[source] War and the global economyECONOMIC OUTLOOK, Issue 1 2002Article first published online: 5 FEB 200 Recent months have seen the adoption of an increasingly belligerent stance towards Iraq on the parts of the US and the UK in particular, signalling a shift from the policy of containment that has been in place since the last Gulf War, and towards a more aggressive policy. As yet, it is unclear whether this increased belligerence will culminate in a renewed full,scale military engagement with Iraq, to achieve the much vaunted ,regime change', or whether the outcome will be an agreement to a new programme of weapons inspections. But the risk of a military conflict appears to have increased. This article assesses the potential economic implications of such a conflict, exploring different scenarios for how it might evolve, and their impact on oil prices and global growth. [source] ,I'm the Boss': testing the feasibility of an evidence-based patient education programme using problem-based learningEUROPEAN DIABETES NURSING, Issue 1 2004K Wikblad FEND Professor in Diabetes Nursing Abstract Patient education programmes have shown only small to modest effects on diabetes self-care and metabolic control. Despite that, almost all diabetes teams agree that patient education is an extremely important part of the treatment of diabetes. It is, therefore, important to identify components of successful patient education as a basis for creating and testing an evidence-based education programme. In a review of controlled studies evaluating patient education such components were identified and these were then used in building up the new programme. This programme, called ,I'm the Boss', is based on the notion that the patient is an active care participant, setting his own self-care goals, and is the one responsible for his own life. The content of the programme did not, therefore, focus on diabetes as such, but on life with diabetes. Six themes were explored during six three-hour weekly sessions. The educational method used was problem-based learning. This method is founded in cognitive theory and views the learner as active in seeking knowledge and able to solve the self-care problems identified. The aim of this study was to explore the feasibility of the programme which was tested in four small groups (five to eight participants) of diabetic patients together with two facilitators. After completing the programme, the patients participated in focus group interviews to evaluate the programme. They identified both positive and negative factors. After each session the two facilitators reflected upon the group dynamics. In particular, problems with allowing patients to be the experts should be highlighted. This programme has been modified according to the evaluation and it is now being tested in a randomised, controlled, multicentre study. Copyright © 2004 FEND. [source] Student satisfaction with curriculum modifications in a French dental schoolEUROPEAN JOURNAL OF DENTAL EDUCATION, Issue 3 2000P. Farge Since 1994, important modifications have been implemented in the dental curriculum in France, and an additional year has been included in the dental curriculum. The 1st year has remained unchanged; it is common to both medical and dental schools and leads to a selection procedure of 1 in 10 dental students. In the new curriculum, the dental student is engaged in a 5-year programme in dental school (years 2 to 6), as opposed to 4 years in the former programme (years 2 to 5). Basically, this new curriculum emphasises research initiation, links with medical training and offers broader clinical experience. During the academic year 1998,1999, dental schools had 2 different types of students: the 4th year students belonging to the new programme along with the 5th year students in their final year of the old curriculum. Using a specially devised questionnaire, we investigated the students' perception of their respective training, their motivation and professional plans. [source] Collection of peripheral progenitor cells in paediatric patients with a new programme for the collection of mononuclear cellsJOURNAL OF CLINICAL APHERESIS, Issue 3 2003R. Moog Abstract When harvesting peripheral progenitor cells (PPC) in children, the special situation of their circulatory system has to be taken into account. Therefore, extracorporeal blood volume and product volume should be small to avoid side effects. Nine children (age 2,14 years, weight 12.8,58.5 kg) with malignancies underwent 10 PPC collections with the MNC programme of the Amicus blood cell separator. The disposable kit was primed with red blood cells (RBCs) or human albumin to avoid circulatory side effects. The children were monitored for blood pressure and heart rate during the whole apheresis procedure. A median blood volume of 4,577 ml (range 3,536,8,596 ml) was processed in a separation time of 270 min (range 176,331 min). The median product weight was 81 g (range 53,107 g) and the yield of CD 34 antigen expressing cells was 12.5 × 106/kg body weight (range 1.8,26 × 106/kg body weight). Only one child had to undergo a second apheresis to collect the desired transplantation dose. The median platelet contamination of the product was 0.32 × 1011 (0.13,0.85 × 1011). No circulatory side effects were observed. Blood flow alarms occurred in seven of ten aphereses and one collection had to be terminated due to insufficient