CANCER INPATIENTS MORPHINE USAGE: A NEW ENGLAND AREA SURVEY

ABSTRACT:,This is a one year study of the use of morphine in cancer patients in 10 inpatient facilities in the New England Area Health Service in the north-west of New South Wales. The study explored 170 admissions relating to 122 patients, most of whom were cared for by their general practitioners. The use of morphine in these cancer patients was compared with the recommendations made by the expert working group of the European Association of Palliative Care.1 Those items which matched the recommendations included the initial doses for new users of morphine and the subcutaneous route being the preferred parenteral route. The data in this study differed from the recommendations in that only half of the patients received the immediate release morphine when first given oral morphine, only 43% had orders for immediate release oral morphine for breakthrough pain (with a variable frequency) and a significant number of orders for parenteral and immediate release oral morphine for breakthrough pain were outside the recommended doses (100% and 86.2%, respectively). Written orders for immediate release oral and parenteral morphine involved a dose range in significant numbers while only 30% of patients had orders for parenteral morphine for breakthrough pain. Anxiety disorders and risk for suicide attempts: findings from the Baltimore Epidemiologic Catchment area follow-up study,

Abstract Our objective was to determine whether the presence of an anxiety disorder was a risk factor for future suicide attempts. Data were drawn from the 13-year follow-up Baltimore Epidemiological Catchment Area survey (n=1,920). Multiple logistic regression analysis was used to determine the association between baseline anxiety disorders (social phobia, simple phobia, obsessive-compulsive disorder, panic attacks, or agoraphobia) and subsequent onset suicide attempts. The presence of one or more anxiety disorders at baseline was significantly associated with subsequent onset suicide attempts (adjusted odds ratio 2.20, 95% confidence interval 1.04,4.64) after controlling for sociodemographic variables and all baseline mental disorders assessed in the survey. These findings suggest that anxiety disorders are independent risk factors for suicide attempts, and underscore the importance of anxiety disorders as a serious public health problem. Bright Ly, emitters at z, 9: constraints on the LF from HizELS,

ABSTRACT New results are presented, as part of the Hi- z Emission Line Survey (HizELS), from the largest area survey to date (1.4 deg2) for Ly, emitters (LAEs) at z, 9. The survey, which is primarily targeting H, emitters at z < 3, uses the Wide Field CAMera on the United Kingdom Infrared Telescope and a custom narrow-band filter in the J band and reaches a Ly, luminosity limit of ,1043.8 erg s,1 over a co-moving volume of 1.12 × 106 Mpc3 at z= 8.96 ± 0.06. Only two candidates were found out of 1517 line emitters and those were rejected as LAEs after follow-up observations. The limit on the space density of bright LAEs is improved by three orders of magnitude, consistent with suppression of the bright end of the Ly, luminosity function beyond z, 6. Combined with upper limits from smaller but deeper surveys, this rules out some of the most extreme models for high-redshift LAEs. The potential contamination of future narrow-band Ly, surveys at z > 7 by Galactic brown dwarf stars is also examined, leading to the conclusion that such contamination may well be significant for searches at 7.7 < z < 8.0, 9.1 < z < 9.5 and 11.7 < z < 12.2. Archaeological prospecting at the Double Ditch State Historic Site, North Dakota, USA

Abstract A four-year programme of remote sensing at the Double Ditch State Historic Site, a fortified earthlodge village in the Great Plains of North Dakota, USA, demonstrates the importance of combined prospecting methods for understanding the multidimensional nature of complex settlements. Aerial survey from a powered parachute acquired high-resolution digital colour and thermal infrared imagery. The former distinguishes houses, borrow pits and ditches from middens and fill areas by changes in vegetation; the latter does the same through temperature variations that also highlight substantial village deposits and century-old excavations. Magnetic gradiometry reveals two previously unknown fortification systems that vastly increase the settlement's area and projected population to perhaps 2000 individuals. Vast numbers of food storage pits that supported that population are also indicated. Vertical soundings and area surveys of magnetic susceptibility aid in the understanding of storage pit magnetism and anthropogenic soil mounding and removal. A high-resolution digital model of topography documents surface expressions caused by ditches, houses, borrow pits and mounds; when combined with overlays of geophysical data understanding of remote sensing responses is improved and relationships between large mounds with village defenses are made clear. Ground-penetrating radar gives insights into mounded midden interior forms and yields details about house interior components, while electrical resistance results improve definition of middens, other depositional areas, houses and borrow pits. This multisensor remote sensing programme reduced excavation costs by allowing a wide variety of archaeological features of interest to be targeted accurately. Excavations confirmed anomaly identifications and established a chronology that documents late fifteenth century origins to an ultimate abandonment about AD 1782.
