Natural Changes in Peritoneal Equilibration Test Results in Continuous Ambulatory Peritoneal Dialysis Patients: A Retrospective, Seven Year Cohort Survey

Abstract: We conducted a retrospective, 7 year cohort survey to examine the natural changes in peritoneal equilibration test (PET) results in patients with long-term uneventful continuous ambulatory peritoneal dialysis (CAPD). Thirty-two (17 males, 15 females) patients on CAPD with two or more standard PETs performed more than 6 months apart, in the absence of peritoneal insult, were included. Changes and pattern of PET results were evaluated by the dialysate to plasma ratio of creatinine (D:P-cre), the fourth h dialysate to instilled glucose ratio (D4:Do) and ultrafiltration volume (UF, ml). The subgroups included high (H), high-average (HA), low-average (LA), or low (L) transporters with the dividing ratios (D:P-cre) of >0.81, >0.65 to 0.81, >0.5 to 0.65, and <0.5, respectively. The median D:P-cre significantly decreased (p = 0.04), but neither the D4:Do nor the final median UF significantly decreased. The change in D:P-cre was strongly and inversely correlated with the initial D:P-cre value (r = ,0.68; p < 0.05). A similar relationship was found between the change in the final D4:Do and the initial D4:Do (r = ,0.752; p < 0.01) and between the change in the final UF and the initial UF (r = ,0.875; p < 0.01). No correlation was found between the change in D:P-cre and the age of the patient, the time interval between PETs, monthly dialysate glucose exposure, or underlying diabetes/non-diabetes. The final peritoneal transport pattern was altered with 5 (15.6%) patients remaining in the extreme subgroups (H or L) and, by contrast, 84.4% (27/32) of the patients now in the averaged (HA or LA) groups (p < 0.01, ,2 test). We demonstrated a natural "centralization" migration of PET results after long-term uneventful CAPD, which may help to explain why patients with extreme PET characteristics, that is, H or L, continued to do well on CAPD. A systematic review of topical skin care in aged care facilities

Aim., This systematic review aimed to evaluate the best available evidence regarding the effectiveness of topical skin care interventions for residents of aged care facilities. Introduction., Natural changes to skin, as well as increased predisposition to pressure sores and incontinence, means residents of aged care facilities readily require topical skin care. A range of interventions exist that aim to maintain or improve the integrity of skin of older adults. Methods., Pubmed, Embase, Current Contents, CINAHL and The Cochrane Library databases were searched, as well as Health Technology Assessment websites up to April 2003. Systematic reviews and randomized or non-randomized controlled trials were evaluated for quality and data were independently extracted by two reviewers. Results., The effectiveness of topical skin interventions was variable and dependent on the skin condition being treated. Studies examined the effectiveness of washing products on incontinence irritated skin. Disposable bodyworns may prevent deterioration of skin condition better than non-disposable underpads or bodyworns. Clinisan, a no-rinse cleanser may reduce the incidence of incontinence associated pressure ulcers when compared with soap and water. Conclusion., In general the quality of evidence for interventions to improve or maintain the skin condition in the older person was poor and more research in this area is needed. Relevance to Clinical Practice., Skin care is a major issue for nurses working with older people. On the basis of this review no clear recommendations can be made. This lack of strong evidence for nurses to base effective practice decisions is problematic. However, the ,best' evidence suggests that disposable bodyworns are a good investment in the fight against skin deterioration. No rinse cleansers are to be preferred over soap and the use of the bag bath appears to be a useful practice to reduce the risk of dry skin (a risk factor for breaches in skin integrity). [source]


Seasonal variations in [3H]citalopram platelet binding between healthy controls and violent offenders in Finland

