Sports Medicine and School Nurses: A Growing Need for Further Education and Appropriate Resources

The use of exercise as a prerequisite for conditioning and proper treatment of injuries was first documented in early Greek civilization with the establishment of the Olympics. Today, sports by their very nature invite injury. In 2000, 2.5 million students participated in varsity sports with 750,000 injuries recorded. These numbers do not account for sports activities outside school or leisure activities. Another area of potential injury is physical education class. These classes are large with limited supervision and encompass students of varying age and abilities. Nurses do not have an extensive knowledge of injury prevention or assessment in their basic nursing education. School nurses, as a subspecialty within nursing, are expected to keep up with the requirements of the adolescent and pediatric populations as well basic nursing skills. Due to work schedules and limited resources for continuing education, school nurses are not afforded much time or benefits to attend classes that would teach them skills needed to assess athletic-type injuries. School nurses need printed resources specific to their setting to help fill this void. Recognizing this need, Sports Medicine Techniques for the School-Based Nurse is a manual in process that will help fill this void. Being developed specifically for school nurses, the manual will provide information on prevention, evaluation, and management of athletic-type injuries commonly seen in the school nurse's office. Understanding Surge Capacity: Essential Elements

As economic forces have reduced immediately available resources, the need to surge to meet patient care needs that exceed expectations has become an increasing challenge to the health care community. The potential patient care needs projected by pandemic influenza and bioterrorism catapulted medical surge to a critical capability in the list of national priorities, making it front-page news. Proposals to improve surge capacity are abundant; however, surge capacity is poorly defined and there is little evidence-based comprehensive planning. There are no validated measures of effectiveness to assess the efficacy of interventions. Before implementing programs and processes to manage surge capacity, it is imperative to validate assumptions and define the underlying components of surge. The functional components of health care and what is needed to rapidly increase capacity must be identified by all involved. Appropriate resources must be put into place to support planning factors. Using well-grounded scientific principles, the health care community can develop comprehensive programs to prioritize activities and link the necessary resources. Building seamless surge capacity will minimize loss and optimize outcomes regardless of the degree to which patient care needs exceed capability. [source]


Habitat utilisation during staging affects body condition in a long distance migrant, Branta bernicla hrota: potential impacts on fitness?

There is considerable evidence to suggest that an animal's ability to access the appropriate resources at one time of year may profoundly restrict its performance at another. For migrants, wintering and breeding periods are often connected by refuelling or staging periods, critical (particularly for females) in attaining the body reserves required to ensure successful breeding. However in many instances there are differences in the extent to which different individuals gain access to the highest quality resources. Here we demonstrate how body condition in brent geese Branta bernicla hrota, during spring staging is related to differences in marine and terrestrial habitat utilisation (inferred from stable isotope analysis). Female birds with high fat scores feed to a greater extent on marine resources. Body mass and condition are also higher in individuals utilising more marine resources. Given that body mass at spring staging is correlated with reproductive success, the extent of marine habitat maybe critical to this population. Combining this with data from previous studies of dark-bellied brent geese Branta bernicla bernicla, we predict the potential impacts of spring staging resource utilisation on future breeding success. Although staging is of short duration compared to the other components of annual cycles of migratory species, our results suggest that the quality of staging grounds may be vitally important to population processes. This article investigates how the existence of non-cooperative social groups that appropriate resources either peacefully or through contest affects economic growth when property rights are unenforceable. For symmetric groups it shows that economic growth is generated only in peaceful societies. For the case of asymmetric groups rebel-equilibria are investigated in which a large majority behaves peacefully although challenged by an aggressive minority. The article explains how conflict intensity and the rate of economic growth depend on social fractionalisation, general productivity of the economy, power of social elites and the ease at which resources are appropriated. INTEGRATION OF PROSPECTIVE QUALITY OF LIFE AND NUTRITIONAL ASSESSMENT AS ROUTINE COMPONENTS OF MULTIDISCIPLINARY CARE OF PATIENTS WITH HEAD AND NECK CANCER

