Directed Attention in Normal and High-Risk Pregnancy

Objective:, To compare the ability to direct attention in women having a high-risk pregnancy with those having an uncomplicated pregnancy. Design:, Descriptive comparative. Setting:, A tertiary-care hospital. Participants:, Women in the 3rd trimester of pregnancy receiving care from perinatologists were recruited for this study and comprised the high-risk group (n= 67). Women in their 3rd trimester of pregnancy with uncomplicated pregnancies and enrolled in prenatal classes were the comparison group (n= 57). Main outcome measures:, Objective measures of directed attention included digit span forward, digit span backward, Trailmaking A, and Trailmaking B. Subjective measures included the Attentional Function Index and Mental Effort in Tasks. Results:, Women having a complicated pregnancy had significantly more difficulty directing attention on all measures than women having normal pregnancies. When all covariates were considered, women having a high-risk pregnancy had significantly more difficulty directing attention as measured by Trailmaking A, Trailmaking B, and Mental Effort in Tasks. Conclusions:, Women having high-risk pregnancies may have more difficulty with activities that require directed attention than women having normal pregnancies. Learning new information and skills, problem solving, and planning may require additional effort for women having complicated pregnancies. JOGNN, 35, 241-249; 2006. THE ROLE OF NONCOGNITIVE SKILLS IN EXPLAINING COGNITIVE TEST SCORES

This article examines whether noncognitive skills,measured both by personality traits and by economic preference parameters,influence cognitive tests' performance. The basic idea is that noncognitive skills might affect the effort people put into a test to obtain good results. We experimentally varied the rewards for questions in a cognitive test to measure to what extent people are sensitive to financial incentives. To distinguish increased mental effort from extra time investments, we also varied the questions' time constraints. Subjects with favorable personality traits such as high performance motivation and an internal locus of control perform relatively well in the absence of rewards, consistent with a model in which trying as hard as you can is the best strategy. In contrast, favorable economic preference parameters (low discount rate, low risk aversion) are associated with increases in time investments when incentives are introduced, consistent with a rational economic model in which people only invest when there are monetary returns. The main conclusion is that individual behavior at cognitive tests depends on noncognitive skills. (JEL J20, J24) [source]


Managing the Unique Size-related Issues of Pediatric Resuscitation: Reducing Cognitive Load with Resuscitation Aids

Abstract A resuscitation is a complicated event that requires for its optimal outcome the effective completion of a distinct series of actions, some simple, some complex, most occurring simultaneously or in close proximity. In children, these actions are determined not only by the clinical situation, but also by a series of age and size factors particular to each child. Different tasks require different levels of cognitive load, or mental effort. Cognitive load describes the mental burden experienced by the decision maker and will be higher when the task is less familiar or more demanding. In the setting of resuscitation, it refers to the cumulative demands of patient assessment, the ongoing decisions for each of the various steps, and decisions around procedural intervention (e.g., intubation). In children, the level of task complexity and, hence, cognitive load is increased by the unique component of variability of pediatric age and size, introducing logistical factors, many of which involve computations. The purpose of this paper is to examine the effects of age/size-related variables on the pediatric resuscitative process and to explore how these effects can be mitigated using resuscitation aids. The concept of cognitive load and its relation to performance in resuscitation is introduced and is used to demonstrate the effect of the various aids in the pediatric resuscitative process. Cerebral Metabolism is Influenced by Muscle Ischaemia During Exercise in Humans

Maximal exercise reduces the cerebral metabolic ratio (O2/(glucose + 1/2lactate)) to < 4 from a resting value close to 6, and only part of this decrease is explained by the ,intent' to exercise. This study evaluated whether sensory stimulation of brain by muscle ischaemia would reduce the cerebral metabolic ratio. In 10 healthy human subjects the cerebral arterial-venous differences (a-v differences) for O2, glucose and lactate were assessed before, during and after three bouts of 10 min cycling with equal workload: (1) control exercise at light intensity, (2) exercise that elicited a high rating of perceived exertion due to a 100 mmHg thigh cuff, and (3) exercise followed by 5 min of post-exercise muscle ischaemia that increased blood pressure by , 20%. Control exercise did not significantly affect the a-v differences. However, during the recovery from exercise with thigh cuffs the cerebral metabolic ratio decreased from a resting value of 5.4 ± 0.2 to 4.0 ± 0.4 (mean ±s.e.m.. P < 0.05) as a discrete lactate efflux from the brain at rest shifted to a slight uptake. Also, following post-exercise muscle ischaemia, the cerebral metabolic ratio decreased to 4.5 ± 0.3 (P < 0.05). The results support the hypothesis that during exercise, cerebral metabolism is influenced both by the mental effort to exercise and by sensory input from skeletal muscles. [source]


