Non-fatal injuries among Pacific infants in Auckland: Data from the Pacific Islands Families First Two Years of Life study

Aims: Child injury is the leading cause of mortality and morbidity in developed countries. While Pacific infant death rates are relatively high in New Zealand, little is known about non-fatal injury rates. We seek to describe maternally reported injury in Pacific infants aged between 0,24 months. Methods: A cohort of Pacific infants born during 2000 in Auckland, New Zealand, was followed. Maternal home interviews were conducted at 6 weeks, 12 months and 24 months postpartum and injury events were recalled. Marginal models using generalized estimating equations (GEEs) were used to analysis the longitudinal data. Results: The inception cohort included 1398 infants at 6 weeks, 1241 infants at 12 months and 1161 infants at 24 months. The age-specific injury incidence per 1000 person-years exposure was estimated at 48 (95% CI: 23, 88) injuries for infants aged 0,6 weeks, 106 (95% CI: 88, 127) injuries for infants aged 7 weeks,12 months and 174 (95% CI: 151, 199) injuries for infants aged 13,24 months. In the multivariable GEE model, older infants (P < 0.001), infants who were male (P = 0.01), born to Pacific Island fathers and non-Pacific Island mothers (P < 0.001), and in higher or unknown income groups (P = 0.01) were significantly more likely to suffer injury events. No significant two-factor interaction with infant age was identified. Conclusions: Among Pacific infants, non-fatal injury is common and injury incidence rates are considerably higher than national levels. Male infants and those born into ethnically mixed families, where the father was of Pacific Island ethnicity and the mother was non-Pacific, were at increased relative risk of injury and might benefit from specific injury prevention targeting. However, given the high injury incidence levels found, we advocate that investigation and targeting of culturally appropriate prevention strategies for all Pacific families with young children is required to reduce injury rates for Pacific infants in New Zealand. A global perspective of migration and occupational health

Abstract Background Global migration has dramatically increased over the past decade and is at an all-time high, approaching 200 million persons per year. Demographics and economic interdependence suggest that immigration will continue for the near future at record high levels. Methods A review of the few studies that have investigated occupational injury and illness rates among immigrant populations. Results Existing data indicate that higher rates of fatal and non-fatal injuries are common compared to native populations. This increase is in part due to immigrants working in higher risk occupations (e.g., agriculture, construction), but occupational morbidity and mortality is higher among immigrants than native-born workers within occupational categories. Conclusions Research is needed to identify the causes of increased risk among immigrants and to provide direction to effective public health interventions. Research methods must be adapted to different epidemiologic characteristics of immigrant populations, including lack of standard sampling frames, different language and culture from the dominant culture, and precarious work status. Utilizing hospital discharge data (HD) to compare fatal and non-fatal work-related injuries among Hispanic workers in New Jersey

Abstract Background This study explores the utilization of Hospital Discharge (HD) data to obtain estimates of work-related non-fatal injuries rates in NJ to determine if Hispanics workers have an increased risk of specific work-related injuries. In addition, HD data are used to compare the rate ratios between fatal and non-fatal injuries in this population to demonstrate the effectiveness of using HD as a surveillance tool for monitoring injury trends and performing evaluations. Methods Several types of fatal and non-fatal injuries were modeled using Poisson regression with the following predictor variables: gender, ethnicity, and year. The estimated number of workers by ethnicity employed in NJ each year was obtained from the U.S. Census Bureau, DataFerrett, Current Population Survey, November 2006, a data mining tool which accesses CPS data. Results These analyses, utilizing estimates of working population at-risk, indicate that Hispanic workers have an increased risk of four particular work-related injuries compared with non-Hispanics, and Hispanics were injured at a younger age than non-Hispanics. In addition the rankings of the rate ratios from the comparison between non-fatal and fatal risk estimates were similar; indicating that occupational surveillance of non-fatal injuries is a viable component to be considered. Conclusions HD data are effective for monitoring trends over time across ethnic groups and injury types. Therefore, non-fatal injury surveillance should be considered for targeting specific worker populations for interventions to reduce exposure to workplace hazards, and can be a valuable surveillance tool in efforts to reduce occupational injuries. Injuries and fatalities to U.S. farmers and farm workers 55 years and older

