SELF-ESTEEM AND JOB PERFORMANCE: THE MODERATING ROLE OF SELF-ESTEEM CONTINGENCIES

Although theoretical perspectives suggest self-esteem level (i.e., high/low) should have main and moderating effects on job performance, empirical and narrative reviews of the literature suggest such effects are either nonexistent or highly variable. To account for these mixed findings, we hypothesized that self-esteem level should only have main and moderating effects on job performance when one's self-esteem is not contingent upon workplace performance. Using multisource ratings across 2 samples of working adults, we found that the importance of performance to self-esteem (IPSE) moderated the effect of self-esteem level on job performance and moderated the buffering interaction between self-esteem level and role conflict in the prediction of job performance. Our results thus support IPSE as an important moderator of both main and moderating effects of self-esteem level. [source]


Twelve-Step Groups, Attributions of Blame for Personal Sadness, Psychological Well-Being, and the Moderating Role of Gender,

Two studies investigated links among 12-step group participation, gender, attributions of blame for personal sadness, and psychological well-being. Study I used a correlational design to examine these links cross-sectionally among substance abusers who identified alcohol as their primary drug problem. Study 2 used an experimental design to examine prospective links among these variables for substance abusers who were also adult children of alcoholics. Females engaged in more blame than did males, and personal blame was negatively related to psychological well-being in Studies 1 and 2. Most significantly, 12-step group participation was associated with lower personal blame among females but not among males across both studies. These results indicate that 12-step groups can reduce personal blame among females who have substance abuse problems. [source]


Exploring the Moderating Roles of Perceived Person,Job Fit and Person,Organisation Fit on the Relationship between Training Investment and Knowledge Workers' Turnover Intentions

Previous studies have documented inconsistent results in terms of the relationship between knowledge workers' perceived training investment and their turnover intentions. In order to clarify the inconsistencies, the present study extends previous research by exploring the moderating roles of perceived demand,ability (D,A) job fit and person,organisation (P,O) fit. Data were collected from 303 research and development (R&D) engineers from 30 high-technology firms in Taiwan. Hierarchical regression analyses were conducted to test the hypotheses. The results show that perceived D,A fit, P,O fit, and perceived training investment interact jointly to predict knowledge workers' turnover intentions. Specifically, while the main effect of perceived training investment on turnover intentions was negative, under situations of extremely high perceived D,A fit and extremely low P,O fit, the relationship between knowledge workers' perceived training investment and their turnover intentions became positive, and under situations of low perceived D,A fit and high P,O fit, the relationship between knowledge workers' perceived training investment and their turnover intentions remained negative. Theoretical and practical implications are also discussed. Les études antérieures ont donné des résultats contradictoires quant aux relations entre la connaissance que les salariés perçoivent de l'investissement dans la formation et leurs intentions de changer. Dans le but de clarifier ces incohérences, la présente étude prend la suite de recherches antérieures en explorant les rôles modérateurs de la compatibilité perçue entre la compétence et les nécessités au travail (DA) et de la compatibilité entre la personne prise dans sa globalité et les caractéristiques de l'organisation qui l'emploie (PO). Les données ont été collectées auprès de 303 ingénieurs en recherche et développement (R&D) de 30 entreprises de haute technologie à Taiwan. Des analyses de régression hiérarchique ont conduit à tester les hypothèses. Les résultats montrent que la compatibilité D-A, la compatibilité P-O et l'investissement perçu dans la formation interagissent conjointement pour prédire les intentions de changement des employés. Plus spécifiquement, alors que le principal effet de l'investissement perçu dans la formation sur les intentions de changement est négatif, dans des conditions de compatibilité D-A perçue comme étant basse et de compatibilité P-O haute, la relation entre la connaissance que les salariés perçoivent de l'investissement dans la formation et leurs intentions de changement reste négative. Differential development of infants at risk for psychopathology: the moderating role of early maternal responsivity

