Responding to formal complaints about the emergency department: Lessons from the service marketing literature

Abstract The ability to respond to formal complaints is a necessary part of emergency medicine practice. In spite of the significance of formal complaints there is little guidance within the medical literature to understand why patients complain or how to provide satisfaction to individuals who complain. Practitioners are usually left to their own devices in the style and substance of complaint responses even when working within a defined complaint management system. This article draws on relatively abundant literature in the service marketing field to provide an understanding of dissatisfaction, complaining and complaint handling. Having developed an appropriate theoretical framework the article provides guidance for applying these concepts in dealing with formal complaints. [source]


Anatomics: the intersection of anatomy and bioinformatics

Abstract Computational resources are now using the tissue names of the major model organisms so that tissue-associated data can be archived in and retrieved from databases on the basis of developing and adult anatomy. For this to be done, the set of tissues in that organism (its anatome) has to be organized in a way that is computer-comprehensible. Indeed, such formalization is a necessary part of what is becoming known as systems biology, in which explanations of high-level biological phenomena are not only sought in terms of lower-level events, but are articulated within a computational framework. Lists of tissue names alone, however, turn out to be inadequate for this formalization because tissue organization is essentially hierarchical and thus cannot easily be put into tables, the natural format of relational databases. The solution now adopted is to organize the anatomy of each organism as a hierarchy of tissue names and linking relationships (e.g. the tibia is PART OF the leg, the tibia IS-A bone) within what are known as ontologies. In these, a unique ID is assigned to each tissue and this can be used within, for example, gene-expression databases to link data to tissue organization, and also used to query other data sources (interoperability), while inferences about the anatomy can be made within the ontology on the basis of the relationships. There are now about 15 such anatomical ontologies, many of which are linked to organism databases; these ontologies are now publicly available at the Open Biological Ontologies website (http://obo.sourceforge.net) from where they can be freely downloaded and viewed using standard tools. This review considers how anatomy is formalized within ontologies, together with the problems that have had to be solved for this to be done. It is suggested that the appropriate term for the analysis, computer formulation and use of the anatome is anatomics. [source]


The antiquity of Madagascar's grasslands and the rise of C4 grassy biomes

Abstract Aim, Grasslands and savannas, which make up > 75% of Madagascar's land area, have long been viewed as anthropogenically derived after people settled on the island c. 2 ka. We investigated this hypothesis and an alternative , that the grasslands are an insular example of the post-Miocene spread of C4 grassy biomes world-wide. Location, Madagascar, southern Africa, East Africa. Methods, We compared the number of C4 grass genera in Madagascar with that in southern and south-central African floras. If the grasslands are recent we would expect to find fewer species and genera in Madagascar relative to Africa and for these species and genera to have very wide distribution ranges in Madagascar. Secondly, we searched Madagascan floras for the presence of endemic plant species or genera restricted to grasslands. We also searched for evidence of a grassland specialist fauna with species endemic to Madagascar. Plant and animal species endemic to C4 grassy biomes would not be expected if these are of recent origin. Results, Madagascar has c. 88 C4 grass genera, including six endemic genera. Excluding African genera with only one or two species, Madagascar has 86.6% of southern Africa's and 89.4% of south-central Africa's grass genera. C4 grass species make up c. 4% of the flora of both Madagascar and southern Africa and species : genus ratios are similar (4.3 and 5.1, respectively). Turnover of grasses along geographical gradients follows similar patterns to those in South Africa, with Andropogoneae dominating in mesic biomes and Chlorideae in semi-arid grassy biomes. At least 16 monocot genera have grassland members, many of which are endemic to Madagascar. Woody species in frequently burnt savannas include both Madagascan endemics and African species. A different woody flora, mostly endemic, occurs in less frequently burnt grasslands in the central highlands, filling a similar successional niche to montane C4 grasslands in Africa. Diverse vertebrate and invertebrate lineages have grassland specialists, including many endemic to Madagascar (e.g. termites, ants, lizards, snakes, birds and mammals). Grassland use of the extinct fauna is poorly known but carbon isotope analysis indicates that a hippo, two giant tortoises and one extinct lemur ate C4 or CAM (crassulacean acid metabolism) plants. Main conclusions, The diversity of C4 grass lineages in Madagascar relative to that in Africa, and the presence of plant and animal species endemic to Madagascan grassy biomes, does not fit the view that these grasslands are anthropogenically derived. We suggest that grasslands invaded Madagascar after the late Miocene, part of the world-wide expansion of C4 grassy biomes. Madagascar provides an interesting test case for biogeographical analysis of how these novel biomes assembled, and the sources of the flora and fauna that now occupy them. A necessary part of such an analysis would be to establish the pre-settlement extent of the C4 grassy biomes. Carbon isotope analysis of soil organic matter would be a feasible method for doing this. Multifaceted functional behavior assessment for students with externalizing behavior disorders

