors relating to land cover and management should also be taken into account. [source] Injection of botulinum toxin before pneumatic dilatation in achalasia treatment: a randomized-controlled trialALIMENTARY PHARMACOLOGY & THERAPEUTICS, Issue 6 2006J. MIKAELI Summary Background Pneumatic dilatation is the first line therapy in achalasia, but half of patients relapse within 5 years of therapy and require further dilatations. Aim To assess whether botulinum toxin injection before pneumatic dilatation is superior to pneumatic dilatation alone in achalasia patients. Methods Newly diagnosed achalasia patients were randomly assigned to receive botulinum toxin 1 month before pneumatic dilatation (botulinum toxin-pneumatic dilatation group: 27 patients with median age of 38) or to undergo pneumatic dilatation alone (pneumatic dilatation group: 27 patients with median age of 30). Response to therapy was assessed by clinical and objective methods at various intervals. Results One-year remission rate of patients in botulinum toxin-pneumatic dilatation group was 77% compared with 62% in pneumatic dilatation group (P = 0.1). In pneumatic dilatation group, the oesophageal barium volume significantly (P < 0.001) decreased at 1 month, but this reduction did not persist over 1-year follow-up. Botulinum toxin-pneumatic dilatation group showed a significant (P < 0.001) reduction in barium volume at the various times intervals post-treatment. In the botulinum toxin-pneumatic dilatation group, 10/11 (91%) patients over 40 were in remission at 1 year, comparing with only five of nine (55%) cases in pneumatic dilatation group (P = 0.07). Conclusion Injection of botulinum toxin before pneumatic dilatation does not significantly enhance the efficacy of pneumatic dilatation. Abstract This paper summarizes successes and limitations of targeted observing field programmes starting from the Fronts and Atlantic Storm-Track Experiment in 1997 through recent programmes targeting winter storms and tropical cyclones. These field programmes have produced average reductions in short-range forecast errors of about 10 per cent over regional verification areas, and maximum forecast error reductions as large as 50 per cent in certain cases. The majority of targeting cases investigated so far involve sets of dropsondes and other observation data that provide partial coverage of target areas. The primary scientific challenges for targeting include the refinement of objective methods that can identify optimal times and locations for targeted observations, as well as identify the specific types of satellite and in situ measurements that are required for the improvement of numerical weather forecasts. The most advanced targeting procedures, at present, include: the ensemble transform Kalman Filter, Hessian singular vectors, and observation-space targeting using the adjoint of a variational data assimilation procedure. Targeted observing remains an active research topic in numerical weather prediction, with plans for continued refinement of objective targeting procedures, and field tests of new satellite and in situ observing systems. Copyright © 2005 Royal Meteorological Society [source] Combining geometric morphometrics and pattern recognition to identify interspecific patterns of skull variation: case study in sympatric Argentinian species of the genus Calomys (Rodentia: Cricetidae: Sigmodontinae)BIOLOGICAL JOURNAL OF THE LINNEAN SOCIETY, Issue 2 2008PEDRO CORDEIRO-ESTRELA Sympatric species of vesper mice Calomys laucha and Calomys musculinus are difficult to discriminate, especially in natural history collections where they are identified by external body measurements and cranial characteristics. Accurate identification of these two species can be important because only one of them, C. musculinus, is a Junin virus reservoir, the aetiological agent of the Argentine Hemorragic Fever. Research has focused into the development of molecular techniques to unambiguously identify these species. We apply statistical procedures from the field of pattern recognition to three-dimensional geometric morphometric data based on skull landmarks to identify sympatric species C. laucha, C. musculinus and Calomys venustus. Pattern recognition techniques correctly identified the three species without any prior information on specimen identity. By contrast to expectations, C. venustus differed from the other two species mainly on the basis of shape and not by its centroid size. The main sources of difference between C. laucha and C. musculinus were of shape, specifically localized at the landmarks defined by: (1) the sutures between the premaxillaries, the nasals and the frontals; (2) the sutures between the parietals, the frontals and the squamosals; and (3) the suture between the parietals and the interparietal. Nevertheless, allometries dominate the patterns of interspecific variation between these latter species and may partly explain past identification difficulties. Morphological evolution is discussed. [source] Defining Systems Expertise: Effective Simulation at the Organizational Level,Implications for Patient Safety, Disaster Surge Capacity, and Facilitating the Systems InterfaceACADEMIC EMERGENCY MEDICINE, Issue 11 2008Amy H. Kaji MD Abstract The Institute of Medicine's report "To Err is Human" identified simulation as a means to enhance safety in the medical field, just as flight simulation is used to improve the aviation industry. Yet, while there is evidence that simulation may improve task performance, there is little evidence that simulation actually improves patient outcome. Similarly, simulation is currently used to model teamwork-communication skills for disaster management and critical events, but little research or evidence exists to show that simulation improves disaster response or facilitates intersystem or interagency communication. Simulation ranges from the use of standardized patient encounters to robot-mannequins to computerized virtual environments. As such, the field of simulation covers a broad range of interactions, from patient,physician encounters to that of the interfaces between larger systems and agencies. As part of the 2008 Academic Emergency Medicine Consensus Conference on the Science of Simulation, our group sought to identify key research questions that would inform our understanding of simulation's impact at the organizational level. We combined an online discussion group of emergency physicians, an extensive review of the literature, and a "public hearing" of the questions at the Consensus Conference to establish recommendations. The authors identified the following six research questions: 1) what objective methods and measures may be used to demonstrate that simulator training actually improves patient safety? 2) How can we effectively feedback information from error reporting systems into simulation training and thereby improve patient safety? 3) How can simulator training be used to identify disaster risk and improve disaster response? 4) How can simulation be used to assess and enhance hospital surge capacity? 5) What methods and outcome measures should be used to demonstrate that teamwork simulation training improves disaster response? and 6) How can the interface of systems be simulated? We believe that exploring these key research questions will improve our understanding of how simulation affects patient safety, disaster surge capacity, and intersystem and interagency communication.