The development of behaviour problems in infants born with biological risk (low birthweight) and psychosocial risk (psychosocially disadvantaged family) was studied in a sample of 347 children (171 males, 176 females) at the ages of 2, 4:6, and 8 years. In the search for factors that moderate the effects of early risks, the role of early responsive caregiving was examined. Results indicate that infants at psychosocial risk exhibited both more externalizing and internalizing problems across ages than infants not at psychosocial risk, while no overall differences were apparent between normal-and low-birthweight groups. With one exception, no interactions between biological and psychosocial risk factors emerged, suggesting that their simultaneous effect is largely additive. Maternal responsivity was found to moderate the effects of low birthweight on hyperkinetic and internalizing problems as well as to influence the consequences of family disadvantage on total problems. These findings stress the importance of early parenting in the behavioural development of at-risk children. [source]


Alternative Knowledge Strategies, Competitive Environment, and Organizational Performance in Small Manufacturing Firms

This study examines the relationship between knowledge strategy (exploration or exploitation) and performance, and the possible moderating role of external environment variables. Results from a sample of small manufacturing firms indicate that exploration and exploitation are distinct and complementary constructs. The relationship between exploration and performance is linear and positive, while the relationship between exploitation and performance is concave, indicating that there is a point at which focusing on exploitation leads to reduced returns. Additionally, we find that the competitive environment moderates the relationship between exploitation and performance, such that exploitation has a stronger impact on performance in stable and high-tech environments than in dynamic and low-tech environments. Exploration also has a stronger impact on performance in high-tech environments than in low-tech environments. [source]


Efficacy of maintenance treatment with naltrexone for opioid dependence: a meta-analytical review

ABSTRACT Aims To determine the efficacy of naltrexone in reducing illicit opioid use and the potential moderating role of treatment retention. ,Design First, randomized controlled trials (RCTs) comparing the regimens of treatment using the opioid antagonist, naltrexone, with controls were analysed by meta-analysis for treatment effect with regard to a range of outcome criteria. The degree of heterogeneity was also determined. The moderating effect of other interventions during naltrexone maintenance was then estimated, particularly with regard to their effect on treatment retention. Participants Fifteen studies involving 1071 patients were found. Measurements All available outcomes were analysed in 10 studies of naltrexone versus control (seven placebo) and six studies of randomized psychosocial/psychopharmacological interventions. Findings Significant heterogeneity was found in the efficacy of naltrexone. Level of retention in treatment was found to be a moderator, explaining most of the heterogeneity found. Overall, naltrexone was significantly better than control conditions in reducing the number of opioid-positive urines. This effect was only present in the high retention subgroup for differences in retention. Contingency management (CM) increased retention and naltrexone use, resulting in a reduced number of opioid-positive urines. Conclusion Retention is important to the effect of naltrexone in treating opioid dependence. Contingency management is a promising method of increasing retention. [source]


Avoidance motivation, risk perception and emotional processing

The present studies examined the moderating role of state motivation on the associations between trait avoidance motivation, risk perception and emotional processing. In Studies 1 and 2, avoidance or approach states were evoked in participants who then completed a risk perception task and a trait avoidance motivation measure. Both studies showed that trait avoidance only correlated with risk perceptions among individuals in approach state. In Study 3, emotional interpretation was measured. State and trait avoidance motivation did not interact in predicting emotional interpretation. The results showed that the effect of state motivation can explain the low correlations found between trait avoidance and risk perceptions, and suggested that the avoidance system may operate on an on,off principle rather than synergistically. Adolescent personality, problem behaviour and the quality of the parent,adolescent relationship,

Abstract The relationship between adolescent personality and problem behaviour has been well established. However, relatively little attention has been given to the role of the social environment in the association between adolescent personality and problem behaviour. We tested the mediating and moderating role of the quality of the parent,adolescent relationship in the associations between adolescents' personality traits and problem behaviour. The sample consisted of 140 adolescents (11 to 18 years of age) and both their parents. Results supported a mediating role of the father/mother,adolescent relationship in the associations between Agreeableness, Emotional Stability, and Conscientiousness and externalizing problem behaviour. The father/mother,adolescent relationships did not mediate the associations between personality traits and internalizing problem behaviour. We also found support for a moderating role of the father/mother,adolescent relationships in the association between Emotional Stability and both externalizing and internalizing problem behaviours. On implicit,explicit consistency: the moderating role of individual differences in awareness and adjustment

