Most Latino smokers in California are low-frequency smokers

ABSTRACT Aims We examine the phenomenon of low-frequency smoking (non-daily smoking or smoking , 5 cigarettes daily) among California Latinos and address its implications for addiction theory and population tobacco control. Design, setting and participants Data gathered in 2001 and 2003 through the California Health Interview Survey (CHIS), the largest general health survey in California. The present study focused on Latino current smokers (n = 1254 for CHIS 2001; n = 946 for CHIS 2003). Measurement Latino smokers reporting either non-daily smoking or smoking ,5 cigarettes daily were identified and grouped into one category: low-frequency smokers. Findings Weighted by population parameters, more than 70% of Latino smokers in California were found to be low-frequency smokers [70.7% (CI = 67.2%, 73.9%) in 2001 and 70.8% (CI = 67.1%,74.2%) in 2003]. This high proportion cut across all demographic dimensions in both surveys, suggesting pervasiveness and reliability of this phenomenon. Proportions for non-daily smokers and low-rate daily smokers were 48.6% and 22.1% in 2001 and 54.9% and 15.9% in 2003. In both surveys, more than 80% of non-daily smokers consumed , 5 cigarettes on their smoking days. Conclusions The fact that most Latino smokers are low-frequency smokers calls for a new theoretical framework,beyond withdrawal-based theories,to account for the prevalence of this behavior on the population level. It also calls into question the harm-reduction approach as a tobacco control strategy for California Latino populations. Strategies emphasizing that every cigarette can hurt, and encouraging complete cessation, seem more fitting for this group of smokers. [source]


Theoretical framework for the histone modification network: modifications in the unstructured histone tails form a robust scale-free network

A rapid increase in research on the relationship between histone modifications and their subsequent reactions in the nucleus has revealed that the histone modification system is complex, and robust against point mutations. The prevailing theoretical framework (the histone code hypothesis) is inadequate to explain either the complexity or robustness, making the formulation of a new theoretical framework both necessary and desirable. Here, we develop a model of the regulatory network of histone modifications in which we encode histone modifications as nodes and regulatory interactions between histone modifications as links. This network has scale-free properties and subnetworks with a pseudo,mirror symmetry structure, which supports the robustness of the histone modification network. In addition, we show that the unstructured tail regions of histones are suitable for the acquisition of this scale-free property. Our model and related insights provide the first framework for an overall architecture of a histone modification network system, particularly with regard to the structural and functional roles of the unstructured histone tail region. In general, the post-translational "modification webs" of natively unfolded regions (proteins) may function as signal routers for the robust processing of the large amounts of signaling information. Diverting with Benevolent Military Force: Reducing Risks and Rising above Strategic Behavior

Research on the diversionary use of force has burgeoned in recent years, but the literature remains divided. This paper attempts to reconcile extant findings by advancing a new theoretical framework for diversionary force centered on the agenda-setting literature. It expands the conventional conception of diversionary behavior and distinguishes the benevolent use of force over low politics issues (which we term socioeconomic interventions, SEI) from belligerent force used over high politics issues (which we term politico-strategic interventions, PSI). This expansion also refines our understanding of strategic conflict avoidance (SCA). Using Zero-Inflated Poisson (ZIP) regression on 140 countries from 1950 to 1996, we find that democracies and mixed regimes tend to use SEI for diversion even though strategic conflict avoidance does not prevent them from using PSI. We further find that autocracies do not externalize their internal problems with either type of armed force and that, surprisingly, strategic conflict avoidance may constrain autocracies suffering economic decline. These outcomes suggest that our theory has utility and that research on both diversion and SCA would benefit from further theoretical refinement. [source]


Developmental pathways in food allergy: a new theoretical framework

Background:, To date, there is no model of psychosocial development based on empirical food allergy (FA) research. This limits the ability of clinicians, researchers and policy-makers to predict and evaluate the real impact of FA on the child, with implications for prevention, treatment, intervention and health policy. Objectives:, To provide an integrated conceptual framework to explain the onset, development and maintenance of FA-related cognitions, emotions and behaviour, with particular attention to transition points. Method:, Fifteen focus groups meetings were held with 62 children (6,15 years). Developmentally appropriate techniques were designed to stimulate discussion, maintain interest and minimize threat to children's self-esteem. Data were analysed using grounded theory. Results:, FA impacts directly on children's normal trajectory of psychological development in both an age- and disease-specific manner. Six key themes emerged from the analysis: ,meanings of food'; ,autonomy, control and self-efficacy'; ,peer relationships'; ,risk and safety'; ,self/identity'; and ,coping strategies'. Conclusions:, Coping with FA is more than simply a strategy, it is a cumulative history of interactive processes (age, gender and disease specific) that are embedded in a child's developmental organization. Clinical Implications:, The early recognition and incorporation of an FA-specific developmental framework into a treatment plan is essential and sets the stage for an effective medical care and the eventual transition from paediatric to adult care. Schooling as a Knowledge System: Lessons from Cramim Experimental School

This article describes an experiment utilizing a research and development strategy to design and implement an innovative school for the future. The development of Cramim Elementary School was a joint effort of researchers from Tel-Aviv University and the staff of the school. The design stage involved constructing a new theoretical framework that defined school as a knowledge system, based on the state of the art, interdisciplinary study of the nature of humans, and the nature of knowledge. A new school design emerged based on this theoretical framework and the school was opened in 1995. Action research followed for 8 years and the results indicated that the school has emerged as a learning organization and successfully integrated knowledge technologies into the learning processes of both students and teachers. Differentiated teaching strategy resulted in a significant increase in achievements (+11% in maths, literacy, and science; +10% in literacy in kindergarten; persistence of higher achievement in junior high schools). The greatest beneficiaries were low-achieving students. As the school is a highly complex system, individual variables contributing to the increased effectiveness could not be isolated. The article's conclusion is that experimental schools are a productive strategy to bring about changes, but unless these schools are part and parcel of the culture of the mainstream education system culture, they are destined to remain isolated cases. Imaging the future: theatre and change within the child protection system

The use of social pedagogy as a paradigm for critically appraising developments within child and family social work has been largely neglected. This paper outlines the work of Augusto Boal and his adoption of social pedagogy as a method for empowering oppres-sed social groups in Brazil. It is argued that Boal's approach can be adapted by using action research techniques to analyse and effect change in situations where child care professionals face daily contradictions in their attempts to both protect children and support families. To demonstrate its relevance to child care practice, a description is provided of how the approach was used with two groups of social work students , one undertaking qualifying training, the other post-qualifying training. The results of this application suggest a new theoretical framework for practice which aims to establish communicative consensus around the needs of children and a mutual appreciation of roles and responsibilities.