Abstract Background Previous studies have shown that older farmers and farm workers have been identified at high risk for farm fatalities, most notably involving tractor overturns. Older farmers also incur more severe non-fatal injuries. Methods Data from two national surveillance systems are presented to describe fatal and non-fatal injuries occurring to older farmers 55+ years of age. Tractor-related fatality investigations for older farmers are examined for characteristics of the tractors not available in the injury surveillance systems. Results Older farmers and farm workers averaged 26,573 lost-time injuries annually in 2001 and 2004, with an injury rate of 4.5 injuries/100 workers/year compared to an overall farming injury rate of 4.8 injuries/100 workers/year. Fatality data show that older farmers accounted for over half of all farming deaths between 1992 and 2004 (3,671 of 7,064 deaths), and had a fatality rate of 45.8 deaths/100,000 workers/year compared to the overall farming fatality rate of 25.4 deaths/100,000 workers/year. Most common mechanisms of fatal injury to older farmers were "tractors" (46%), "trucks" (7%), and "animals" (5%). Conclusions Although older farmers and farm workers are at lower risk of overall injury compared to their younger counterparts, injuries to farmers 55 years and older tend to be much more severe. To effectively minimize the risk faced by older farmers, prevention programs must encourage safe work behaviors and practices and the implementation/installation of appropriate safety devices and equipment. Work-related non-fatal injuries among foreign-born and US-born workers: Findings from the U.S. National Health Interview Survey, 1997,2005

Abstract Background Millions of foreign-born workers are employed in the US. Population-based surveys have value in describing the non-fatal work-related injuries that these workers suffer. Methods Using data from the 1997,2005 National Health Interview Survey, we compared the rates of non-fatal work-related injuries among foreign-born and US-born adult workers. Logistic regression was used to produce work-related injury odds ratios (ORs) and 95% confidence intervals (95% CI) by nativity and years of residence while controlling for sex, age, race/ethnicity, education, poverty, family size, insurance status, delayed medical care, and alcohol use. Industry-specific rates were also compared. Results Foreign-born workers reported a lower rate of work-related injuries than US-born workers, 50 per 10,000 foreign-born workers versus 89 per 10,000 US-born workers (P,<,0.01). After controlling for confounding variables, the OR of work-related injuries for foreign-born workers as compared to US-born workers was 0.50 (95% CI,=,0.38,0.66). The construction, agriculture/forestry and fisheries, and manufacturing industries had the highest work-related injury rates for both groups of workers. Conclusions Foreign-born workers had a lower overall rate of work-related injury when compared to US-born workers. Both US-born and foreign-born workers face significant injury risks, especially in certain industries. Interventions tailored with ethnic and cultural differences in mind are still warranted. Am. J. Workers' compensation experience of Colorado agriculture workers, 2000,2004,

Abstract Background Agriculture is among the most hazardous of occupations. The lack of information regarding agriculture injuries or fatalities has been recognized as an obstacle for effective injury prevention. Workers' compensation claims data for non-fatal injuries among agriculture and agri-business workers in the State of Colorado between the years of 2000 and 2004. Methods Workers' compensation claims are utilized to estimate injury claim incidence rates, determine the distributions of sources, causes, types and body locations of injuries, and estimate the costs of these injuries. Results Colorado agriculture and agri-business workers (e.g., cattle dealers, cattle or livestock raisers, dairy farmers) have high rates of injury claims, especially in sectors that involve interaction with animals or livestock. Grain milling operations had a high rate of injury claims among agri-business operations. Injuries related to animals, strains, machinery, and falls or slips were the most frequent among all occupations analyzed. Conclusions Understanding the occurrence of injuries among Colorado agriculture and agri-business workers is critical to implementing and evaluating effective intervention programs for specific agriculture-related occupations. The development of safety interventions that address the worker,animal interface, fall protection systems, machinery usage, and overexertion prevention strategies is recommended. Am. J. Ind. Workplace risks and wages: Canadian evidence from alternative models

Three alternative models of compensating wage premiums for risk are estimated: the conventional OLS wage regression; an endogenous risk model that accounts for the simultaneity that may occur if workers of high potential earnings prefer safer jobs; and a self-selection model to account for the possibility that workers sort into jobs based on unobserved tolerance for risk that affects their productivity in dangerous work environments. The results suggest that the existing Canadian estimates, which have been based on the basic model, may seriously underestimate the wage premium for risk and hence the implied cost of fatal and non-fatal injuries.