A moderated process model is presented that attempts to explain the consistency between implicit and explicit indicators as a function of awareness, i.e. the degree to which persons become aware of their implicit attitude, and adjustment, i.e. the degree to which they adjust for the explicit response. In two experiments on attitudes of West Germans toward East Germans and Turks, a number of dispositional moderators pertaining to awareness and adjustment were tested. Concerning moderators affecting awareness, no reliable first-order effects were found for Private Self-Consciousness or Attitudinal Self-Knowledge. However, Attitude Importance generated the expected effect. Concerning moderators influencing adjustment, consistent effects were obtained for Motivation to Control Prejudiced Reactions. Social Desirability and Self-Monitoring did not moderate the implicit,explicit relationship in the expected direction. Some evidence was found for a second-order moderator effect between awareness and adjustment, suggesting that adjustment effects may be more pronounced under conditions of high awareness. Stereotype threat: the moderating role of Locus of Control beliefs

The goal of the present study was to test the moderating role of Locus of Control beliefs on performance deficits typically associated with stereotype threat. The results from Experiment 1 were consistent with predictions. First, consistent with the Stereotype Threat model, participants showed a decrease in performance when the task was perceived as a potential test of the in-group's negative stereotype (lacking logical mathematical intelligence in the case of women and lacking social intelligence in the case of men). Most important, participants' Locus of Control beliefs were found to moderate participants' vulnerability to stereotype threat: individuals with an Internal Locus of Control, although generally performing better, showed a sharper decrease in the stereotype threat condition compared to individuals with External Locus of Control beliefs. Experiment 2 replicated the results from Experiment 1. Findings are discussed in relation to the psychological characteristics of Internal Locus of Control that may render individuals more vulnerable to the negative effects of stereotype threat. When is a minority a minority?

Active versus passive minority advocacy, social influence
Some conceptions of minority influence have stressed the impact of the mere existence of an unpopular, deviant position. Others (e.g. Moscovici, 1980) have emphasized the active opposition of a committed minority to a powerful majority. An active advocate is defined as one that is aware of the level of support for his/her position, expresses his/her position openly, and whose outcomes may depend on others' agreement/disagreement. In the present study, the potential moderating role of an advocates' active/passive status on opinion change was examined. When the issue was highly relevant to the target of influence, all that mattered was the quality of the source's arguments (i.e. majority, minority, active source,=,passive source). When the issue was not highly relevant to the target, though, active and passive sources had different impact: (1) active sources prompted attention to argument quality (for minorities) and heuristic compliance (for majorities); (2) passive sources prompted insensitivity to both the popularity of the position and to the quality of the source's arguments. Perceived Risk and Efficacy Beliefs as Motivators of Change

Use of the Risk Perception Attitude (RPA) Framework to Understand Health Behaviors
Evidence of a direct correlation between risk perception and self-protective behavior is ambiguous at best. Witte's (1992, 1994) extended parallel process model (EPPM) explains many contradictory findings by pointing out the moderating role played by efficacy beliefs. Working from the EPPM, this article introduces the risk perception attitude (RPA) framework that categorizes individuals into one of four attitudinal groups: responsive (high risk, high efficacy), avoidance (high risk, low efficacy), proactive (low risk, high efficacy), and indifference (low risk, low efficacy). We conducted two studies to test our hypotheses that these groups differ in their self-protective motivation, intention to seek information, behavioral intention, knowledge acquisition, and time spent seeking information. Results, though not entirely consistent, suggest that, when risk and efficacy are made salient (Study 1), people's risk perception guides most of their subsequent actions, but in a natural context (Study 2), risk and efficacy jointly affect subsequent action. Worker flexibility and its perceived contribution to performance: The moderating role of task characteristics