flow. PPC can be efficiently collected in children with the MNC programme without circulatory side effects. The platelet contamination of the product was low due to the elutriation principle of the collection process, thereby avoiding thrombocytopenic bleeding episodes in the patients. J. Clin. (2010) Journal of Nursing Management 18, 234,244 Preceptors' experiences of clinically educating and assessing undergraduate nursing students: an Irish context Aim, To explore preceptors' views and experiences of preceptoring undergraduate nursing students. Background, Undertaking a preceptoring role is acknowledged internationally as complex and challenging. With the introduction of the undergraduate degree programme in Ireland (2002), preceptors were assigned a more formal role in the teaching and assessing of students. As this was a new programme for students and an additional responsibility for preceptors, it was important to investigate how preceptors found this new experience. Methods, Data were collected using a mixed methods descriptive approach. Results, Many preceptors wanted to become a preceptor and enjoyed the role. The majority of preceptors found the role stressful and burdensome and did not feel adequately supported by their clinical managers. Preceptors expressed the need for protected time, support, feedback and recognition from management for undertaking this role. Conclusion, Findings validate problems experienced with preceptoring in other English-speaking countries and contribute further to building a case for vital change in this component of nursing education and nursing practice. Implications for nursing management, Managers should focus on the challenges expressed by preceptors in this study and identify appropriate strategies to carefully select, monitor and support preceptors in this complex role. [source] Delivering ,Gold Standards' in end-of-life care in care homes: a question of teamwork?JOURNAL OF CLINICAL NURSING, Issue 12 2009Alistair Hewison Aim., This paper reports findings from a study which investigated the introduction of the Gold Standards Framework for improving end-of-life care into care homes in England. Background., The Gold Standards Framework was developed in primary care to improve the care provided for people at the end-of-life. Following its successful introduction to this setting it was adapted and implemented in care homes. Design., A case study approach was appropriate for this study of a care programme into a ,real life' setting. Method., Fourteen managers participated in an initial telephone interview. Ten of these homes participated in the case study phase and 61 staff participated in individual or group interviews. Seven residents and three relatives participated in face to face interviews. Qualitative data were analysed in line with the template approach. Survey data were also analysed and a validated Teamworking Questionnaire was used. Results., It was found that teamwork is central to the successful introduction of the Gold Standards Framework in Care Homes. Good staffing levels and management support were also perceived to be key factors in homes where the Framework became established. Conclusion., Effective teamwork was necessary for changes in end-of-life care to be achieved in the care homes. If end-of-life care and other improvements in practice are to continue, teamwork will need to be supported and developed. Relevance to clinical practice., Effective teamwork appears to be a pre-requisite for successful implementation of new programmes of care. Organisations wishing to implement such programmes should assess the quality of teamwork and may need to address this first. [source] Report of the Council for the session 2006,2007JOURNAL OF THE ROYAL STATISTICAL SOCIETY: SERIES A (STATISTICS IN SOCIETY), Issue 4 2007Council Report President's foreword., This year's annual report shows another very successful year for the Society. The range of the Society's new initiatives bears testament to our vigour and to the energy and enthusiasm of Fellows and staff. It is difficult to summarize all of these but I offer a brief overview of some of the highlights. This year we have awarded the first annual prize for ,Statistical excellence in journalism'. It is too easy to bemoan the general quality of coverage of statistical issues in the press and other media. But simply moaning does not improve the situation. As a positive step, on the instigation of Sheila Bird and Andrew Garratt, the Society decided to initiate an award for the best journalistic coverage of a statistical issue. This year first prize was awarded to Ben Goldacre of The Guardian. I hope that these annual awards will offer a positive focus on good coverage and help us to promote best practice. This year, also, we have set up the Professional Development Centre to act as a focus for statistical training both for statisticians and for others who use statistical methods as part of their work. It thus reflects our support for continuing professional development for our Fellows and at the same time provides outreach to members of the statistical user community who want to improve their statistical skills. We welcome Nicola Bright as the Director of the Centre and wish her every success. I am pleased to say that it is not just the Society centrally that has taken new activities this year. The Manchester Local Group have initiated a prize for final year