Recent federal mandates have increased interest in the use of functional behavior assessment as a necessary part of initial and ongoing work with students with externalizing behavior disorders. A multifaceted approach to functional behavior assessment provides a comprehensive assessment of both behavioral excesses and deficits commonly found in externalizing behavior disorders and also provides for the linkage of assessment information to educational interventions that can be implemented and monitored in educational settings for students with behavior disorders. The authors, using a variety of empirically derived methods, describe specific components of a multifaceted approach to functional behavior assessment in the context of the widely accepted behavioral excess/deficit model for students with externalizing behavior disorders. Empirically derived and validated methodologies (i.e., indirect data collection and analysis, direct observation and recording of behavior, and use of probes to establish baseline rates and measure impacts of intervention) are described. Technology-assisted data collection and analysis using computer-assisted functional behavior assessment interviews and direct observation of behavior using personal digital assistant (PDA) based software are also presented as a means to improve efficiency and reduce time needed to conduct adequate functional behavior assessments. Specific strategies to address academic skill and performance deficits as well as social skills deficiencies in the context of a multifaceted functional behavior assessment are also presented for students with externalizing behavior disorders. © 2002 Wiley Periodicals, Inc. Cervical thymic anomalies,The Texas Children's Hospital experience

Abstract Objectives/Hypothesis: To review the presentation and management of cervical thymic cysts and ectopic thymic tissue at Texas Children's Hospital over the last 25 years. Study Design: Case report and case series using retrospective chart review. Methods: A case report is presented of a recently diagnosed thymic cyst highlighting diagnostic, management, and treatment strategies available for optimizing management of patients with significant mediastinal extension. We then present a retrospective review of cervical thymic anomalies at a tertiary academic medical center over a 25-year span (1983,present). Data extracted include patients' characteristics, clinical presentation, diagnostic workup, surgical management, and postoperative complications. Results: Fifteen patients were found to have a pathological diagnosis of cervical thymic cyst, and 10 patients had a diagnosis of ectopic thymic tissue in the neck. This is the largest case series of cervical thymic anomalies presented in the literature to date. Patients' characteristics, diagnostic techniques, and treatment strategies are discussed. Conclusions: Cervical thymic anomalies are a rare but necessary part of the differential diagnosis of a cervical mass. Computed tomography scan can both narrow the preoperative differential diagnosis and aid in surgical planning for thymic cyst excision. Cues they use: clinicians' endorsement of risk cues in predictions of dangerousness

Clinical predictions of violence are a necessary part of clinical practice despite extensive literature validating the use of actuarial rather than clinical prediction. The current study examined clinicians' use of risk cues in predictions of violence. Clinicians identified several risk cues as significant in clinical assessments of risk, including a history of assaults, hostility, medication noncompliance, paranoid delusions, presence of psychosis, and family problems. DEADLOCK AVOIDANCE FOR FLEXIBLE MANUFACTURING SYSTEMS WITH CHOICES BASED ON DIGRAPH CIRCUIT ANALYSIS

ABSTRACT Due to existence of concurrent part flows and resource sharing in modern automated flexible manufacturing systems (FMS), deadlock is a common problem and its occurrence causes loss of productivity. When a manufacturing system is modeled by a digraph, existence of circuits in such a graph is a necessary condition for deadlock. Our previous work further showed that the knot and order of a circuit is closely related to impending deadlocks , a type of deadlock that is more difficult to detect. In this paper, we extend our previous work on deadlock avoidance for flexible manufacturing systems to allow choices in process flows (a.k.a., flexible part routing). Due to introduction of choices, part flow dynamics become more sophisticated and our previous results are no longer valid. A systematic circuit analysis is performed in this paper. New concepts such as broken circuit, basic circuit, choice circuit and supremal circuit are introduced to reduce significantly the number of circuits thus improving efficiency of our approach. The extended method is highly permissive with the adjusted effective free space calculation to capture more necessary parts flow dynamics, especially when multiple knots exist in the digraph model. The online policy runs in polynomial time once the set of basic circuits of the digraph is computed offline. Simulation results on selected examples are given. [source]