This study examined the relationship between worker flexibility in team-based work and its perceived contribution to efficiency, work quality, and innovation, and the moderating role of task autonomy, skill utilization, and task monotony. Four-hundred ninety-four employees from 113 teams in 15 organizations completed and returned questionnaires. Skill utilization proved to be positively related to perceived contribution of flexibility to efficiency, work quality, and innovation. Furthermore, skill utilization strengthened the positive relationships between worker flexibility and its perceived contribution to efficiency and work quality and weakened the negative relationship between worker flexibility and its perceived impact on innovation. Task monotony was negatively related to the perceived quality benefits of being flexible. In addition, it weakened the positive relationship between worker flexibility and its perceived contribution to both efficiency and quality and strengthened the negative relationship with the perceived impact of flexibility on innovation. Task autonomy was positively related to the perceived contribution of flexibility to innovation and weakened the negative relationship between worker flexibility and the perceived innovation benefits of being flexible. The authors conclude that skill utilization and lack of monotony are important issues in relation to worker flexibility on all performance objectives that were considered, whereas task autonomy seems especially relevant when innovation is a key issue. © 2007 Wiley Periodicals, Inc. Shy and soft-spoken: shyness, pragmatic language, and socio-emotional adjustment in early childhood

Abstract The goal of this study was to examine the moderating role of pragmatic language in the relations between shyness and indices of socio-emotional adjustment in an unselected sample of early elementary school children. In particular, we sought to explore whether pragmatic language played a protective role for shy children. Participants were n=167 children aged 6,7 years, recruited from grade 1 classrooms in public elementary schools. Multi-source assessment was used to measure child shyness, pragmatic language ability, and indices of social and emotional difficulties at school. Results indicated several significant shyness-by-pragmatic-language interactions in the prediction of outcome variables. The pattern of results indicated a clear buffering effect of pragmatic language, with associations between shyness and adjustment difficulties exacerbated at lower levels of pragmatic language, and attenuated at higher levels. Results are discussed in terms of the specific positive benefits of pragmatic language for shy children and the implications for ameliorative intervention programs. Interdependence as a Moderator of the Relationship Between Competitiveness and Objective Sales Performance

In this study, we investigated the moderating role of interdependence (a personality trait that measures the extent to which individuals desire working in a group-based, cooperative setting) on the relationship between competitiveness and one-year objective sales performance. On the basis of data from 133 sales representatives, results indicated that: (a) competitiveness was related to objective sales performance; and (b) interdependence moderated the relationship between competitiveness and objective sales performance such that competitiveness predicted objective sales performance more strongly for those who scored low in interdependence versus those who scored high in interdependence. Implications for sales selection and reward systems and directions for future research are considered. [source]


Ethnicity-Related Stressors and Mental Health in Latino Americans: The Moderating Role of Parental Racial Socialization

The current study examined the moderating role of proactive and reactive parental racial socialization on the relationship between Latino American college students' mental health and 3 ethnicity-related stressors: perceived racial discrimination, stereotype confirmation concern, and own-group conformity pressure. In hierarchical regression, greater stereotype confirmation concern predicted more mental distress in 105 Latino American college students. Both proactive and reactive racial socialization moderated the effect of own-group conformity pressure on anxiety and loss of behavioral/emotional control. Specifically, greater proactive and reactive racial socialization predicted more mental distress under greater own-group conformity pressure. Findings are interpreted in a college environment of ethnic diversity and a context of acculturative stress. [source]


Posttraumatic growth in accident survivors: openness and optimism as predictors of its constructive or illusory sides

Abstract Posttraumatic growth (PTG), the phenomenon of self-reported positive outcomes of trauma, is assumed to consist of two sides: a constructive and an illusory side. This study investigates the relationship between PTG and its possible illusory and constructive predictors, as well as the moderating role of posttraumatic stress disorder (PTSD) severity. One-hundred two motor vehicle accident (MVA) survivors with full, subsyndromal, and without PTSD were assessed by multiple psychometric measures targeting PTSD severity, posttraumatic growth, optimism, and openness to experience. Hierarchical regression analysis yielded differential interaction effects between PTSD severity and optimism, as well as openness facets pointing to the moderating role of PTSD severity in the prediction of an illusory and a constructive factor in PTG. © 2008 Wiley Periodicals, Inc. Third-Person Effects and the Environment: Social Distance, Social Desirability, and Presumed Behavior