undergraduates from any higher education institute in the north-west. At a time when there are concerns about the number of well-qualified graduates coming into the statistics profession this seems an excellent way to attract the attention of final year undergraduates. I wish this initiative every success. Another development to which the Society has contributed is the Higher Education Funding Council for England project ,more maths grads' which is designed to promote participation in undergraduate degrees in the mathematical sciences. A good supply of mathematically trained graduates is essential to the UK economy in general and to the health of the statistics discipline in particular. It is good that the Society is involved in practical developments that are aimed at increasing participation. The final new initiative that I shall draw attention to is the ,first-in-man' report which is concerned with the statistical design of drug trials aimed at testing novel treatment types. The working party was set up as a result of the adverse reactions suffered by healthy volunteers to a first-in-man trial of monoclonal antibodies and who were subsequently admitted to Northwick Park hospital. The report makes a series of recommendations about the design of such trials and will, I hope, contribute to the safety of future trials. I would like to thank Stephen Senn and the members of the working party for their considerable efforts. As well as these new initiatives there were, of course, many other continuing activities that are noteworthy. The annual conference in Belfast was a great success with many lively sessions and a good number of participants. In particular it was good to see a high number of young statisticians participating in the conference, reflecting the continuing impact of the Young Statisticians Forum on which I commented in the previous annual report. Another continuing activity for the Society is the statistical legislation going through Parliament as I write. The Society has long campaigned for legislation for official statistics. The issue now is to try to get good legislation which will have the required effect and will help the Government Statistical Service and other statistical producers to produce high quality, authoritative statistics in an environment that commands public confidence. As first published, the Society was disappointed with the Bill but we have worked to build support for amendments that, in our view, are essential. Time alone will tell how effective the final legislation will be in meeting our aims. I would like to draw attention to the success of the Membership Services team. We, although with other statistical Societies, have experienced a decline in membership in recent years but the team have turned this round. They are helping to recruit new Fellows and to retain the commitment of existing Fellows. This is a fine achievement and I would like to thank Nicola Emmerson, Ed Swires-Hennessy and the whole team. Finally we have, at last, reached a conclusion in our dealings with the Privy Council and will implement the second phase of constitutional changes. In future our business year, financial year and year for elected appointments will all coincide on a calendar year basis. There will be transitional arrangements but in due course all our administrative arrangements will coincide and will improve efficiency and co-ordination. This has been a long journey, steered effectively by our Director General, Ivor Goddard, and I congratulate him for a successful outcome on your behalf. As you read this report, I hope that you will share my impression of a Society that is lively and spawning many new programmes. We have a dual commitment: to the well-being of statistics as a discipline and to the promotion of statistical understanding and practice to the benefit of Society at large. In both respects I feel that the Society is in good health. This is due to the unstinting efforts of a large number of individual volunteers, including in particular our Honorary Officers and also, of course, the staff at Errol Street. Tim Holt [source] Childhood disadvantage and health inequalities: a framework for policy based on lifecourse researchCHILD: CARE, HEALTH AND DEVELOPMENT, Issue 6 2004H. Graham Abstract Tackling health inequalities is moving up the policy agenda of richer societies like the UK, with governments looking for evidence to guide policy review and development. Observational studies of how childhood disadvantage compromises health in adulthood are an important part of the evidence base, but are largely inaccessible to the policy community. We develop a framework which captures the findings of these studies. Our framework highlights how disadvantage in childhood adversely affects both socio-economic circumstances and health in adulthood through a set of interlocking processes. Key among these are children's developmental health (their physical, cognitive and emotional development) and health behaviours, together with the associated educational and social trajectories. In breaking down the link between childhood disadvantage and adult health into its constituent elements, the framework provides a basis for understanding where and how policies can make a difference. The paper argues that the process of policy review and development needs to include both new programmes and the mainstream policies in which they are embedded. [source] |