Abstract Monthly binding densities (Bmax) of [3H]citalopram to the platelet serotonin transporter (SERT) was measured longitudinally over 1 year in a control group of 18 healthy Finnish male volunteers. Single platelet samples were also analysed from 33 men who were incarcerated for violent crimes during the same calendar year. A statistically significant seasonal variation in SERT Bmax was observed in both data sets, and bi-monthly floating averages for SERT Bmax were calculated and then fit to an annual sinusoidal curve for both groups. The Bmax for platelet [3H]citalopram binding showed a statistically significant (p,=,0.001) seasonal variance between a winter (January,February) maximum of 1590 fmol/mg protein and a summer (July,August) minimum of 1216 fmol/mg protein for the control group, with an R2 of 70% for the annual sinusoidal curve fit. A statistically significant (p,=,0.007) seasonal variance was also observed between a winter (January,February) maximum of 1980 fmol/mg protein and an autumnal (August,September) minimum of 1234 fmol/mg protein for the violent offenders, again with an R2 of 70% for the annual sinusoidal curve fit. This observation lends additional support to the idea that violent human behavior and impulsivity may be directly linked to values of SERT Bmax, which can be affected by various psychoactive drugs and also varies with the natural change of seasons. Water problems and hydrological research in the Yellow River and the Huai and Hai River basins of China

Abstract This paper deals with hydrological research in regard to the water resources crisis in the vulnerable areas found in the northern part of China. This area includes three main river basins, namely the basins of the Yellow (Huang) River, the Hai River and the Huai River. Several water problems are becoming very severe. Among them, two are the most critical: the Yellow River has been drained dry in the main course of its lower reaches and along its major tributaries, and the groundwater table has rapidly declined in the floodplains of the three rivers' downstream areas. To counter the problems, particularly the critical issues mentioned above, hydrological research, which serves as the basis of water development and management, has been carried out in the last two decades. This paper addresses three basic scientific problems in North China, namely: (a) water consumption and the capacity for saving water; (b) the changes in hydrological processes and water resources caused by natural change and human activities; and (c) the ability to supply water resources and water safety in terms of both quantity and quality within a changing environment. However, opportunities and challenges for ameliorating the problems exist, and new ideas and methodology to solve the problems have been proposed, such as the interface process study on the interactions in the soil,root interface, the plant,atmosphere interface, the soil,atmosphere interface, and the interface of soil water and groundwater. In order to manage water resources in a sustainable manner, the study of water resources' renewal ability as affected by natural change and human activity is addressed from the viewpoint of both water quantity and quality, and their integration. To reduce the vulnerability of water resources in regional water management, a paradigm of sustainable water resources utilization is also proposed, using water,heat balance, water,salt balance, water,sediment balance, and water supply,demand balance. Latent difference score approach to longitudinal trauma research

In this article, the authors introduce a latent difference score (LDS) approach to analyzing longitudinal data in trauma research. The LDS approach accounts for internal sources of change in an outcome variable, including the influence of prior status on subsequent levels of that variable and the tendency for individuals to experience natural change (e.g., a natural decrease in posttraumatic stress disorder [PTSD] symptoms over time). Under traditional model assumptions, the LDSs are maximally reliable and therefore less likely to introduce biases into model testing. The authors illustrate the method using a sample of children who experienced significant burns or other injuries to examine potential influences (i.e., age of child,adolescent at time of trauma and ongoing family strains) on PTSD symptom severity over time. An Initial Analysis of River Discharge and Rainfall in Coastal New South Wales, Australia Using Wavelet Transforms

In many coastal catchments of south eastern New South Wales, Australia, changes in river morphology are a response to human impact superimposed on spatial and temporal patterns of variability in precipitation and discharge. Understanding, and preferably quantifying, spatial and temporal patterns of hydrologic variability are essential to understanding natural changes, and to separate these from artificial changes in river systems. Prediction and management of water resources are also dependent upon this understanding. We assess the variability in precipitation and discharge using the wavelet transform which projects the time series of data into a three dimensional surface of frequency, amplitude and time. The analysis reveals that changes across time often reflect changes in individual seasons and may be linked to changes in particular seasonal atmospheric circulation systems. Strong perturbations in the analysis of one catchment are consistent with documented, geomorphically-effective, flooding sequences. The characteristics of the series in the transformed data reveal interesting differences at certain times and scales which may be a reflection of changes in larger scale atmospheric processes.