Previous research has documented third-person effects (persons presuming that others will be more susceptible to media effects than they themselves are) and explored moderators such as social desirability (the effect reverses when the media effects are undesirable) and social distance (the effect increases as the social distance from the self increases). In a study of environmental news coverage, the authors observed the general third-person effect and the moderating role of social desirability; however, they also found that social distance affected presumed influence in complex ways reflecting varying perceptions of issue relevance for the comparison groups. A new variable, presumed behavior (the presumed effect of media coverage on others' behavior), was found to be independent of presumed influence and to offer improved prediction of perceivers' behavioral intentions. Unemployment and aggression: the moderating role of self-awareness on the effect of unemployment on aggression

Abstract In February 2005, the unemployment rate in Germany surpassed the 10% mark. Derived from the revised version of the frustration,aggression hypothesis [Berkowitz, 1989], the present studies investigated the association between unemployment and aggression, as well as the moderating role of the self in this context. Because previous research on unemployment and aggression has been plagued by the cause-and-effect issue, the present research employed both an experimental and a correlational field approach. Three studies revealed that participants who expected to be unemployed after their degree (Studies 1 and 3), or who were currently unemployed (Study 2), reported stronger aggressive inclinations than participants who expected not to be unemployed or who were not unemployed at the time of data collection. However, this aggression-eliciting effect of expected or real unemployment only occurred for participants with low self-awareness. Visual aesthetics in store environment and its moderating role on consumer intention

The paper goal is to answer the following questions: What are the factors of centrality of visual aesthetic design (CVSA) in store environment? What factor explains better the construct? Is there a relationship between CVSA and consumers' intentions, such as loyalty, satisfaction, minutes, item, and $ spent inside store? What is the moderating role of store visual aesthetic design on consumer intentions? The research design was a 2,×,2 design, between subjects, which we manipulated store arousal (high vs. low) and CVSA (high vs. low) in two studies. The findings supported the theory that there are three CVSA dimensions; that there is a positive relationship between CVSA and consumer satisfaction, loyalty, items bought, minutes visiting the store, and $ spent; and that high (vs. low) CVSA consumers were more discriminating in their intentions, when the environment had a high arousal. The original value is to support the notion that consumers also evaluate, beyond products, visual aesthetic components in retail and that it plays a moderating role on consumer intention. The mediating role of overall fairness and the moderating role of trust certainty in justice,criteria relationships: the formation and use of fairness heuristics in the workplace

Theory suggests that perceptions of overall fairness play an important role in the justice judgment process, yet overall fairness is insufficiently studied. We derived hypotheses from fairness heuristic theory, which proposes that perceptions of overall fairness are influenced by different types of justice, are more proximal predictors of responses than specific justice types, and are used to infer trust when trust certainty is low. Results from Study 1 (N,=,1340) showed that employees' perceptions of overall fairness in relation to a senior management team mediated the relationships between specific types of justice and employee outcomes (e.g., affective commitment). In Study 2 (N,=,881), these mediated effects were replicated and trust certainty moderated the effect of overall fairness on trust as hypothesized. Study 2 also showed that, relative to procedural and informational justice, distributive and interpersonal justice had stronger effects on overall fairness. To explore how the organizational context may have influenced these findings, we performed qualitative analyses in Study 3 (N,=,268). Results suggested that, consistent with the quantitative findings from Study 2, some types of justice were more salient than others. Is it pay levels or pay raises that matter to fairness and turnover?

