Symptom Perception and Adherence to Asthma Controller Medications

Purpose: To explore asthma symptom perception and the relationship between asthma symptom perception and adherence to asthma treatment. Design: Adult patients (N=120) of asthma/allergy specialty clinics, taking AdvairŪ as a controller medication, were enrolled in this cross-sectional descriptive study. Methods: Ninety-seven participants completed 4 weeks of daily diaries to assess subjective symptom perception and measured peak expiratory flow rates (PEFR), both done twice daily. Individual perceptual accuracy scores (PAS) were determined by correlating the subjective symptom perception scores with the PEFRs. Measures included demographic variables, illness identity (personal control and treatment control, consequences, and timeline-cyclical subscales of the IPQ-R), asthma severity (FEV1 percentage) and a single-item indicator of perceived asthma severity. Adherence was measured by the Medication Adherence Report Scale (MARS) and by an AdvairŪ dose count (percentage of doses taken as prescribed). Findings: Independent t tests comparing adherence rates of good versus poor perceivers were not significant, using either the percentage AdvairŪ dose count or the MARS. Multiple regression analyses showed that years with asthma, illness identity, and peak flow variability were all significant explanatory variables for perceptual accuracy. Conclusion: Peak flow variability adds complexity to the relationship between perceptual accuracy and adherence that warrants further investigation. [source]


The Danish version of the Medication Adherence Report Scale: Preliminary Validation in Cancer Pain Patients

Abstract Objective: To examine the psychometric properties of the Danish version of the Medication Adherence Report Scale (DMARS-4) adapted to measure adherence to analgesic regimen among cancer patients. Methods: The validated English version of the Medication Adherence Report Scale was translated into Danish following the repeated back-translation procedure. Cancer patients for the study were recruited from specialized pain management facilities. Thirty-three patients responded to the DMARS-4, the Danish Barriers Questionnaire II, The Danish version of Patient Perceived Involvement in Care Scale measuring the quality of patient-physician pain communication, and the Danish Brief Pain Inventory pain severity scale. Results: A factor analysis of the DMARS-4 resulted in one factor. Mean (SD) score on the cumulative scale ranging from 4 to 20, with higher scores indicating better medication adherence, was 17.8 (0.42). The DMARS-4 scores were related to the measures of patients' concerns about pain management and patients' pain communication. The internal consistency of the DMARS-4 was 0.70. Conclusions: The DMARS-4 seems to be a valid and reliable measure of self-reported adherence to analgesic regimen in the context of cancer pain. [source]


Influence of disease features on adherence to prophylactic migraine medication

Objectives,,, Randomized controlled trials of prophylactic treatments for migraine focus on the effects in an ideal situation and underestimate the impact of non-adherence, which in this study was examined in a natural setting. Materials and methods,,, A sample of 174 adult migraineurs with a current prescription of pharmacological prophylaxis were consecutively recruited at a specialist clinic. Logistic regression analysis was performed to analyse the association between adherence (self-reported with the Medication Adherence Report Scale) and number of years with migraine, frequency of attacks, number of days with migraine per month, attack duration, presence of cardinal features, mean intensity of pain, and recovery between attacks. Results,,, One third (35%) were non-adherent. Neither demographic characteristics nor any of the disease specific variables were significantly associated with adherence. Conclusion,,, Characteristics of the disease per se did not predict non-adherence which was also observed among patients with severe migraine. The full benefit of drugs cannot be realized at currently achieved levels of adherence. [source]