Estimating nurses' workload using the Diagnosis Procedure Combination in Japan

Aim:, To examine the methods used to estimate nurse staffing levels in acute care settings with Diagnosis Related Groups, which in Japan are called the Diagnosis Procedure Combination (DPC). Methods:, For estimating staffing requirements, the study used four DPC groups: (1) acute or recurrent myocardial infarction (AMI) with stenting, (2) angina pectoris with coronary artery bypass grafting (CABG), (3) sub-arachnoid haemorrhage (SAH) with clipping surgery, and (4) cerebral infarction with carotid endarterectomy (CEA). Registered nurses with more than 3-year nursing experience in nine university hospitals in the Tokyo metropolitan area completed self-report questionnaires in order to obtain nursing care time and care intensity per each DPC. The concordance rate was measured by Kendall's coefficient of concordance. The relationship between the care time and the care intensity was examined by a time series graph per DPC. Care intensity consisted of professional judgement, mental effort for helping patients, professional skill, physical effort for providing activities of daily living support, and nurse stress, based on the Hsiao and colleagues' model of resource-based relative value scale. Results:, Twenty-five nurses in nine university hospitals answered for a hypothetical typical patient with AMI and with CABG, and 28 nurses in nine university hospitals answered for a hypothetical typical patient with SAH and with CEA. Kendall's coefficient of concordance was 0.896 for AMI, 0.855 for CABG, 0.848 for SAH, 0.854 for CEA. The time series data of the care time and the care intensity items showed different patterns for each DPC. Conclusion:, The DPC for cardiovascular and cerebral surgical procedures can be used for estimating nurses' workload. [source]


User preference: A measure of query-term quality

The goal of this research is to understand what characteristics, if any, lead users engaged in interactive information seeking to prefer certain sets of query terms. Underlying this work is the assumption that query terms that information seekers prefer induce a kind of cognitive efficiency: They require less mental effort to process and therefore reduce the energy required in the interactive information-seeking process. Conceptually, this work applies insights from linguistics and cognitive science to the study of query-term quality. We report on an experiment in which we compare user preference for three sets of terms; one had been preconstructed by a human indexer, and two were identified automatically. Twenty-four participants used a merged list of all terms to answer a carefully created set of questions. By design, the interface constrained users to access the text exclusively via the displayed list of query terms. We found that participants displayed a preference for the human-constructed set of terms eight times greater than the preference for either set of automatically identified terms. We speculate about reasons for this strong preference and discuss the implications for information access. The primary contributions of this research are (a) explication of the concept of user preference as a measure of query-term quality and (b) identification of a replicable procedure for measuring preference for sets of query terms created by different methods, whether human or automatic. All other factors being equal, query terms that users prefer clearly are the best choice for real-world information-access systems. [source]


Instructional Efficiency of Performance Analysis Training for Learners at Different Levels of Competency in Using a Web-Based EPSS

ABSTRACT The measure of performance improvement potential (Gilbert, 1978) in human performance technology uses an exemplary performance as a criterion against which to measure the potential improvement in the performance of a workforce. The measure is calculated based on the performance efficiency which compares expended resources to productivity. The same notion is used to measure the efficiency of instructional conditions, based on learners' mental effort invested in a learning task compared to their performance. This article compares the efficiency of an instructional condition for three groups of students differentiated by their use of an electronic performance support system (EPSS) to conduct a performance analysis project. The results indicated that the instruction was most efficient for those learners who showed greater competency in using the EPSS. Implications for using this technique as a means of formative evaluation of a course are discussed. [source]


Revisiting search task difficulty: Behavioral and individual difference measures

Search task characteristics are recognized as important factors that affect search process and its outcomes. We examine the relationships among operational measures of searcher's behavior, individual cognitive differences, subjective task difficulty and mental effort assessed by dual-task performance. A web-based information study was conduced in a controlled experimental setting. Forty eight study participants performed six search tasks of varying type and structure. Subjective task difficulty was found to be influenced by the searcher's effort measured as the number of result pages and individual documents visited, the number of documents marked as relevant, as well as by individual cognitive differences, and mental effort assessed by performance on the secondary task. In contrast to previous studies, no strong effects of user navigation graph structure were found. [source]


Effects of incentive on working memory capacity: Behavioral and pupillometric data

Abstract We evaluated the hypothesis that individual differences in working memory capacity are explained by variation in mental effort, persons with low capacity exerting less effort than persons with high capacity. Groups previously rated high and low in working memory capacity performed the reading span task under three levels of incentive. The effort hypothesis holds that low span subjects exert less effort during task performance than do high spans. Subjects' pupil sizes were recorded online during task performance as a measure of mental effort. Both recall performance and pupil diameter were found to be increased under incentives, but were additive with span (incentives increased performance and pupil diameter equivalently for both span groups). Contrary to the effort hypothesis, task-evoked pupillary responses indicated that if anything, low span subjects exert more effort than do high spans. [source]


Effects of task difficulty and invested mental effort on peripheral vasoconstriction

Abstract We ran two experiments to investigate whether peripheral arterial tone reflects changes in mental effort. Finger pulse wave amplitude, interpulse interval, and pulse variability in the mid- and high-frequency bands were recorded by means of a newly developed finger plethysmograph during both rest and cognitive performance. Using a modified version of the Sternberg memory task, we selectively manipulated either the difficulty of the task (Experiment 1) or the subjects' level of engagement in the task (Experiment 2). We found a significant difference in finger pulse wave amplitude between rest and task periods, suggesting that the measure reflects changes in sympathetic activity due to task engagement. In addition, our results suggest that reduced pulse wave amplitude, signaling vasoconstriction, occurs when subjects are investing effort. [source]