Two studies examined the relationship between actual pay and distributive and procedural justice, and the extent to which these perceptions were related to two important pay satisfaction dimensions, pay level and pay raise, and ultimately, impacted turnover. For each study the measures of pay and justice variables were obtained on a cross-sectional basis, while the measure of turnover was necessarily lagged. Results showed that distributive justice mediated the relationship between pay and both pay level satisfaction and pay raise satisfaction. Furthermore, distributive justice was a stronger predictor of pay level satisfaction; whereas procedural justice was a stronger predictor of pay raise satisfaction. Procedural justice also played a moderating role in Study 2. The study also showed that only pay raise satisfaction was significantly and negatively related to turnover in Study 1, and to turnover via turnover intention in Study 2. Job stress, incivility, and counterproductive work behavior (CWB): the moderating role of negative affectivity,

The current study was designed to replicate findings from previous research regarding the relationships between job stressors, negative affectivity, and counterproductive work behavior (CWB) using peer-reported data and to assess the effects of workplace incivility on employee satisfaction and CWB. Results indicate that incivility, organizational constraints, and interpersonal conflict were negatively related to job satisfaction and positively related to CWB. Support was also found for the role of negative affectivity as a moderator of the relationship between job stressors and CWB, although only one significant moderator was found using peer-reported CWB. In general, the relationships between job stressors and CWB were stronger for individuals high in negative affectivity than for individuals low in negative affectivity. Transformational leadership and organizational commitment: mediating role of psychological empowerment and moderating role of structural distance

Using a sample of 520 staff nurses employed by a large public hospital in Singapore, we examined whether psychological empowerment mediated the effects of transformational leadership on followers' organizational commitment. We also examined how structural distance (direct and indirect leadership) between leaders and followers moderated the relationship between transformational leadership and organizational commitment. Results from HLM analyses showed that psychological empowerment mediated the relationship between transformational leadership and organizational commitment. Similarly, structural distance between the leader and follower moderated the relationship between transformational leadership and organizational commitment. Naltrexone Selectively Elevates GABAergic Neuroactive Steroid Levels in Heavy Drinkers With the ASP40 Allele of the OPRM1 Gene: A Pilot Investigation

Background:, Preclinical studies have implicated GABAergic neurosteroids in behavioral responses to alcohol. Naltrexone is thought to blunt the reinforcing effects of alcohol, and a few studies have found that the effects of naltrexone are moderated by the Asn40Asp polymorphisms of the OPRM1 gene. The present study seeks to integrate these lines of research by testing (i) the moderating role of the functional Asn40Asp polymorphism of the OPRM1 gene on naltrexone-induced alternations in GABAergic neurosteroid levels, namely (3,,5,)-3-hydroxypregnan-20-one (allopregnanolone, ALLO); and (ii) the combined effects of naltrexone or genotype with alcohol administration on neurosteroid levels in a sample of at-risk drinkers. Methods:, Participants were 32 (9 females) nontreatment-seeking heavy drinkers who completed a placebo-controlled laboratory study of naltrexone (50 mg/d for 3 days) and provided complete sets of serum samples for ALLO assays before and after alcohol administration under both naltrexone and placebo conditions. Results:, Naltrexone treatment raised ALLO levels among carriers of the Asp40 allele, but not homozygotes for the Asn40 allele. The Asn40Asp polymorphism did not moderate effects of naltrexone on cortisol levels. Ethanol infusion modestly reduced ALLO levels in all subjects, independent of genotype or naltrexone exposure. Conclusions:, Naltrexone increased ALLO levels among individuals with the Asn40Asp allele suggesting a potential neurosteroid contribution to the neuropharmacological effects of naltrexone among Asp40 carriers. The Role of Socioregional Factors in Moderating Genetic Influences on Early Adolescent Behavior Problems and Alcohol Use