Background: Quality of life (QOL) and nutritional assessment of patients with head and neck cancer can provide additional information about the effects of treatment beyond the standard measures of disease control and survival. Integrating a prospective evaluation program into a multidisciplinary service may ensure that a more holistic model of care is developed. Methods: Prospective evaluation of QOL and nutrition before and after treatment for head and neck cancer was implemented in 2001. All patients enrolled in the program were treated with curative intent. Patients completed the European Organisation for Research and Treatment of Cancer Core QOL Questionnaire and Head and Neck Specific Module before treatment and at 3, 6 and 12 months after completion of therapy. In conjunction, patients underwent nutritional assessment by body mass index, biochemical parameters and the patient-generated subjective global assessment tool. Results: Among 288 patients who consented to participate in this study, 134 patients completed the QOL assessment criteria and were eligible for evaluation. Examples of QOL and nutritional data for patients with cancers of the oral cavity, oropharynx, nasopharynx, larynx, hypopharynx, parotid gland and paranasal sinus, and also unknown primary cancers are given. Implementation of this prospective assessment program required appropriate resources and was hampered by time constraints, logistics with blood tests and patient compliance. Conclusions: Despite difficulties with implementation, the information concerning QOL and nutritional status obtained in this study provided an appreciation of the long-term functional effects of treatment for head and neck cancer. Prospective QOL assessment and nutritional evaluation should become integral components of the care of patients with cancers of the head and neck. [source]

This review focuses on clinical bacteriology and by and large does not cover the detection of fungi, viruses or parasites. It discusses two completely different but complementary approaches that may either supplement or replace classic culture-based bacteriology. The latter view may appear provocative in the light of the actual market penetration of molecular genetic testing in clinical bacteriology. Despite its elegance, high specificity and sensitivity, molecular genetic diagnostics has not yet reached the majority of clinical laboratories. The reasons for this are manifold: Many microbiologists and medical technologists are more familiar with classical microbiological methods than with molecular biology techniques. Culture-based methods still represent the work horse of everyday routine. The number of available FDA-approved molecular genetic tests is limited and external quality control is still under development. Finally, it appears difficult to incorporate genetic testing in the routine laboratory setting due to the limited number of samples received or the lack of appropriate resources. However, financial and time constraints, particularly in hospitals as a consequence of budget cuts and reduced length of stay, lead to a demand for significantly shorter turnaround times that cannot be met by culture-dependent diagnosis. As a consequence, smaller laboratories that do not have the technical and personal equipment required for molecular genetic amplification techniques may adopt alternative methods such as fluorescence in situ hybridization (FISH) that combines easy-to-perform molecular hybridization with microscopy, a technique familiar to every microbiologist. FISH is hence one of the technologies presented here. For large hospital or reference laboratories with a high sample volume requiring massive parallel high-throughput testing we discuss matrix-assisted laser desorption ionization time-of-flight mass spectrometry (MALDI-TOF) of nucleic acids, a technology that has evolved from the post-genome sequencing era, for high-throughput sequence variation analysis (1, 2). What stresses remote area nurses?

Abstract Objective:,Review and synthesise the literature identifying the stresses experienced by remote area nurses (RANs). Identify interventions implemented to address identified stresses. Explore the use of the job demands,resources (JD-R) model. Methods:,A comprehensive literature review was conducted using the meta-databases Ovid and Informit. Setting:,Remote Australian primary health care centres. Results:,The reported demands experienced by RANs can be grouped into four themes: (i) the remote context; (ii) workload and extended scope of practice; (iii) poor management; and (iv) violence in the workplace and community. In this high-demand, low-resource context, the JD-R model of occupational stress is particularly pertinent to examining occupational stress among RANs. The demands on RANs, such as the isolated geographical context, are immutable. However, there are key areas where resources can be enhanced to better meet the high level of need. These are: (i) adequate and appropriate education, training and orientation; (ii) appropriate funding of remote health services; and (iii) improved management practices and systems. Conclusion:,There is a lack of empirical evidence relating to stresses experienced by RANs. The literature identifies some of the stresses experienced by RANs as unique to the remote context, while some are related to high demands coupled with a deficit of appropriate resources. Use of models, such as the JD-R model of occupational stress, might assist in identifying key areas where resources can be enhanced to better meet the high level of need and reduce RANs' levels of stress.