Background:, Twin and family studies have demonstrated that adolescent alcohol use and behavior problems are influenced by a combination of genetic and environmental factors. More recently, studies have begun to investigate how genetic and environmental influences may interact, with efforts underway to identify specific environmental variables that moderate the expression of genetic predispositions. Previously, we have reported that community-level factors, including urban/rural residency, migration rates, and prevalence of young adults, moderate the importance of genetic effects on alcohol use in late adolescence (ages 16 to 18). Here, we extend these findings to test for moderating effects of these socioregional factors on alcohol use and behavior problems assessed in a younger sample of adolescent Finnish twins. Methods:, Using data from the population-based Finnish twin study, FinnTwin12, biometric twin models were fit to data on >1,400 twin pairs to examine the significance of each of the socioregional moderating variables on alcohol use measured at age 14, and behavior problems, measured at age 12. Results:, We find no evidence of a moderating role of these socioregional variables on alcohol use; however, there was significant moderation of genetic influences on behavior problems, with effects limited to girls. Genetic influences assumed greater importance in urban settings, communities with greater migration, and communities with a higher percentage of slightly older adolescents. Conclusions:, The moderation effects observed on behavior problems in early adolescence paralleled the effects found on alcohol use late in adolescence in an independent sample, providing further support for the idea that behavior problems may represent an earlier manifestation of the predisposition to subsequent alcohol problems. Our findings also support the growing body of evidence suggesting that females may be more susceptible to a variety of environmental influences than males. [source]


Social Exchange in Work Settings: Content, Process, and Mixed Models

abstract Social exchange theory has provided the dominant basis for understanding exchange relationships in organizational settings. Despite its predominance within the management field, there are a number of unaddressed issues. This special issue seeks to further social exchange research in work settings. We differentiate social from economic exchange and highlight the moderating role of cultural and individual differences in explaining the outcomes associated with social exchange relationships. We introduce the ideas of content, process, and mixed models of exchange to reflect the different emphases given to the amount and type of resources exchanged, the quality of the relationship, and a combination of both. The five papers in this special issue illustrate these models. We discuss the applicability of social exchange theory across cultural contexts and present suggestions for future research. [source]

Response to Cardiac Resynchronization Therapy: Is It Time to Expand the Criteria?

Background: Cardiac resynchronization therapy (CRT) is a promising treatment for a subgroup of patients with advanced congestive heart failure and a prolonged QRS interval. Despite the majority of patients benefiting from CRT, 10,40% of patients do not respond to this treatment and are labeled as nonresponders. Given that there is a lack of consensus on how to define response to CRT, the purpose of this viewpoint is to discuss currently used definitions and their shortcomings, and to provide recommendations as to how an expansion of the criteria for CRT response may be useful to clinicians. Methods and Results: Analysis of the literature and case reports indicates that the majority of established measures of CRT response, including New York Heart Association functional class and echocardiographic, hemodynamic, and neurohormonal parameters, are poor associates of patient-reported symptoms and quality of life. Moreover, the potential moderating role of psychological factors in determining health outcomes after CRT has largely been neglected. Conclusions: It is recommended to routinely assess health status after CRT with a disease-specific questionnaire in standard clinical practice and to examine its determinants, including psychological factors such as personality traits and depression. This may lead to improved (secondary) treatment and prognosis in CHF patients treated with CRT. [source]


The Relationship between Empathy and Estimates of Observed Pain

ABSTRACT Objective., Recent research suggests that higher scores on measures of empathy correlate with a stronger response to observed pain, as well as higher estimates of pain intensity. Little work to date has examined the impact of empathy on evaluations of different levels of expressed pain, or how empathy may alter the accuracy of interpreting these painful facial expressions. This study examines the role of empathy in rating the intensity of facial expressions of pain, and the accuracy of these ratings relative to self-reported pain. The potential mediating role of available pain cues or the moderating role of gender on this relationship are also examined. Methods., Undergraduate participants (observers, N = 130) were shown video clips of facial expressions of individuals from a cold presser pain task (senders), and then asked to estimate that pain experience. This estimate was compared with the video sender's actual pain ratings. Results., Higher empathy was associated with an overall increase in estimates of senders' pain, which was not mediated by video subject or participant gender or the duration of painful facial expressions. Further analyses revealed that high empathy was associated with greater accuracy in inferring pain on only one of three inferential accuracy indices. Conclusions., While observers with greater empathy may infer greater pain in senders, resulting in a smaller underestimation bias overall, they are not necessarily more accurate in estimating pain on any given stimuli. The importance of these potential differences in perceived pain for clinical assessment and interpersonal relationships are discussed. [source]
